Schedule H, Part V, Section B, Line 3EThe significant health needs are a prioritized description of the significant health needs of the community and identified through the CHNA.
Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - Nicklaus Children's Hospital. Online Key Informant Survey To solicit input from key informants, those individuals who have a broad interest in the health of the community, an Online Key Informant Survey also was implemented as part of this process. A list of recommended participants was provided by University of Miami Health System, Jackson Health System, and Nicklaus Children's Hospital; this list included names and contact information for physicians, public health representatives, other health professionals, social service providers, and a variety of other community leaders. Potential participants were chosen because of their ability to identify primary concerns of the populations with whom they work, as well as of the community overall. Key informants were contacted by email, introducing the purpose of the survey and providing a link to take the survey online; reminder emails were sent as needed to increase participation. In all, 91 community stakeholders took part in the Online Key Informant Survey. Through this process, input was gathered from several individuals whose organizations work with low-income, minority, or other medically underserved populations. In the online survey, key informants were asked to rate the degree to which various health issues are a problem in their own community. Follow-up questions asked them to describe why they identify problem areas as such and how these might better be addressed. Community Feedback on Prioritization of Health Needs On January 14, 2021, the sponsors of the community feedback on prioritization of health needs assessment convened an online meeting attended by 49 community stakeholders (representing a cross-section of community-based agencies and organizations) to evaluate, discuss and prioritize health issues for community, based on findings of this Community Health Needs Assessment (CHNA). Professional Research Consultants, Inc. (PRC) began the virtual meeting with a presentation of key findings from the CHNA, highlighting the significant health issues identified from the research (see Areas of Opportunity above). Following the data review, PRC answered any questions. Finally, participants were provided an overview of the prioritization exercise that followed. In order to assign priority to the identified health needs (i.e., Areas of Opportunity), a wireless audience response system was used in which each participant was able to register his/her ratings using a small remote keypad. The participants were asked to evaluate each health issue along two criteria: Scope & Severity - The first rating was to gauge the magnitude of the problem in consideration of the following: How many people are affected? How does the local community data compare to state or national levels, or Healthy People 2020 targets? To what degree does each health issue lead to death or disability, impair quality of life, or impact other health issues? Ratings were entered on a scale of 1 (not very prevalent at all, with only minimal health consequences) to 10 (extremely prevalent, with very serious health consequences). Ability to Impact - A second rating was designed to measure the perceived likelihood of the hospital having a positive impact on each health issue, given available resources, competencies, spheres of influence, etc. Ratings were entered on a scale of 1 (no ability to impact) to 10 (great ability to impact). Individuals' ratings for each criteria were averaged for each tested health issue, and then these composite criteria scores were averaged to produce an overall score. This process yielded the following prioritized list of community health needs: 1. COVID-19 2. Access to Health Care Services 3. Mental Health 4. Nutrition, Physical Activity & Weight 5. Heart Disease & Stroke 6. Diabetes 7. Substance Abuse 8. Sexual Health 9. Housing 10. Cancer 11. Injury & Violence 12. Tobacco Use
Schedule H, Part V, Section B, Line 6a Facility , 1Facility , 1 - Nicklaus Children's Hospital. Every three years not-for-profit hospitals are required to conduct a Community Health Needs Assessment (CHNA) of the local populations they serve. This assessment was completed in collaboration with University of Miami Health System (U Health) and Jackson Health System. The Hospital goal in conducting the assessment was to better understand the health status, behaviors, and needs of children and adolescents in South Florida so that we as a community - patients, families, Nicklaus Children's, and partner organizations - can optimally support their health and wellbeing.
Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - Nicklaus Children's Hospital. Access to Healthcare Telehealth: The last two years have been transformational for healthcare access, due to the adoption of telehealth for many outpatient appointment services during the pandemic. In 2020, the number of telehealth appointments increased by 7,641 percent, compared to 2019. In 2020, demand continued for telehealth consultations with subspecialists, enhancing appointment efficiency and supporting children in gaining access to the services they need. Completed a comprehensive assessment of the hospital's access to care (incl. Access to our scheduling services, Scheduling gaps from the provider perspective, Coordinating patient access) and launched a plan to improve access. Initiatives so far have resulted in a reduction of our call center wait times down from 6 minutes to ~2.5 minutes and our hotline abandonment rates down from 6.1% to 2.8% In 2020: Cardiology and Orthopedic surgery both implemented and sustained processes to increase access with greater appointment availability and same day appointment access. Location and providers: Nicklaus Children's continues to enhance access to subspecialty care through provision of services at outpatient centers and offices in locations throughout South Florida. The hospital currently has 20 locations offering subspecialty consults (from southern Miami Dade to Martin county), including several new locations that opened within the last few years. Collectively the entire network of centers bring a broad range of pediatric subspecialty services to Miami-Dade, Broward, Palm Beach and Martin counties, including cardiology and fetal cardiology, gastroenterology, allergy and immunology, endocrinology and diabetes, nephrology, neurosurgery, plastic and reconstructive surgery, rheumatology, spinal disorders, sports health and more. Hired 58 Providers in 2021, 12 in 2020 Diabetes Access to pediatric endocrinology clinicians is key to supporting the care needs of children with diabetes in our community. Nicklaus Children's continues to expand locations where children can receive care with an endocrinologist. In addition to the hospital's main campus near Coral Gables, the following Nicklaus Children's locations in Miami-Dade, Broward and Palm Beach counties now offer endocrinology consults: Aventura, Boynton Beach, Coral Springs, Doral, Palm Beach Gardens, Pinecrest, West Kendall and Weston. Shared a video of Dr. Carillo explaining Type 2 Diabetes with the community https://www.nicklauschildrens.org/news-events/video-center/what-is-type-2-diabetes 2021: House-wide Diabetes Education on wheels hosted by 3NE CS team and Diabetes Educators this week in support of Diabetes Awareness Month in Nov Asthma & Other Respiratory Conditions Nicklaus Children's provides care for children with complex respiratory disorders, including asthma, chronic lung diseases, cystic fibrosis, pneumonia and tuberculosis. The Pulmonology Division offers care for the most complex conditions at the hospital's main campus. Services include an Asthma Center and Cystic Fibrosis Center. In addition, the Division of Allergy and Immunology offers care for children with asthma and allergies at locations throughout the region, including Boynton Beach in Palm Beach County, Miami Lakes and the main campus near Coral Gables. Potentially Disabling Conditions Nicklaus Children's offers a broad range of multidisciplinary specialty services to help children with potentially disabling conditions. It is a regional leader in treatment and management of a full spectrum of conditions, including spasticity and movement disorders, epilepsy, developmental disabilities, metabolic disabilities and degenerative disorders. Cerebral palsy is among the most common disabling conditions in children. The hospital's spasticity and movement disorders program strives to enhance quality of life and movement for these children. New offerings to support children with potentially disabling conditions included MRI-focused ultrasound, an incisionless form of surgery to ablate seizure-producing brain tissue, and single-event multilevel surgery (SEMLS). This approach to surgery aims to address many issues associated with cerebral palsy in one or two procedures, offering an enhanced standard of care. 2020- Launch of Pediatric Advanced Care Team (PACT) palliative service for the CICU/PICU. Mental & Emotional Health The need for mental healthcare services for children and teens has grown in recent years, exacerbated by the pandemic. Starting in 2020, Nicklaus Children's division of behavioral health enhanced access by offering appointments via telehealth. When demand for services exceeded available clinical staffing, the team developed a network of experts in the community to which it could refer children in need. These steps made it possible for the program to address the needs of more children. Nonetheless, demand far exceeds the hospital's ability to serve. Thus, Nicklaus Children's continues to seek opportunities to expand offerings to meet the mental health needs of children and has several collaborations in development. Code Bear - Autism Care -- a collaborative effort with Rehabilitation Services; Nursing and the Brain Institute. Development of a program to follow communication and/or behaviorally vulnerable patients (including Autism) through the continuum of care and beyond a single one-time visit. The Code Bear - Autism Care is a behavioral management program that is designed to identify, alleviate, and if necessary, manage anxiety and fear during hospitalization. Funding provided by the DeLuca Foundation to support an Autism Program navigator, Autism Clinical Coordinator, and equipment. In 2019, the hospital established an intensive outpatient program to support families of children recently hospitalized due to mental health issues. The program helps the family continue to make progress, reducing the incidence of readmission. 2020: PCC, Department of Psychology and Dr. Rivero-Conil collaborated on the PCORI (Patient- Centered Outcomes Research Institute)/Anxiety Study. PCC, Department of Psychology and Center for Children and Families at FIU partnered on a Parenting Study (Preventive Intervention for Behavior Problems in Infants from High-Risk Families) funded by the NIH. New bilingual series, "Let's Talk" was created in response to the pandemic to assist in dealing with stress, resiliency in families and children, communication skills and setting goals and structuring the home environment. Nutrition, Physical Activity, & Weight Nicklaus Children's GiveMe5 program, is a community outreach program, funded by Kohl's, to promote healthy lifestyles for children and families. (see attached PDF for details and data) Representatives of the program participated (pre-COVID-19) in dozens of community events in South Florida, sharing healthy food samples, educational games and fun giveaways to focus children on healthy lifestyles. A core focus of GiveMe5 is promoting consumption of five fruits or vegetables per day. When children follow this guideline, they are less likely to fill up on unhealthy snack foods. During the pandemic, the program transitioned to providing health information through blogs, social media posts and media interviews to encourage children and families to maintain good health habits during lock down. https://www.nicklauschildrens.org/patient-resources/community-outreach/giveme5 An additional pivot of this program during the pandemic conducted screenings for food insecurity. 5% of those families screened positive for food insecurity and were provided with gift cards to local grocery stores. Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceThe organization utilized IRS Worksheet 2 for the calculation of the cost to charge ratio in Part I, Line 7.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountFOR FINANCIAL STATEMENT PURPOSES, the organization HAS ADOPTED ACCOUNTING STANDARDS UPDATE NO. 2014-09 (TOPIC 606). IMPLICIT PRICE CONCESSIONS INCLUDES BAD DEBTS. THEREFORE, BAD DEBTS ARE INCLUDED IN NET PATIENT REVENUE IN ACCORDANCE WITH HEALTHCARE FINANCIAL MANAGEMENT ASSOCIATION STATEMENT NO. 15 AND BAD DEBT EXPENSE IS NOT SEPARATELY REPORTED AS AN EXPENSE.
Schedule H, Part III, Line 3 Bad Debt Expense MethodologySince the organization HAS ADOPTED ACCOUNTING STANDARDS UPDATE NO. 2014-09 (TOPIC 606), BAD DEBTS are included as an implicit price concession in Net Patient Revenue. As a result of this, there are no bad debt expenses attributable to patients eligible for free or discounted services under the financial assistance policy.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteThe System estimates the transaction price for patients with deductibles and coinsurance and from those who are uninsured based on historical experience and current market conditions. The initial estimate of the transaction price is determined by reducing the standard charge by any contractual adjustments, discounts and implicit price concessions. Subsequent changes to the estimate of the transaction price are generally recorded as adjustments to patient service revenue in the period of the change. For the years ended December 31, 2021 and 2020, there was no additional revenue recognized due to changes in its estimates of implicit price concessions, discounts and contractual adjustments for performance obligations satisfied in prior years. Subsequent changes that are determined to be the result of an adverse change in the patient's ability to pay are recorded as bad debt expense.
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsVARIETY CHILDREN'S HOSPITAL ("VCH") ACCEPTS ALL MEDICARE PATIENTS WITH THE KNOWLEDGE THAT THERE MAY BE SHORTFALLS AND OPERATES TO PROMOTE THE HEALTH OF THE COMMUNITY. VCH BELIEVES THAT THE MEDICARE SHORTFALL SHOULD BE TREATED AS A COMMUNITY BENEFIT BECAUSE MEDICARE DOES NOT FULLY COMPENSATE HOSPITALS FOR THE COST OF PROVIDING HOSPITAL CARE TO MEDICARE BENEFICIARIES. VCH EXPERIENCED LOSSES ($107,256) FOR 2021) DUE TO UNDERPAYMENT FOR THE TREATMENT OF MEDICARE PATIENTS. UNREIMBURSED MEDICARE COSTS REPRESENT A SIGNIFICANT VALUE THAT NONPROFIT HOSPITALS PROVIDE TO THE COMMUNITY AS THE HOSPITAL RELIEVES THE FEDERAL GOVERNMENT OF A FINANCIAL BURDEN WHEN IT PROVIDES ESSENTIAL HEALTHCARE SERVICES TO MEDICARE COVERED PATIENTS. THE MEDICARE COST REPORT AND THE ORGANIZATION'S COST ACCOUNTING SYSTEM ARE USED TO DETERMINE THE MEDICARE ALLOWABLE COSTS.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistanceHospital patient accounts may be subject to the following collection actions to be conducted by the Hospital or an authorized party thereof: 1. Following the initial bill, monthly statements and collection letters are mailed and phone calls are completed in an effort to collect all outstanding balances due. 2. The patient responsible portion of an outstanding balance may be transferred to a bad debt collection agency. 3. Payment plan arrangements may be made for patients who indicate they may have difficulty paying their balance in a single installment. If an individual submits a complete FAP application and NCH determines the individual is FAP-eligible for the care, NCH will refund to the individual any amount he or she has paid for the care (whether to the hospital facility or any other party to whom the hospital facility has referred or sold the individual's debt for the care) that exceeds the amount he or she is determined to be personally responsible for paying as a FAP-eligible individual, and take all reasonably available measures to reverse any ECA taken against the individual to obtain payment for the care.

Form 990, Part III, Line 4a CONTINUED PROGRAM DESCRIPTION(CONTINUED FROM PART III) IN IN 2021, 5,127 PATIENTS WERE TRANSFERRED FROM HOSPITALS THROUGHOUT FLORIDA, CENTRAL AND SOUTH AMERICA AND THE CARIBBEAN. WHEN CHILDREN NEED THE SERVICES OF A SPECIALIST, IT IS IMPORTANT THAT SUCH SERVICES BE PROVIDED BY A PRACTITIONER WITH SPECIFIC TRAINING AND EXPERTISE IN PEDIATRIC CARE. THE CARE OF CHILDREN WITH CERTAIN COMPLEX CHRONIC CONDITIONS, SUCH AS JUVENILE DIABETES, CANCER, OR CYSTIC FIBROSIS IS BEST MANAGED BY A PEDIATRIC SPECIALIST IN ORDER TO ELIMINATE REFERRAL DELAYS AND EXPEDITE POSITIVE TREATMENT. CHILDREN ARE MORE VULNERABLE TO COMPLICATIONS DUE TO DELAYS THAN ADULTS. THEY DEVELOP SERIOUS COMPLICATIONS MORE RAPIDLY THAN ADULTS BECAUSE THEY HAVE LESS PHYSIOLOGICAL RESERVES AND LESS DEVELOPED ORGAN SYSTEMS. IN 2021 THE HOSPITAL HAD APPROXIMATELY 286 PROVIDERS TO SUPPORT THE ONGOING FUNCTIONS OF NICKLAUS CHILDREN'S HEALTH SYSTEM AND NICKLAUS CHILDREN'S HOSPITAL IN THE FOLLOWING SUB-SPECIALTIES: ADOLESCENT MEDICINE, ALLERGY/IMMUNOLOGY, CARDIOLOGY, CARDIOVASCULAR SURGERY, CLINICAL GENETICS AND METABOLISM, CRITICAL CARE MEDICINE, DENTISTRY, EMERGENCY MEDICINE, ENDOCRINOLOGY, GASTROENTEROLOGY, GENERAL PEDIATRICS, INFECTIOUS DISEASES, MATERNAL FETAL, NEPHROLOGY, NEUROPSYCHOLOGY, NEUROSURGERY, NUTRITION, OPHTHALMOLOGY, ORTHOPEDICS, OTOLARYNGOLOGY, PALLIATIVE CARE, PLASTIC AND RECONSTRUCTIVE SURGERY, PSYCHIATRY AND PSYCHOLOGY, RADIOLOGY/SEDATION AND RHEUMATOLOGY.THIS CRITICAL MASS OF PHYSICIANS SIGNIFICANTLY CONTRIBUTES TO THE QUALITY AND RANGE OF PEDIATRIC SERVICES AVAILABLE AT THE HOSPITAL. 2021 STATISTICAL DATA INCLUDES TOTAL INPATIENT DAYS: 60,971 TOTAL INPATIENT ADMISSIONS: 9,509 INPATIENT AVERAGE DAILY CENSUS: 167.0 TOTAL OUTPATIENT VISITS (MAIN CAMPUS): 182,433 TOTAL OUTPATIENT VISITS (AMBULATORY CENTERS): 289,416 TOTAL AMBULATORY URGENT CARE VISITS: 163,695 TOTAL SURGICAL CASES: 9,668 AVERAGE LENGTH OF STAY: 6.4 DAYS THE HOSPITAL IS INTERNATIONALLY RECOGNIZED FOR CARE EXCELLENCE. Five Nicklaus Children's Hospital specialty programs are again identified among the best in the nation, according toU.S. News & World Report's 2021-22 Best Children's Hospitals rankings.Nicklaus Children's has more ranked pediatric programs than any other hospital in South Florida. NICKLAUS CHILDREN'S 2021-22 RANKINGS ARE AS FOLLOWS: CARDIOLOGY & HEART SURGERY, DIABETES & ENDOCRINOLOGY NEONATOLOGY, NEUROLOGY & NEUROSURGERY ORTHOPEDICS AND PULMONOLOGY. Nicklaus Children's Hospital is included in Newsweek's list of World's Best Specialized Hospitals 2022. This recognition is presented by Newsweek and Statista Inc., a statistics portal and industry-ranking provider. No other South Florida pediatric hospital was recognized in the ranking. In the 2022-23 U.S. News & World Report Best Children's Hospitals Nicklaus Children's Hospital is ranked as the #1 children's hospital in Florida and is the only children's hospital that is top-ranked in South Florida. Forty-three pediatric specialists from the Nicklaus Children's Hospital medical staff have been designated among America's Top Doctors for 2022 by Castle and Connolly, a New York City-based research and information company dedicated to identifying top healthcare resources. Nicklaus Children's Hospital completed the ISO 9001 Quality Management System certification by DNV GL. Nicklaus Children's is the second freestanding children's hospital in the nation to obtain this certification. Since the hospital's initial DNV GL accreditation in 2016, Nicklaus Children's and its network of outpatient centers have participated in annual accreditation site visit evaluations to ensure steady progression toward ISO 9001 certification. ISO 9001 sets forth criteria for quality management that include a strong customer focus, engagement of top management, a process approach and continual improvement. The ultimate impact of ISO within hospitals is the reduction or elimination of variation, so that critical work processes are done consistently and the "best ideas" aren't by one person or department, but are ingrained in the organization itself. Nicklaus Quality Program is data driven and dedicated to the development and implementation of consistency in the service we provide. Our quality measures are based on national standards and are formed around the patient centered framework put forth by the Institute for Healthcare Improvement (IHI) which aligns to the principles of safety, equity, effectiveness, timeliness, and efficiency. Using industry benchmarks, internal standards, and the resources of national survey and compliance services, we seek to constantly improve our pediatric care. The quality department serves as a vital part of improvement and resource management throughout the institution. Our team of dedicated physicians, nurses, quality and accreditation specialists, and data analysts are working to achieve customer care excellence, standardization of practice, transparency in performance, a Just Culture for improvement, and assurance to ourselves and the patients and families we serve that we are delivering the best possible pediatric care. Nicklaus monitors internal performance in real time and formally on a monthly basis using an extensive, metric driven, nationally benchmarked scorecard. Improvement opportunities and action planning are addressed using a structured, collaborative, multidisciplinary approach in which quality becomes the interest and responsibility of every employee. Quality performance and action planning are monitored at a monthly cadence through the multidisciplinary Performance Improvement Council with regular oversight by the Quality Committee of the System Board.
