Schedule H, Part V, Section B, Line 3EThe significant health needs are a prioritized description of the significant health needs of the community and identified through the CHNA.
Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - Merrick Medical Center. Merrick Medical Center participated in the Community Health Needs Assessment that was completed in 2021 and led by the Central District Health Department. We used that report as a basis for this Community Health Needs Assessment. In efforts to learn more about the impact of COVID-19 on communities in the Central District Health Department (CDHD) launched a 5-question survey. The survey was developed by the Nebraska Association of Local Health Directors (NALHD) as an open-ended survey designed intended to allow respondents to tell LHDs their experience related to their health and the health of their community to identify emerging issues in the community. The NALHD made the survey accessible to all LHDs across Nebraska to identify statewide impact and trends. The NALHD partnered with representatives from CDHD with knowledge and expertise relevant to the needs of the medically underserved and low income populations. This survey will assist CDHD by highlighting community themes and strengths that may not be identified solely with the use of secondary data sources. The survey assessed experiences of community members related issues for them or their family, what it means to be healthy, top health concerns, and ways to be healthy in their community and was available in English, Spanish, Somali, and Arabic by print and online. Participants of the survey were residents of Merrick County, Hamilton County, and Hall County. The survey was also distributed to CDHD partners, including Multicultural Coalition, area hospitals, and others. Additional, a resource inventory survey was launched to partners of CDHC in August 2021 as a way to provide insight into available medical resources, resources that help people prevent and manage personal health risks, and resources that help people thrive. June 15, 2021, 35 partners partners participated in a focused discussion to identify forces that impact health in communities within the CDHD area as part of this community health assessment. The participants in the focused discussion revealed the following through the surveys of the Top 3 health concerns from the overall respondents: 1.) Cancer 2.) Diabetes and 3.) Getting enough exercise.
Schedule H, Part V, Section B, Line 6b Facility , 1Facility , 1 - Central District Health Department. The Central District Health Department serves a three county area including Merrick County. As a health department, it offers the following services: breastfeeding peer counselor programs, immunization clinics, public health messaging, refugee services, health services and screenings, Women, Infants & Children (WIC), in addition to environmental health services, emergency preparedness and various other health projects.
Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - Merrick Medical Center. On September 2, 2021 the CDHD held a meeting to discuss the findings from the five questions survey. 33 participants attended this meeting from Merrick, Hall, and Hamilton Counties. The survey revealed the following top 3 health concerns: 1.) Cancer. 2.) Diabetes 3.) Getting enough exercise. 1.) Cancer Cancer is the leading cause of death in the CDHD district and across the state of Nebraska. In the CDHD region, female breast cancer was the leading type of cancer diagnosed. Prostate cancer followed as a close second for the CDHD district. Notably, Merrick County residents experience more cancer than their counterparts in Hall or Hamilton counties and the state. Lung (and bronchus) cancer was the leading type of cancer that resulted in death in the CDHD district. Tobacco smoking remains the leading cause of lung cancer and other causes include exposure to secondhand smoke and radon. 2.) Diabetes Diabetes is a chronic disease that impacts how a body gets energy from food. Diabetes is the 7th leading cause of death in the IS with more than 88 million US adults diagnosed with diabetes. Overweight/obesity and age are factors that impact the risk of diabetes. Generally, diabetes rates in the CDHD region are similar to the state rate, except for Merrick County which experienced a slightly higher diabetes rate than the other counties within CDHD due to a higher proportion of an aging population in this county. African American/Black (15%), American Indian/Alaskan Native (16%), and Hispanic (14%) populations experience almost 2 times the rates of diabetes compared to non-Hispanic, Whites. As affirmation to the prevalence and factors contributing to diabetes, respondents to the CDHD Community Survey identified diabetes as one of the top three health concerns. 3.) Physical Activity and Nutrition According to the Nebraska BRFSS, healthy eating and active living was not a routine behavior for many adults in the CDHD district. 40% of the adults in this area reported consuming fruits less than 1 time per day and about 1 in 4 adults consumed vegetables less than 1 time per day. 85% of adults in the CDHD region indicated that they have access to safe places to walk in their neighborhoods, 1 in 3 adults reported no-leisure time physical activity in the past 30 days. Non-White, Hispanic respondents to the CDHD survey identified challenges getting healthy and affordable food as one of the top three health concerns. The CDHD district experienced shortages in primary care, dental, and mental health professionals, and further reducing access to needed health services. Merrick Medical Center will have a shared responsibility to their community and other hospitals within the CDHD to create a CHI plan. Each hospital will share their success and challenges for others to replicate. The tri-county wide hospital planning will begin on January of 2022. Once the focus has been identified, the hospitals will bring it to the community stakeholders to vet and create actionable tactics that complement our strategic plan, community, and county residents.
