DescConcordia Visiting Nurses (CVN) program service accomplishments are driven by the core mission to provide excellent in-home health services to those who need them, in a faithful environment. CVN is part of a large continuum of care striving to succeed with an aging-in-place philosophy by providing a wide breadth of services in the home, preventing rehospitalization, enhancing home safety and monitoring chronic disease states. During the fiscal year ending June 30, 2020, approximately 15,376 patients were served across thirteen counties in Western Pennsylvania with an average daily census of 2,731. A total of approximately 305,801 visits (including 5,165 non billable visits associated with spiritual care, nutrition, etc.) were completed by various disciplines. The non reimbursed cost of the non billable spiritual care visits was $112,257 for the fiscal year. Additionally, as part of the prevention efforts for the deterioration of existing health conditions, CVN provides Tele-Health monitoring free of charge to patients with chronic diseases like congestive heart failure. Tele-Health monitoring is a non billable service that costs the agency $421,763.00 annually. Tele-Health has also been utilized to provide additional monitoring for COVID-19 positive patient vital sign trending. In response to the pandemic, the Tele-Health program has expanded to include Tele-Video visits in order to provide collaborative care with physicians. In April 2020, the agency purchased an additional 50 monitors that included the virtual visit technology. CVN has also been working with our partner hospitals to provide Tele-Video visits during the public health emergency. On January 31, 2020, acting HHS Secretary Alex Azar first declared a public health emergency following the World Health Organization's recognition of the novel strain of coronavirus, COVID-19, as a pandemic. In response to this coronavirus outbreak, the state and federal governments have taken preventative or protective actions, such as imposing restrictions on travel and business operations and advising or requiring individuals to limit or forego their time outside of their homes. Daily high-level leadership meetings were established to assess and provide guidance to staff through every changing regulation surrounding the pandemic. Throughout the pandemic, education and guidance related to best practices and CDC protocol was provided by our corporate compliance team. This allowed our frontline workers to remain safe and able to provide quality care to many COVID affected patients and families. Employees were awarded a daily appreciation bonus for each shift worked. Even with decreased census when surrounding hospitals shut down, there were no layoffs as a result of the pandemic. A call center was established to call any current or referred patients even if they were declining visits to ensure there wasn't an exasperation of symptoms to avoid rehospitalizations. The agency worked with a corporate purchasing team to ensure appropriate levels of personal protective equipment (PPE), laboratory equipment, tests and other related COVID response items were purchased to provide care is the safest way possible. The agency purchased $106,394 of PPE. CVN serves residents in the following counties; Allegheny, Armstrong, Beaver, Butler, Greene, Lawrence, Westmoreland, Washington and parts of Clarion, Fayette, Indiana, Mercer and Venango counties. Over the last year, the agency has been working to achieve accreditation through the Accreditation Commission for Health Care (ACHC). The initial survey was completed in May 2020 with completion of the accreditation process in July 2020. By pursuing and achieving accreditation, CVN has demonstrated that it meets the standards for quality and is committed to pursuing excellence. This accreditation represents the highest level of accreditation that can be given to an organization and shows our organization's substantial conformance to the ACHC standards. CVN employs a team of Transitional Nurse Liaisons who serve the community by providing a variety of information, screenings, speaking topics on healthy living and disease prevention. They also team up with CVN nurses, and social workers to provide Health Chats. Each month our Transitional Nurse Liaisons schedule and attend several community gatherings to discuss our "Concordia Visiting Nurses Healthy Topic" of the month at health fairs, community speaker programs and events. Due to the pandemic many of these events were held virtually. Our goal is to reach as many people as possible in the community to promote wellness and teach prevention. Ideally, we would like to prevent the deterioration of existing health conditions and identify and teach early intervention for exacerbations of diseases and careful management to prevent hospitalizations. In providing screenings, we are attempting to be proactive in the early diagnosis and treatment of chronic disease conditions. Any senior adult with questionable screening results or symptoms are always referred to their physician for further testing and treatment and timely follow-up occurs with the senior by a member of our team. We are receptive to and always looking for ways to enhance Concordia's exposure throughout the community. These free education and screening events are conducted at a variety of apartments, housing centers, senior centers, events at local malls, churches, civic centers, and other health care facilities. 964 flu shots were provided by CVN to employees and to the community at several Flu Clinics which were held at various sites across our service area during the 2019-2020 fiscal year. During the 2019-2020 fiscal year, the following (but not limited to) organizations have received direct financial support from CVN through donations and sponsorships of various events: ACMH Foundation, Alzheimer's Association, American Cancer Society, Armstrong County Health Care Consortium, Butler County Healthcare Consortium, Butler Health System Foundation, Center for Hope, CLM Foundation-Charitable Fund, CLM Foundation-Concordia at Villa St Joseph, CLM Foundation-GSH Purse Gala, Community Health Center, Concordia Haven Apartments - Veteran's Luncheon, County of Butler Children & Youth Agency, Faith Restorations, Heritage Valley Beaver Foundation, Hero's Angels, Inter Church Food Bank, Knoch Cross-Country Booster Association, Knoch Softball Boosters, Knoch Track and Field Boosters, Meals on Wheels, Ohio Valley Wine and Dine Celebration, St Clair Hospital, St Luke Lutheran Church, Terry George Fund, Washington Health System Foundation, We Care Street Outreach. Total donations and sponsorships made during fiscal year ended 6/30/20, equated $38,933. Additional funding via distributions to 501(c)(3) member hospital systems at a total of $5,500,000 have expanded like-minded mission within the geographical communities we serve. Many employees of CVN volunteer their time to serve at the Annual Summer Festival (July 2019) and Octoberfest (October 2019), both of which are sponsored by Concordia Lutheran Health and Human Care and are open to the public as a way of providing entertainment, food and refreshments and a feeling of community. Employees of CVN were given the opportunity to participate in several mission events organized by the parent company Concordia Community Support Services. These are part of our ongoing Mission Committee that was formed to organize opportunities for employees to serve the community. In the summer of 2019, CVN employees partnered with Freeport High School to assist with their Blessings in a Backpack program. This program supplies the families of children on the free lunch program with groceries during the summer months when school is not in session. Our employees worked in teams which picked up groceries from Walmart and filled 20-25 bags with the non-perishable items and dropped off the completed bags at the school. There were three food packing days that fell in the fiscal year ending June 30, 2020. A Servant Day/House Build was held on Sept 21, 2019 when employees partnered with St Luke Lutheran Church to construct the frame for a 1,500 square foot house. The team was assisted by the Center for Community Resources from Butler, PA and Crossroads Mission-Help Build Hope from Louisville, KY. Other service opportunities available that day were making fleece blankets for Ronald McDonald House, sewing dresses for girls in Tanzania, making greeting cards for Military families, preparing dry soup mix for local food pantries. For the Christmas 2019 season, employees received the Christmas wish lists of 24 children through a partnership with Butler County Children and Youth. Employees purchased gifts and wrapping paper to give to the families in the program. In March 2020, CVN participated in a clothing drive for the homeless in Washington County. These events are some of the ways we serve God by providing for the needs of His people.Desc

ExplanationThe Organization is a not-for-profit corporation as described in Section 501(c)(3) of the Internal Revenue Code (Code) and has been recognized as tax exempt under Section 501(a) of the Code. Accordingly, no provision for income taxes has been provided. Accounting principles generally accepted in the United States of America require management to evaluate tax positions taken by the Organization and recognize a tax liability or asset if the Organization has taken an uncertain position that more likely than not would be sustained upon examination by the Internal Revenue Services (IRS). The Organization has concluded that as of June 30, 2020 and 2019, there are no uncertain positions taken or expected to be taken that would require recognition of a liability, asset, or disclosure in the financial statements. Generally, tax returns for years ended June 30, 2017, and thereafter remain subject to examination by federal and state tax authorities.ExplanationTxt
