The March 2010 passage of the Patient Protection and Affordable Care Act (ACA) introduced reporting requirements for private, not-for-profit hospitals. To meet these new federal requirements, the information gathered in this assessment is used to guide the strategic planning process in addressing health disparities. A Community Health Needs Assessment gives information to health care providers to make decisions and commit resources to areas of greatest need, making the greatest impact on community health status. This assessment incorporates data from within the community, such as individuals served and health organizations, as well as vital statistics and other existing health-related data to develop a tailored plan which targets the needs of the county. METHODS An assessment team comprised of McLeod Health's Community Health and Communication and Public Information staff reviewed literature, data and publications from public sources. Members of the assessment team represented each of the seven acute care hospital facilities within McLeod Health and were assigned to collect data that represented indicators of community health status or its socioeconomic determinants. Therefore, focus was placed on identifying locally-appropriate indicators, benchmarks, and pertinent health issues. Pre-existing databases containing local, state and national health and behavior data were used for comparisons when possible. Data collection was limited to the most recent publicly available resources and some primary data from qualitative and quantitative investigation. As a result, this document portrays a partial picture of the health status of the community served. Data analysis included demographic, socioeconomic and health determinant measures. Data analysis included demographic, socioeconomic and health determinant measures. When possible, data also was analyzed according to age, gender and/or race to offer insight into health disparities that may affect specific subgroups in the community. A summary of county data is reflected as a comparison to state and national data when available to indicate community health concerns. HEALTH DETERMINANTS AND DISPARITIES What are the determinants of health? Health behaviors had the majority overall impact on future health outcomes (i.e., smoking, diet, drug & alcohol use, physical activity, other lifestyle behaviors) and account for 40% of causes for premature death. Genetic predisposition is responsible for 30%, Social circumstances 15%, and Health care for only 10% (i.e., access to physician and other health services) of health risk for premature death. Behavioral Determinants (40%) Examples: - Diet - Physical activity - Alcohol, cigarette, and other drug use - Hand washing Genetic Determinants (30%) Examples: - Age - Sex - HIV status - Inherited conditions, such as sickle-cell anemia, hemophilia, and cystic fibrosis - Carrying the BRCA1 or BRCA2 gene, which increases risk for breast and ovarian cancer - Family history of heart disease, cancer, etc. Social Determinants (15%) Examples: - Availability of resources to meet daily needs, such as educational and job opportunities, living wages, or healthful foods - Social norms and attitudes, such as discrimination - Exposure to crime, violence, and social disorder, such as the presence of trash - Social support and social interactions - socioeconomic conditions, such as concentrated poverty - Quality schools - Transportation options - Public safety Health Care Determinants (10%) Examples: - Quality, affordability, and availability of services - Lack of insurance coverage - Limited language access Environmental Determinants (5%) Examples: - Quality of food, water, and air - Worksites, schools, and recreational settings - Housing, homes, and neighborhoods - Exposure to toxic substances and other physical hazards - Physical barriers, especially for people with disabilities What are health disparities? "Health disparity" refers to a higher burden of illness, injury, disability, or mortality experienced by one population group relative to another group. Health disparities can involve the medical care differences between groups in health insurance coverage, access to care, and quality of care. While disparities are commonly viewed through the lens of race and ethnicity, they occur across many dimensions, including socioeconomic status, age, location, gender, and disability status. Poor health status is often linked with people without health insurance, those who have poor access of care (i.e., limited transportation), lower socioeconomic status, lower education obtainment, and those among racial minority groups. Beyond the provision of health care services, eliminating health disparities will necessitate behavioral, environmental, and social-level approaches to address issues such as insufficient education, inadequate housing, exposure to violence, and limited opportunities to earn a livable wage. Health disparities have persisted across the nation and have been documented for many decades and, despite overall improvements in population health over time, many disparities have persisted and, in some cases, widened. Moreover, economic downturns contributed to a further widening of disparities. The Community Health Needs Assessment attempts to identify and quantify the health disparities within a defined county population that are at disproportionately higher in incidence of disease, disability, or at risk of experiencing worse health outcomes. Within these identified disparities and availability of health resources, gaps can be identified and prioritized based on need so that health resources can be targeted. Planning initiatives to address community health needs take in consideration the existing initiatives, the available resources that we are aware of, and where future improvements can be anticipated to make meaningful impact on improving community health. What are Key Initiatives to reduce disparities? IN 2010, THE U.S. DEPARTMENT OF HEALTH AND HUMAN SERVICES (HHS) ESTABLISHED A VISION OF, "A NATION FREE OF DISPARITIES IN HEALTH AND HEALTH CARE,SET OUT A SERIES OF PRIORITIES, STRATEGIES, ACTIONS, AND GOALS TO ACHIEVE THIS VISION. STATES, LOCAL COMMUNITIES, PRIVATE ORGANIZATIONS, AND PROVIDERS ALSO ARE ENGAGED IN EFFORTS TO REDUCE HEALTH DISPARITIES. Federal, state, and local agencies and programs work along with local hospitals, often in cooperation, to provide access to needed health care services. Within constraints of limited resources, each of these entities generally target populations with specific services offered within the county. This study attempts to incorporate their input into determining the priorities among health disparities and look for opportunities for collaboration. Preventative Care Preventative care includes medical services such as screen

SCH H. PART III, LINE 4NET PATIENT REVENUES ARE REPORTED AT THE ESTIMATED NET REALIZABLE AMOUNT RECEIVED, OR TO BE RECEIVED, FROM PATIENTS, THIRD PARTY PAYOR, AND OTHERS FOR THE SPECIFIC SERVICES AND SUPPLIES RENDERED, INCLUDING ESTIMATED RETROACTIVE ADJUSTMENTS UNDER REIMBURSEMENT AGREEMENT WITH THIRD PARTY PAYOR. FOR THE AMOUNTS REPORTED IN LINES 2 AND 3, WE USED THE IRS METHOD TO CALCULATE THE RATIO OF PATIENT COST TO CHARGES, AS SHOWN ON PAGE 13 OF THE INSTRUCTIONS FOR SCHEDULE H (WORKSHEET 2). THE CALCULATED RATIO WAS THEN APPLIED TO THE GROSS CHARGES WRITTEN OFF TO BAD DEBT (NET OF RECOVERIES).
SCH H, PART III, LINE 8THE ORGANIZATION FEELS THAT THE TOTAL SHORTFALL OF MEDICARE REIMBURSEMENT COMPARED TO COMPUTED MEDICARE ALLOWABLE COSTS SHOULD BE TREATED AS A COMMUNITY BENEFIT. THE HOSPITAL IMPROVES ACCESS TO PATIENT CARE BY PROVIDING SERVICES REGARDLESS OF A PATIENT'S ABILITY TO PAY OR THE HOSPITAL'S ABILITY TO RECEIVE FULL COST REIMBURSEMENT FOR SERVICES. THE HOSPITAL ALSO RELIEVES THE GOVERNMENT OF A FINANCIAL BURDEN WHEN IT PROVIDES CARE TO PUBLICLY-INSURED PATIENTS WHERE REIMBURSEMENT IS LESS THAN THE COST OF PROVIDING THE SERVICE.
SCH H, PART III, LINE 9BMCLEOD'S CHARITY POLICY OUTLINES THE CRITERIA USED TO DETERMINE PATIENTS WHO QUALIFY FOR CHARITY. WHEN PATIENTS HAVE FURNISHED THE REQUIRED INFORMATION, IT IS REVIEWED AND A DETERMINATION IS MADE. IF APPROVED FOR CHARITY CARE, THEIR ACCOUNT BALANCES ARE ADJUSTED BASED ON THE PERCENTAGE THEY QUALIFY FOR USING A CHARITY ADJUSTMENT CODE. IF ALL REQUIRED INFORMATION IS NOT FURNISHED, THE PATIENT IS NOTIFIED THAT THEIR CHARITY APPLICATION WAS NOT APPROVED DUE TO FAILURE TO PROVIDE THE NECESSARY INFORMATION. FOLLOWING THAT NOTIFICATION, THE ACCOUNT GENERALLY TRANSFERS TO BAD DEBT FOR FURTHER COLLECTION ACTION.
SCH H, PART VI, LINE 2In addition to the CHNA described above for McLeod Loris Seacoast Hospital. The hospital has a community board that consists of local physicians and other influential community leaders. This board meets monthly (alternating between in-person and virtual meetings) and the leaders provide input from various parts of the community to assist the coastal region in assessing the health care needs of the community.
SCH H, PART VI, LINE 3UNINSURED PATIENTS ARE SCREENED AT THE TIME OF REGISTRATION FOR THEIR ABILITY TO PAY FOR THEIR HEALTHCARE SERVICES. IF THE PATIENT HAS NO ABILITY TO PAY AND IS DEEMED INELIGIBLE FOR GOVERNMENTAL PROGRAMS (MEDICARE, MEDICAID, ETC.) THEN THEY ARE INFORMED OF THE HOSPITAL CHARITY PROGRAM. THEY ARE PROVIDED WITH AN APPLICATION AND A LISTING OF THE APPROPRIATE DOCUMENTS NECESSARY TO ESTABLISH ELIGIBILITY FOR THE HOSPITAL CHARITY PROGRAM.
SCH H, PART VI, LINE 4MCLEOD LORIS SEACOAST HOSPITAL (MLSH), TOGETHER WITH ITS RELATED ORGANIZATIONS MCLEOD REGIONAL MEDICAL CENTER OF THE PEE DEE, INC., MCLEOD MEDICAL CENTER-DILLON, MCLEOD HEALTH CLARENDON, MCLEOD HEALTH CHERAW, AND MCLEOD PHYSICIAN ASSOCIATES II CONSIDERS ITS PRIMARY SERVICE AREA (PSA) AS THE SOUTH CAROLINA COUNTIES OF FLORENCE, DARLINGTON, CHESTERFIELD, DILLON, CLARENDON, HORRY, MARION, AND MARLBORO, AND ITS SECONDARY SERVICE AREA (SSA) AS THE SOUTH CAROLINA COUNTIES OF GEORGETOWN, LEE, SUMTER, AND WILLIAMSBURG. THESE TWELVE COUNTIES MAKE UP THE NORTHEASTERN PORTION OF SOUTH CAROLINA. MLSH HAS THE GREAT MAJORITY OF ITS DISCHARGES FROM THE COUNTY OF HORRY.
SCH H, PART VI, LINE 5Community Benefit Report: McLeod Loris Seacoast Hospital primarily serves northern Horry County and the bordering North Carolina geographies. The population of Horry County is approximately 374,000. Horry County is in the Northeastern corner of South Carolina and is a county assigned to the Pee Dee region of the state. The county's per capita income has lagged both the US and South Carolina averages, and the percentage of its population living below the poverty level is much higher than the national average. In Horry County, the number of uninsured has reached 26%. McLeod Loris Seacoast Hospital is one hospital with three campuses within Horry County. The medical staff consists of more than 200 active and affiliate physicians serving these communities. Short summaries of the three campuses in Horry County follow: McLeod Health Loris: Founded in 1950, McLeod Health Loris is a fully-accredited acute care facility with 50 licensed beds. It offers a wide range of inpatient and outpatient services. McLeod Health Loris has been a vital part of Loris for more than 70 years. As healthcare needs have changed, Mcleod Health Loris has changed to offer new services and technology for its patients. Our team consists of compassionate professionals dedicated to providing quality care and services. McLeod Health Loris specializes in general surgery, dialysis access, women's services, urological surgery, and physical/occupational/speech therapies. The 24/7 Emergency Department, medical/surgical inpatient beds, intensive care unit (ICU), and advanced diagnostic imaging provides surrounding communities with access to hospital care that services an important community benefit. In January 2020, McLeod Health Loris provided another key health care service to the community: a dialysis access center for patients in need of medical repair of their access area where they receive their dialysis treatments. Dialysis is life-sustaining medical treatment for so many who suffer from kidney disease. This treatment aids in preventing delays for dialysis treatment because of medical complications associated with access. McLeod Health Seacoast: McLeod Health Seacoast, located in Little River, SC in the North Myrtle Beach area. McLeod Health Seacoast opened as an outpatient facility in October 2000. In July 2011, McLeod Health Seacoast underwent its first major expansion and added 50 hospital beds. Today, McLeod Health Seacoast offers a wide range of high-quality services, including general, vascular, and orthopedic surgery, cardiology, and cancer care. In 2016, Seacoast expanded its emergency department as part of the first phase in an overall construction plan that included a new inpatient tower and operating rooms. The second phase of construction, completed in March 2017, included a new fast track area to treat less complex emergency cases. In March 2016, McLeod Health opened the first building on the new McLeod Health Carolina Forest campus. This campus is designed to provide comprehensive outpatient services in the heart of Myrtle Beach and Carolina Forest communities. Four primary care physicians as well as a urology practice are in building one. In November 2017, the second building was opened on the McLeod Health Carolina Forest campus. This building is home to McLeod Cardiology Associates Carolina Forest as well as a shared medical specialists suite that allows for regular rotations from our area medical specialists. Plans for outpatient imaging in that building are currently underway. In September 2018, McLeod Health Seacoast opened its new patient tower following the completion of a $110 million dollar expansion that more than doubled its patient capacity. The patient tower added 55 beds bringing total capacity to 105 beds. In addition to the increased patient rooms, the expansion includes six new operating rooms increased total operating room capacity to 10 rooms providing the community with more access to surgical care for emergent and elective surgeries. McLeod Health Seacoast completed construction on its new diagnostic cardiac catheterization and electrophysiology labs in June 2019. These labs were added to meet the cardiac needs of the growing community that McLeod Health Seacoast serves. McLeod Health Carolina Forest In March 2016, McLeod Health started the 43-acre Carolina Forest campus. This campus was designed to provide comprehensive outpatient services in the heart of Myrtle Beach to the Carolina Forest community, one of the fastest growing areas in Horry County. Currently, the campus houses two medical office buildings providing primary care services along with several medical and surgical specialists to support the growing medical needs of this community. Access to outpatient physician care is key to improving the health status of the community. In addition to physician medical practices, in October 2019, McLeod Health opened a 13,000 square foot provider-based emergency department. X-ray, CT, and lab services support the emergency department but also are available on an outpatient basis for patients. In addition, an outpatient rehabilitation department provides physical, speech, pediatric, and occupational health services. This campus continues to expand to meet the needs of the community with a third building currently under construction.
