Group Reynolds Memorial Hospital Line Part V, Section B, Line 5The CHNA was prepared by Reynolds Memorial Hospital, Inc with assistance from the WVU School of Public Health. It focused on its primary service area, Marshall County, WV and to a lesser degree, the surrounding counties of Ohio and Wetzel, West Virginia Belmont, Ohio and Greene, Pennsylvania. The collection and analysis of primary and secondary data included the input from both public and private organizations, such as faith-based organizations, government agencies, educational systems, and health and human services. In total, the primary data collection phase resulted in the contribution of thirteen community stakeholders/leaders as well as many community residents. Primary data was collected in the form of an online and paper survey which was used to obtain feedback from community residents and the hospital staff. Secondary data was also utilized to provide additional insight and knowledge about the community and stakeholders RMH serves. The CHNA can be found here https//wvumedicine.org/wp-content/uploads/2020/11/RMH-2017-CHNA.pdf.
Group Reynolds Memorial Hospital Line Part V, Section B, Line 11Consistent with the 2014 CHNA the three biggest priority health needs identified in our 2017 CHNA were chronic disease, substance abuse, and cancer. Reynolds Memorial Hospital has taken various actions to assist those who battle with chronic diseases. RMH has employed a cardiologist, is currently developing an oncology center, has implemented a Telestroke program and has introduced a new service for asthma patients. To combat substance abuse, Reynolds Memorial Hospital has partnered with Breakthru to implement a medical drug withdrawal program and with local law enforcement by providing them with biohazard sharps containers for hypodermic needle disposal. RMH has hired three primary care physicians, opened an urgent care and established an additional primary care physician in the area. RMH partnered with Health Net Life to station a life flight helicopter in the county, offered educational materials through the cancer resource center, offered educational programs such as Safety Town for children, provides CPR classes and participates with Partners in Education with two Marshall County Schools.
Group Reynolds Memorial Hospital Line Part V, Section B, Line 11Several issues were identified as reasons individuals were unable to access care. The inability to pay deductibles and copays, lack of availability of doctors, no insurance and the inability to pay for health care, fear of facing health problems, and transportation were top problems. The prioritization team noted this as an area to be aware of, but it was not deemed a priority for the purpose of this needs assessment. The full assessment and implementation policy can be found online https//wvumedicine.org/wp-content/uploads/2020/11/RMH-2017-CHNA.pdf.
Group Reynolds Memorial Hospital Line Part V, Section B, Line 13bReynolds Memorial Hospital offers discounted care only to individuals who qualify for free care. Reynolds Memorial Hospital offers a Prompt-Pay Discount Program - patients who have no third party coverage uninsured or uninsurable may be eligible for a 50 self-pay discount while patients who have a guarantor balance may be eligible for a 20 discount. To be eligible for either prompt pay discount, the patient/guarantor must meet certain requirements.
Group Reynolds Memorial Hospital Line Part V, Section B, Line 24All patients are charged an amount equal to gross charges regardless of payment method. Once FAP eligibility is confirmed, charge amounts are moved to Charity Care and no longer charged to the patient.
Group Reynolds Memorial HospitalPart V Line 12b - On March 25, 2020, the IRS unveiled The People First Initiative Initiative, which was the IRSs effort to help taxpayers during the COVID-19 Pandemic. IRS Commissioner Chuck Rettig described the Initiative as follows The IRS is pursuing unprecedented actions to ease the burden on people facing tax issues. During this difficult time, we want people working together, focused on their well-being, helping each other and others less fortunate. The Initiative contained some specific measures. For instance, the IRS allowed taxpayers to suspend installment agreements, stopped the issuance of liens and levies, and suspended all face-to-face meetings with field revenue agents. The IRS has made clear that the initiative was and is broader than these types of specific measures. In other words, the purpose of the Initiative was to accommodate and provide leniency to taxpayers, in particular taxpayers who were hit hardest by the pandemic.
Group Reynolds Memorial HospitalPart V line 12b - Moreover, Treas. Reg. section 1.501r-2b establishes that the excise tax under I.R.C. section 4959 will not be assessed if a taxpayer has reasonable cause for failure to meet the CHNA. The fact that a hospital has established procedures to comply with the CHNA is a factor supporting reasonable cause. Treas. Reg. section 1.501r-2b4. All tax law support that reasonable cause requires only ordinary care and prudence.
Group Reynolds Memorial HospitalPart V line 12 - In short, the Taxpayer is a hospital that could not reasonably be expected to timely complete its 2020 CHNA because of the pandemic. The assessment of a 50,000 excise tax runs contrary to both reasonable cause and the IRSs own Initiative.
Group Reynolds Memorial HospitalPart V line 12b - Prior to 2020, the Taxpayer always complied with its CHNA filing obligation. The Taxpayer had every intention of complying in 2020 and took specific measures to complete its CHNA. The Taxpayer hired a third party consulting firm to assist with its CHNA in early 2020. The Taxpayer had meetings to establish a CHNA Leadership Team, provided its final survey to the consulting firm for community distribution, ordered printed copies where required, received and processed digital survey responses, maintained a database, and continually followed up with its third party consulting firm. Despite the Taxpayers efforts, the Taxpayer was unable to complete its CHNA due to the Pandemic. The Pandemic caused a host of issues, including but not limited to - overall time compression at the hospital, constant loss of personnel, nonresponsiveness by the third party consulting firm, quarantines, the cancellation of various school and community events where data is ordinarily procured, the inability to procure paper surveys in waiting rooms, mailing complications and delays, and the inability to visit members of the community in person. The Taxpayer is actively working to finalize its CHNA, and anticipates completion by December 7, 2021.

Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Part I Line 3cDiscounted services, for those over 200 FPG, are determined by evaluating income, assets, and liabilities on a case by case basis. The hospital also offers discounts to self pay patients for prompt payment, and also accepts payment plans over time without charging interest.
Part I Line 6aRMH community benefit numbers are reported in total with the other hospitals within West Virginia United Health System. The most recent amounts can be found on WVU Medicines website at https//wvumedicine.org/about/leadership-and-more/community-benefit/
Part I Line 7Total community benefit expense for 2020 is 20,068,397 and is 23 of total net expenses. To calculate net expense, provision for doubtful accounts of 6,785,534 was deducted from total expenses of 94,041,684 as shown in Part IX line 25 of the core Form 990, for a net expense of 87,256,150.
Part IIRMH did not report any community building acitivies for 2020.
Part III Line 2Bad Debt Expense at cost was calculated by multiplying provision for doubtful accounts of 6,785,534 by our cost to charge ratio of 38.56 derived from Worksheet 2 in the IRS Schedule H instructions for a total of 2,616,502 less DSH allocations to self-pay patient accounts of 1,280 for a total of 2,615,222.
Part III Line 3Estimated bad debt attributable to patients eligible for charity care was calculated by running a report within our patient revenue software of all bad debt account balances greater than 15,000. The total of that report was 201,809 which we then multipled by our cost to charge ratio of 38.56 for a total of 77,817.
Part III Line 3The estimated bad debt attributable to patients eligible for charity care of 77,817 should be considered community benefit due to the fact that anyone with outstanding balances of 15,000 or greater usually qualifies as catastrophic if the patient completes the application process. In our charity care policy, we define catastrophic care as any illness or injury that will likely require continuous or frequent treatment for more than one year. Regardless, that bad debt should be considered community benefit as we provide services to those in need regardless of their ability to pay.
Part III Line 4Reynolds Memorial Hospitals financial statements are prepared on a consolidated basis as a member of the WV United Health System. The footnote for Accounts Receivable is as follows Patient accounts receivable are reported at net realizable value. For accounts receivable associated with services provided to patients who have third-party coverage, the System estimates net realizable value based on the estimated contractual reimbursement percentage, which in turn is based on current contract provisions and historical paid claims by payor. For self-pay accounts, including uninsured and patient responsibility accounts, the net realizable value is determined using historical collection experience, adjusted for estimated conversions of patient responsibility portions, expected recoveries and changes in trends to estimate implicit price concessions. Management continually reviews the estimated net realizable value of accounts receivable by monitoring cash collections, economic conditions and trends, changes in payor mix, changes in federal or state healthcare coverage and other matters. The System grants credit without collateral to its patients, most of whom are local residents and are insured under third-party payor agreements, primarily with Medicare, Medicaid and various commercial insurance companies. The System records accounts receivable net of estimated price concessions and such amounts have historically been within managements expectations. The mix of accounts receivable at December 31, 2020 from patients and third-party payors is as follows Medicare - 29, Medicaid - 17, Blue Cross - 21, Commercial, managed care and other - 28, Patients - 5, totaling 100.
Part III Line 4For receivables associated with self-pay patients, which includes both patients without insurance and insured patients with deductible and copayment balances, the System records a significant provision for bad debts in the period of service on the basis of its past experience, which indicates that many patients are unable or unwilling to pay the portion of their bill for which they are financially responsible. The difference between the billed rates and the amounts actually collected after all reasonable collection efforts have been exhausted is charged off against the allowance for doubtful accounts.
Part III Line 8The amount reported in Part III Line 6 45,059,640 was calculated using the total Medicare allowable cost from the Medicare cost report, less Medicare reimbursement of direct GME.
Part III Line 8Reynolds shortfall, 4,264,950 of Medicare program reimbursement should be considered a community benefit because we are relieving a government burden by providing care in excess of our costs to these patients.
Part III Line 9bRMH does have a debt collection policy. When a patient has been approved for financial assistance under our charity care policy, they will not be sent to bad debt. Additionally, if a patient is being evaluated for charity, the patient will not be sent to bad debt agency pending charity guarantor status until the pending status has been finalized approved/denied. All other patients with outstanding balances will be processed through billing and collections pursuant to our Financial Policy.
Part VI Line 2With assistance from the WVU School of Public Health, Reynolds Memorial Hospital completed a Community Health Needs Assessment CHNA in 2017. The CHNA can be found here https//wvumedicine.org/wp-content/uploads/2020/11/RMH-2017-CHNA.pdf.
Part VI Line 2The CHNA process for Reynolds Memorial Hospital included the collection and analysis of primary and secondary data. Both public and private organizations, such as faith-based organizations, government agencies, educational systems and health and human services entities were engaged to assess the needs of the community. RMH believes that the health status of a community depends on many factors, including quality of health care, social and economic determinants, individual behaviors, heredity, education and the physical environment. Population and demographic data is obtained through WVU Medicines population health and enterprise analytics department.
Part VI Line 3The hospital employs one resource finding clerk to assist patients in obtaining benefits from Medicare, Medicaid, Vocational Rehab, and other federal and state sources.
Part VI Line 3Our charity care eligibility guidelines are also listed on our website at https//wvumedicine.org/bill-pay/financial-assistance/.
Part VI Line 4Reynolds Memorial Hospital RMH defines their community service area as Marshall County. Marshall County has a total population of 30,591, a median household income of 48,557 and an employment rate of 49.3. 14.5 of Marshall County residents live in poverty and 6.7 are without health care coverage. Surrounding communities in Ohio and Wetzel Counties, WV Belmont County, OH and Greene County, PA are also served by RMH. Ohio County, WV had a total population of 42,425, a median household income of 50,584 and 4.3 of residents reported they did not have health care coverage. Wetzel County, WV had a total population of 14,442 a median household income of 43,107 and 6 of residents reported they did not have health care coverage. Belmont County, OH had a total population of 66,497 with a median household income of 49,780 and 6.6 of residents reported they did not have health care coverage. Greene County, PA had a total population of 35,954 with a median household income of 54,776 and 4.6 of residents reported they did not have health care coverage. Population and demographic information was obtained from the United States Census Bureau https//data.census.gov/cedsci/.
Part VI Line 5RMH has taken various actions to promote community health within its defined community service area. In an effort to offer more specialty care, RMH has employed a cardiologist and a new oncology center is being developed. RMH has also hired three primary care physicians and opened an urgent care office to make health care services more convenient and readily available. RMH has partnered with HealthNet Life to station a life flight helicopter in Marshall County in an effort to improve critical emergency care.
Part VI Line 5The Reynolds Memorial Hospital board of directors is a community board, with thirteen members, most of which reside in or around Glen Dale, WV.
Part VI Line 5Reynolds Memorial Hospital extends privileges to all qualified medical staff in Marshall County and surrounding communities to meet its healthcare needs.
Part VI Line 6Reynolds Memorial Hospital is a part of the WV United Health System WVUHS. WVUHS is a not-for-profit corporation formed to serve as part of an integrated health science and healthcare delivery system. WVUHS serves as the parent corporation to an affiliated group of healthcare providing entities. The strategic plan of the System states intent to build a regional health care delivery system in its services area, while offering a variety of options for providers who want to participate. The System maintains a demonstrated commitment to assist rural communities in preserving and improving the health care available to the patients it serves.
Part VI Line 6System management is focused on recruitment of staff and employees to meet the growing needs of the aging population in the Systems service areas. Other hospitals in the System include West Virginia University Hospitals, Inc. in Morgantown, WV, United Hospital Center in Bridgeport, WV, Camden-Clark Memorial Hospital in Parkersburg, WV, City Hospital, Inc. in Martinsburg, WV, Jefferson Memorial Hospital in Ranson, WV, Potomac Valley Hospital in Keyser, WV, St. Josephs Hospital of Buckhannon, Inc. in Buckhannon, WV, West Virginia Healthcare Cooperative, doing business as, Summersville Regional Medical Center in Summersville, WV, Braxton County Memorial Hospital in Gassaway, WV, Community Health Association, doing business as, Jackson General Hospital in Ripley, WV, and Wetzel County Hospital, Inc. in New Martinsville, WV.
