Desc74 bed general medical surgical hospital providing inpatient and outpatient services with the following service statistics for 2020; 7,250 inpatient days, 657 observation visits, 625 new born days, 1,961 admissions, 336 nursery admissions, 16,596 emergency room visits, 85,407 outpatient visits and 3,640 day surgery visits. In January 2020, the Secretary of the U.S. Department of Health and Human Services (HHS) declared a national public health emergency due to a novel strain of coronavirus (COVID-19) and in March 2020 the World Health Organization declared the spread of the virus to be a pandemic. In early March 2020, the Governor of New York also declared a state of emergency which provided various orders by the State, Department of Health, and other state agencies to respond to COVID-19. Among such orders, all hospitals were required to cancel non-essential elective procedures and non-urgent procedures effective March 25, 2020 in order to protect patients and the healthcare workforce, conserve resources including personal protective equipment, and increase available beds to service COVID-19 cases. In coordination with State health authorities, the Hospital initiated emergency measures to prevent avoidable transmission of the illness, including canceling non-urgent elective surgeries, procedures, and clinic visits. The Hospital also implemented significant new safety measures, expanded critical care bed capacity, acquired personal protective equipment, expanded testing capabilities, and redeployed clinical and nonclinical staff to work in areas where the need was most urgent and transition to telemedicine visits to continue to provide primary clinic services in a touchless environment. The cancelation of elective procedures and non-urgent ambulatory visits resulted in a significant reduction in patient volumes. In addition to the lost revenue that accompanied the lower patient volumes, the Hospital experienced a significant increase in operating expenses associated with the purchase of personal protective equipment, certain pharmaceuticals, COVID-19 testing supplies, and other overhead and patient specific costs. In response to COVID-19, the federal government took various actions intended to assist healthcare providers and in March 2020 the Coronavirus Aid, Relief, and Economic Security (CARES) Act became law. The CARES Act included Provider Relief Funds (PRF), which are not subject to repayment if certain terms and conditions provided by HHS are met. Generally, those terms and conditions require PRF payments will only be used to prevent, prepare for, and respond to coronavirus and shall only reimburse the recipients for health care-related expenses or lost revenues that are attributable to coronavirus, so long as such expenses and lost revenues have not been reimbursed by other sources. During 2020, the Hospital received $9.6 million of PRF payments which were recognized in other operating revenue in the statement of operations.Desc

Schedule H, Part V, Section B, Line 5-Nathan Littauer Hospital AssociationThe Ad Hoc Data Sub-Committee of the Adirondack Rural Health Network (ARHN) drafted a survey to obtain input from the community. ARHN surveyed key informants in the seven county service area, to provide the CHA Committee with input on regional health care needs and priorities. The survey was developed using Survey Monkey; it included 14 community health questions, as well as several demographic questions. The CHA Committee provided a list of health care, social service, education, government, and service providers by county to survey. The collected distribution list totaled 807 key informants. In early January 2019, the CHA Committee partners sent an email to community stakeholders to introduce the survey and provide a web-based link. ARHN staff sent a second email approximately two weeks later after the initial reach out. CHA Committee members received the names of all non-respondents for additional follow up, at partner discretion. The survey requested that key informants identify the top two New York State Prevention Agenda priority areas from a list of five. Key informants also gave insight on what they felt were the top health concerns and what contributing factors were most influential for those specific health concerns. Chosen priority areas were analyzed by totaling the responses by county. The priority area that received the most responses is the First Choice, followed by the second most responses listed as Second Choice. Fulton County informants identified Promote Well-Being and Prevent Mental and Substance Use Disorders as their top priority and Prevent Chronic Disease as their second choice.
Schedule H, Part V, Section B, Line 6a-Nathan Littauer Hospital AssociationThe Community Health Assessment Committee is comprised of representatives from hospitals; Adirondack Health, Glens Falls Hospital, University of Vermont Health Network- Alice Hyde Medical Center, University of Vermont Health Network - Champlain Valley Physicians Hospital, and University of Vermont Health Network- Elizabethtown Community Hospital.
Schedule H, Part V, Section B, Line 6b-Nathan Littauer Hospital AssociationThe Community Health Assessment Committee is comprised of representatives from organizations other than hospital facilities including; Clinton County Health Department, Essex County Public Health, Franklin County Public Health, Fulton County Public Health, Hamilton County Public Health, Warren County Health Service and Washington County Health Services.
Schedule H, Part V, Section B, Line 11-Nathan Littauer Hospital AssociationAfter reviewing the survey results and determining the ability to affect the focus areas, Nathan Littauer Hospital and Fulton County Public Health Department selected the following priorities; #1-Prevent Chronic Disease with a focus area of Preventive Care and Management and #2-Prevent Communicable Diseases with a focus area of Antimicrobial Resistance and Healthcare-Associated Infections for our 2019-2021 service plan. To address the Prevention of chronic disease priority, Nathan Littaer Hospital offers evidence-based self-management programs. Data shows the programs increase self-efficacy in managing one's health, are effective in increasing or maintaining independence, positive health behavior and or mobility. Reduction in disability (fewer falls, later onset or fewer years of disability, etc.) along with pain reduction are other outcomes. Mental health (including delays in loss of cognitive function and positive effects on depressive symptoms) and over-all quality of life are also improved. Rural residents have limited access to care due to fewer options for public transportation, socioeconomic conditions, access to mass media and emerging technologies, built environment, and some physical barriers. It is our goal to improve self-management skills for individuals with chronic disease, including asthma, arthritis, cardiovascular disease, diabetes, prediabetes and obesity. The benefits of utilizing evidence-based programs carry over from the community members to Community-Based and Health Care Organizations. Efficient use of available resources occur as it facilitates partnerships and clinical linkages, which lead to greater opportunity for varied funding sources. Fewer hospital and doctor visits, lower health care costs, better health outcomes and a more positive health care experiences. Nathan Littauer Hospital has staff trained to facilitate the following evidence-based programs; Chronic Disease Self-Management Program (CDSMP), Diabetes Self-Management Program (DSMP), National Diabetes Prevention Program (NDPP), Chronic Pain Self-Management Program (CPSMP), The Butt Stops Here (TBSH), Active Living Every Day (ALED), Healthy Eating Every Day (HEED), and Tai Chi for Arthritis and Fall Prevention. We plan to expand access to evidence-based self-management interventions for individuals with arthritis, asthma, cardiovascular disease, and diabetes We will measure the results by looking at; the number of workshops delivered, the number of participants who complete 4 out of 6 sessions, and the number of sites and or counties where workshops are held. Concerning Priority #2 the Prevention of Communicable Diseases, The CDC has identified certain disparities increase the risk of antibiotic-resistance. These include; increased age, people residing in a health care setting, living in a rural area and lower socioeconomic status. The data suggests that Fulton County would be at an increased risk. The population of people 65 years of age and older is higher than that in other areas of New York State, and the household income is much lower, with more people living below the Federal Poverty Level. Nathan Littauer Hospital & Nursing Home plans to build a team of clinicians and support personal that will collaborate with Fulton County Public Health to decide on educational material to disseminate into the community. Visiting nursing agencies and local pharmacies asked to collaborate with us on developing a cohesive messaging plan. Our hospital's marketing team, including our graphic designers will consult with our team to develop the final material. We will utilize our in-house print shop to produce the materials. The identified priority not chosen by Nathan Littauer Hospital was Promote Well-Being and Prevent Mental and Substance Use Disorders. Stakeholders identified this priority as the second largest need in our area. Due to limitations, Nathan Littauer Hospital & Nursing Home did not choose to focus on this priority. Our organization does not have the clinical infrastructure to address mental health issues at the hospital level for on-going treatment. We do refer to appropriate providers and hospitals in the area and our trained clinicians can meet the needs of a patient with a mental health issue in a medical setting. We have cooperative relationships with the Family Counseling Center, even offering them office space in exchange for quick mental health referrals for our patients in crisis. We refer to a neighboring hospital's mental health treatment facilities available. On the Primary Care level, we are able to provide situational mental health assistance; providing medication as indicated and/or referral to appropriate local and regional resources. The ARHN CHA voted unanimously to address this as our regional priority. Nathan Littauer Hospital & Nursing Home will work in conjunction with ARNH to implement meaningful NY Prevention Agenda interventions to the best of our abilities. Additionally we will work with Fulton County Department of Public Health as members of the Mohawk Valley Population Health Improvement Program (MVPHIP) identified behavioral health as a priority and has a workgroup to address stigma and provide mental health resources to the region.
Schedule H, Part V, Section B, Line 20d-Nathan Littauer Hospital AssociationThe Hospital does not have adequate resources to accurately predict those who would qualify for its Financial Assistance Program.

Schedule H, Part I, Line 7The costing methodology used to calculate the amounts reported in the Table for Line 7 is the ratio of patient cost to patient charges.
Schedule H, Part I, Line 7, Column fBad Debt Expense was not included in Form 990, Part IX, line 25, Column (A), and not subtracted for purpose of calculating the percentage in Schedule H Part I, Line 7 column (f).
Schedule H, Part I, Line 7gSubsidized health services reported included details from the Primary/Specialty Care Centers and licensed clinics. The Centers operate despite financial losses to the Organization for at least the last fifteen years. The Hospital is committed to operate the Clinics as a benefit to the community in order address an identified need and to improve access to healthcare, including our remote rural communities.
Schedule H, Part III, Section A, Line 2Due to the adoption of ASU No. 2014-09- Revenue from contracts with Customers (Topic 606) bad debt expense is no longer reported on the audited financial statements. Rather, it is treated as a price concession. Revenue from Contracts with Customers (Topic 606), which outlines a single comprehensive model for entities to use in accounting for revenue arising from contracts with customers and supersedes most current revenue recognition guidance, including industry-specific guidance, and requires significantly expanded disclosures about revenue recognition. The core principle of the revenue model is that an entity recognizes revenue to depict the transfer of promised goods or services to customers in an amount that reflects the consideration to which the entity expects to be entitled in exchange for those goods or services.
Schedule H, Part III, Section A, Line 3Due to the adoption of ASU No.
Schedule H, Part III, Section A, Line 4The Hospital provides care to patients who meet certain criteria under its charity care policy without charge or at amounts less than its established rates. Because the Hospital does not pursue collection of amounts determined to qualify as charity care, they are not reported as revenue. The Hospital maintains records to identify and monitor the level of charity care it provides. Those records include the amount of charges for services and supplies furnished to patients at no charge. The level of charity care, measured on the basis of cost, is approximately $204,000 and $247,000 for 2020 and 2019, respectively. The cost of charity care provided was determined based on the application of the ratio of the Hospital's overall cost to patient charges. Included in net patient service revenue for 2020 and 2019 is approximately $112,000 and $52,000, respectively in net uncompensated care distributions received from New York State to support the cost of charity care. The Hospital annually conducts a comprehensive assessment of the community and charitable benefits it provides. A copy of the Nathan Littauer Hospital Community Service Report is available on the Hospital's website.
Schedule H, Part III, Section B, Line 8Medicare allowable costs were calculated using a cost to charge ratio based upon the cost finding methodology in the Medicare 2252-10 Cost Report. Community Benefit rationale: The Medicare shortfall should be considered a community benefit because our mission is to promote health in the community and we do not limit the care available to any patient, including those covered by Medicare. We are relieving a government burden by providing care to Medicare patients even though costs exceed reimbursements by $2,905,318. Tax exempt hospital are expected to participate in the Medicare program. For many of the medical services provided by the Hospital, Medicare does not provide sufficient reimbursement to cover the cost of providing care to our patients, forcing the Hospital to use other funds to cover the deficit. Medicare Cost Reports that hospitals are required to file do not include the costs required to treat all Medicare patients. Therefore, the amount reflected on the Medicare Cost Report differs from actual losses. In 2020 the Hospital realized an additional shortfall of $6,716,580 resulting from Medicare Advantage and non-PPS Medicare patients. The total shortfall for all types of Medicare patients was $9,621,898.
Schedule H, Part III, Section C, Line 9bRequests for Financial Assistance may be made at any point before, during or after the provision of care. Anyone seeking Financial Assistance will be given an application. Financial Assistance requests may be proposed by sources other than the patient, such as patient's physician, family members, social services or hospital personnel. Pending final eligibility determination, Nathan Littauer Hospital will not initiate collection efforts or requests for deposits provided that the responsible party is cooperative with Nathan Littauer Hospital's efforts to get payment from other sources. If initial screening does not disqualify someone for Financial Assistance they will be given an application, which includes instructions on how to apply.
Schedule H, Part VI, Line 2The Mohawk Valley Population Health Improvement Program (MVPHIP) partnered with Fulton County Public Health Department and Nathan Littauer Hospital & Nursing Home to complete their 2019-2021 Community Health Needs Assessment utilizing key informant survey data compiled by Adirondack Rural Health Network. The key informants represent a broad range of sectors, community interests, and includes organizations that represent the medically underserved, low-income, and minority populations. In addition to the key informant responses, the assessment includes quantitative data sources collected and published by New York State, as well as health indicators included on the MVPHIP website compiled by Healthy Communities Institute. The site includes a comprehensive dashboard of community indicators covering over 20 topics in the areas of health, social determinants of health, and quality of life from public secondary data sources. Fulton County Public Health Department and Nathan Littauer Hospital & Nursing Home also collected a small sample of community members' views on the health of the community.
Schedule H, Part VI, Line 3NLH educates our patient population for our Financial Assistance policy in the following ways: NLH has provided a "Frequently Asked Question" section on our website (www.nlh.org) to answer questions our patients may have. We have posted the complete Charity/Financial assistance policy on our website (www.nlh.org). NLH posts in their registration areas the availability of Financial Assistance and the appropriate contact information to obtain and complete the application process. NLH also provides pamphlets in all of our inpatient and outpatient registration areas literature providing information on insurances and again, providing information on our Financial Assistance program and contact information. NLH has four facilitated enrollers available to assist patients with their application for Medicaid, Quality Health Plan and Child Health Plus. They will also assist patients in completing our charity care application. Our staff has been trained in assisting our patients with obtaining application and providing direction and contact information for the completion of same. Our in-patient statements and our billing agency provides information advising patients to contact our offices to request the application and assistance in its completion.
