DescCabinet Peaks Medical Center operates a 25 bed critical access hospital providing medical services to the residents of Lincoln County. Due to the remote location of Lincoln County, Cabinet Peaks serves a critical role in caring for the medical needs of its community, as the next closest emergency room is over 50 miles away and the nearest regional hospital is 90 miles away. As such Cabinet Peaks makes it a priority to provide an Intensive Care Unit, 24/7 Emergency Services, Birthing Services, and extensive Surgery Services. Cabinet Peaks also offers a complement of outpatient services, including primary care and OB/GYN services, a variety of rehab and therapy services, infusion and chemotherapy, imaging, and lab. Continued on Schedule O.Patients often do not have the luxury of time or money or the physical strength necessary to make the 90 mile trip to Kalispell to receive these services, which is why Cabinet Peaks recognizes the value of providing these services close to home. In fiscal year 2020, Cabinet Peaks Medical Center provided services to 12,849 unique patients amounting to a total of 17,373 adjusted patient days. Just over 952 surgical cases were performed. Additionally, Cabinet Peaks Medical Center delivered 97 babies during fiscal year 2020. As Lincoln County consistently has one of the top unemployment rates in the State and is an economically depressed area, many of Cabinet Peaks' patients are uninsured or underinsured, meaning Cabinet Peaks provides a significant amount of uncompensated care. The Hospital provides health care services to patients who meet certain criteria under its charity care policy without charge or at amounts less than established rates. Since the Hospital does not pursue collection of these amounts, they are not reported as patient service revenue. The amounts of charges forgone, based on established rates, were $666,373 and $518,569 during the years ended March 31, 2020 and 2019, respectively. The estimated cost of providing these services was approximately $365,000 and $282,000 for the years ended March 31, 2020 and 2019, respectively, calculated by multiplying the ratio of cost to gross charges for the Hospital by the gross uncompensated charges associated with providing charity care to its patients.Desc

Cabinet Peaks Medical CenterPart V, Section B, Line 5: In July 2017, surveys were mailed out to the residents in Cabinet Peak Medical Center's service area. Additionally, two focus groups were held to identify important local healthcare issues, how to improve the health of the community and gaps in health services. It was intended that this research would help determine the awareness of local programs and services, as well as the level of satisfaction with local services, providers, and facilities.A Steering Committee was convened to assist Cabinet Peaks Medical Center in conducting CHSD. A diverse group of community members representing various organizations and populations within the community (ex. public health, elderly, uninsured) came together in May 2017. The Steering Committee met twice during the CHSD process; first to discuss health concerns in the community and offer their perspective in designing the survey instrument and again to review results of the survey and focus groups and to assist in the prioritization of health needs to address. The Steering Committee was comprised of public leaders including members of other healthcare organizations, school system, athletic clubs, local politicians, governmental organizations, public health, community service organizations, etc.An Implementation Planning Committee was comprised of Cabinet Peaks Medical Center's leadership team.
Cabinet Peaks Medical CenterPart V, Section B, Line 11: Goal 1: Assure access to healthcare services in Lincoln County through increasing community knowledge of primary and specialty care services. In FY20, We have created Same Day appointment slots (a minimum of 4 per provider per day) that are available. Marketing has worked diligently to disseminate a message to the community about the availability of Same Day Appointments. Psychiatry via telehealth will now be available for utilization at the Family Medicine clinic for their patient base to provide consultations and follow up care collaboratively with the Primary Care Providers. Additional actions were accomplished in previous fiscal years.Goal 2: Enhance community collaboration to promote community health in Lincoln County. A Cabinet Peaks employee serves as a member of the Lincoln City/County health board and continues to ensure appropriate stakeholders/projects are included through this forum. CPMC continues to participate in the Lincoln County Health Alliance Project working to improve population health through integrated health information exchange platforms. LCHA received an Outreach Grant for 2018-2021 and completed multiple projects aimed at improving interoperability within the Alliance. Additional actions were accomplished in previous fiscal years.Goal 3: Increase Cabinet Peaks Medical Center's education and outreach to improve the community's health as well as knowledge of available and affordable healthcare services. CPMC has continued our employment of Patient Account representatives to assist patients. Additionally, registration In-Services have been held to educate staff about insurances. Additional actions were accomplished in previous fiscal years. CPMC continues to participate in events such as CHC Fundraiser, Youth Runnerfell, PIP Fun Run, STOKR ,etc and plan to continue to do so for the foreseeable future. CPMC hosted our annual Community Health Fair. The Fair was well received and over 500 community members were served. Cabinet Peaks hosted annual "Paint it Pink" event. This year, the month held a fun run, logger week, dessert, PNAC week, Bling a bra, Hava Java, Flocking Season, Ladies Night Out, Jeans Day, Brews for Benefits, Paint the Town Pink Day, Chick Flick Night, Bracelet Sales, Etc. Proceeds from this year Paint it Pink events went towards providing mammograms and breast ultrasounds for the uninsured in our services area. Additionally, Cabinet Peaks hosted its annual Breast Health Awareness Dessert. Cabinet Peaks also continued its medical mentorship program for area high school students.Goal 4: Enhance outreach to the community by promoting healthy living for Seniors in Lincoln County. CPMC continues to educate the community on the new behavioral health program 'Senior Life Solutions.' CPMC partnered with LCCP to develop a senior resource directory and assisted with dissemination of the directory. The Palliative Care program is up and running at Cabinet Peaks Medical Center.Goal 5: Enhance and support community programs and activities that address behavioral health, mental health and chemical dependency services in Lincoln County. CPMC continues its 'Senior Life Solutions' program focusing on intensive outpatient therapy for seniors (65+). CPMC continues its participation on Unite for Youth Board and Coalition. CMPC continues its participation on Lincoln County Mental Health Coalition. CPC started providing telepsychiatry services November 2019 for direct patient care and BHC consultations. CPMC meets monthly with other healthcare/community partners with local chemical dependency program to enhance care transitions for patients in need. CPMC calls AA and Al-Anon to provide onsite support and education at CPMC as needed. CPMC continues supporting the prescription disposal program.

Part I, Line 3c:Cabinet Peaks Medical Center also uses an asset test. A patient must meet both the income and asset criteria.
Part I, Line 6a:The Organization's community benefit report can be found on its website at www.cabinetpeaks.org.
Part I, Line 7:Charity care expense was converted to cost using an overall cost-to-charge ratio addressing all patient segments. Community health improvement services, line 7e, health professions education, line 7f, and cash and in-kind contributions, line 7i, are reported based on actual expenses. The cost for subsidized health services on line 7g was determined using the Medicare cost report.
Part I, Line 7, Column (f):The Bad Debt expense included on Form 990, Part IX, Line 25, Column (A), but subtracted for purposes of calculating the percentage in this column is $ 1,033,928.
Part II, Community Building Activities:The Hospital is involved in various community activities that are indirectly related to the health and well-being of the community. Employees actively participate on the boards of other non-profits and organizations that promote economic development. Community building activities are essential to the economic growth of the community which in turn affects access to healthcare and overall better health for all individuals in the area.
Part III, Line 2:Bad debt is the unpaid obligation for care provided to patients who may be unable or unwilling to pay or who do not qualify for charity care. Prior to any account being classified as bad debt, all payments are applied including contractual allowances and discounts from Medicare, Medicaid, and insurance companies. Accounts from patients qualifying for charity care or the SharePay sliding scale financial assistance program are not written off to bad debt. Every effort is made with the patient in an attempt to collect the outstanding balance. Payments on accounts that were written off as bad debt reduces bad debt expense. The Organization's bad debt is reported as charges incurred during the year.
Part III, Line 3:Cabinet Peaks Medical Center budgets for and monitors the level of charity care it provides. The estimated amount of the Organization's bad debt expense attributable to patients eligible under the Organization's charity care policy cannot be estimated due to the expectation that all eligible individuals would apply for financial assistance.
Part III, Line 4:The footnote to the Organization's financial statements that describes bad debt expense is located on pages 12-13 of the attached financial statements.
Part III, Line 8:The expenses represent an overall shortfall for the Medicare patient population. The costs themselves are a community benefit as the community would not receive these services if we did not provide them.Total revenue received from Medicare is the gross reimbursement plus settlement. Both total revenue received from Medicare and the Medicare allowable costs are reported from the Medicare Cost Report. The Medicare Cost Report is completed based on the rules and regulations set forth by Centers for Medicare and Medicaid Services.
Part III, Line 9b:There are no collection efforts for patients that qualifed for Share Pay. However, there may be collection actions on their personal portion not covered by Share Pay. Our sliding scale financial assistance program is designed to meet the medical financial obligations of the uninsured and underinsured.
Part VI, Line 2:CPMC utilizes a third party to collect patient input and perspective. This company calls a sample of our patients who received services in Acute Care, Emergency Department and Surgery to see if we are meeting their expectations. CPMC reviews the compiled data and comments monthly. Additionally, CPMC holds a yearly Patient and Family Advisory Council (PFAC) Focus Group meeting to listen to what our patients have to say about the care and services they receive. This group participates by openly answering questions about their experience, what worked and what did not as well as an opportunity for them to make suggestion on ways we can improve in the future.Thirdly, annually we invite a patient who received less than excellent care to attend one of CPMC's Board of Director's meetings to speak about their experiences and offer insight into ways we can improve. In addition to these formal methods for feedback, we are continuously rounding with patients to ensure their needs are met and we regularly attend community events and meetings to stay abreast on our community's needs and initiative. A complete community health needs assessment was conducted and completed during the 2018 fiscal year. Cabinet Peaks Medical Center participated in the Community Health Services Development (CHSD) process, a Community Health Needs Assessment (CHNA), conducted by the Montana Office of Rural Health (MORH). Community involvement in steering committee meetings and focus groups enhanced the community's engagement in the assessment process. The hospital also completed and the Board approved our implementation plan with MORH for the CHNA in FY18. Over the past two years CPMC has made significant progress on achieving the items identified in the implementation plan.
Part VI, Line 3:CPMC publicizes the availability of financial assistance through the following methods:A. Postage of signage in the Patient Admitting and Registration areas as well as the Emergency Department and other areas throughout the Hospital's facilities offering meaningful visibility;B. Posting the Financial Assistance Policy, the Financial Assistance Application, and the Plain-Language Summary on CPMC's website;C. Including the Plain-Language Summary in patient registration materials and inpatient handbooks; andD. Including the Plain-Language Summary in materials offered to each patient as part of the intake or discharge process.CPMC also includes in each billing statement sent to the patient a statement regarding the availability of financial assistance, including a website address where the Financial Assistance Policy, Financial Assistance Application, and the Plain Language Summary can be found as well as a phone number for information about the Financial Assistance Policy and application process. In addition, the hospital employs self-pay account representatives to assist patients in determining if they qualify for free or discounted care. The account representatives also provide hands-on assistance to patients applying for other financial assistance such as CHIP's, heating assistance, Medicaid, and WIC. Applications for charity care are also discussed at registration and discharge and with patients who have bills that are past due.
Part VI, Line 4:Cabinet Peaks Medical Center is located in Libby, a very remote town in northwestern Montana. Libby is 72 miles south of the Canadian border, 89 miles northwest of Kalispell, Montana, and 35 miles east of the Idaho border. Libby is the county seat for Lincoln County. Due to its remote nature, Libby relies upon transport services, including helicopter and fixed wing transports, to ensure patients requiring specialized care can travel the longer distances to access larger facilities, usually in Spokane, Kalispell, or Missoula. The closest hospital is Bonner General Hospital about 47 miles from Libby in Sandpoint, ID, which is also of a somewhat limited scope of practice. The next largest hospital with more specialized and high acuity services is Kalispell Regional Medical Center 90 miles from Libby.CPMC has been designated a Medically Underserved Population and a Health Professional Shortage Area due to our economic challenges, lack of resources and remote location. We are a relatively poor county, with 22.8% of Libby population being at or below the poverty level, compared to a state average of 15.2%.
Part VI, Line 5:Cabinet Peaks Medical Center operates a 25 bed critical access hospital providing medical services to the residents of Lincoln County. Cabinet Peaks also offers a complement of outpatient services, including a variety of rehab and therapy services, infusion and chemotherapy, family medicine, imaging, and lab. Patients often do not have the luxury of time or money or the physical strength necessary to make the 90 mile trip to Kalispell to receive these services, which is why Cabinet Peaks recognizes the value of providing these services close to home. As Lincoln County consistently has one of the top unemployment rates in the State and is an economically depressed area, many of Cabinet Peaks' patients are uninsured or underinsured, meaning Cabinet Peaks provides a significant amount of uncompensated care. Emergency and other medically necessary services are provided to all persons, regardless of their ability to pay.In addition to the services provided within the Hospital campus, Cabinet Peaks also does a considerable amount of outreach to promote the health of the community. Organizing and funding a community health fair, providing discounted services for mammography and laboratory tests, providing speakers for health lectures, promoting hospital tours for the schools, and providing internship opportunities for the schools are just a few ways Cabinet Peaks partners to promote the health of the community. Cabinet Peaks' mission is to provide quality compassionate care to its community. A majority of Cabinet Peaks' Board of Directors is comprised of residents and/or business owners of Lincoln County. With the exception of the Medical Staff representative on the Board, none of the Board members are Cabinet Peak's contractors or employees, meaning they truly do serve the needs of the community. In order to ensure the appropriate medical staff and services are available for patients, Cabinet Peak's medical staff privileges are open to all qualified, licensed physicians in the community and are also extended to physicians who visit Lincoln County in order to offer additional services to our residents. Additionally, in order to ensure the future success of Cabinet Peaks, any and all surplus funds generated by the Hospital are reinvested into operations, capital equipment and infrastructure, including facility upkeep.
