Schedule H, Part V, Section B, Line 3ETHE HOSPITAL FACILITY ANALYZED SEVERAL HEALTH NEEDS OF THE COMMUNITY AND HAS PRIORITIZED THOSE OF MOST CONCERN. THE PRIORITIZATION OF THE TOP SIGNIFICANT COMMUNITY HEALTH NEEDS IS DESCRIBED IN THE CHNA.
Schedule H, Part V, Section B, Line 5 Facility A, 1Facility A, 1 - Monroe Clinic. The hospital facility designed and administered a community health survey to assess the top health-related needs perceived in the county. A total of 350 responses to the survey, which was distributed via paper and online, were collected. The survey was taken by community members, healthcare providers, key stakeholders, and community advisors. Secondary data from multiple sources was analyzed and revealed health disparities and key issues impacting social determinants of health. The community survey results and secondary data were presented to the coalition who then used the information to identify community health themes and issues. Secondary data from multiple sources also revealed health disparities and issues. This secondary data and the community perception survey results were presented to a group of community stakeholders, who used the data to make final selections to identify community health priorities.
Schedule H, Part V, Section B, Line 6b Facility A, 1Facility A, 1 - Monroe Clinic. The facility conducted its 2018 CHNA through extensive collaboration with Green County (WI) Public Health.
Schedule H, Part V, Section B, Line 11 Facility A, 1Facility A, 1 - Monroe Clinic. The hospital identified various health needs in the 2018 CHNA. In order to make meaningful impact, and to use its finances most effectively and efficiency, the hospital will place primary focus on the following key priorities: - Substance abuse - Mental health - Physical activity - Food security and breastfeeding Substance abuse Multiple factors contribute to substance abuse: mental health needs, poor economic conditions, high poverty rates, lower educational attainment levels, higher percentages of individuals living alone, and easy access to alcohol and other substances. Additionally, statistics show: - Alcohol is the most widely-used and widely-abused drug - Two-thirds of adults regularly consuming alcohol - 13 percent of the US population is classified as binge drinkers The hospital has taken the following steps to improve substance abuse in the community: - Implemented a "Drug Court" system in Green County - Hosted seven community Narcan trainings - Provided new services to residents for AODA & Mental Health - Implemented a Teen Intervene program in two schools - Distributed 80 medication lock boxes, drug disposal kits, and promoted use of drug drop off boxes - Monroe Clinic implemented a patient pain reduction contract and chronic pain education class to reduce the use of opioids - Hosted five opioid movie screenings and a variety of media and health promotions - Initiated a Youth to Youth group in Brodhead - Received grant funds to provide counseling, case management, nursing care, medication and assisted treatment, detoxification, and residential treatment services to the county Mental health One in five Americans suffer from a mental health issue each year. Mental health includes our emotional, psychological, and social well-being. It impacts general health and quality of life at every stage. Locally, mental health professions are challenged by a need for services that exceeds availability. The result is mental health care in Green County is largely focused on providing crisis intervention and follow-up services. The hospital has taken the following steps to improve mental health in the community: - Collaborated with "Better Brodhead" in expanding behavioral health services to the Brodhead area - Facilitated psychosocial skills groups at Brodhead High School and Monroe Middle School - Conceptualized a "Mental Health Navigator" role and partnered with the UW School of Medicine and Public Health for a feasibility study - Collaborative partner with the Southern Wisconsin Behavioral Health Partnership - Monroe Clinic sponsored two Mental Health First Aid trainings a year and have trained over 200 individuals Physical activity A sedentary lifestyle impacts health on many levels and is often a key contributor to obesity. The percentage of individuals considered overweight or obese continues to rise on a national and local level. Increased physical activity is proven to lower obesity rates as well as decrease risks for health conditions, such as heart disease, stroke, diabetes, depression and certain types of cancer. Additionally, statistics show: - Green County's obesity rate is 35% - Wisconsin's state-wide obesity rate is 31% The hospital has taken the following steps to improve physical activity in the community: - Surpassed goal of 20 participating businesses for company to company physical activity challenge in second year of "Let's get Moo-Ving Green County" - 1,391 participants recorded 526,473 minutes of physical activity in "Let's Get Moo-Ving Green County" challenge in 2018 - Promoted use of Badger State Trail by creating Green County Bike Resource Guide/Trail Map and held events on the trail - Supported addition of mile markers to Badger State Trail Food security and breastfeeding Food security is met when all people, at all times, have physical, social, and economic access to sufficient, safe, and nutritious food. A changing climate, growing global population, rising food prices, and environmental stressors may continue to have undetermined impacts on food security. Statistics show that Green County's food insecurity rate is 9 percent. At birth and the months following, the best source of food security for babies is breastfeeding. Breastmilk helps fight infections and respiratory illnesses in infants. It can also lower the risk of sudden infant death syndrome in the first year of baby's life. The hospital has taken the following steps to improve food security and breastfeeding in the community: - The Monroe Farmers Market saw a 35% increase in the number of vendors - EBT (Electronic Benefit Transfer) machine was added to the farmers market - 55 different SNAP clients bought from the Farmers Market - Farmers Market vendors donated 4,380 pounds of food to local food pantries in the past three years - Two breastfeeding resources were developed, endorsed and used regionally by community partners - Five certified lactation counselors are now available to provide support and education to breastfeeding families - 48 Green County employers received tool-kits with breastfeeding policies and practices - Monroe Clinic/SSM Health and and Green County Government adopted a workplace breastfeeding policy The hospital has no plans to discontinue other community benefit efforts addressing the remaining CHNA-identified needs and address additional community needs within its efforts. The following community needs were identified but have not been prioritized due to the hospital's limited resources at this time (additional descriptions available in the 2018 CHNA): - Dementia/aging population - Housing - Access to care
Schedule H, Part V, Section B, Line 11 Facility A, 2Facility A, 2 - Monroe Clinic (continued). During 2020, the following activities were performed to help address the issues identified in the CHNA: - To better foster dialoguearound racial disparities and healthcare, Monroe Clinic hosted aMeet & Greet event inJuly2021to initiate thoughtful community conversations. The Spanish-speaking event was heldata local middle school as an open discussion for Hispanic families to bring forward questions around health care. - Questions and comments at the event included vaccine information, the experience of scheduling and going to an appointment if you only speak Spanish, blood pressure and glucose screenings, and the request to hold meetings like this more often and in different locations. Participants feel that people are interested and will continue to come with their questions. - This opportunity was brought forward by Ramon Viveros, the middle school's dean, who shared that their Hispanic population needed more health care related information. This is the beginning of Monroe Clinic's plans to address comments and concerns that were made at the eventand help eliminate barriers for their health care needs.More Meet & Greet type of community events are being planned along with visits to churches and local employers.

Schedule H, Part VI, Line 2 Needs assessmentSSM Health (SSMH) participates in Community Benefit according to our vision, Through our participation in the healing ministry of Jesus Christ, communities, especially those that are economically, physically, and socially marginalized, will experience improved health in mind, body, spirit and environment. In the tradition of our founders, the Franciscan Sisters of Mary, caring for those in greatest need remains our organizational priority. Today our System Board monitors Community Benefit efforts, and views achievement of our vision as a primary responsibility. The purpose of SSMH's Community Benefit program is to assess and address community health needs. Making our communities healthier in measurable ways is always our goal. To fulfill this commitment, SSMH's Community Benefit is divided into two parts: 1) Community Health Needs Assessment (CHNA), and 2) Community Benefit Inventory for Social Accountability (CBISA). The CHNA is an assessment and prioritization of community health needs and the adoption and implementation of strategies to address those needs. A CHNA is conducted every three years by each hospital according to the following steps: * Assess and prioritize community health needs: Gather CHNA data from secondary sources; obtain input from stakeholders representing the broad interests of the community through interviews and focus groups; use data to select top health priorities; and complete written CHNA. * Develop, adopt, and implement strategies to address top-health priorities: Establish strategies to address priorities; complete Strategic Implementation Plan; obtain Regional/Divisional Board approval; and integrate strategies into operational plan. * Make CHNA widely available to the public: Publish CHNA and summary document on hospital's website. * Monitor, track, and report progress on top health priorities: Collect data and evaluate progress; report to Regional/Divisional Board every six months and System Board every year; share findings with community stakeholders; and send results to finance for submission to the Internal Revenue Service (IRS). System Office staff and leaders oversee and monitor SSMH's Community Benefit Program, and ensure reporting is in compliance with IRS regulations. In collaboration with community stakeholders and partner organizations, SSM Health Care Corporation also identifies needs based on assessments and research, and SSMH facilities also involve case managers and care team staff to pinpoint critical health issues in the community. All hospital CHNAs are completed, approved, and integrated into the organization's strategic plan. We continue to monitor and assess the progress of our local efforts in the spirit of caring for others and improving community health.
Schedule H, Part VI, Line 3 Patient education of eligibility for assistanceEach entity providing medical service shall provide information to the public regarding its charity care policies and the qualification requirements for each of its facilities. When standard system notices and communication regarding charity care are available, these must be used. Modifications to the standard may be made to comply with state and local laws, as well as reflect culturally sensitive terminology for the policy. All notices are easy to understand by the general public, culturally appropriate and available in those languages that are prevalent in the community. They provide information about: * The patient's responsibility for payment, * The availability of financial assistance from public programs and entity charity care and payment arrangements, * The entity's charity policy and application process, and * Who to contact to get additional information or financial counseling. The following types of notices to the public are provided: * Signs in the emergency department, website resources, and public waiting areas. * Brochures or fliers provided at time of registration and available in the financial counseling areas. * Notices sent with or on patient bills or communications sent to patients and guarantors related to medical services. * Applications provided to uninsured patients at the time of registration. The application for charity care, together with any instructions, must clearly state the policies regarding charity care, including excluded services, eligibility criteria and documentation requirements. Information about the entity's charity policies is also provided to public agencies.

Our Community Health programs provide compassionate and competent care while they promote health improvement by reaching directly into the community to ensure that low-income and under-served persons can access health care services.
