Form 990, Part III, Line 1Benaroya Research Institute at Virginia Mason BRI is one of the few research institutes in the world devoted to finding causes and cures for autoimmune disease and other diseases of the immune system including type 1 diabetes, multiple sclerosis, rheumatoid arthritis, lupus, inflammatory bowel disease, cancers, allergies and infectious diseases. BRI is affiliated with the Virginia Mason Health System, which provides governance oversight, and development support through the Virginia Mason Foundation. BRIs bold mission is to predict, prevent, reverse and cure diseases of the immune system to achieve our vision of a healthy immune system for everyone. BRI is led by President Jane H. Buckner, MD, and Executive Director/COO Margaret McCormick, PhD. Discoveries at BRI are accelerated by our talented researchers, collaborative culture and application of new technologies and data analytics in a quest to understand what can cause the immune system to fall out of balance and what we can do to restore it to health. To that end, scientific activities are organized into four Centers of Discovery, a model that allows for knowledge sharing and for each Center and core lab to build on discoveries from individual labs 1 Fundamental Immunology, directed by Daniel Campbell, PhD 2 Translational Immunology, directed by Karen Cerosaletti, PhD 3 Interventional Immunology, directed by Carla Greenbaum, MD and 4 Systems Immunology, directed by Peter Linsley, PhD. In 2020 we recruited and integrated three new investigators into our institute Hamid Balouri, PhD, and John Ray, PhD into the Center for Systems Immunology and Carmen Mikacenic, MD, into the Center for Translational Immunology. In the year 2020, our vision of achieving a healthy immune system for every person took on a new urgency as we pivoted our expertise and research skills to help solve the mysteries of COVID-19. With our clinical research expertise and the help of many volunteers, we served as a trial site for testing the Pfizer vaccine for COVID -19 as well as a trial site for the drug remdesivir. We were also able to complete volunteer enrollment and push forward our Sound Life Project to help us understand what constitutes a healthy immune system and what causes it to move from health to disease. Through remote work, in the labs with strict COVID safety protocols, and in partnership with personnel and patients at Virginia Mason, we kept our momentum of research making progress against everything from devastating viruses to Crohns disease to type 1 diabetes.
Form 990, Part III, Line 1 continuedDonor and NIH support of COVID Disease and Immune Response. For expertise in immunology research key to understanding symptoms caused by COVID-19, BRI received more than 5.8 million dollars from the National Institutes of Health NIH to fund studies involving lung tissue and COVID, immune response to the virus, and the phenomenon of cytokine storm. This funding would not have been possible without the support and confidence of our donors in giving more than 365,000 in seed money for initial research, enabling us to obtain larger government grants. Discoveries from BRI helped identify a new cellular protection pathway that targets a vulnerability common to several different pandemic viruses. Principal investigator Adam Lacy-Hulbert, PhD, was lead author on a study showing that this pathway can protect cells from infection by Ebola virus and coronaviruses, such as SARS-CoV-2. Published in Science, the findings provided a better understanding of cellular mechanisms involved in viral resistance to inform future treatments and therapies for infectious diseases. The research illuminated a completely new role for the two genes identified and a unique approach to inhibiting virus fusion and entry into human cells - bringing us one step closer to the next generation of antiviral therapies. A team collaboration with the University of Washington yielded evidence that mild cases of COVID-19 may trigger long-term immunity from the virus. The researchers, including Dan Campbell, PhD and Jessica Hamerman, PhD, performed an analysis of SARS-CoV-2 specific immune responses at one and three months following the onset of symptoms in patients who had recovered from mild COVID-19. Most of the participants developed antibodies, B and T cells that remained present for at least three months it appeared the recovered patients had formed virus-specific memory cells showing signs of antiviral immunity.
Form 990, Part III, Line 4One unique quality of BRI is the close integration of three types of medical research - laboratory research, translational research, and clinical research - to improve lives. Our scientists collaborate to explore basic science as well as clinical applications, which is the best way to design studies with the highest potential for success. At BRI, discovery starts in the laboratory but is driven by patient issues addressed in the clinic.
Form 990, Part III, Line 4a2020 advancements of the Immune Tolerance Network are highlighted as follows Funding for the Immune Tolerance Network ITN was awarded to BRIs Dr. Jerry Nepom in 2014 from the National Institute of Allergy and Infectious Disease NIAID of the National Institutes of Health NIH. The 7-year award, totaling 27 million annually support the ITN, a research consortium established in 1999 with a primary focus on the development of new tolerogenic approaches for the treatment and prevention of disease in three clinical areas asthma and allergic diseases autoimmune diseases and immune-mediated rejection of transplanted solid organs, tissues and cells. As the Prime recipient of this grant, BRI is responsible for the management of the overall grant from NIAID including oversight of all aspects of the Networks infrastructure. The major activities in 2020 were diverse, but continued to center on preserving the ITNs cohesive project focused environment and its framework which has proven so successful in the management of a large cooperative agreement grant. Key outcomes in 2020 include the following 1 Grant Renewal The ITN was notified in late 2020 that the ITN grant renewal had been awarded to Dr. Jerry Nepom at the Benaroya ReZsearch Institute. This is a 7-year, 189M award starting in 2021. 2 Cross Network Integration The COVID 19 pandemic in 2020 presented a temporary shift in ITN priorities as personnel spread across the ITNs three main operational sites moved to a work from home WFH model. This shift was largely seamless due to past focus on careful planning and emphasis on collaboration across the ITNs major centers, located in Seattle, San Francisco and Bethesda. No changes to existing network integration were needed as the current model continued to successfully support the overall ITN program. Establishing new relationships and vital connections within the research community continued to be an ITN goal. 3 Operations and Clinical Trial Support The ITN program team at BRI issued 211 sub-awards in 2020 in support of 22 active ITN clinical trials and numerous supporting ITN mechanistic trials. 4 Supplemental Funding / Partnerships ITN central administration continued to focus on identifying additional opportunities for supplemental funding and/or funding partnerships. In 2020, the ITN received supplemental funding as the contracting component for several NIAID sponsored COVID-19 clinical trials. a Supplemental Funding Opportunities The ITN Program Team at BRI submitted four supplemental funding requests in 2020. these requests were awarded, bringing in an additional 5.9M in funding to the ITN BRI. b Partnerships. The ITN continued its relationship with two pharmaceutical partners for partial funding on two ongoing ITN studies. NIAID is the clinical sponsor on both of these trials. 5 Publication Activity In 2020, 31 ITN publications were published in prestigious research journals and were presented at domestic and international research conferences. These publications represented ITN research in all therapeutic focus areas of the ITN Type 1 Diabetes, Transplant, Autoimmune disease and Allergy and Asthma.
Form 990, Part III, Line 4a continuedBRIs advances in Interventional Immunology research are highlighted as follows 1 Testing a vaccine for COVID-19. BRI served as one of about 150 Pfizer COVID-19 vaccine testing sites across six countries and 39 states. First, researchers opened enrollment for the trial and spread the word that they were looking for volunteers to participate in the study. Some participants received the vaccine, and others received a placebo. Frontline workers, several Virginia Mason ER doctors, and physicians from other local hospitals were among the volunteers. Led by Carla Greenbaum, MD, the research team had only three weeks to give the first vaccine dose to more than 80 participants - while social distancing meant they could have only a few people at a time in the Clinical Research Center. To make it happen, the team mapped out detailed schedules and worked nights and weekends. At the end of each day, they entered data about potential COVID-19 illnesses and vaccine side effects, which were monitored closely by Pfizer. When enough data had been collected worldwide, Pfizer announced its safety and efficacy endpoints had been met, and the FDA independently reviewed the data to decide whether to authorize the vaccine for emergency use. In late 2020, the research team finally got word from Pfizer that the FDA had approved the vaccine for use during the pandemic emergency. 2 A Strategy to Prevent Celiac Disease in Children. A study involving BRI scientists found that introducing high doses of gluten into infants diets could prevent them from developing celiac disease. Results from the Enquiring About Tolerance EAT study, published in JAMA Pediatrics, suggest this discovery could prove an effective prevention strategy for the disease. The EAT study investigated effects of gluten - in the form of wheat-based cereal - along with breastfeeding from the age of four months. Researchers compared the results with those from infants who avoided allergenic foods and consumed only breast milk until the age of six months. The results showed that among babies who delayed gluten introduction, celiac disease at three years of age developed at higher rates than expected. In contrast, there were no cases of the disease among children who had received gluten from four months of age. Celiac disease is an autoimmune disease in which eating gluten causes the bodys immune system to attack its own tissues, causing a range of potentially severe symptoms in the gut and elsewhere. 3 Type One Diabetes Research. BRI is dedicated to seeking ways to eliminate type 1 diabetes by finding ways to predict the disease before clinical symptoms occur and to identify therapies that prevent, reverse and intervene in the disease at any stage. BRI is an international leader in type 1 diabetes research and has investigated it for more than 30 years, starting with identification of a genetic marker for the disease.
Form 990, Part III, Line 4a continuedAdvancements in 2020 included the following 1 BRI supports Type 1 Diabetes TrialNet, a National Institutes of Health-funded clinical trial network for type 1 diabetes prevention and early treatment, serving as the TrialNet hub of 18 clinical centers worldwide working with more than 150 screening and clinical research sites. 2 Immunotherapy Can Delay T1D - Carla Greenbaum, MD, chair of TrialNet, furthered findings that immunotherapy can delay a type 1 diabetes diagnosis in people at high risk. Findings from TrialNets Teplizumab anti-CD3 Prevention Study drawing from multiple sites showed the drug that targets the immune system can delay type 1 diabetes a median of two years in children and adults at high risk. Samples collected in the original trial were then studied to help Dr. Greenbaum and others understand why some people respond to the drug better than others - and how its benefits may be extended. Dr. Greenbaum, who in 2020 directed BRIs Diabetes Research Program and served as principal investigator for its TrialNet clinical site, noted the findings and additional insights represent great news for millions with T1D, as well as relatives who are at a high risk for the disease. 3 The TrialNet Pathway to Prevention study screens relatives of people with type 1 diabetes to find out if these family members are at risk for developing diabetes. Relatives of people with type 1 diabetes are at 15 times greater risk of developing type 1 diabetes. Those who qualify have the opportunity to enroll in a prevention trial. Those with two or more autoantibodies are closely monitored for early detection of type 1 diabetes. Close monitoring of at-risk family members can reduce the chances of being diagnosed in diabetic ketoacidosis DKA, a potentially life-threatening complication. 4 As home to the JDRF Core for Clinical Assay Validation CAV, BRI plays an important role in identifying type 1 diabetes biomarkers and in helping outside researchers get the most accurate test results. The CAV is pinpointing key biomarkers, such as those indicating which patients will keep making insulin for years and which patients wont. The CAV also helps maintain quality control for a worldwide network of research labs by asking them to perform specific tests and reviewing their results.
Form 990, Part III, Line 4bTranslational Immunology research observations are made in the basic research lab, then quickly tested in the clinic with patients, with results brought back to the lab again in an expedient and systematic way. This type of research translates new methods to improve our ability to predict disease risk, prevent onset of disease, slow disease progression and improve safety and efficacy of treatments. At BRI, translational research addresses autoimmune and immune-mediated diseases. The BRI biorepository is a collection of blood, serum and tissue samples, as well as medical histories, from volunteers with and without diseases involving the immune system. BRI scientists and physician collaborators work together to study tissue, blood and serum samples along with the medical and demographic data. BRIs biorepository samples date back to 2000 and include multiple disease categories and a biorepository of healthy people for comparison purposes. Advancements in 2020 included the following 1 Sound Life Project - BRI completed volunteer enrollment for this study in which researchers are tracking healthy immune systems over time. Seattle-area adults in two age cohorts - 25-35 and 55-65 - are participating in this groundbreaking study to build a baseline of knowledge about the human immune system to understand disease. The goal is to lay the groundwork for better ways to diagnose, treat and prevent immune system diseases such as rheumatoid arthritis, type 1 diabetes and multiple sclerosis. Volunteers provide blood samples and health information through in-person visits and a phone app developed at BRI, allowing researchers to track their immune systems over the course of two years. The project is the initial phase of a research partnership with the Allen Institute for Immunology. BRI is providing detailed information about healthy immune systems to serve as a foundation for existing and future disease research programs. 2 Autoimmune Disease Research in the Down Syndrome Population. BRIs Bernard Khor, MD, PhD, and Virginia Mason pediatrician Rebecca Partridge, MD, continued their work leveraging a grant from the National Institutes of Health to build a Down syndrome DS biorepository to research the connection between DS and autoimmune disease. The collection includes blood, serum and tissue samples, as well as medical histories, from volunteers, people with DS and their relatives, with and without autoimmune diseases. The DS population has long been underrepresented in medical research, as nearly 50 percent of this population is living with at least one autoimmune disease. BRI is among the first to be awarded a grant dedicated to autoimmune disease in the DS population. 3 Type 1 Diabetes Fast Progressors and Prevention Strategies - Alice Long, PhD, and colleagues pursued their landmark discovery that is leading to new treatment strategies for type 1 diabetes T1D. In some people newly diagnosed with the disease, it progresses so quickly that their pancreas stops making insulin within a year. For others, the process is slower and this can make the disease easier to manage. The researchers discovery enables early identification of fast progressors to potentially match them with treatments to help them stay healthier for much longer. The researchers identified important differences between fast progressors and people whose disease progresses slowly. Dr. Longs team showed that slow progressors have higher levels of exhausted CD8 T cells - cells that are worn out from attacking the pancreas. The discovery could lead to a test that identifies how quickly individual patients will lose their ability to make insulin. Then, Dr. Long received additional funding to investigate why CD8 T cells become exhausted. For people with T1D, a therapy that would slow down or even stop attacker cells would prolong their ability to make insulin, which makes their lives much easier and significantly reduces their long-term health risks. Peter Linsley, PhD, led BRIs bioinformatics team in helping to analyze a vast amount of data from the cells. While researchers are typically only able to look at two or three aspects of each cell, Dr. Linsleys team used innovative technology to help Dr. Long and colleagues collect and analyze data on up to 30 aspects of each cell. 4 In 2020, BRI also received a 5.4 million contract award from the NIH to improve treatments for patients diagnosed with rheumatoid arthritis.
Form 990, Part III, Line 4c1 Fundamental Immunology The Center for Fundamental Immunology investigates the immune system to understand how it works in health and disease. At BRI, we employ a variety of methods to examine the immune system function at the genetic, molecular and cellular levels. These include developing novel model systems of autoimmune and inflammatory disease, used to determine how activities of the immune system are regulated and how regulatory mechanisms break down in autoimmunity, allergy, asthma and COVID-19. We also leverage samples from healthy subjects and patients with immune-mediated diseases to study immune system function and identify potential therapeutic targets. 2 Systems Immunology With the explosion of data from genome studies and molecular profiling technologies, huge amounts of biological information are now available. BRI scientists in this Center use complex systems theory and high throughput techniques as well as mathematical and computational tools to understand the functioning of the immune system in health and disease.
Form 990, Part III, Line 4dBRI oversees the Clinical Research Program at Virginia Mason Medical Center, uniquely combining the expertise of a world-renowned medical research institute with the care of a healthcare quality leader. The Clinical Research Program supports Virginia Mason clinical investigators in studies across a wide variety of autoimmune and immune-mediated diseases and conditions, as well as a broad range of other diseases such as cancer and cardiac disease. The Clinical Research Program staff annually enrolls approximately 1500 study participants into Virginia Mason trials, but numbers were fewer in year 2020 due to the pandemic. 1 Testing a New MS Therapy - a research study at BRI and Virginia Mason is seeking to find out if an experimental oral drug, similar to a currently available MS medication, would be just as effective as the current one, but with fewer gastrointestinal side effects. In an earlier study, half the research participants took this experimental drug and half took the current drug. Participants who finished the five-week study then enrolled in a two-year study of the experimental drug.
