Franciscan Hospital for Children, IncPart V, Section B, Line 5: To develop a comprehensive picture of the Allston/Brighton neighborhood, the assessment team reviewed existing data from national, state, and local sources. The types of data included demographics, vital statistics, public health surveillance, and self-reported health behaviors. Data sources included but were not limited to: the U.S. Census, Massachusetts Department of Public Health, Boston Public Schools, and Boston Public Health Commission, among others. Data analyses, such as significance testing, were conducted by the original data source (e.g., U.S. Census, Boston Public Health Commission). Much of the U.S. Census data derive from the American Community Survey, which is comprised of data from a sample of a given geographic area. Per the U.S. Census recommendations, HRIA used 5-year aggregated data (e.g., the 2018-2022 data combined) for these indicators to yield a large enough sample size to examine results by municipality and census tract. In addition to quantitative data from the Allston/Brighton neighborhoods, the assessment team gathered qualitative information through interviews with leaders from organizations serving the Allston/Brighton community and youth living in Allston/Brighton. These conversations helped develop a more nuanced understanding of the perceptions of community strengths and assets, health concerns, and suggestions on what programming or services are most needed to address these concerns.During March-May 2024, the planning team conducted 16 interviews with 19 providers serving Allston/Brighton and the Greater Boston area. The needs assessment also included one focus group with 12 Allston/Brighton youth. In selecting both youth and service providers, the assessment team engaged in a careful and deliberate process to ensure that 1) the community was properly engaged in the development of the CHNA, and 2) Franciscan Children's can gain a clear understanding of the needs in the community from the CHNA.This CHNA included 16 interviews with 19 Allston/Brighton service providers representing a range of sectors including public health, education, childcare, housing/community development, social services, and healthcare. The interviews explored participants' perceptions of the community, priority health concerns, and suggestions for future programming and services to address perceived health issues. Interviewers used a semi-structured interview guide across all discussions to ensure consistency in the topics covered. Interviews were approximately 40-60 minutes in length. A list of the service providers interviewed can be found in the Appendix. One focus group was conducted with 12 Allston/Brighton youth who access services through the Jackson Mann Community Center. Similar to conversations with service providers, discussions with youth explored perceptions of the Allston/Brighton community, health concerns, and suggestions for how to address these concerns. More specifically, participants were asked to elaborate on their perceptions of the mental health needs among youth. The focus group utilized a semi-structured interview guide and lasted approximately 75 minutes.NVivo analysis software coded the qualitative information collected and analyzed the information collected thematically for main categories and sub-themes. The assessment team identified key themes that emerged across the qualitative data, as well as the unique issues noted for specific populations. Frequency and intensity of discussions on a specific topic were key indicators used for extracting main themes. Selected quotes without personal identifying information are included in the report to further illustrate points within topic areas.
Franciscan Hospital for Children, IncPart V, Section B, Line 7d: The complete Franciscan Children's Community Health Needs Assessment report is available on the hospital's website at https://franciscanchildrens.org/wp-content/uploads/2024/09/Franciscan-Childrens-2024-CHNA-Report.pdf. In addition to the report, there is also an executive summary, and data on the findings, and the hospital's Community Health Implementation Plan.
Franciscan Hospital for Children, IncPart V, Section B, Line 11: In 2024, Franciscan Children's conducted a community health needs assessment (CHNA) of the community it serves. Franciscan's Executive Leadership Team reviewed the findings of the 2024 Community Health Needs Assessment and prioritized areas where the institution could successfully engage and intervene. The Franciscan team prioritized the areas identified based on: 1) demonstrated need in the community as evidenced by CHNA findings; 2) the expertise Franciscan Children's has in the area identified; 3) the feasibility of Franciscan Children's addressing this need and making an impact. Areas not selected did not meet the aforementioned criteria. Franciscan's executive leadership team identified the following as priorities:-Mental health-Workforce development-Community engagement-Dental careMental Health:-Increase school-based behavioral health (through Children's Wellness Initiative)-Provide summer group therapy programs in local community centers-Launch telehealth group therapy program-Expand outpatient behavioral health servicesWorkforce Development:-Establish workforce academy to train health care behavioral health professionals-Grow behavioral health trainees onsite-Expand recruiting partnerships with colleges and universities-Continue to diversify workforce, in partnership with the City of BostonCommunity Engagement:-Launch health related social needs (HRSN) survey institution-wide to identify gaps-Collaborate with Boston Children's Hospital to connect patient families to local programs in the community (e.g., food bank)-Provide education, expertise, and strategic guidance to community groups (e.g., Allston/Brighton Health collaborative)-Expand local volunteer opportunities on campus-Offer adaptive sports camps for children with special needsDental Care:-Provide specialized support to kids with special health care needs (e.g. esensitization clinic for children with developmental disabilities)-Expand access for dental care
Franciscan Hospital for Children, IncPart V, Section B, Line 20e: If patients have outstanding balances, the hospital's patient accounting department initially sends out three statements. If the balance remains unpaid after these attempts, the account is forwarded to a collection agency, which then attempts to collect the outstanding amounts.

The IRS counts the following as community benefits while the AG does not: Medicaid shortfalls, indirect costs, health professions education, and research funded by tax-exempt and government sources. Franciscan Children's AG report is publicly available and can be accessed directly on the AG's website, www.mass.gov/AG.
Part II, Community Building Activities:Children's Wellness Initiative (CWI): CWI is an ongoing program that provides school-based mental health counseling and psychiatry services, combined with outreach, education, and parent consultation. In FY24, CWI provided services in 22 Boston school sites, serving over 700 students. The goal of the program is to increase access to mental health services for children and families, as well as build connections between schools and families and de-stigmatize accessing mental health services. The need for more school-based mental health services was identified in our 2024 Community Health Needs Assessment. The program continues to grow to meet the mental health needs of children and adolescents in Boston.Adaptive Sports Programming: Franciscan Children's served 450 children from the surrounding community in FY24 with adaptive sports programming directed toward children with special health care needs in the Boston area. Programs consisted of adaptive track, basketball, gymnastics, dance, bicycle, skating and sports camp, as well as wheelchair track, basketball, and tennis. All programs included therapeutic components. The hospital also worked to offer additional sports through community partners.Volunteer Program: Franciscan Children's coordinates opportunities for volunteers for the surrounding community. Careers in Child and Adolescent Mental Health Summer Course: Franciscan Children's partnered with Northeastern University to run a summer course on careers in child and adolescent mental health featuring speakers in psychology, nursing, social work, among mental health disciplines.
Part III, Line 2:Bad debt expense is recorded to report accounts receivable at their net realizable value. Showing the bad debt as adeduction from revenue more clearly represents the amount of revenue that the Franciscan Hospital actually expects to collect, therby improving the quality and comparability of the financial statements.
Part III, Line 4:Franciscan Children's audited financial statements do not contain any footnotes describing bad debt.
Part III, Line 8:The costing methology used to determine Medicare allowable cost was the cost to outpatient ancillary charge ratios as reported on the Medicare 2552 cost report for FYE 9/30/2024.
Part III, Line 9b:The collection process encompasses billing the patient's health insurance(s), filing appeals for non-payment andbilling the patient. The statement/data mailers are processed by the hospital and an outside company. They are sent once a month, one business day after the month-end close. Statements/data mailers are sent to the patient's responsible party each month for service not covered by the patient's insurance company. There will be three statements for self and self-pay after insurance and five for self payment. There is a different massage on each statement indicating status of account contact. Low income families who meet the following criteria are eligible for partial free care:1. Massachusetts residents whose family income is from 201% - 400% of the federal poverty income guidelines will be eligible for financial assistance for that portion of the bill which exceed their family's annual deductiable. This will be determined by a sliding fee scale. 2. Any participant in the Children's medical security plan whose family inocme is from 201% - 400% of the federalpoverty income guidelines will be eligible for finanical assistance for that portion of the bill which exceedstheir family annual deductible.3. Any participant in health start whose family income is from 201% - 400% of the federal poverty inocme guidelines will be eligible for that portion of the bill which exceeds their family annual deductible.Note: The annual deductible for partial free care equals 40% of the difference between the applicant's familyincome and 200% of poverty income guidelines. The patient is responsible for payment for all services provided up to this deductible amount. There is only one deductible per approved period. Therefore, it is the responsibility of the patient to provide documentation that their obligation has been met.
Part VI, Line 2:While Franciscan Children's patients come from across New England, the institution is located in the Allston/Brighton neighborhood of Boston, Massachusetts. Therefore, the community of focus for this CHNA is defined as the Allston/Brighton neighborhood. This geographic area was selected for the CHNA because Franciscan Children's recognizes the importance of focusing efforts directly in the neighborhood where the hospital is located and addressing the health needs of the local community. Where possible, neighborhood-level data for Allston/Brighton are presented. Boston and Massachusetts data are also shown to provide more context and/or where neighborhood-level data are not available. The 2024 CHNA builds off the work from the previous assessments conducted in 2018 and 2021. Priority areas identified in the 2021 CHNA included mental health for youth, marijuana and e-cigarette use among youth, child wellness (e.g., nutrition, obesity, physical activity), domestic violence, dismantling barriers to primary, mental, and oral health care for children and adolescents, expanding prevention education, centralization of information in the community, and continued engagement with Allston/Brighton institutions. Franciscan Children's and its partners developed and implemented a range of strategies to address these identified needs. These strategies can be found in the Appendix.
Part VI, Line 3:Inpatient:We review all potential patients insurance policies prior to addmission for length of stay and coverage guidelines and limitations. Most patients who have private insurance exhaust their rehabilitation benefits while here. We meet with parents/guardians to educate them on their options and if necessary assist them with applying for government sponsored assistance programs. For those who do not apply or do not qualify for government assistance, the hospital provides medical and financial hardship assistance based on the federal and state poverty guidelines.Outpatient:We review insurance eligibility for each visit and contact parent/guardian if there is an issue. We also assist patients with MassHealth enrollment where needed. The hospital provides medical and financial hardshipassistance based on the federal and state poverty guidelines. Allston-Brighton has a predominantly caucasion population. It is also home to a large number of diverse groups including many recent immigrants from Latin America, Brazil, China, and Russia. The median income of Allston-Brighton residents is $40,400.
Part VI, Line 4:Allston/Brighton remains a diverse and unique community with a large young adult population. According to the American Community Survey, the population of Allston/Brighton in 2022 was 64,443 residents (down from 67,479 in 2019). More than half (57.1%) of the Allston/Brighton population represented adults aged 20-34. Children under age 0-9 years and 10-19 years represented 4.3% and 5.7% of the Allston/Brighton population respectively. Regarding racial and ethnic diversity, nearly two in five (40.1%) Allston/Brighton residents in 2022 were people of color. Specifically, Asian residents comprised the largest proportion (18.4%), followed by Hispanic or Latinx residents (11.4%). In 2022, more than one in three (36.5%) residents spoke a language other than English at home. Spanish and Chinese were the most commonly spoken languages, with about one in ten residents reporting one of those as their primary language at home.
Part VI, Line 5:Since opening in 1949, Franciscan Children's has become a premier, nationally recognized pediatric specialty care provider offering services critical to families in Boston and throughout the region. Franciscan Children's is a health care institution dedicated to serving (i) medically complex children, (ii) children with behavioral health conditions requiring inpatient services, short-term residential, school-based, ambulatory and/or community-based services, (iii) children who need specialized dental services, including dental surgery under general anesthesia, and (iv) children with specialized educational needs due to their physical and/or cognitive condition. Franciscan Children's is the only pediatric chronic disease and rehabilitation hospital in Massachusetts and one of the largest providers of pediatric behavioral health services in the state. It also operates the Kennedy Day School, a special education school for children with complex needs ages 3 to 22 years from more than 30 cities and towns throughout Massachusetts. Franciscan's patients and students arrive from acute care hospitals, emergency departments, school systems, and other providers from across the Commonwealth and New England.Located in the Allston/Brighton neighborhood of Boston, Franciscan Children's is an integral institution to the city. In addition to serving children from many different neighborhoods in Boston across its various programs, Franciscan Children's is an important employer in the community, with over 730 staff members. Franciscan Children's also offers several community-based programs, including in Boston Public Schools, to meet the growing needs of children and families in the city. In addition, Franciscan Children's routinely sponsors events in the local community, and is a frequent collaborator with other Boston-based organizations.
Part VI, Line 6:As sister organizations, Boston Children's Hospital (BCH) and Franciscan Children's (FC) are able to offer a fully integrated care system providing children with care where and when they need it, all with one point ofcontact for families. The organizations are affiliated as sister hospitals to more effectively confront an acute children's mental health crisis in Massachusetts and across the country, and advance equitable access to care and eliminate disparities.By joining together, the two organizations work to improve the lives of children and families by:-Improving care by modernizing programs and facilities. This affiliation will modernize the FC campus, which dates back to the 1940s. This will include a new, state-of-the-art, net carbon-neutral building that housesthe full continuum of care programs for medical rehabilitation and mental health services.-Offering the full range of integrated care that children need. BCH and FC offer a fully integrated continuum of care across inpatient, outpatient, primary care, and school-based settings.-Building a new generation of caregivers. By 2030, the organizations intend to hire 150-200 new full-time employees related to care for children, and invest in programs to build an employee pipeline from highschool through medical school. They will also increase training programs for students and staff.-Increasing the number of beds available. BCH and FC will increase the number of available beds for inpatient services from 98 to 127, and nearly double the behavioral health bed capacity. This will reduce the number ofkids forced to board in emergency rooms for medical and mental health needs.-Expanding services. They are creating and expanding intensive outpatient therapy and partial hospitalization programs that reduce the likelihood that mental health patients reach crisis levels.-Increasing sustained investments in research. Research is the future of children's mental health because the underlying causes are still poorly understood. The organizations continue to make sustained investments inresearch, better integrate research and clinical faculty, and hire 3-4 new clinician scientists.-Expanding community and school-based prevention programs. BCH and FC plan to expand their community, early education and school-based programs.Additionally, FCH has maintained a relationship with Boston University School of Dental Medicine's outpatient pediatric dental program. Residents of that program complete a rotation in the dental clinic at the hospital.
