Cameron Regional Medical Center, Inc.Part V, Section B, Line 5: The Medical Center conducted a town hall meeting and invited local hospital, public health community, mental health community, free clinics, community-based clinics, service providers, local residents, community leaders, opinion leaders, school leaders, business leaders, local government, faith-based organizations and persons (or organizations serving them), people with chronic conditions, uninsured community members, low income residents, and minority groups.
Cameron Regional Medical Center, Inc.Part V, Section B, Line 11: The following is the description of the actions taken during the most recent fiscal year to address the significant needs identified through the CHNA:General Notes Addressing Multiple Priorities/Objectivesa) CRMC's 17th Annual Health Fair entitled "Ride the Wave to Better Health" was held in 2019 and was again expanded after soliciting feedback from the public. The free 2019 offerings included: Extensive blood work (anemia, blood sugar, thyroid, cholesterol panel, iron, GFR, and BUN/Creatinine), skin cancer screenings, Ear, Nose & Throat consultations, medication counseling, prescription disposal education, kidney function screenings, podiatry screenings, shingles and pneumonia vaccine education, hearing screenings, depression and anxiety screenings, women's health counseling, education on mental health and available resources, vision screenings, exercise guidelines for older adults, stroke education, oral cancer screenings, glucose testing, blood pressures, parent training under the Missouri Parents Act (MPACT), diabetes/nutrition education, and pulse oximetry. All of the above were provided to the public free of charge. Flu vaccinations were offered for a nominal charge. Approximately 400 participants from over 16 counties attended this comprehensive event; andb) A Regional Resource Directory for CRMC's primary service area was developed several years ago and is updated/maintained on a regular basis. It is distributed to area residents upon request and disseminated at certain public meetings/events (such as Community Health Needs Assessment meetings) as sponsored by the Medical Center. Covering CRMC's four-county primary service area (Clinton, Caldwell, Daviess and DeKalb), the directory includes mental health services, medical clinics, alcohol and drug treatment programs, wellness/fitness centers, medical services, and several other categories affecting the health and well-being of area residents.Priority/Object No. 1 - Mental HealthDue to a continuing shortage of mental health providers in rural northwest Missouri and the ever-increasing demand for this service, Cameron Regional Medical Center ("CRMC") remains committed to maintaining its offerings in this specialty. Five part-time Psychiatrists, including a Child Psychiatrist and a Geriatric Psychiatrist, provide inpatient and outpatient services at CRMC on a regularly-scheduled basis. Mental health services are marketed throughout CRMC's service area, and free transportation is provided to those area residents in need of same via a Patient Courtesy Van system. CRMC's 18 satellite clinics serve as both referral and education centers for the Medical Center's mental health services. A calendar denoting mental health offerings is placed in 15 area publications on a monthly basis, as well as on the Medical Center's website. CRMC greatly underwrites/subsidizes this program in order to assure the above-referenced mental health services are readily available to all patients/payors (Medicare, Medicaid, self-pay, etc.). CRMC continues to maintain its 10-bed GeroPsychiatric Unit (BHU). A major remodel of the BHU has been completed to bring it current with anti-ligature regulations. TelePsych services are utilized one day per week in this Unit to enhance physician coverage. Based on demand/need, a two-bed expansion of the BHU is planned for late 2020. A financial feasibility study was performed concerning the potential for implementing a ten-bed (inpatient) Adult Mental Health Unit at CRMC; however, a high Medicaid and self-pay payor mix may preclude construction of such a unit unless a corporate partner can be identified. Adult Mental Health First Aid Training sessions are held annually in the Cameron area by Preferred Family Healthcare, a division of the Behavioral Health Funded Prevention Resource Center in Kirksville, Missouri. CRMC staff members from the E.R., Home Health, Hospice, and Behavioral Health Unit have attended these sessions. Through the Community Health Needs Assessment process, Preferred Family Healthcare has become a valued partner for CRMC, providing resources otherwise unavailable in the area for mental/behavioral health andsubstance abuse (see next section).Priority/Objective No. 2 Substance Abuse (Alcohol/Drugs/Prescriptions/Tobacco)Detailed information on smoking cessation continues to be included in all CRMC admission packets, and in discharge packets for those patients who smoke. The Medical Center continues to assist its employees financially with smoking cessation products. CRMC, through its network of primary care clinics, physicians, mid-levels, and hospital Pharmacy, continues to facilitate and promote concerted efforts against prescription drug/medication abuse. The Medical Center's eprescribing software and communication protocols with area pharmacies aid in monitoring and tracking "doctor shopping" efforts by patients suspected of abusing the system. The Medical Center assists the Cameron Police Department with marketing its annual "Prescription Drug Take-Back Day" held in conjunction with the DEA. In terms of health care professionals, the CRMC Medical Staff donates a substantial sum on an annual basis to the Missouri Association of Osteopathic Physicians and Surgeons' (MAOPS') Physicians Health Program (PHP). PHP provides addiction treatment resources and coordinates the care of the affected health care professional throughout his/her treatment program, with the goal of facilitating and expediting his/her return to health and a normal personal and professional life. In March 2019, the CRMC Medical Staff and Board of Directors voted to prohibit medical marijuana on the CRMC campus.Priority/Objective No. 3 Obesity/Nutrition/FitnessFree bi-monthly training/education classes on diabetes, nutrition, food labels, and healthy eating are held by one of CRMC's Registered Dieticians, with excellent participation rates and feedback. From fitness/wellness, youth activities, and anti-obesity standpoints, CRMC and its Medical Staff have both made sizeable donations toward the relocation/expansion of the Cameron YMCA under terms of five-year pledges ($26,000 in FYE 2019). CRMC continues to provide a regimented physician-assisted weight loss program at lower-than-normal rates via one of its satellite clinics. CRMC and the CRMC Foundation continue to sponsor their 4-Mile Run, 2-Mile Walk, and Children's Fun Run on the Fourth of July each year. Approximately 250 runners, walkers, and children participate each year in this fitness/wellness event. The 31st Annual Run/Walk was held in 2019. CRMC's Fitness Committee sponsors fitness challenges for CRMC employees at least twice a year. CRMC continues to financially assist its employees and their families with membership fees at area fitness centers. The CRMC Foundation continues to operate a one-mile Wellness/Fitness Trail on the CRMC campus. This facility is available free-of-charge to the public 365 days per year during daylight hours. Annual maintenance expenses for the Trail run approximately $9,000 - $10,000. CRMC's Wellness Nurse continues to expand the offerings made to area employers. These services include: Blood pressure and pulse checks, pulse oximetry, blood glucose and cholesterol, weight and height with BMI calculations, free educational presentations, mini health fairs, lab work, and flu shots. Starting in the early fall of 2018, CRMC became a remote site for the University of Kansas's "Midwest Diet and Exercise Trial." This weight management program continues for area residents and was made possible via a grant the University received. In response to the closing of the Cameron Nutrition Site (Stella Grinstead Center) due to a lack of State funding, CRMC assumed operation of the Meals on Wheels Program to benefit homebound Cameron residents living in Clinton County beginning in June 2018. This program continues to rave reviews. Not only do participants receive hot, nutritious meals, but they anticipate the socialization and interaction offered by the program's volunteer drivers. As an adjunct to the Meals on Wheels program, seniors residing in the area have a standing invitation to eat breakfast and/or lunch in the CRMC Dining Room at greatly-reduced rates five days per week. These two affordable approaches are nutritionally, physically, and mentally benefitting area seniors via hot, healthy meals and the camaraderie/fellowship afforded those choosing to partake. Financial assistance forms are available for those participants experiencing monetary constraints.
Cameron Regional Medical Center, Inc.Part V, Section B, Line 11: Priority/Objective No. 4 Parenting and Social SupportWhile CRMC does not delve into the particulars of parenting, the Medical Center recognizes the financial stresses placed on area families by medical costs and today's general environment. From a social support aspect,CRMC's collection policies remain extremely lenient. The Medical Center and its satellite clinics offer a prompt pay discount on all self-pay balances. Charity care is available if the patient makes a good-faith effort, meets U.S. Federal Poverty Guidelines, and completes the applicable paperwork. CRMC's temperate collection policies are widely publicized in area media. Monthly audits are conducted to determine the public's satisfaction with CRMC's billing and collection practices. CRMC's high school scholarship program for FYE 2019 represented its 30th annual offering. As another avenue of social support, a total of $11,500 was awarded to 21 high school seniors entering a health care field of study. These awards are available to students residing throughout five counties and are renewable for an additional three years to address undergraduate studies. Renewals each year average a total of approximately $14,000. CRMC also makes available a total of $10,000 to second-year Allied Health Care students at North Central Missouri College in Trenton, Missouri on an annual basis. As a third prong, the CRMC Medical Staff awards a $5,000 scholarship each year to a promising first-year medical student. CRMC continues to sponsor free prenatal, childbirth, and breastfeeding classes periodically. Immunization education is provided to young families via CRMC's physicians, mid-levels, satellite (primary care) clinics, and Obstetrics Unit. From a safety standpoint, CRMC provides free Athletic Trainer services during area high schools' sporting events. CRMC maintains a close working relationship with area Ministerial Alliances, particularly in terms of families in need. The Medical Center is a strong supporter of the Cameron Women's Domestic Abuse Shelter. As previously mentioned under Priority/Objective No. 1, and as a means of social support, CRMC operates five full-time Patient Courtesy Vans throughout eight counties in rural northwest Missouri. Free transportation is provided Monday through Saturday to patients in need of medical services, but who have no other viable means of travel. Patients are transported to and from the Medical Center, physicians' offices, CRMC's satellite clinics, and/or CRMC's outpatient specialty clinics. Based on need, transportation to tertiary centers is also made available under certain circumstances. CRMC continues to provide free public education seminars on Medicare, Medicaid, Medicare Advantage, managed care, Part D, the PPACA, commercial insurance, and other third-party payors two to three times per year. These question-and-answer sessions cover all facets of public health care concerns. The Medical Center provides individuals with free enrollment assistance for both Medicaid and Medicare upon request.Priority/Objective No. 6 Urgent Care ServicesAs feasibility studies show the area's population is not sufficient to support an Urgent Care Center in Cameron, CRMC initiated an After-Hours Care Program at Walnut Medical Clinic several years ago (2011). Over the years, the hours have been expanded in phases, enabling the clinic to begin seven-day-a-week operation effective April 1, 2019. As demand for After-Hours services continued to increase, CRMC entered the design phase in May 2019 for a new, larger Walnut Medical Clinic to be located in DeKalb County, and ground-breaking took place in the fall of 2019. (Current fiscal year update: Opening in the spring of 2020, the new 5,200 square-foot facility features increased providers and 14 exam rooms for greater access to care.) Early morning hours are still available five days per week at Westside Medical Offices in Cameron. CRMC provides primary care services via 18 satellite clinics located throughout five counties in rural northwest Missouri. The addition of Saturday hours is being considered at certain clinics. CRMC's 24-hour Emergency Room sees all patients, regardless of ability to pay and circumstances.Needs NOT Addressed for FYE 9-30-19Priority/Objective No. 5 Child CareAs child care is not a central component of CRMC's mission or operations, the Medical Center plays a SUPPORTING ROLE ONLY in addressing this need. The Medical Center cooperates in economic development efforts toward promoting new businesses whenever feasible in the hope of realizing additional pre-schools and child care centers in rural northwest Missouri. CRMC has been examining the Cameron YMCA's needs for additional child-care facilities to see if the Medical Center can assist in any way. CRMC Administration has held discussions with the Missouri State Representative for District 008 concerning the prohibitive nature of state child care licensing laws.

Part I, Line 7:A cost-to-charge ratio derived from Worksheet 2 was used to calculate the amounts reported on line 7.
Part I, Line 7g:The costs of subsidized health services include costs attributable to multiple physician clinics in the amount of $9,381,482.
Part III, Line 4:See note B (beginning on page 11) of the audited financial statements regarding the allowance for uncollectible accounts.Part III Line 2:This amount was determined by multiplying a cost-to-charge ratio derived from Worksheet 2 by the Medical Center's bad debt expense for the tax year.Part III, Line 3:This amount was determined based on the overall income level of patients that submit all required documentation under the Medical Center's financial assistance policy. As a tax-exempt hospital, we must provide necessary services regardless of the patient's ability to pay for that care. Making quality patient care available to all in our community, regardless of their economic means, qualfies these bad debts as a community benefit.
Part III, Line 8:Medicare costs were obtained from the Medical Center's as-filed Medicare cost report for the tax year. The shortfall is a benefit to the Medical Center's community since it represents health care provided at less than cost to elderly and needy residents of the Medical Center's service area.
Part III, Line 9b:At the time of admission and before discharge, each patient will receive a notice that the Medical Center has a Financial Assistance Policy (with a brief description of such policy) and directions for obtaining additional information about such policy. All itemized statements sent to the patient include a notice that the Medical Center has a Financial Assistance Policy and directions for obtaining additional information about such policy. If a patient submits a Financial Assistance Policy application, all payment notifications and collection actions are suspended until it is determined whether the patient is eligible for the Financial Assitance Policy (and if so, notifications and collection actions will cease).
Part VI, Line 2:The Medical Center relies on the feedback and requests taken from patients of the Medical Center and its numerous clinics, as well as residents from its primary service area.
Part VI, Line 3:The Medical Center employs a social worker that informs all patients of all available payment options.
Part VI, Line 4:Cameron Regional Medical Center and its ten rural health clinics are located in Northwest Missouri. The primary service area is comprised of four rural counties consisting of Daviess, Caldwell, DeKalb, and Clinton. There are approximately 50,232 individuals that reside in the primary service area. Of these residents approximately 12.7% of them have an annual income below the federal poverty level. The median annual income per household for the four counties is approximately $51,731. Approximately 39% of the total services provided are for Medicare patients and 12% are for Medicaid patients. The nearest surrounding hospitals are located in Smithville, Excelsior Springs, and Richmond. The distance between these facilities and Cameron Regional Medical Center is approximately 28, 28, and 31 miles respectively.
Part VI, Line 5:Cameron Regional Medical Center's governing body is comprised of residents from its primary service area. The Medical Center extends medical privileges to all local physicians who meet the requirements of the medical staff bylaws. All surplus funds are used to maintain and improve the medical facility and medical equipment in order to provide the best medical treatment possible to every patient.
