Facility: - Part V, Section B, Line 5The 2019 CHNA data collection process can be summarized as follows: 1. review of prior CHNA reporting efforts; 2. Collection of most recently available demographic, socioeconomic and health indicator data. 3. Data gathering from community membersSteering Committee: A steering committee was formed to: a) advise Islander Institute on the factors that impact health in Hawaii and the different communities affected; b) participate in or help plan community engagement; and c) provide input and analysis in the development of findings. The committee included a number of willing participants from HAH and member facilities, and provided an opportunity for them to forge new relationships and shared understandings that will hopefully benefit the future implementation phases of this CHNA. The steering committee met a total of three times in 2018 on May 21, July 27, and September 19. In between meetings, steering committee members were called on for advice and for help in making community connections. Some attended community meetings, and some planned and facilitated meetings based on their own relationships with groups.Community Meetings: Islander Institute convened 21 community meetings in total to engage everyday people in discussions around health in their communities. A specific effort was made to learn about the experience of health in vulnerable communities, including disadvantaged minorities, rural residents, youth, seniors, places that are medically underserved, and people with low incomes. The meetings were not limited to these populations and included a diverse sample of geographies, ages, ethnicities, and socioeconomic backgrounds. Special attention was given to ensure the meetings were not merely extracting information from people and communities. Rather, the purpose of the meeting.Key Informant Interviews: Islander Institute created an initial list of people who could shed light on questions of health, social determinants, and the circumstances in various communities. The initial list included experts and authorities with whom Islander Institute had existing relationships based on its many years of working in Hawaii. Throughout the months of conversations, people suggested others to talk to and helped make connections. In total, Islander Institute spoke with roughly 200 people from six islands in person or over the phone, in a one-on-one format or in small groups, and in formal or informal settings. Among the key informants, Islander Institute also met with representatives of each of the 19 participating hospital facilities to best understand their engagement with the CHNA and the communities they serve. These conversations helped illuminate interests, capacities, and missions of each facility and shed light on limitations and uses of previous CHNAs and the potential for this CHNA. Additionally, Islander Institute obtained the informant interview notes from the past two CHNAs, reviewed them, and incorporated those views that were still timely into this process.Secondary data used for the CHNA was used in context with data collected from sources above. Some core county-level data is presented through the community lens that was developed from community conversations and key informant feedback. Additionally, administrators of the CHNA used data from Hawaii Health Matters, Data USA, HealthLandscape, and Community Commons.
Facility: - Part V, Section B, Line 6aNineteen HAH member hospitals contributed to the production of this CHNA.The names of those hospitals follow: Adventist Health Castle, Kahi Mohala, Kahuku Medical Center, Kaiser Foundation Hospital Honolulu, Kapiolani Medical Center for Women & Children, Kuakini Medical Center, Kula Hospital, Lanai Community Hospital, Maui Memorial MedicalCenter, Molokai General Hospital, North Hawaii Community Hospital, Pali Momi Medical Center, The Queens Medical Center, The Queens Medical Center West Oahu, Rehabilitation Hospital of the Pacific, Shriners Hospitals for Children Honolulu, Straub Medical Center, Wahiawa General Hospital, and Wilcox Medical Center.
Facility: - Part V, Section B, Line 11Adventist Health Castle Medical Center identified the following areas of significant needs in the 2016 CHNA: diabetes and access to health care. The main goal was to improve the overall health and wellness of our communities through provision of services, community collaboration and innovation. Priority Need 1 - Diabetes Adventist Health Castle saw the A1C improvement of 1.24 in 2019 from 1.05 in 2018. Average weight loss was 5.7 pounds in 2019, which is a decrease from 6.6 pounds in 2018. We continued to have a focus of increasing A1C compliance along with controlling blood pressure. In 2019, we had 79% compliance with our patients with A1C.Interventions:1. The Wellness and Lifestyle Medicine Center receives referrals for children, adolescents, and adults diagnosed with obesity and/or diabetes. On a continual basis throughout the year, a morning and evening 5-week Self-Management Education Diabetes Program class was offered. Number of Community Members Served: 1,166 diabetic patient encounters were served in our American Association of Diabetes Educators accredited program 2. Increasing awareness and participation in self-management education: We reached out to our community to encourage enrollment in the Self-Management Education Diabetes Program where participants learn to adopt healthy behavior changes. We also encourage family referrals. As way of serving the community and creating greater awareness of the services available, we participated in community events including Veg Fest, I Love Kailua Town Party, and Convoy of Hope. At Convoy of Hope, 1,319 people were provided health services over the course of a single day. In late 2019, we revised our referral forms so physicians may select to have medical management for their patients by our Nurse Practitioner who is a Certified Diabetes Educator. In 2019, we expanded access to classes beyond Kailua to include a Kaneohe location throughout the year. Having more than one location has been well received, and we will continue to have classes in both the Kailua and Kaneohe locations. Number of Community Members Served: 254 unique patients. Our 2019 goal was to increase from a completion rate of 87% to a 90% completion rate and we successfully had a completion rate of 91% with at least 300 unique patients.3. We mailed a post card to community residential addresses and have received a positive response from patients who directly want to enroll in classes. We are continuing this communication on a quarterly basis. Number of Community Members Served: 55,000 homes every quarter.4. Our social media presence expanded by 80% on Instagram (IG) and Facebook #castlewellnesshawaii to reach new and existing participants. Number of Community Members Served: from 350 followers in 2018 to 640 followers in 2019 with a goal of 1,0005. As of October 2017, we hired a Certified Diabetes Educator (CDE) who is a Nurse Practitioner (NP) and she receives referrals for all diabetes and pre-diabetes patients inpatient and outpatient. The CDE/NP establishes an introduction between the patient and his/her family with our diabetes program, and as of 2019 a clinic follow-up appointment with the NP is scheduled at bedside before the patient returns home. This real time scheduling has decreased the barrier of delayed education post hospital discharge. Additionally, before the patient leaves the hospital, the NP is prescribed a glucometer, learns how to use the glucometer, and given a glucose food log. For the patient, education begins bedside and there is a bridge to outpatient education. Number of Community Members Served: 1,258 diabetes patientsPriority Need 2 - Access to Care Castle Health Group has had a focus on access and worked with physician members to expand office hours including evenings and weekend hours. This has been very successful and allowed community members to seek care outside of their normal working hours. We have also partnered with local Urgent Cares to ensure notes on patients are sent to the primary care providers to coordinate care. In 2019, we had 26,696 Office visits throughout our clinics. By continuing to have same day access that we implemented in 2018, we have increased our visits by 6,282. Interventions:1. Added ENT, Cardiology, and Dental services to our Rural Health Clinic in Laie. Number of Community Members Served: 2,432 unique people were served in our Rural Health Clinic 2. Hired 1 Interventional Cardiologist to serve the Windward Community. Number of Community Members Served: 875 people served for same day visits.Priority Areas Not Addressed:The following 20 areas were identified as top health priority needs in our 2016 CHNA: access to health services, cancer, diabetes, disabilities, economy, education, environment, exercise, nutrition & weight, family planning, heart disease & stroke, immunizations & infectious diseases, injury prevention & safety, maternal, fetal & infant health, mental health & mental disorders, older adults & aging, oral health, respiratory diseases, social environment, substance abuse & lifestyle, and transportation. After investigating the twenty topic areas identified in the report, the CHNA committee followed an iterative process in which the focus was narrowed to increasingly fewer topics, each of which was investigated further to determine its fit with selection criteria. After internal discussion and consultation with community stakeholders, our CHNA committee chose diabetes and access to health care as our top community needs.

Part I, Line 7 - Explanation of Costing MethodologyThe costs were determined by using a cost-to-charge ratio. The cost-to-charge computation is based on hospital specific data included in the system-wide combined audited financial statements. The formula used for computation equals financial statement data as follows:(Total expenses - (Provision for bad debts + Other revenue + Interest income))/Gross patient charges)
Part III, Line 2 - Methodology Used To Estimate Bad Debt ExpenseUncollected patient accounts are analyzed using written patient financial services policies that apply standard procedures for all patient accounts. The result of the analysis is what is recognized as bad debt expense. For example, all self-pay patients receive a discount. If the discounted account is unpaid after collection efforts, the unpaid balance is classified as bad debt. The cost-to-charge ratio is multiplied times the hospital's bad debt expense. The resulting figure has been reported as bad debts at cost on Part III, Line 2.
Part III, Line 3 - Methodology of Estimated Amount & Rationale for Including in Community BenefitThe portion of the bad debts attributed to charity care as reported on Part III, Line 3 was calculated by an independent third-party consulting firm. This is an estimate of additional charity care that would have been granted if patients had cooperated by furnishing family financial information. A statistically valid sampling of patient accounts written-off was evaluated. The evaluation used various factors to determine which patients would have been eligible for charity care. Had the hospital obtained sufficient information from all patients who qualified for financial assistance, these additional accounts would have been recorded as charity care instead of bad debt.
Part III, Line 4 - Bad Debt ExpenseThe system-wide combined audited financial statements do not contain a footnote describing bad debt expense.
Part III, Line 8 - Explanation Of Shortfall As Community BenefitThe Medicare cost report apportions the hospitals costs on the basis of inpatient days and ancillary and outpatient charges to establish the costing methodology.Healthcare delivery by hospitals is a complex, highly regulated business in the United States. Healthcare unit cost inflation is driven by compliance with ever-expanding regulatory requirements, shortages of highly skilled labor and evolving medical and information technology. The health care market basket is unrelated to that of the average individual consumer. Since the 1997 Balanced Budget Act, Medicare annual payment updates have fallen behind actual healthcare cost inflation to the point that Medicare payments too many U.S. hospitals are well below the cost of providing care. These unreimbursed costs are a community benefit for seniors and others in the community as these individuals are continuing to receive care without which many would become dependent on other governmental resources such as Medicaid. The benefit to the community for healthier Medicare recipients is no different than those benefits the community realizes for uninsured and underinsured patients who are eligible for partial and full charity care. Medicare is a safety net for seniors and others. Without Medicare coverage, many individuals would undoubtedly qualify for charity care.In addition to the mismatch between Medicare payment increases and healthcare cost inflation, the highly complex Medicare payment systems and formulas produce disparate payment levels from one hospital to another for the same service. These disparate payment levels create disparate results within groups of hospitals.Reconciliation of Medicare Revenue from the hospital's Medicare Cost Report to GL*Medicare Cost Report Revenue $32,504,526Prior Year Settlements 2,940Cost Report Reimbursable Bad Debts 287,132Estimates and Accrual Variances 229,882Other 1,887,617 ___________Total Medicare Revenue $34,912,097*Note: The Medicare Cost Report Revenue includes the patient co-pay and deductible amounts, but not the bad-debt reimbursement. Adding the bad-debt reimbursement would have duplicated the revenue and deductible amounts.
Part III, Line 9b - Provisions On Collection Practices For Qualified PatientsWhen a patient has requested screening for charity care, the hospital must immediately cease collection activity and place the account in a charity pending status. If 100% charity is approved, the entire account balance is written off to charity care. If the patient has a sliding scale liability based on the federal poverty guidelines, they are billed only for that liability. If the patient fails to pay their after-charity liability, they are assigned to a collection agency with an identifier that indicates to the agency that the patient is "low income" and the following criteria must be followed by the agency: 1. They may not report the patient to a credit bureau 2. They may not file a lawsuit to recover the outstanding liability 3. They may not charge interest
Part VI, Line 2 - Needs AssessmentThe hospital's 2019 CHNA, the 2020 Community Health Plan (CHP) Update for fiscal year 2019, and the 2020 Implementation Strategy are posted on the hospital's website at: https://www.adventisthealth.org/castle/about-us/community-benefit/The two most recent Community Health Needs Assessments, two most recent Implementation Strategy reports and the most recent Community Health Plan Updates are also available on the Adventist Health Corporate website athttps://www.adventisthealth.org/about-us/community-benefit/In addition to the CHNA, our hospital engages in a process of continuous quality improvement, focusing on questions such as: are our interventions making a difference in improving health outcomes; are we providing the appropriate resources in the appropriate locations; and how are we using technology to track our health improvements and provide relevant feedback at the local level? Building a healthy community requires multiple stakeholders working together with a common purpose. With that in mind, our hospital leadership serves on a variety of community boards and committees focused on addressing community-specific needs, allowing for ongoing responsiveness to the health care needs of the community and collaboration with local agencies and organizations, enabling maximum effectiveness through collective impact.
Part VI, Line 3 - Patient Education of Eligibility for AssistanceThe plain language summary of the Financial Assistance Policy (FAP) is posted along with the complete FAP policy and FAP Application on the hospital's website at https://www.adventisthealth.org/documents/financial-assistance/ENG_PFS-112_Financial-Assistance-Policy.pdf. These documents are available in multiple languages. At the time of registration, patients who are uninsured and underinsured are provided information about government healthcare programs. Patients are also orally informed of their right to request charity assistance. Signs are displayed in the patient business office, patient registration areas and the emergency room in multiple languages informing patients of this right as well. The hospital also provides a brochure during the registration process that explains the hospital billing and collection procedures, and how to request financial assistance. In addition, every billing statement sent to patients contains information on how to request financial assistance.
Part VI, Line 4 - Community InformationO'ahu is a place of tourism and gentrification. It is a place where many cultures are able to practice their traditions and share them. It is a place of visible disparities between rich and poor, urban and rural, sheltered and unsheltered. It is a place of rural living where you know your neighbors and care for each other. It is a place of workplace stress, traffic and long commutes, sedentary lifestyles, poor sleep, and people locked in on their phones. It is a place of convenience, job opportunities, and healthcare accessibility for most. It is a place of commercial activity and material abundance. It is a place of connection to lo'i, fishponds, beaches, hiking, and fishing. A steady stream of development continues around rail, luxury condominiums, shopping centers, and suburban neighborhoods. Many feel policies regarding public areas are made for the safety and wellbeing of visitors first, thus exacerbating a crisis for houseless people in large part caused by a lack of sufficient affordable housing and insufficient services for those with mental illnesses. Care for houseless individuals, particularly those in urban Honolulu, falls primarily to government and nonprofits rather than appealing to a broader notion of all caring for all.The population on Oahu is 990,060, of which 1 in 10 at any given time are visitors. Of the population, 242,500 identify as Hawaiian. Almost one in four residents identifies as two or more races, which is much more than in the U.S. overall, and the largest single race group in Hawaii is Asian. 4500 residents of Oahu are homeless, 86,900 are living in poverty, 23,500 are children living in poverty and 21,800 are unemployed. Those living without health insurance number 37,600. 78,400 residents are identified as veterans. Oahus communitys highest needs include addressing mental and physical well-being for families, developing traffic solutions to reduce commute times, resisting gentrification and inequality of conditions exacerbated by development, and engaging the public in developing and implementing solutions for the houseless population.
Part VI, Line 4 - Community Building ActivitiesThe hospital is involved in numerous community building activities which promote the health of the communities it serves. Numerous community concerns are addressed, including health improvement, education, poverty, workforce development and access to care. We also encourage our employees to serve on community collaboration boards, health advocacy programs, and physical improvement projects to promote the health of the communities we serve. In addition, we work with neighborhood programs, including schools, work sites and safety net providers to promote health and wellness and prevent disease. These activities are not included elsewhere on Schedule H.
Part VI, Line 5 - Promotion of Community HealthOur hospitals mission is, Living Gods love by inspiring health, wholeness and hope. Our community benefit work is rooted deep within our mission and merely an extension of our mission and service. We have also incorporated our community benefit work to be an integral component of improving the triple aim. The Triple Aim concept broadly known and accepted within health care includes:1) Improve the experience of care for our residents.2) Improve the health of populations.3) Reduce the per capita costs of health care. Our strategic investments in our community are focused on a more planned, proactive approach to community health. The basic issue of good stewardship is making optimal use of limited charitable funds. Defaulting to charity care in our emergency rooms for the most vulnerable is not consistent with our mission. An upstream and more proactive and strategic allocation of resources enables us to help low income populations avoid preventable pain and suffering; in turn allowing the reallocation of funds to serve an increasing number of people experiencing health disparities.Hospitals and health systems are facing continuous challenges during this historic shift in our health system. Given todays state of health, where cost and heartache is soaring, now more than ever, we believe we can do something to change this. These challenges include a paradigm shift in how hospitals and health systems are positioning themselves and their strategies for success in a new payment environment. This will impact everyone in a community and will require shared responsibility among all stakeholders. As hospitals move toward population health management, community health interventions are a key element in achieving the overall goals of reducing the overall cost of health care, improving the health of the population, and improving access to affordable health services for the community both in outpatient and community settings. The key factor in improving quality and efficiency of the care hospitals provide is to include the larger community they serve as a part of their overall strategy. Population health is not just the overall health of a population, but also includes the distribution of health. Overall health could be quite high if the majority of the population is relatively healthyeven though a minority of the population is much less healthy. Ideally such differences would be eliminated or at least substantially reduced.Community health can serve as a strategic platform to improve the health outcomes of a defined group of people, concentrating on three correlated stages:1) The distribution of specific health statuses and outcomes within a population; 2) Factors that cause the present outcomes distribution; and 3) Interventions that may modify the factors to improve health outcomes.Improving population health requires effective initiatives to: 1) Increase the prevalence of evidence-based preventive health services and preventive health behaviors, 2) Improve care quality and patient safety, and 3) Advance care coordination across the health care continuum. We will work together with our community to ensure the community health improvements are identified and then targeted for programs to influence behaviors to obtain improved health within the whole community.
Part VI, Line 6 - Affilated Health Care SystemThe hospital is a member of Adventist Health, a health care system which provides healthcare services in diverse markets within the Western United States. A member hospital may share some services with other member hospitals in its geographic area, such as clinical, management and support services. Using today's technology, hospitals outside the geographic area can provide support through remote services such as telepharmacy and robotics surgery. The Corporate Office provides important shared administrative support for member hospitals' rural health clinics and home care agencies, quality of care, other clinical needs, financing and risk management, and shared clinical and financial information technology. As many experienced and new physicians search for alternatives to independent practice, there is also corporate administrative support for hospital affiliated medical groups that engage physicians through employment or other contracts. This provides stability and growth of qualified physicians across many specialties, which is very important to make healthcare services available and to maintain and improve health within the communities served by all member hospitals.
