ExplanationKey informant interviews were conducted with approximately 80 community stakeholders from throughout Bergen County. These interviews confirmed and/or refined the findings from quantitative data sources and provided valuable insight on community need, community health priorities, segments of the population most at-risk, and community health assets. Individual interviews were conducted by phone using a structured interview guide developed by John Snow, Inc. (JSI), a public health research and consulting firm, and the Steering Committee. At the outset, JSI worked with the Steering Committee to identify a representative list of key informants that could provide a deep and broad perspective on the health-related needs of the County. This list included administrative and clinical representatives from each of the hospitals and Bergen County Department of Health Services (BCDHS), as well as representatives from across many sectors, including health, public health, social service, academic, and business. Detailed notes were taken for each interview. For a list of interviewees, their organizational affiliations, interview dates, and the interview guide, please see Appendix A of the Community Health Needs Assessment. Key themes and findings from these interviews are included in the narrative sections of this report. During this Phase, JSI staff worked with the Steering Committee to develop a Resource Inventory. This inventory was meant to inform what services are available in Bergen County to address community needs as well as to determine the extent to which there are gaps in health-related services. The Community Health Improvement Partnership (CHIP) of Bergen County and BCDHS staff supported this effort by providing a list of community partners and known resources from across the broad continuum of services, including clinical health care services, community health and social services, and public health resources. This was done primarily by compiling information from existing resource inventories and partner lists from the CHIP, BCDHS, hospitals, and other service providers. The Resource Inventory can be found in Appendix C of the Community Health Needs Assessment. Part V, Section B, Line 6A Bergen New Bridge Medical Center, Englewood Health, Hackensack Meridian Health Hackensack University Medical Center, Hackensack Meridian Health Pascack Valley Medical Center, Holy Name Medical Center and The Valley Hospital. Part V, Section B, Line 7a https://christianhealthcare.org/community/community-health-needs-assessmen t/ Schedule H Part V Section B Line 10a https://christianhealthcare.org/community/community-health-needs-assessmen t/ Part V, Section B, Line 11 Ramapo Ridge Psychiatric Hospital is committed to improving the health status and well-being of all residents living in Bergen County - certainly all geographic, demographic, and socioeconomic segments of the population face challenges that may impede their ability to access care or maintain good health. Regardless of age, gender, race/ethnicity, income, family history, or other characteristics, everyone is impacted in some way by health-related disparities. With this in mind, RRPHs Implementation Strategy includes activities that will support all residents, across all segments of the population. However, based on the assessments quantitative and qualitative findings, there was agreement that RRPH should prioritize certain demographic and socio-economic segments of the population that have complex needs or face especially significant barriers to care, service gaps, or adverse social determinants of health, which put them at greater risk. RRPHs community health priorities have been framed broadly to ensure that the full breadth of unmet needs and community health issues are recognized. The priorities have been identified to maximize impact, reduce disparities, and promote collaboration and cross-sector partnership. Based on the findings from CHNA activities, and RRPHs clinical expertise, leadership opted to prioritize the following community health issues: behavioral health (mental health and substance use disorder), chronic/complex conditions and risk factors, social determinants of health and health disparities. It is important to note that there are community health needs that were identified through the Community Health Needs Assessment that were not prioritized for inclusion in the Implementation Strategy. Reasons for this include: * Feasibility of Ramapo Ridge having an impact on this issue in the short or long term * Clinical expertise of the organization * The issue is currently addressed by community partners in a way that does not warrant additional support * Poverty/employment, housing stability, and transportation were identified as community needs, but were deemed to be outside of Ramapo Ridges primary sphere of influence. Ramapo Ridge Psychiatric Hospital remains open and willing to work with hospitals and other public and private partners to address these issues should an opportunity arise. ExplanationChristian Health Care Center uses the cost to charge ratio to calculate the amount presented in Part 1 line 7. This cost to charge ratio methodology addresses all patient segments throughout the Christian Health Care Center community. Part II Christian Health Care Center provides health education classes to residents in the community and on-site education training to students which promote healthy living and lifestyle choices to community residents. PART III, LINE 2 Christian Health Care uses the amount of bad debt expense calculated in the cost report. The benefit to the community at large is their ability to obtain healthcare and treatment they might have not been able to receive due to their ability to pay. Part III, Line 3 Christian Health uses the patient ability to pay to the Medicare fee schedule to estimate the bad debt expense for patients under its financial assistance policy. PART III, LINE 4 Christian Health Care Center's bad debt expense footnote appears on the top of page 17 in the 2019 audited financial statements. PART III, LINE 8 Christian Health Center uses the cost to charge ratio to determine the amount that is included in line 6. Christian Health Care Center is treating a severely mentally ill population who otherwise would not be able to receive treatment if the Center did not provide this benefit to its inpatient and outpatient residents. The Medicare shortfalls should be considered a community benefit because Christian Health Care Center is providing care to these patients/residents with full knowledge that Medicare reimbursement will not cover the full cost of care to these residents. PART III, LINE 9B In connection with its Mission Statement and subject to the availability of resources, the Center provides outpatient, inpatient and partial hospitalization clients/patients free care or sliding fee scale to uninsured, under insured or otherwise ineligible insured patients/clients who are unable to pay for services due to their financial situation. Free Care or Sliding Fee Scale shall refer to the provision of health care services and supplies by employees of the Center. Free Care or a Sliding Fee Scale shall not include cash payment in any form, such as the payment of any individual's health insurance premiums, or free goods not otherwise furnished in the ordinary course of the Center's operations. Part VI, 2 NEEDS ASSESSMENT Christian Health Care Center assess the health-care needs of the community through market research conducted by organizations specializing in senior markets. They also track and trend referrals and admissions from acute care facilities, other nursing facilities and directly from the community at large. Whereas, trending these numbers shows the organization which programs will require additional resources to operate at a high level. Part VI, 3 PATIENT EDUCATION OF ELIGIBILITY FOR ASSISTANCE The Center informs and educates patients and persons who may be billed for patient care about their eligibility for assistance under federal, state, or local government programs or under the organization's charity care policy. The Center posts its Charity Care Policy, or a summary thereof, and financial assistance contact information in the admissions areas, waiting rooms and by the reception areas, and other areas of the Organization's Facilities in which eligible patients are likely to be present or seek such information. The Center provides a copy of the policy, or a summary thereof, as well as financial assistance contact information to patients as part of the intake and discharge process. The above materials are also included, in patient bills; and/or discussed with patient with the availability of various government benefits, such as Medicaid or State programs, and this assists the patient with the qualifications for such programs where applicable. Part VI, 4 COMMUNITY INFORMATION DESCRIPTION Christian Health Care Center serves two distinct communities; older adults and the mentally ill. The service area for Elder-Care programs includes Northwest Bergen County and Southern Passaic County. The service area for inpatient psychiatric services is much larger and includes most of Northern New Jersey. Also, the age of the psychiatric patients spans from eighteen years old to geriatric. The Center implements awareness and education in external based-settings to help the community. Part VI, 5 PROMOTION OF COMMUNITY HEALTH The Center provides meals on wheels to services, social outreach to community members who are housebound as well as CPR classes to local residents. Part VI, Line 6 Christian Health Care Center is not part of an affiliated health care system. Part VI, Line 7 New JerseyExplanationTxt
