DescOver the course of 2019, America's Agenda: Health Care for All (AAHCA) program services continued to promote and expand delivery of higher quality patient care, while achieving more affordable overall health costs and a substantially improved patient experience. Throughout 2019, AHCA programs continued to focus on two core areas of high value care delivery transformation: 1) delivery of high-value, broad scope primary care, and 2) improved affordability of prescription medications. During the course of 2019, AAHCA was able to apply its work in these two areas with particular focus on the State of New Jersey as a first adopter by virtue of Governor Murphys appointment of AAHCA Executive Director Mark Blum to serve as a member of the Governors Taskforce on Health Benefits Quality and Value and Americas Agendas engagement as a health policy advisor to the New Jersey legislature.AAHCAs continued its emphasis on transformation to advanced, high-value primary care design for two principal reasons: a) because robust provision of well-designed, broad scope primary care is the most cost effective means of preventing and cost-effectively managing chronic disease which accounts for 84% of overall health care spending in the US and b) scaling of well-designed, broad scope primary care offers significant leverage toward high-value transformation of secondary and tertiary health care delivery where, in the US today, most health care dollars are spent. AAHCAs added focus on affordability of prescription medicines over the last several years arises from the emergence of prescription medications as the most rapidly growing component of overall health costs. Attainment of the AAHCA mission of securing affordable health care for every American requires a solution to growth in cost of pharmaceutical medicines that currently outpaces significantly, not only US wages and GDP, but also the rate of growth in overall health costs.Below are key AAHCA initiatives and accomplishments in these areas over the course of 2019:Detailed Program Activities:1.Stregnthening high-value delivery care both through primary care transformation and improved health plan administration -- in the State of New Jersey Principally through its work in support of the New Jersey Governors Taskforce on Health Benefits Quality and Value, AAHCA and several public sector unions represented on the Taskforce succeeded in winning Taskforce support for a) extending the duration for several more years of the Direct Primary Care Pilot Program that AAHCA had designed and guided through adoption in 2015-16 with a greater emphasis placed on a) expansion of enrolled lives through coordinated education about the offering to New Jersey public employees and b) more robust measurement and analysis of outcomes.Concurrently, Americas Agenda was able to win Taskforce support for innovative bifurcation of third party administrator or TPA bill-paying functions from health care provider network management functions that have traditionally been unified in the hands of the commercial insurance carrier that administers the State of New Jersey public employee health plans. The separation of bill-paying from network management responsibilities would. eliminate costly (to the State) insurance carrier conflicts of interest that undermine third-party the administrators fulfilment of responsibilities as a fiduciary to the State.Independent of the Task Forces deliberations on this issue, AAHCA served as advisor to State legislative leadership on addressing the matter in statute. S-3042), that was enacted and signed into law by Governor Murphy in July 2019.2.Expanded savings for the State of New Jersey to cumulative of $2.5 billion in projected prescription drug savings over 5 years through conduct of an expedited, AAHCA-proposed second reverse auction - In 2017, the State of New Jersey implemented the Americas Agenda-designed strategy to reduce prescription drug costs through creation of a dynamically competitive marketplace for selection of a pharmacy benefits manager, contracting of pharmacy services, and pricing of the prescription medicines. The result was 25% savings on prescription drug spending after the first year following implementation of the Americas Agenda-proposed strategy. The magnitude of New Jersey public workers prescription drug savings was so great that, according to a 2019 report by Governor Murphy, overall health care premiums in the States self funded health plans fell by as much as 14% in the case of the New Jersey public school employees plan.As the performance of the new prescription drug plan progressed, it became evident to AAHCA that expedited conduct of a second PBM reverse auction after only 2 years of a 3-year duration contract with the incumbent PBM could enable the State of New Jersey to further deepen its prescription drug plan saving by substantial amounts. Through educating New Jersey policy makers about the improved savings opportunity, AAHCA helped unify State leadership in a coordinated strategy for conducting a second PBM reverse auction in 2019. The initiative was very successful, and a recent projection has extended expected savings to the state to a total of $2.5 billion in savings over a cumulative 5-year contract period -- that is, by a sustained average of $500 million in prescription drug savings per year. 3. Export of the successful AAHCA strategy for prescription drug cost reduction to other States Buoyed by the magnitude of the State savings achieved in New Jersey, AAHCA recruited several allied organizations to support efforts in other states to apply the AAHCA approach and aligned policies to significantly reduce prescription drug spending without relying on cost-shifting or cutting public employees benefits. As a result of this initiative, AAHCA and several peer organizations launched the PBM Accountability Project with the goal of serving as a resource clearing house to support state initiatives to effectively lower costs of prescription medicines. The first state project to be launched was the PBM Accountability Project of New Hampshire, a statewide education and advocacy initiative led by a broad range of in-state organizations committed to a shared goal of lowering New Hampshire prescription drug spending without shifting the cost burden health plan beneficiaries. In launching their NH initiative, a bi-partisan group of Granite State business and union leaders, municipal mayors, and patient advocates expressed their shared objective of enacting legislation in the 2019-20 session that would benefit all of New Hampshire health plan purchasers of prescription drugs, both public and private sector, with the ultimate goal of providing security about the affordability of prescription medicines to patients, taxpayers, and employers by adopting and broadening access to the kind of PBM reverse auction strategy that has been so successful in New Jersey. The emphasis of the New Hampshire initiative on a bi-partisan free market solution to reducing prescription drug costs, amplified with AAHCA health policy and communications support, has earned an unusually broad spectrum of interest from New Hampshire leaders and organizations that are not frequently allied in matters of public health policy. In the State of Maryland, AAHCA has also worked during 2019 to educate State legislators, public employee representatives, and members Marylands new State Prescription Drug Affordability Board of the about the potential of the AAHCA-designed state strategy to address long-standing State concerns about the rising cost of prescription medicines.Desc
