| Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistance | THE COST TO CHARGE RATIO USED WAS BASED ON THE COST REPORT AND IS CONSISTENT WITH THE FINANCIAL STATEMENTS. |
| Schedule H, Part II Community Building Activities | COMMUNITY BUILDING ACTIVITIES - PROGRAMS AND SERVICES THAT WHILE NOT DIRECTLY RELATED TO HEALTH CARE, ADDRESS THE ROOT CAUSES OF HEALTH PROBLEMS SUCH AS POVERTY, CRIME, AND ENVIRONMENTAL ISSUES. EXAMPLES ARE PHYSICAL IMPROVEMENTS, ECONOMIC DEVELOPMENT, COMMUNITY SUPPORT, COALITION BUILDING, COMMUNITY HEALTH IMPROVEMENT ADVOCACY, AND WORKFORCE DEVELOPMENT ACTIVITIES. HEALTH EDUCATION IS PROVIDED ON THE HIGH PRIORITY HEALTH NEEDS IDENTIFIED IN THE CHNA, INCLUDING INFORMATION ON STRESS MANAGEMENT AND MENTAL HEALTH; OBESITY AND WEIGHT CONTROL; DIABETES AND KIDNEY DISEASE; HYPERTENSION AND OTHER CARDIOVASCULAR DISEASES; AND NUTRITION AND HEALTH BEHAVIORS. EDUCATION IS TAILORED TO VULNERABLE POPULATIONS IN NEIGHBORHOOD SETTINGS. FOR EXAMPLE, THE POPULATION HEALTH DEPARTMENT AT LAKELAND HAS DEVELOPED NEIGHBORHOOD-CENTERED HEALTH HOMES AT ELITE BARBERSHOP (A BARBERSHOP THAT PRIMARILY SERVES MEN) AND HARBOR TOWERS (A LOW-INCOME HOUSING FACILITY) IN BENTON HARBOR. THIS CATEGORY ALSO ENCOMPASSES ALLIANCES WITH COMMUNITY PARTNERS. FOR EXAMPLE, DISASTER READINESS PREPARATIONS AS PART OF THE FIFTH DISTRICT MEDICAL RESPONSE COALITION. LAKELAND ASSESSES THE HEALTH CARE NEEDS OF OUR COMMUNITIES IT SERVES BY CONDUCTING A COMMUNITY HEALTH NEEDS ASSESSMENT EVERY THREE YEARS. THE FIRST STEP IN CONDUCTING THE CHNA IN 2019 WAS TO STRATIFY THE POPULATION ACCORDING TO TWO PROXY INDICATORS OF HEALTH: AGE-ADJUSTED MORTALITY AND LIFE EXPECTANCY. THIS LED TO THE IDENTIFICATION OF SIGNIFICANT HEALTH INEQUITIES ACROSS CENSUS TRACTS IN BERRIEN COUNTY. FOR INSTANCE, BENTON HEIGHTS HAS AN AGE-ADJUSTED MORTALITY RATE THREE TIMES HIGHER THAN SHOREHAM. SIMILARLY, THE LIFE EXPECTANCY OF RESIDENTS IN PARTS OF BENTON HARBOR IS ABOUT NINETEEN YEARS SHORTER THAN THAT OF PEOPLE WHO LIVE IN BERRIEN SPRINGS AND LINCOLN TOWNSHIP. PEOPLE WHO LIVE IN AND AROUND THE CITIES OF BENTON HARBOR AND NILES DIE EARLIER THAN PEOPLE WHO LIVE IN AND AROUND THE CITIES OF ST. JOSEPH, STEVENSVILLE AND BERRIEN SPRINGS. MOREOVER, 75% OF THE CENSUS TRACTS WITH THE HIGHEST DEATH RATES (I.E., THE LOWEST QUARTILE) ARE IN AND AROUND THE CITY OF BENTON HARBOR. THESE FINDINGS OF SIGNIFICANT HEALTH INEQUITIES FORMED THE FOUNDATION UPON WHICH THE PROJECT TEAM PLANNED AND EXECUTED THE CHNA. BY DIRECTING THE DATA COLLECTION PROCESS, THESE FINDINGS HELPED THE PROJECT TEAM ENSURE THAT MEDICALLY UNDERSERVED, LOW-INCOME AND MINORITY POPULATIONS PROVIDED THEIR PERSPECTIVES ON COMMUNITY HEALTH. | Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amount | THE TOTAL BAD DEBT EXPENSE OF $17,030,304 EQUALS THE AMOUNT INCLUDED ON FORM 990, PART IX, LINE 24, COLUMN (A). |
| Schedule H, Part III, Line 3 Bad Debt Expense Methodology | THE ESTIMATED AMOUNT OF THE ORGANIZATION'S BAD DEBT EXPENSE ATTRIBUTABLE TO PATIENTS WHO LIKELY WOULD QUALIFY UNDER THE HOSPITAL'S FINANCIAL ASSISTANCE POLICY IS DETERMINED BY MULTIPLYING THE TOTAL BAD DEBT EXPENSE BY A RATIO OF ADJUSTED PATIENT CARE COST DIVIDED BY ADJUSTED PATIENT CARE CHARGES. |
| Schedule H, Part III, Line 4 Bad debt expense - financial statement footnote | The Net Patient Service Revenue and Patient accounts receivable footnote, which contains bad debt expense and allowance for uncollectible accounts is on pages 17-20 of the organization's consolidated financial statements. |
| Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costs | The methodology described in the instructions to schedule H, Part III, Section B, Line 6 does not take into account all costs incurred by the hospital and does not represent the total community benefit conferred in this area. Reasons why Medicare shortfall should be treated as community benefit are: (1) absent the Medicare program, it is likely many of the individuals would qualify for charity care or other needs-based government programs; (2) by accepting payment below cost to treat these individuals, the burdens of government are relieved with respect to these individuals; (3) there is a significant possibility that continued reduction in reimbursement may actually create difficulties in access for these individuals; and (4) the amount spent to cover the Medicare shortfall is money not available to cover charity care and other community benefit needs. In determining Medicare shortfalls the organization uses a ratio of cost-to-charges. In determining the ratio of cost-to-charges the organization adjusts for bad debt expenses, non-patient care activities, Medicaid provider taxes and community benefits accounted for, and or reported, elsewhere. The ratio of cost-to-charges is applied to Medicare charges to determine shortfalls in Medicare reimbursements. |
| Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistance | Patients who qualify for financial assistance are eligible for free care, and thus no collection actions are taken. If financial assistance eligibility would be discovered after commencement of a collection action, then such collection action would be suspended or reversed. |

A COMPREHENSIVE MENTAL HEALTH-RELATED CURRICULUM WAS DEVELOPED AND IS BEING DELIVERED THROUGH A REGIONAL MULTI-STAKEHOLDER COLLECTIVE IMPACT INITIATIVE. MENTAL ILLNESS IS LESS STIGMATIZED AS EVIDENCED BY THE SIGNIFICANT INCREASE IN THE NUMBER OF COMMUNITY MEMBERS AND ORGANIZATIONS SEEKING EDUCATION AND TRAINING. * INCREASED COMMUNITY HEALTH EDUCATION PROVIDED IN "NON-TRADITIONAL" SETTINGS. THIS WAS ACHIEVED THROUGH THE DEVELOPMENT AND EXECUTION OF A NEIGHBORHOOD-BASED HEALTH HOME MODEL IN WHICH HEALTH EDUCATION IS PROVIDED IN "NON-TRADITIONAL" SETTINGS (E.G., BARBERSHOPS, PUBLIC HOUSING COMPLEXES COMMUNITY FOOD NETWORKS) THAT SERVE MEDICALLY UNDERSERVED, LOW-INCOME AND MINORITY POPULATIONS. * DEVELOPED CULINARY SKILLS OF AND EXPANDED ACCESS TO NUTRITIOUS FOODS FOR MEDICALLY UNDERSERVED, LOW-INCOME, MINORITY, AND LIMITED ENGLISH PROFICIENCY RESIDENTS. THIS WAS ACHIEVED THROUGH THE IMPLEMENTATION OF TWO PROGRAMS: THE RX FOR HEALTH PROGRAM PROVIDED NUTRITION EDUCATION AND INCREASED ACCESS TO FRESH FRUITS AND VEGETABLES; AND THE COMMUNITY KITCHEN CLUB ADMINISTERED A CO-DESIGNED CURRICULUM THAT ENHANCED PARTICIPANTS' CULINARY SKILLS. * INCREASED COMMUNITY TRUST IN LAKELAND ASSOCIATES. THIS WAS ACHIEVED THROUGH THE CO-DESIGN AND IMPLEMENTATION OF COMMUNITY-DIRECTED PROGRAMMING. * INCREASED COMMUNITY CAPACITY FOR ADVOCACY AROUND ISSUES OF HOUSING AND PUBLIC SAFETY. WITH SUPPORT AND GUIDANCE FROM THE POPULATION HEALTH TEAM, RESIDENTS OF A NEIGHBORHOOD-BASED HEALTH HOME DEVELOPED THE CAPACITY TO HOLD PUBLIC AUTHORITIES ACCOUNTABLE FOR MEETING STATUTORILY MANDATED HOUSING NEEDS. * INCREASED ACCESS TO FRESHER AND HIGHER QUALITY SCHOOL MEALS. THIS RESULTED FROM A PLATE WASTE PROJECT UNDERTAKEN BY HIGH SCHOOL STUDENTS, THE RESULTS OF WHICH INFORMED A REVISION OF THE SCHOOL MEAL PROGRAM. |
