Schedule H, Part V, Section B, Line 5 Facility 1, 1Facility 1, 1 - UM St. Joseph Medical Center. Community engagement and feedback were an integral part of the CHNA process. A key informant survey sought input from 56 community stakeholders including public health and health care professionals, social service providers, non-profit leaders, business leaders, faith-based organizations, and other community leaders (See complete list below). The survey was designed to assess pressing health issues in the community, missing resources/services, health care access, underserved populations, and community assets and opportunities. It took approximately 10-15 minutes to complete and was delivered online. Brief community surveys were distributed at free flu clinics throughout Baltimore County to gain more information on health and socioeconomic concerns of residents. Feedback was received and compiled from 779 community members. Community focus groups were arranged to delve further into some of the health needs identified in the area. Useful insights and helpful suggestions were received from 34 community members across three sessions, including Spanish-speaking individuals, seniors and the general community. Topics of conversation included access to health care services, personal health and wellness and chronic diseases. Following the completion of the CHNA research, the health issues were prioritized and implementation plans were drafted with the help of 28 partners including health care providers, public health experts, health and human service agencies, and other community representatives (see complete list below). Schedule H, Part V, Section B, Line 11 Facility 1, 1Facility 1, 1 - UM St. Joseph Medical Center. In our most recent CHNA, the following priorities were identified: Access to Care, Mental Health and Substance Abuse, Chronic Disease- Cardiovascular Disease/Obesity, Cancer, and Fall Prevention. Our full CHNA with supporting documentation of these needs can be found here: www.umms.org/sjmc/community. Below is a summary of our initiatives according to priority area. For the full implementation strategy, please visit www.umms.org/sjmc/community. In response to Access to Care, the following actions are being taken: * Continue to provide increased access to health care in the uninsured population through St. Clare Medical Outreach. * Continue to support multidisciplinary follow-up care for high-risk patients through the Transitional Care Center. * Identify and address barriers related to transportation and psychosocial needs among high-risk discharged patients. * Offer onsite appointments with HealthCare Access Maryland representatives to individuals in need of health insurance. * Increase the use of telehealth kits among eligible heart failure patients, allowing these individuals to self-monitor and report regularly to a cardiologist through at home use of a tablet, blood pressure cuff, pulse ox, and scale. * Increase availability of free seasonal flu vaccines. In response to mental health and substance abuse, the following actions are being taken: * Continue to provide diagnostic and transitional treatment planning including comprehensive case management and intensive relapse prevention to at risk patients through the Behavioral Health Center. * Continue to devote four office hours a week to mental health services at St. Clare Medical Outreach. * Increase availability of peer recovery specialists through ongoing partnership with the Baltimore County Department of Health. * Increase mental health trainings and awareness efforts in partnership with local organizations. * Continue to offer Chronic Pain Self-Management workshops. * Educate youth on substance abuse, performance enhancement, and social responsibility. In response to chronic disease (cardiovascular disease/obesity), the following actions are being taken: * Implement a Heart Failure Clinic to ensure timely access to comprehensive, specialty care and to address medication management, education, and other barriers. * Continue to offer Prevent T2 lifestyle change program proven to prevent or delay type 2 diabetes. * Continue to offer Stanford Living Healthy with Hypertension workshop to help individuals with high blood pressure learn how to better manage their condition. * Increase cardiovascular screenings and education. * Provide free exercise programs for the community. * Partner with Hungry Harvest to host Produce in a SNAP, reduced-cost community markets that provide access to fresh, affordable fruits and vegetables. In response to cancer, the following actions are being taken: * Increase early detection through screenings and education. * Increase access to survivorship support and services in the new Wellness and Support Center. * Reduce the rate of adults who smoke. In response to fall prevention, the following actions are being taken: * Continue to offer Stepping On, an evidence-based fall prevention program. * Increase social support for older adults with the addition of new programs. * Partner with local safety and security teams to increase pedestrian safety efforts. During our most recent CHNA, there were a few community needs identified that are not being addressed directly by UM SJMC at this time. UM SJMC has not pursued initiatives related to oral health, as we do not provide dental services. We do make referrals to local resources such as Baltimore County Department of Health/Dental Access Program, Liberty Family Resource Center, Eastern Family Resource Center, Chase Brexton Health Care, CCBC, and Mission of Mercy for those in need. We also lack the resources to address dementia at this time; however, we do support and spread awareness on the efforts and expertise available through the Alzheimer's Association.
Schedule H, Part V, Section B, Line 13 Facility 1, 1Facility 1, 1 - UM St. Joseph Medical Center. THE FINANCIAL ASSISTANCE POLICY EXPLAINS SEVERAL ELIGIBILITY CRITERIA, INCLUDING PARTICIPATION IN MEDICAID/MEDICARE PROGRAMS AS WELL AS ELIGIBILITY UNDER VARIOUS STATE REGULATIONS. IN ADDITION TO FPG, THE INCOME LEVELS DEFINED BY THE MARYLAND STATE DEPARTMENT OF HEALTH AND MENTAL HYGIENE (MD DHMH) ARE USED TO DETERMINE ELIGIBILITY FOR FINANCIAL ASSISTANCE. THE MD DHMH INCOME LEVELS ARE MORE GENEROUS THAN THE FPG INCOME LEVELS.

Schedule H, Part I, Line 3c CRITERIA FOR FREE OR DISCOUNTED CAREUMSJMC IS COMMITTED TO PROVIDING FINANCIAL ASSISTANCE TO PERSONS WHO HAVE HEALTH CARE NEEDS AND ARE UNINSURED, UNDERINSURED, INELIGIBLE FOR A GOVERNMENT PROGRAM, OR OTHERWISE UNABLE TO PAY, FOR MEDICALLY NECESSARY CARE BASED ON THEIR INDIVIDUAL FINANCIAL SITUATION. THE FAP ALSO USES A FINANCIAL HARDSHIP THRESHOLD WHEN DETERMINING ELIGIBILITY. A PATIENT WITH MEDICAL DEBT EXCEEDING 25% OF FAMILY ANNUAL HOUSEHOLD INCOME MAY BE ELIGIBLE FOR FINANCIAL ASSISTANCE.
Schedule H, Part I, Line 6b COMMUNITY BENEFIT REPORTTHE ORGANIZATION ANNUALLY FILES A COMMUNITY BENEFIT REPORT AS REQUIRED BY THE MARYLAND HSCRC. THE REPORT CAN BE FOUND AT HTTPS://HSCRC.STATE.MD.US/PAGES/INIT_CB.ASPX.
Schedule H, Part VI, Line 5 Promotion of Community HealthSocial Determinants/Other Efforts and Partnerships UM St. Joseph Medical Center mentors clinical students from a variety of health fields and schools. Our employees recognize the importance of providing hands on experiences for these students, and the medical center appreciates that these labor hours foster more qualified graduates joining the workforce. UM St. Joseph Medical Center is a part of the Corporate Internship Program through Cristo Rey Jesuit High School located in Baltimore City. Along with our financial contribution, we commit to hosting four high school students in various departments across the medical center each year. The students are onsite five days a month throughout their school year. UM SJMC also provides work-based learning internships for students with special needs through Humanim. During FY19, UM SJMC hosted 53 Humanim students. UM St. Joseph Medical Center actively engages with several local partners to promote the health of the community. We have an ongoing partnership with our local Y in Central Maryland, the Orokawa Y in Towson. In addition to a sponsorship that benefits The Y Campaign For a Better Us (assists children, families and individuals who cannot otherwise afford Y membership and programs), we plan monthly health events such as free lectures delivered by experts, health fairs and screenings, and flu immunizations. We provide further support and expertise for their initiatives such as family health events, fitness challenges, and awareness efforts. This partnership allows for greater access to our shared community which includes underserved and high risk populations. The collaboration and shared resources have helped both organizations to better assess health needs and gain trust within the community. Our existing programs have been widely promoted to their membership and we have been able to offer tailored programs to meet the needs of their members. We work collaboratively with the Baltimore County Department of Aging (BCDA) to determine timely topics and services for the senior population. On an annual basis, we travel to the majority of the senior centers throughout the county offering free services such as bone density screening, body composition analysis, and presentations on topics like sleep, hypertension and stroke. UM St. Joseph Medical Center is represented in the Baltimore County Health Coalition as well as the University of Maryland System Health Improvement Committee which allows our team to remain well-informed and involved in new initiatives in Baltimore County and surrounding communities. To help better understand the needs of our community, UM St. Joseph also operates a Patient and Family Advisory Council that helps guide hospital direction for community services as well as raising operational and experience concerns. UM SJMC operates under a unique regulatory system that caps hospital revenues and provides adjustments to allow for population health improvement investments. Under Maryland's global budget revenue model, UM SJMC reports on a regular basis how it is investing in efforts to improve population health. UM SJMC provides on-going services that are fundamental to addressing the identified community health needs that demonstrate the extent to which our commitment to serve our community is integrated into our care delivery model. We have a strong focus on treating patients with chronic conditions. UM SJMC works to coordinate care, ensure smooth transitions and promote disease self-management strategies at every step of a patient's journey - whether at home, in the community, or within our hospital. We operate specialized clinics to enable access to routine outpatient care. Our transitional care services help patients newly diagnosed or those that have had a recent hospitalization transition safely back to the community. Transitional nurse navigators provide patients with disease education and self-management strategies, connect them to primary care providers and specialists, and help them overcome any barriers to making follow up appointments. An urgent care center to address off hours visits and low acuity emergencies is also available. Resources are embedded throughout the hospital and in the community so that every patient receives the same level of care coordination. Care managers are embedded within our emergency department, hospital, community settings and provide in-home visits as needed. Community health workers reach out to patients who are identified as being high risk for readmission or in need of critical resources. Close relationships are maintained with skilled nursing facilities and home health agencies so that transfers from the hospital to these facilities are smooth, and the highest quality of care is constantly delivered. UM SJMC also recognizes behavioral health as a critical need in our community and provides patient outpatient services in addition to our hospital-based services. We operate a Bridge Clinic, which assists patients who are in need of psychiatric assistance and support immediately following discharge from an inpatient behavioral health treatment program. For up to 90 days, the Bridge Clinic "bridges the gap" between a patient's discharge and his or her first connection with a permanent mental health provider in the community. Additionally, we recognize transportation assistance is a common barrier for patients. We are proudly partnered with Lyft, providing rides to patient follow up appointments. We are proud to serve our community and appreciate our partners who allow us to fulfill our mission. We will always do what is right for the patient, no matter where they are in their health journey, and will always strive to have our patients receive care in the community they reside. As a part of the University of Maryland Medical System (UMMS) we are shaping a new paradigm in care delivery that we believe should be the future standard of care.
Schedule H, Part I, Line 7g Subsidized Health ServicesIncluded on Line 7g as subsidized health services is $7,386,173 costs attributable to primary care physicians.
Schedule H, Part I, Line 7 Bad Debt Expense excluded from financial assistance calculation10535266
Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceMaryland's regulatory system creates a unique process for hospital payment that differs from the rest of the nation. The Health Services Cost Review Commission, (HSCRC) determines payment through a rate setting process and all payors, including governmental payors, pay the same amount for the same services delivered at the same hospital. Maryland's unique all payor system includes a method for referencing Uncompensated Care in each payors' rates, which does not enable Maryland hospitals to breakout any offsetting revenue related to Uncompensated Care. Community benefit expenses are equal to Medicaid revenues in Maryland, as such, the net effect is zero. Additionally, net revenues for Medicaid should reflect the full impact on the hospital of its share of the Medicaid assessment.
Schedule H, Part II Community Building ActivitiesUMSJMC ACTIVELY ENGAGES WITH SEVERAL LOCAL PARTNERS TO PROMOTE THE HEALTH OF THE COMMUNITY. WE HAVE AN ONGOING PARTNERSHIP WITH OUR LOCAL Y IN CENTRAL MARYLAND AND THE OROKAWA Y IN TOWSON. IN ADDITION TO A SPONSORSHIP THAT BENEFITS THE Y'S GIVE EVERY CHILD A CHANCE CAMPAIGN, WE PLAN MONTHLY HEALTH EVENTS SUCH AS FREE LECTURES DELIVERED BY EXPERTS, HEALTH FAIRS AND SCREENINGS, AND FLU IMMUNIZATIONS. UMSJMC IS ALSO PROUD TO SUPPORT THE EFFORTS OF POWERED BY ME!, A PROGRAM THAT ENCOURAGES POSITIVE DECISION MAKING AMONG THE YOUTH IN OUR COMMUNITY. THIS INCLUDES AN ANNUAL CONFERENCE ATTENDED BY APPROXIMATELY 500 STUDENT ATHLETES, COACHES, AND ADMINISTRATORS FROM PUBLIC AND PRIVATE SCHOOLS IN BALTIMORE COUNTY, BALTIMORE CITY, HOWARD COUNTY, HARFORD COUNTY, PRINCE GEORGES COUNTY, AND ANNE ARUNDEL COUNTY. THE FOCUS OF THE CONFERENCE IS PERFORMANCE ENHANCEMENT, SUBSTANCE ABUSE, AND SOCIAL RESPONSIBILITY. THE MESSAGE IS DELIVERED BY PROFESSIONAL ATHLETES, HEALTH EXPERTS, COUNTY REPRESENTATIVES, AND OTHER STUDENTS. THE GOAL IS TO EMPOWER THESE YOUNG ADULTS TO ACT AS ROLE MODELS AND REDUCE THE PREVALENCE OF DRUG ABUSE, EATING DISORDERS, PARTNER VIOLENCE, AND BULLYING IN OUR COMMUNITY. UMSJMC IS A PART OF THE CORPORATE INTERNSHIP PROGRAM THROUGH CRISTO REY JESUIT HIGH SCHOOL LOCATED IN BALTIMORE CITY. ALONG WITH OUR FINANCIAL CONTRIBUTION, WE COMMIT TO HOSTING FOUR HIGH SCHOOL STUDENTS IN VARIOUS DEPARTMENTS ACROSS THE MEDICAL CENTER EACH YEAR. CURRENTLY, STUDENTS ARE PLACED IN DEPARTMENTS SUCH AS EMPLOYEE HEALTH, MOTHER BABY, BUILDING OPERATIONS, AND PATIENT TRANSPORT. THE STUDENTS ARE ONSITE 1-2 DAYS A WEEK THROUGHOUT THEIR SCHOOL YEAR. THEY LEARN VALUABLE SKILLS IN THE WORK SETTING AND GAIN EXPOSURE TO POTENTIAL CAREERS. WE WORK COLLABORATIVELY WITH THE BALTIMORE COUNTY DEPARTMENT OF AGING TO DETERMINE TIMELY TOPICS AND SERVICES FOR THE SENIOR POPULATION. ON AN ANNUAL BASIS, WE TRAVEL TO THE MAJORITY OF THE SENIOR CENTERS THROUGHOUT THE COUNTY. MOST RECENTLY, WE DELIVERED PRESENTATIONS ON HOW MEDICATIONS CAN AFFECT SLEEP AND THE AGING BRAIN AT 14 OF THE BALTIMORE COUNTY SENIOR CENTERS. THE GOAL IS TO REDUCE DRUG ABUSE, DEPENDENCY, AND FALLS. ADDITIONALLY, WE CONTINUE TO OFFER FREE BONE DENSITY SCREENINGS TO SUPPORT BONE HEALTH AWARENESS AND REDUCED INJURIES FROM FALLS. UMSJMC IS REPRESENTED IN THE BALTIMORE COUNTY HEALTH COALITION AS WELL AS THE UNIVERSITY OF MARYLAND SYSTEM HEALTH IMPROVEMENT COMMITTEE WHICH ALLOWS OUR TEAM TO REMAIN WELL-INFORMED AND INVOLVED IN NEW INITIATIVES IN BALTIMORE COUNTY AND SURROUNDING COMMUNITIES.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountTHE HEALTH SERVICES COST REVIEW COMMISSION (HSCRC) STARTED SETTING HOSPITAL RATES IN 1974. AT THAT TIME, THE HSCRC APPROVED RATES APPLIED ONLY TO COMMERCIAL INSURERS. IN 1977, THE HSCRC NEGOTIATED A WAIVER FROM MEDICARE HOSPITAL PAYMENT RULES FOR MARYLAND HOSPITALS TO BRING THE FEDERAL MEDICARE PAYMENTS UNDER HSCRC CONTROL. IN 2014, MARYLAND'S WAIVER WITH MEDICARE WAS RENEGOTIATED AND UPDATED TO REFLECT THE CURRENT HEALTHCARE ENVIRONMENT. UNDER THIS NEW WAIVER, SEVERAL CRITERIA WERE ESTABLISHED TO MONITOR THE SUCCESS OF THE SYSTEM IN CONTROLLING HEALTHCARE COSTS AND THE CONTINUANCE OF THE WAIVER ITSELF: 1. REVENUE GROWTH PER CAPITA 2. MEDICARE HOSPITAL REVENUE PER BENEFICIARY 3. MEDICARE ALL PROVIDER REVENUE GROWTH PER BENEFICIARY 4. MEDICARE READMISSION RATES 5. HOSPITAL ACQUIRED CONDITION RATE BECAUSE OF THIS SYSTEM, THE HOSPITAL IS UNABLE TO ESTIMATE HOW MUCH OF THE AMOUNT REPORTED IN LINE 2 IS ATTRIBUTED TO PATIENTS WHO WOULD APPLY UNDER THE FAP.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteTHE CORPORATION RECORDS REVENUES AND ACCOUNTS RECEIVABLE FROM PATIENTS AND THIRD-PARTY PAYORS AT THEIR ESTIMATED NET REALIZABLE VALUE. REVENUE IS REDUCED FOR ANTICIPATED DISCOUNTS UNDER CONTRACTUAL ARRANGEMENTS AND FOR CHARITY CARE. AN ESTIMATED PROVISION FOR BAD DEBTS IS RECORDED IN THE PERIOD THE RELATED SERVICES ARE PROVIDED BASED UPON ANTICIPATED UNCOMPENSATED CARE, AND IS ADJUSTED AS ADDITIONAL INFORMATION BECOMES AVAILABLE. THE PROVISION FOR BAD DEBTS IS BASED UPON MANAGEMENT'S ASSESSMENT OF HISTORICAL AND EXPECTED NET COLLECTIONS CONSIDERING HISTORICAL BUSINESS AND ECONOMIC CONDITIONS, TRENDS IN HEALTHCARE COVERAGE, AND OTHER COLLECTION INDICATORS. PERIODICALLY THROUGHOUT THE YEAR, MANAGEMENT ASSESSES THE ADEQUACY OF THE ALLOWANCE FOR UNCOLLECTIBLE ACCOUNTS BASED UPON HISTORICAL WRITE-OFF EXPERIENCE BY PAYOR CATEGORY. THE RESULTS OF THIS REVIEW ARE THEN USED TO MAKE MODIFICATIONS TO THE PROVISION FOR BAD DEBTS AND TO ESTABLISH AN ALLOWANCE FOR UNCOLLECTIBLE RECEIVABLES. AFTER COLLECTION OF AMOUNTS DUE FROM INSURERS, THE CORPORATION FOLLOWS INTERNAL GUIDELINES FOR PLACING CERTAIN PAST DUE BALANCES WITH COLLECTION AGENCIES. FOR RECEIVABLES ASSOCIATED WITH SERVICES PROVIDED TO PATIENTS WHO HAVE THIRD-PARTY COVERAGE, THE CORPORATION ANALYZES CONTRACTUALLY DUE AMOUNTS AND PROVIDES AN ALLOWANCE FOR BAD DEBTS, ALLOWANCE FOR CONTRACTUAL ADJUSTMENTS, PROVISION FOR BAD DEBTS, AND CONTRACTUAL ADJUSTMENTS ON ACCOUNTS FOR WHICH THE THIRD-PARTY PAYOR HAS NOT YET PAID OR FOR PAYORS WHO ARE KNOWN TO BE HAVING FINANCIAL DIFFICULTIES THAT MAKE THE REALIZATION OF AMOUNTS DUE UNLIKELY. FOR RECEIVABLES ASSOCIATED WITH SELF-PAY PATIENTS OR WITH BALANCES REMAINING AFTER THE THIRD-PARTY COVERAGE HAD ALREADY PAID, THE CORPORATION RECORDS A SIGNIFICANT PROVISION FOR BAD DEBTS IN THE PERIOD OF SERVICE ON THE BASIS OF ITS HISTORICAL COLLECTIONS, WHICH INDICATES THAT MANY PATIENTS ULTIMATELY DO NOT PAY THE PORTION OF THEIR BILL FOR WHICH THEY ARE FINANCIALLY RESPONSIBLE. THE DIFFERENCE BETWEEN THE DISCOUNTED RATES AND THE AMOUNTS COLLECTED AFTER ALL REASONABLE COLLECTION EFFORTS HAVE BEEN EXHAUSTED IS CHARGED AGAINST THE ALLOWANCE FOR DOUBTFUL ACCOUNTS.
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsTHE ORGANIZATION FILES ANNUALLY A COMMUNITY BENEFIT REPORT WITH THE STATE OF MARYLAND'S HEALTH SERVICES COST REVIEW COMMISSION (HSCRC). THE HSCRC, WHICH OPERATES UNDER A MEDICARE WAIVER, DOES NOT CONSIDER MEDICARE SHORTFALL AS COMMUNITY BENEFIT. THE COSTING METHODOLOGY USED BY THE ORGANIZATION IS A COST-TO-CHARGE RATIO.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistanceTHE ORGANIZATION EXPECTS PAYMENT AT THE TIME THE SERVICE IS PROVIDED. OUR POLICY IS TO COMPLY WITH ALL STATE AND FEDERAL LAW AND THIRD PARTY REGULATIONS AND TO PERFORM ALL CREDIT AND COLLECTION FUNCTIONS IN A DIGNIFIED AND RESPECTFUL MANNER. EMERGENCY SERVICES WILL BE PROVIDED TO ALL PATIENTS REGARDLESS OF ABILITY TO PAY. FINANCIAL ASSISTANCE IS AVAILABLE FOR PATIENTS BASED ON FINANCIAL NEED AS DEFINED IN THE FINANCIAL ASSISTANCE POLICY. THE ORGANIZATION DOES NOT DISCRIMINATE ON THE BASIS OF AGE, RACE, CREED, SEX OR ABILITY TO PAY. PATIENTS WHO ARE UNABLE TO PAY MAY REQUEST A FINANCIAL ASSISTANCE APPLICATION AT ANY TIME PRIOR TO SERVICE OR DURING THE BILLING AND COLLECTION PROCESS, EVEN IN EXCESS OF 240 DAYS FOLLOWING THE FIRST POST-DISCHARGE BILLING STATEMENT. THE ORGANIZATION MAY REQUEST THE PATIENT TO APPLY FOR MEDICAL ASSISTANCE PRIOR TO APPLYING FOR FINANCIAL ASSISTANCE. THE ACCOUNT WILL NOT BE FORWARDED FOR COLLECTION DURING THE MEDICAL ASSISTANCE APPLICATION PROCESS OR THE FINANCIAL ASSISTANCE APPLICATION PROCESS. NO EXTRAORDINARY COLLECTION ACTIONS (ECAS) WILL OCCUR EARLIER THAN 120 DAYS FROM SUBMISSION OF FIRST BILL TO THE PATIENT AND WILL BE PRECEDED BY NOTICE 30 DAYS PRIOR TO COMMENCEMENT OF THE ACTION. AVAILABILITY OF FINANCIAL ASSISTANCE WILL BE COMMUNICATED TO THE PATIENT AND A PRESUMPTIVE ELIGIBILITY REVIEW WILL OCCUR PRIOR TO ANY ACTION BEING TAKEN. IF A PATIENT IS DETERMINED TO BE ELIGIBLE FOR FINANCIAL ASSISTANCE AFTER AN ECA IS INITIATED, THE ORGANIZATION WILL TAKE REASONABLE MEASURES TO REVERSE THE ECAS AGAINST THE PATIENT ACCOUNT.

Schedule H, Part VI, Line 2 Needs assessmentBeginning in the summer of 2018, the University of Maryland St. Joseph Medical Center (UM St. Joseph) undertook a comprehensive community health needs assessment (CHNA) to evaluate the health needs of individuals living in Baltimore County, Maryland. The aim of the assessment is to reinforce St. Joseph's commitment to the health of residents and align its health prevention efforts with the community's greatest needs. The assessment examined a variety of health indicators including chronic health conditions, access to health care and social determinants of health. UM St. Joseph contracted with Holleran Consulting, a research firm based in Wrightsville, Pennsylvania, to execute this project. The completion of the CHNA enabled UM St. Joseph to take an in-depth look at its community. The findings from the assessment were utilized by UM St. Joseph to prioritize public health issues and develop a community health implementation plan focused on meeting community needs. UM St. Joseph is committed to the people it serves and the communities where they reside. Healthy communities lead to lower health care costs, robust community partnerships, and an overall enhanced quality of life. UM SJMC also has representatives that serve on many different coalitions charged with various health issues in the county including the Baltimore County Health Coalition, Smoke Free Baltimore County, and the Maryland Falls Coalition. This helps us to remain current on local trends and work collaboratively on initiatives with partners. Additionally, UM SJMC routinely surveys program participants to identify health concerns and ensure the relevance of our offerings.
Schedule H, Part VI, Line 3 Patient education of eligibility for assistanceOur financial assistance policy and the communication about our financial assistance policy is regularly reviewed to make sure it is available to our patients in a variety of formats and that it is available in culturally/linguistically sensitive manner and at a reading comprehensive level appropriate to the population of our Community Benefit Service Area. The availability of financial assistance for patients who would otherwise be billed for services about their eligibility for assistance under federal, state or local government programs is communicated to patients in multiple ways: At all our points of registration in the hospital (general registration, Emergency Department) and in our specialized service areas (Perinatal Center, Cancer Institute, etc.) large signs are posted informing the patient that if they face problems in paying for their care, they may apply for financial assistance. The phone number is posted for them to contact one of our financial counselors. When patients are registering in the hospital for inpatient treatment or outpatient treatment, they are given the Patient Financial Information Sheet that is printed on two sides in English and Spanish. This Patient Financial Information Sheet is available at every point of entrance to the hospital and every point of service delivery. It is also included in the patient information packet given to each patient as well as online. When patients are inpatients and do not have any health insurance, one of our financial counselors visits them in their room to discuss the availability of various government benefits such as Medicaid or state programs offering health care assistance and assist with applying. When patients receive outpatient services and do not have any health insurance, the financial counselor sends them information about their potential eligibility for various government benefits such as Medicaid or state programs offering health care assistance, and invites them to call (Spanish and English-speaking financial counselors are available) to discuss applying for these programs. When a patient applies for financial assistance, our bilingual financial assistance counselor works with the patient to gather appropriate documents and submit their application for financial assistance. Our Financial Assistance Policy is available in the following languages: English, Spanish, French, Russian, Chinese, Korean, Vietnamese, Tagalog.
Schedule H, Part VI, Line 4 Community informationThe Community Benefit Service Area for UM SJMC encompasses all of Baltimore County. Current health priorities such as obesity, cancer, substance abuse, and fall prevention extend across all communities in the area. The most recent Community Health Needs Assessment conducted by UM SJMC included all of Baltimore County. Within the county, there are more vulnerable populations where more targeted efforts occur. Zip codes with the highest utilization rates include: 21234, 21093, 21239, 21286, 21136, 21204. Zip codes with the greatest socioeconomic needs include: 21227, 21222, 21221, 21220, 21207, 21234, 21237, 21204, 21136, 21030 (CNI, 2018). Schedule H, Part VI, Line 5 Promotion of community healthUM St. Joseph is committed to serving as a health resource and promoting the well-being of the community. This is reflected in our collaborative efforts to address the following health needs: Access to Care Founded on a mission of providing care to the sick and the underserved, UM St. Joseph operates the St. Clare Medical Outreach program, a primary care clinic that serves a mainly Hispanic population with no access to health care. The staff of a medical director, nurse manager, physician assistant, registered nurses, medical assistants, health educator and office coordinator also provide immunizations, pharmaceuticals, health education, mental health counseling, and referrals to specialists that participate in the program. In FY19, the program took care of 4,099 visits and added 55 new patients to its roster. To help improve patients' health through physical activity, staff initiated a program called Sunny Cyclers/Winter Walkers which has about 30 participants. UM SJMC's longstanding free influenza immunization program is offered at the medical center and more than 20 vaccination clinics in the community every fall, providing 2,715 flu vaccinations in FY19. Due to unsatisfactory Baltimore County vaccination rates and unmet needs, we expanded offsite immunization clinics with extended hours and strategic locations. Many individuals cited copays and school and work schedules as barriers to access. Mental Health and Substance Abuse Among our behavioral health initiatives is a partnership with Baltimore County Department of Health to promote access to Peer Recovery Specialists. Peer recovery specialists provide 1:1 peer outreach and coaching as well as linkages to resources that include detox and other mental health services or facilities, assistance with supportive housing, transportation, medication, insurance, and any other ancillary support for clients to be successful. An ongoing system-wide initiative that further supports these efforts targeting mental health and substance abuse is the Not All Wounds Are Visible series which began in June 2017 to promote community conversations, awareness and resources around a wide range of related topics. In FY19 the UMMS hospitals coordinated two seminars, one focused on depression in men and the other on Adverse Childhood Experiences (ACEs). Each included professional speakers, local partners, and panel discussions to encourage dialogue between advocates and community members. The events were free and widely publicized. Attendees were invited to participate at the main location downtown or through a live stream. This series will continue in FY20 with more conversations surrounding suicide and ACEs interventions and resilience. UM St. Joseph Medical Center is also proud to support the efforts of Powered by ME!, a program that encourages positive decision making among the youth in our community. Chronic Disease- Cardiovascular Disease/Obesity Hypertension is a serious issue in Baltimore County, where heart disease and stroke rates are higher compared to the state of Maryland and the nation. According to the MD Behavioral Risk Factor Surveillance System, one-third of adults in the county suffer from high blood pressure. Twelve free Living Healthy with High Blood Pressure Workshops were held in multiple locations, including six Baltimore County senior centers, reaching 122 participants. Sixty people attended a free Annual Heart Health Event at UM SJMC on Valentine's Day, receiving blood pressure and cholesterol screenings, cholesterol body composition analysis, education, and resources. In FY 2019, we performed blood pressure screenings for 247 people with 92 referred for follow-up. With diabetes being a national health epidemic, UM SJMC adopted the CDC's Prevent T2 Program. Seven staff employees trained to become lifestyle coaches who work with participants at risk for type 2 diabetes to make lasting changes to improve their health. The first year-long group began in June with 12 individuals with prediabetes committed to losing 5-7% of their body weight and increasing physical activity to prevent or delay the onset of type 2 diabetes. Being part of a group enables participants to overcome obstacles together. Participants also learn to eat healthy, manage stress, and understand and respond to food cues. To encourage mothers who are interested and able, the Childbirth and Family Education department offers a free bimonthly breastfeeding class by a lactation specialist. Breast milk contains antibodies, which help fight illness. According to the CDC, studies reveal that breastfed babies have a lower chance of getting a variety of health problems, including asthma, obesity, diabetes, ear and respiratory infections, and SIDS. In FY19, there were 589 parents who attended the classes. We also offer Mothering Matters, a free networking group for new mothers and babies meeting weekly on Thursday mornings at Towson United Methodist Church. The Healthy Kids Running Series (HKRS) is a five-week national running program for youth from pre-K to 8th grade sponsored by UM SJMC-affiliated Towson Sports Medicine. Two series are offered, one in spring and one in fall. We partnered with Loch Raven High School to host HKRS. The goal is a positive, age-appropriate, educational and fun experience. The series attracts up to 200 youth, takes place once a week and features the 50-yard dash, 75-yard dash, 1/4 mile, 1/2 mile, and one-mile runs. Health benefits include better self-esteem, strength, sleep and concentration, and increased activity and prevention of obesity in young people. Cancer We performed these free cancer screenings: 38 skin screenings, 151 breast screenings, 27 prostate screenings, and 42 cervical screenings for community residents in addition to performing 39 colorectal cancer screenings for UM SJMC staff and family members. We have representation on the Baltimore County Tobacco Coalition to help expand smoking cessation and awareness efforts. In recent years, we partnered with About Faces Salon, giving skin cancer education presentations to their staff at four locations so they could provide education to their clients. Sun safety education and sunscreen were distributed at health fairs, camps and events. Fall Prevention According to the CDC, one-quarter of adults age 65 and over fall each year. Accompanying injuries and loss of quality of life are significant health concerns. Going into its fourth year at UM SJMC, Stepping On is an evidence-based fall prevention program to help seniors achieve better balance and increase overall strength, self-confidence, and sense of independence. The class features group strength and balance exercises, as well as education about medication management, home safety, proper footwear, vision, and mobility. We offered five Stepping On programs in FY19, including two at area senior living facilities. UM SJMC offers free yoga classes to adults of all ages and fitness levels, including anyone with physical limitations or chronic illnesses. Participants report improvements in concentration, balance, flexibility, strength, ability to relax and to breathe deeply. In FY 2019, we held 95 classes with 1,091 participants. Studies show that the health benefits of yoga range from relieving anxiety and tension to lowering blood pressure and improving fitness. Yoga also promotes strength, balance, pain relief, and relaxation. Two classes are offered a week: gentle, therapeutic yoga on Mondays, 4:30 - 5:30 pm, and a chair yoga class on Wednesdays, 12 noon - 12:30 pm. We also recently adopted the evidence-based program Tai Ji Quan: Moving for Better Balance, which will be offered at no charge onsite at the hospital as well as offsite at a local church.
Schedule H, Part VI, Line 6 Affiliated health care systemThe University of Maryland Medical System Corporation (UMMS) is a private, not-for-profit corporation providing comprehensive healthcare services through an integrated regional network of hospitals and related clinical enterprises. UMMS was created in 1984 when its founding hospital was privatized by the State of Maryland. Over its 30-year history, UMMS evolved into a multi-hospital system with academic, community and specialty service missions reaching primarily across Maryland. As part of the University of Maryland Medical System (UMMS), UM St. Joseph Medical Center understands that health care goes beyond the walls of the hospital and into the community it serves. UMMS hospitals are committed to strengthening their neighboring communities. In doing so, UM SJMC assesses the community's health needs, identifies key priorities, and responds with services, programs and initiatives which make a positive, sustained impact on the health of the community. With representation from all UMMS hospitals, the Medical System's Community Health Improvement Council coordinates the effective and efficient utilization and deployment of resources for community-based activities and evaluates how services and activities meet targeted community needs within defined geographic areas. UM SJMC is committed to health education, advocacy, community partnerships, and engaging programs which focus on health and wellness with the goal of eliminating health care disparities in the Baltimore community.
