ExplanationSCHEDULE H, PART V, SECTION B, LINE 2: On April 1, 2017 Winthrop-University Hospital (WUH) completed an affiliation agreement with NYU Langone Health System (NYUHS) and NYU Langone Hospitals, whereby the NYUHS became the sole corporate member of Winthrop-University Hospital. As a result of the acquisition, WUH changed its name to NYU Winthrop Hospital (NYU Winthrop). NYUHS is also the sole corporate member of NYU Langone Hospitals. The affiliation had two phases: in the first phase of the agreement NYU Winthrop Hospital (www.nyuwinthrop.org) remained a separate entity, responsible for its own liabilities and obligations. In the second phase of the affiliation, which occurred on August 1, 2019, Winthrop merged with and into NYU Langone Hospitals. SCHEDULE H, PART V, SECTION B, LINE 5: The broad interests of the community were addressed through participation in the collaborative assessment that was conducted by the Long Island Population Health Improvement Program, the grant initiative of the Long Island Health Collaborative (LIHC), which is an expansive membership of Long Island hospitals, the Nassau and Suffolk County Departments of Health, academic partners, community-based organizations, physicians, health plans, schools, libraries, local municipalities, and other community partners. This was the basis for the creation of NYU Winthrop's 2019 Community Health Needs Assessment. In April 2019, results of the collaborative Community Needs Assessment were shared with NYU Winthrop's Community Cultural Advisory Council, comprised of the following: EOC of Nassau County, Family & Children's Association, Girl Scouts of Nassau county, Hempstead Hispanic Civic Association, Hispanic Brotherhood of Rockville Centre, Hispanic Counseling Center, Long Island Asthma Coalition, Nassau BOCES, Nassau county Coordinator Agency for Spanish Americans, Nassau County Department of Health, North Shore Child and Family Guidance, Noticia, Project Independence, SHIP/AHEC, and St. Brigid's Church. SCHEDULE H, PART V, SECTION B, LINE 6A: The hospital's CHNA was developed from a collaborative effort initiated by the Long Island Health Collaborative (LIHC). The other hospitals involved include the following member hospitals of LIHC: Catholic Health Services of Long Island,(Mercy Medical Center, St. Francis Hospital, St. Joseph Hospital, Good Samaritan Hospital Medical Center, St. Catherine of Sienna Medical Center, St. Charles Hospital); Northwell Health System (Glen Cove Hospital, Long Island Jewish-Valley Stream, North Shore University Hospital, Plainview Hospital, South Oaks Hospital ,Syosset Hospital, Huntington Hospital, Mather Memorial Hospital, Peconic Bay Medical Center, Southside Hospital) ; Stony Brook Medicine (Stony Brook University Hospital, Southampton Hospital, Eastern L.I. ExplanationPART I, LINE 3c: NYU Winthrop Hospital uses FPG to determine eligibility for free and discounted care (eligibility up to 800% of FGP). PART II, LINE 7: The Hospital utilized a cost-to-charge ratio derived from Worksheet 2, Ratio of Patient Care Cost-to-Charges, of Form 990. PART 1, LINE 7f: Total bad debt expense of $27,985,466 is excluded from these calculations (see PART III, LINE 4 below). PART I, LINE 7g; There are no subsidized health service costs attributable to physician clinics in the amount reported. PART II; Through coalitions such as the Community Cultural Advisory Committee, NYU Winthrop Hospital joins with community based organizations and social agencies to collaborate and exchange information to help the underserved population in the community. PART III, LINE 2: The total bad debt expense shown on this line represents actual amounts written off during the year related to balances due on patient accounts for which payment was expected and pursued by the Hospital and/or its collection agencies but never received. PART III, LINE 3; The Hospital utilizes a process to identify amounts that are in a bad debt status but could qualify for financial assistance under the Hospital's financial assistance policy. These accounts are removed from bad debt and reviewed. If the criteria for financial assistance are met then they are reclassified from bad debt. These amounts are included in Part I of Schedule H. PART III, LINE 4: The Hospital presents the provision for bad debt expense, net totaling $27,985,466 for the year ended July 31, 2019 as a reduction of net patient service revenue in the consolidated statement of activities. PART III, LINE 8: Medicare cost and charge information was extracted from the Medicare cost report and an RCC methodology was applied. The shortfall represents unreimbursed costs incurred in providing services to Medicare recipients and therefore should be treated as a community benefit. PART III, LINE 9b: Patients identified as being eligible for consideration of financial assistance are referred by the debt collector to the Credit and Collection Department of the Hospital for review; Debt collection activities are suspended. 2) NEEDS ASSESSMENT: Community needs are based on assessments conducted by Federal, State, and County agencies as well as studies performed by the Hospital and community organizations. 3) PATIENT EDUCATION OF ELIGIBILITY FOR ASSISTANCE: Notification is provided on the Hospital's website, through signage posted at all access points, through notification on hospital bills, and through counseling during the intake and discharge processes. 4) COMMUNITY INFORMATION: NYU Winthrop Hospital serves primarily Nassau County, which is located on Long Island in New York. Nassau County is home to 1.3 million people and makes up approximately 7% of the State of New York's total population. Nassau is comprised of, 59.3% of White persons (not Hispanic), 17.2% persons of Hispanic or Latino origin, 13% Black persons, and 10.5% Asian persons based on census data. 5) PROMOTION OF COMMUNITY HEALTH: The Board of Directors of NYU Winthrop Hospital is comprised of community members who are not compensated for their duties. Winthrop extends medical staff privileges to all qualified physicians in its service area. The hospital provides an array of educational seminars and preventive health services to the community including a smoking cessation program, diabetes education, health fairs, free blood pressure screenings, fall prevention programs, cancer care programs, prostate screenings, genetic counseling, health updates for seniors, Meals on Wheels, and other programs. Surpluses are reinvested into Winthrop's operations, research, medical education and capital improvement initiatives to enhance its abilities to provide quality health care to its service areas. 6) AFFILIATED HEALTH CARE SYSTEM: The NYU Langone Health System (NYUHS) is the sole corporate member of NYU Winthrop Hospital. NYUHS is also the sole corporate member of the NYU Langone Hospitals. Both Hospitals provide services to their respective communities and collaborate to ensure that their communities are provided a full array of inpatient and outpatient services of the highest quality. 7) STATE FILING OF COMMUNITY BENEFIT REPORT: New YorkExplanationTxt

Form And Line ReferenceSCHEDULE J SUPPLEMENTAL INFORMATIONFormAndLineReferenceDesc
ExplanationSCHEDULE J, LINE 4B: The organization maintains a supplemental executive retirement plan (SERP) for their senior and long term executives of the organization, including Mr. Collins. The plan seeks to provide a replacement income at retirement equal to 60% of the employee's base salary, when considering all organization-sponsored retirement income sources, after 20 years of service. The benefits under this plan are forfeitable in the event of voluntary termination before retirement age. The organization's outside compensation consultant has opined that this plan is within market norms. NYU Winthrop adopted a supplemental retirement plan ("SERP II") for 3 executives who were not eligible under the SERP described above but who were negatively impacted by limits under NYU Winthrop's standard retirement plans (limits not applicable to employees generally). Two of the 3 executives are included in Part VII and Schedule J. The primary purposes of SERP II are to provide a supplement to the standard retirement plan and to encourage retention. SERP II covers Ms. Cataliotti and Ms. Pfeffer, and the 2018 calendar year contribution amounts for them were $113,877 and $82,020, respectively. SERP II is forfeitable by these executives if they terminate before December 31, 2021 (and additional service requirements may apply). The following directors participated in a supplemental non-qualified retirement plan ("SERP") during calendar year 2018. The amounts listed below represent the employer contributions to this plan for calendar year 2018. These amounts are reported as shared costs between NYU Langone Hospitals and NYU Grossman School of Medicine: Joseph Lhota - $174,731; Daniel J. Widawsky - $101,257. Part I, Line 7: The organization maintains an incentive compensation program that pays variable compensation based on the satisfaction by the organization and the executive, if and to the extent satisfied, of pre-established quality of care, program, financial, and similar goals established at the beginning of the year prior to payment. The incentive compensation program awards are made by the compensation committee at its discretion. Part II: David Halpern, M.D., Steven Carsons, M.D., and John Aloia, M.D. are employed physicians of the organization and also serve as uncompensated members of the Board of Directors.ExplanationTxt

MISSION STATEMENTFORM 990, PART III, LINE 1: It is the mission of NYU Winthrop to provide high-quality, safe, culturally competent, and comprehensive healthcare services in a teaching and research environment, which improve the health and well-being of the residents of Nassau County and contiguous county areas, based on a profound commitment to an enduring guiding principle - "Your Health Means Everything". The Hospital's Community Service Plan is guided by and reflects its mission statement - to improve the health and well-being of the residents it serves. FORM 990, PART III LINE 3: On April 1, 2017 Winthrop-University Hospital (WUH) completed an affiliation agreement with NYU Langone Health System (NYUHS) and NYU Langone Hospitals, whereby the NYUHS became the sole corporate member of Winthrop-University Hospital.
FORM 990, PART III, Line 4COMMUNITY PARTNERSHIPS - NYU Winthrop remains engaged with the community by maintaining relationships with community- and faith-based organizations, private organizations, local school districts, and legislators from the village, town, county, state and federal government. Our partnerships have been strengthened through quarterly Community Cultural Advisory Committee meetings addressing the needs of the underserved, as well as increased efforts to reach out to the community to achieve goals and objectives identified through the Community Health Needs Assessment. On a broader scale, the hospital is also an active member of the Long Island Health Collaborative (LIHC), a coalition that includes the Nassau and Suffolk County health departments, all Long Island hospitals, health and social service community-based organizations, health plans, academic institutions, local government, and other groups that are working together toward a healthier Long Island. By using the collective impact model in Population Health efforts, it is hoped that public awareness and education will help motivate individuals to make the changes necessary to develop a healthy lifestyle and improve outcomes. NYU Winthrop hospital participated in the collaborative assessment that was done by the Long Island Population Health Improvement Program, the grant initiative of LIHC.
HEALTH INITIATIVES SUPPORTING THE NYS PREVENTION AGENDAINTERVENTIONS: In June 2019 NYU Winthrop's Board of Directors approved its triennial Community Health Needs Assessment (CHNA). From this, a Community Service Plan was developed. Selection of targeted strategies is data driven, supported by research and discussion with community partners. Our primary focus areas include: 1) Prevention and Management of Chronic Disease, and 2) Promotion of Well-Being and prevention of Mental Health and Substance Abuse. Additionally, the Plan will include focus on 3) Promoting a Healthy and Safe Environment, 4) Birth-Related Disparities, 5) Improvement of Health Literacy, and 6) Healthy Foods The following explains initiatives taken to address the needs identified in NYU Winthrop's 2016 Community Health Needs assessment, many of which will continue in support of the updated CHNA: A) NATIONAL DIABETES PREVENTION PROGRAM - NYU Winthrop offered the evidence based National Diabetes Prevention Program at no cost at the Diabetes Education Center. Three classes began in 2017 and ended mid 2018:39 participants, 71% attendance rate, average weight loss 13 lbs. B) TAI CHI FOR ARTHRITIS - Targeting Chronic Disease, Stress Management & Fall Prevention NYU Winthrop offered the evidenced-based program Tai Chi for Arthritis & Fall Prevention Program at no cost. Medical studies have shown that Tai Chi relieves pain and improves the quality of life for people living with arthritis, though participants do not need to have arthritis to participate. The response to the program was tremendous, with community members calling to request classes. There were a total of 150 participants. Post satisfaction surveys indicate an increased feeling of wellness, improved energy, posture, and confidence. C) CHILDHOOD OBESITY SCREENINGS IN THE COMMUNITY - NYU Winthrop collaborates with the Head Start communities located in Hempstead and Westbury to bring the 5-2-1-0 healthy lifestyle program to their families and initiated this same program at its Hempstead Children's Health Clinic. The program encourages 5 servings of fruits and vegetables each day, no more than 2 hours of screen time, 1 hour of exercise and 0 sugary drinks. The interventional strategy includes documenting each child's BMI, giving participants a packet of the 5-2-1-0 guidelines, and offering nutritional counseling for families of children identified as having an unhealthy weight. Children were measured 10/17, 3/18, and 7/18. We captured the following statistics from the 99 children who attended all three sessions: 10/17 - 56.1% Normal, 18.35% High, and 24.45% Obese; 3/18 - 73.1% Normal, 17.8% High, and 17.8% Obese; 7/18 - 58.65% Normal, 23.35% High, and 23.5% Obese. Although we offered nutritional counseling in the form of a support group, parents did not respond. Head Start's nurse has been supportive of the program. Note: at the request of Head Start we expanded the program to Roosevelt and Freeport, two additional communities with health disparities. D) PROMOTE EXCLUSIVE BREASTFEEDING - Cited as a "Strategy to Prevent Obesity and Other Chronic Diseases" by the CDC, NYU Winthrop, designated "Baby Friendly" in 2014, educates new mothers about breastfeeding and provides supportive services to improve the percentage of newborns who only receive breast milk when discharged from the hospital. Our records show that, from Janaury 2017 through April 2019 an average of 41% of babies were only receiving breast milk when discharged from NYU Winthrop. To encourage this health initiative NYU Winthrop offers a free breastfeeding support group, led by a lactation consultant. E) INCREASE EFFORTS TO RAISE PARTICIPATION IN COLORECTAL CANCER & BREAST CANCER SCREENINGS - NYU Winthrop participated in the American Cancer Society's 80% by 2018 Colorectal Cancer Screening Initiative and in the NAPBC (National Accreditation Program for Breast Centers) Patient Navigation Initiative. Colorectal Cancer - Nurse Navigators contacted 18 community-based organizations to encourage participation among their clients; 2008 screening colonoscopies were completed; 60 positive findings. Breast Cancer - 991 women were contacted; .500 appointments scheduled; 397 completed mammograms (75% completion rate);' 20 abnormal findings; 8 cases receiving treatment. .Challenges for both initiatives are the same: overcoming barriers to screening, such as social challenges including transportation, lack of insurance, work and family responsibilities. Barriers continue to be addressed through patient navigation services.
HEALTH INITIATIVES (CONTINUED)F) FALL PREVENTION PROGRAM - NYU Winthrop's Fall Prevention Program for seniors includes a beginner four-part fall prevention workshop, followed by exercise classes to improve balance, flexibility and strength. NYU Winthrop performs balance assessments for individuals interested in taking a fall prevention class. Those who are deemed unfit to take classes are advised to check with their physician or physical therapist. These services were provided free of charge.During fiscal year 2019, 11 four part series were held at the Winthrop Welcome Center with a total of 126 attendees; 2 four part series were held off-site, with 56 attendees. A total of 198 balance assessments were held. Forty five exercise classes were held at the Welcome Center with 400 participants. Twenty six exercise classes were held in the community, with 608 participants. Participants fill in post-satisfaction forms. They acknowledge that the classes have given them more confidence and helped awareness of what to do to prevent a fall. In addition, they have made modifications at home, see an eye doctor once a year, and review their medications. G) TOBACCO CESSATION - The hospital's once per week, four week Tobacco Cessation Program was expended to a six week program, which includes a behavioral health component. It is facilitated by a social worker specializing in addiction and a nurse practitioner from Behavior Health, as well as a community health nurse. The program is free-of-charge. The results for 2018 are: 1) 42 individuals registered; 26 attended, 2) 10 quit smoking, 3) Post-program - 13 individuals responded to follow up phone calls; 77% of those who did not quit wish to try again and recommended the program; 39% who did not quit cut down on their smoking.
IMPROVING ACCESS TO CAREWOMEN'S HEALTH: NYU Winthrop's Women's Wellness Center in Hempstead addresses the health disparities of this select community by offering a wide variety of obstetrical and gynecological services. Free walk-in pregnancy testing is available Monday through Saturday. Services include on-site risk assessment, smoking cessation support, genetic counseling, nutritional counseling and healthy weight programs. High risk pregnancy patients may be seen by onsite maternal fetal specialist. Gestational diabetes education and monitoring are provided by the nurse practitioner. To meet the needs of the community, the site offered a Medicaid Enrollment Specialist for uninsured patients; Financial assistance and self-pay rates are available as well. The Center participates with NYS Medicaid and most Medicaid Managed Care HMO plans. The Financial Services Department meets with patients to facilitate Medicaid enrollment; this does not include those already enrolled in Medicaid. Additional services include: a) Psychosocial counseling, depression screening at each visit, post partum depression screening, bereavement counseling, education on domestic relationship abuse, assessment of living conditions, and cultural diversity; b) Breast feeding education performed throughout the pregnancy with focus on helping moms achieve optimal breastfeeding goals; c) Patients are counseled and educated on a variety of family planning options. Young moms are provided support by means of referral to home care and social work services coordinated through NYU Winthrop Hospital; d) GYN services include STD screening and education for prevention, education on many types of contraception options as well as vaccinations to prevent cervical cancer. The providers perform GYN procedures on-site, such as biopsy, colposcopy, and LEEP. Patients are provided referrals to NYU Winthrop specialists, e) For those in need of food, maternity and infant clothing, or infant necessities, WWW has a network of resources within the community that provide these items at no cost. WWW collaborated with the Nassau County Infant Car Seat program to provide education on the installation of car seats as well as adult and seat belt education. CHILDREN'S HEALTH SERVICES - A) The Hempstead High School Health Center (HHHC) is a school-based Health Center in a community with health disparities. Comprehensive preventive, episodic and confidential healthcare is provided. The center includes specific programs needed by the school's population, including: 1) Behavioral Health Initiatives that screen each patient at the center once a year for substance abuse and depression, utilizing the SBIRT Screening evidence based model (Screening, Brief Intervention, Referral to Treatment), and PHQ9 - a multipurpose instrument for screening, diagnosing, monitoring and measuring the severity of depression. Four staff members are also trained in NARCAN, 2 Youth Support Groups in English and Spanish that address common adolescent issues such as acculturation, adolescent adjustment, alternative lifestyles, teen pregnancy and prevention, parenting teens, and violence prevention, including gang violence, 3) an Asthma Management Program that monitors and tracks students with asthma. This effort is particularly significant in Hempstead, where asthma rates are nearly double those of adjacent neighborhoods. Both staff nurses in the HHHC are certified asthma educators. Collaboration with primary care physicians in the community facilitates treatment that meets current medical standards, trains high school nurses and staff about asthma, offers access to medications and assists with obtaining health insurance for uninsured students. There is a noticeable difference among students with asthma, in that fewer students are presenting at the clinic with difficulties, and 4) an Oral Health program that assesses dental needs and applies fluoride varnish. It also provides dental supplies: toothbrushes, paste, floss, and referral to local dental providers. B) The NYU Winthrop Hospital Pediatric Center, a Patient-Centered Medical Home since 2014, draws patients primarily from the Hempstead community but also serves children, from newborn to age 21, from the surrounding neighborhoods experiencing health disparities (Elmont, Freeport, Uniondale and Roosevelt). In fiscal year 2019, a total of 20,339 patients were treated at the facility. NYU Winthrop's Pediatric Specialty Center in Mineola sends a Pediatric Pulmonologist, an Endocrinologist, and a Genetic Counselor to the Hempstead location on a regular basis to allow for easier access to these services. The Behavioral Health Department currently has 2 social workers assigned to the Hempstead location and a Child Psychiatrist visits weekly to address the varied concerns of the patient population. The practice focuses on obesity. To reduce obesity, the Center utilizes the 5-2-1-0 program as a way of teaching parents and children a healthy lifestyle. The objective is to teach children and parents the basics of a healthy lifestyle; the intended goal is to achieve a long-term positive impact on their health. Providers are encouraged to consistently document Body Mass Index (BMI), provide lifestyle counseling, develop individual care plans and follow-up measures, and provide culturally relevant messaging to parents. 944 children within the practice were determined to be obese (22%); 34 children were morbidly obese; 100% of those who were obese received 5-2-1-0 counseling; those with a BMI of 95% or more received more specific dietary counseling and surveillance; an endocrinologist is now at the practice one or two Saturdays per month to provide easier access for patients - 164 patients were referred to an endocrinologist; compliance rate of families for those requested to return for 3 month follow-up visits for obesity - 44%. Statistics and quality monthly meetings review the impact of asthma education on the patient population. Regarding asthma, the Pediatric Center in Hempstead had the highest prevalence area of asthma on Long Island at 18% of the population. Health literacy, part of the 2019-2021 Community Service Plan, plays a major role in the management of chronic conditions. Asthmatic children with parents of low health literacy are at risk for worse asthma-related outcomes, as the written Asthma Action Plan (AAP) has a large amount of writing and can be complex. To address this, a pediatric pulmonologist spends 3 days per week, with one evening session, at the Center. Three full time nurses have become Certified Asthma Educators. A Pictorial Asthma Action Plan (PAAP) was developed in March 2018. By July 2018 the distributed picture plans had increased from 2% to 25%. The recall of medical instructions and steps to take when an exacerbation occurs was better understood when using the PAAP. when an exacerbation occurs was better understood when using the PAAP.
