Schedule D, Part X, Line 2 FIN 48 (ASC 740) footnoteThe Medical Center accounts for uncertain tax positions in accordance with ASC Topic 740-10, Income Taxes - Overall. ASC Subtopic 740-10 addresses the determination of how tax benefits claimed or expected to be claimed on a tax return should be recorded in the consolidated financial statements. Under ASC Subtopic 740-10, The Medical Center must recognize the tax benefit from an uncertain tax position only if it is more likely than not that the tax position will be sustained on examination by the taxing authorities based on the technical merits of the position. The tax benefits recognized in the consolidated financial statements from such a position are measured based on the largest benefit that has a greater than 50% likelihood of being realized upon ultimate settlement. ASC Subtopic 740-10 also provides guidance on derecognition, classification, interest and penalties on income taxes, and accounting in interim periods and requires increased disclosures. As of December 31, 2019 or 2018, The Medical Center does not have a liability for unrecognized tax benefits. The Medical Center is a not-for-profit corporation as described in Section 501(c)(3) of the Internal Revenue Code and is exempt from federal income taxes on related income pursuant to Section 501(a) of the Code.

THOSE FOUR PROCESSES ARE COMMUNITY HEALTH STATUS ASSESSMENT (CHSA): FORCES OF CHANGE ASSESSMENT (FOCA); LOCAL PUBLIC HEALTH SYSTEM ASSESSMENT (LPHSA); COMMUNITY THEMES & STRENGTHS ASSESSMENT (CTSA); THESE FOUR ASSESSMENTS WERE USED TO DEVELOP THE KANKAKEE COUNTY DATA SHARING/STRATEGIC PRIORITIES. FOR THIS MOST RECENT COMMUNITY HEALTH NEEDS ASSESSMENT, THIS GROUP BEGAN IN 2017 AND CONTINUED THROUGH 2018 TO DEVELOP A REFRESHED PLAN FOR 2019-2021. THE PARTNERSHIP FOR A HEALTHY COMMUNITY CONTRACTED WITH THE ILLINOIS PUBLIC HEALTH INSTITUTE TO FACILITATE THE ASSESSMENT AND DEVELOPMENT OF ACTION PLANS.
Schedule H, Part V, Section B, Line 11 Facility A, 1Facility A, 1 - RIVERSIDE MEDICAL CENTER. Introduction: The 2019-2021 Community Health Needs Assessment Plan was developed following the MAPP process over more than two years with a broad variety of community stakeholders. Riverside helped fund the assessments and participated as a leader on the steering committee and with strong expert representation across the various assessments. Riverside's website (www.riversidehealthcare.org) displays the full 2019-2021 Community Health Needs Assessment, the associated four assessments that informed the plan, and Riverside Medical Center's approved three-year Implementation Plan. Three Prioritized Strategic Areas The MAPP process used by the Partnership for a Healthy Community allowed the community partners to identify three areas of priority to improve residents' health. For reference, these are listed below in alphabetical order. I. Access to Care II. Behavioral Health III. Education and Employment Priority #1: Access to Care Prevention and Primary Care Riverside will increase access to care in the community by actively recruiting primary and specialty care providers to the 150-member Riverside Medical Group, educate and encourage preventative services for patients, and leverage technology through tools like MyChart and video visits to enhance patient/doctor communication and accessibility. Social Determinants: Consistent with the Democracy Collaborative's Anchor Mission Playbook, Riverside will continue to measure and improve its positive impact through local sourcing, local hiring, and place-based investing. Further, we will convene and collaborate with other similarly-minded, local anchor institutions to share best practices and encourage similar benefit to the community by others. Chronic Disease: As the region's leader in services treating the most prolific chronic diseases, Riverside will further support the area's largest multi-disciplinary cardiovascular team with a multi-million-dollar catheterization lab investment and continued expansion of our Heart Failure Clinic, with expanded community education, screenings, promotion, and accessibility of our Oncology Services, and with the ongoing commitment to stroke prevention and care from our neurologists and neurosurgeons. Also, Riverside will continue to lead the community in diabetes education, treatment, and prevention efforts and with programs that encourage healthy lifestyles and reduce obesity in children and adults. Priority #2: Behavioral Health Substance Use: Riverside will continue as a leader in securing grant funding for substance abuse initiatives. In collaboration with community partners, Riverside will establish education programs for the prevention of drug use and abuse across the lifespan and will improve access to community- based substance abuse services by initiating the provision of opioid treatment clinics and growing existing programs that assist residents and patients in reducing and eliminating dependence. Mental Health: Riverside is committed to making Mental Health services more accessible to the community. With the recent launch of a Centralized Intake process for all Behavioral Health Services, Riverside will provide screening, assessment, and referral services to the community linking residents with the appropriate type of treatment and level of care. Riverside will increase the availability of services through continued investment in the recruitment and retention of mental health providers and growth across the full continuum of high- quality inpatient and outpatient services for mental health. To address the increased demand for services caused by readmissions, non-compliance, readmissions, and medication problems, Riverside will employ a discharge planning and aftercare initiative to assist patients in maintaining functioning and sustaining after-treatment gains. Trauma Awareness and Prevention: Riverside will continue to provide trauma-informed care to children and adolescents and will extend this model to adult services. Riverside will invest in resources to enhance our ability to care for those who have experienced trauma while working collaboratively with local social services organizations to prevent future events. We will partner with community providers and first responders to address their staff education and training needs to improve trauma- sensitive interventions. We will build upon current services which include a SANE-trained nurse in the Emergency Department, active counseling services, and screening questions during well-check exams. Priority #3: Employment and Education Job Skill: As the region's largest employer, Riverside will invest in job skill training for employees via internal programs and through philanthropic and organizational scholarships for formal education and certifications. Employability: Riverside will continually align our orientation and job-training to improve the service level of all staff for the betterment of our patients and as an investment in the local workforce. Career Ladder: Riverside will work collaboratively with local K-12 education and higher education institutions to promote and coordinate practical, dynamic career ladders. As an example, Riverside launched a specific program with Kankakee Community College to advance current entry level employees to CNAs. Further, because of partnerships already in place, Riverside employees benefit from tuition discounts at many area colleges. Conclusion: In 2019, Riverside Healthcare supported the communities we serve with $53.5 million in total community benefit. Categories of support include: Subsidized health services, Uncompensated care, Government sponsored indigent care, Charity care, Volunteer services, Education, Donations, Language assistance services and other support. This 2019 update indicates those major activities and initiatives Riverside undertook which add to the activities, investments and initiatives of the previous two years, making up the three years of the CHNA plan term. Questions: Questions regarding any items related to Riverside's commitment to the Community's Health Needs can be directed to the Vice President of Institutional Advancement at 815-935-7502.

Schedule H, Part I, Line 7g Subsidized Health ServicesNET COMMUNITY BENEFIT EXPENSE FROM PHYSICIAN CLINICS WAS $17,242,989.
Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceTHE HOSPITAL USES A COST ACCOUNTING SYSTEM FOR ALL PATIENT SEGMENTS, INCLUDING INPATIENT, OUTPATIENT, EMERGENCY ROOM, PRIVATE INSURANCE, MEDICAID, MEDICARE, UNINSURED, OR SELF PAY. IN ARRIVING AT THE AMOUNTS REPORTED IN SCHEDULE H, PART I, LINE 7, THE HOSPITAL USED A COST TO CHARGE RATIO, DERIVED FROM WORKSHEET 2, RATIO OF PATIENT CARE COST-TO-CHARGES.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountBad debt expense is determined using the cost to charge ratio. Payments on patient accounts are a reduction of bad debt expense.
Schedule H, Part III, Line 3 Bad Debt Expense MethodologyNone of the amount the organization reported as bad debt was attributable to patients eligible under the organization's financial assistance policy.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteGenerally, patients who are covered by third-party payors are responsible for related deductibles and coinsurance, which vary in amount. The Medical Center also provides services to uninsured patients and offers those uninsured patients a discount, either by policy or law, from standard charges. The Medical Center estimates the transaction price for patients with deductibles and coinsurance and from those who are uninsured based on historical experience and current market conditions. In evaluating the collectibility of patients' accounts receivable, The Medical Center analyzes its past history and identifies trends for each of its major payor sources of revenue to estimate the variable consideration. Management regularly reviews data about these major payor sources of revenue in evaluating the variable consideration. The Medical Center uses a portfolio approach to account for categories of patient contracts as a collective group rather than recognizing revenue on an individual contract basis. The Medical Center believes that revenue recognized using this approach approximates the revenue that would be recognized if an individual contract approach were used. The initial estimate of the transaction price is determined by reducing the standard charge by any contractual adjustments, discounts, and implicit price concessions. Subsequent changes to the estimate of the transaction price are generally recorded as adjustments to patient service revenue in the period of the change.
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsTHE COSTING METHODOLOGY USED IS COST TO CHARGE RATIO. THE COST TO CHARGE RATIO IS FIGURED BY TAKING THE TOTAL EXPENSES EXCLUSIVE OF BAD DEBT AND DIVIDING THAT BY GROSS PATIENT REVENUE AND OTHER OPERATING REVENUE. ANY SHORTFALLS THAT RESULT FROM MEDICARE ALLOWABLE COSTS EXCEEDING MEDICARE REVENUE CAN BE CONSIDERED A COMMUNITY BENEFIT IF THE ACTIVITY IS SOMETHING THAT the MEDICAL CENTER IS DOING TO CREATE THE AWARENESS AND WELL-BEING OF THE HOSPITAL COMMUNITY.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistanceWhen a patient has been approved under the hospital's Charity Care Policy for a partial discount, the hospital will work with the patient or responsible party to establish a reasonable payment plan. The plan will take into account available income and assets, the amount of the discounted bill(s), and any prior payments. The hospital will also advise the patient of his or her responsibilities under these collection practices guidelines. Riverside may offer prompt payment discounts to charity care patients who are eligible for partial discounts on their hospital bills. The hospital will not pursue legal action for non-payment of bills against charity care patients who have clearly demonstrated that they have neither sufficient income nor assets to meet their financial obligations. Legal action, including the garnishment of wages, may be taken by the hospital to enforce the terms of the payment plan. Such measures would only take place when there is evidence that the charity care patient or responsible party has sufficient income and/or assets to meet his or her obligation. The hospital will not place a lien on a charity care patient's primary residence. The hospital will not execute a lien by forcing the sale or foreclosure of a charity care patient's primary residence to pay for an outstanding medical bill. The hospital will not use body attachment to require the charity care patient or responsible party to appear in court, however Riverside recognizes that the court system may take this action independently. The hospital will ensure that the guidelines outlined above are followed by any external collection agency engaged to assist in obtaining payment on outstanding bills from charity care patients. Additionally, external collection agencies will identify patients that may qualify for charity care and will assist in communicating Riverside's financial assistance policy and mail applications to patients requesting assistance.

Schedule H, Part VI, Line 2 Needs assessmentIn addition to conducting a Community Health Needs Assessment (CHNA) every three years, the organization continually assesses the needs of the communities it serves. Specifically, the organization assesses whether progress has been made on the specific areas identified in the Implementation Plan, as determined by the CHNA. Each year the organization revisits the needs identified in the CHNA and documents progress made towards addressing these needs. For more information regarding these assessments, please see Schedule H, Part V, Section B, Line 11.

Examples include, but are not limited to: Signage, Brochures, Placing a note on the hospital bill and statements regarding how to request information about financial assistance. This includes statements from our bad debt agency. Designated individuals who can explain the hospital's charity care policy, including a Spanish-speaking representative. Reference in our patient handbook Notice on in-house television Lobby Kiosks Website Employee Charity Lesson Riverside also screens identified patients for eligibility for various federal, state or local government programs. We use both internal and external staff to assist in the identification of patients and to assist with the education of our patients.

Four key indicators of well-being are assessed in each of Illinois' 102 counties: high school graduation rates, unemployment rates, teen birth rates, and poverty rates. Kankakee's Poverty Warning List placement reverts back to a Watch status. Sixty-five percent of the Medical Center's services are provided to the elderly and the poor-some of the area's most vulnerable citizens.
Schedule H, Part VI, Line 5 Promotion of community healthRiverside Medical Center is governed by a community board comprised of key community leaders who volunteer their time to assist in directing the organization.
