DescNewYork-Presbyterian/Queens (NYP/Q), located in the Borough of Queens in New York City, is a 535 bed licensed, not-for-profit medical center. NYP/Q has received a determination letter from the Internal Revenue Service that it is exempt from federal income taxes as an organization described in Section 501(c)(3) of the Internal Revenue Code. The Hospital is a voluntary, not-for-profit acute care community teaching hospital located in the Flushing section of Queens, New York. Presently, the Hospital provides medical and surgical services and has a certified bed capacity of 535. It was incorporated in 1974 as Booth Memorial Medical Center and changed its name to The New York Hospital Medical Center of Queens in 1992 in conjunction with its affiliation with The Society of the New York Hospital (now, The NewYork and Presbyterian Hospital). On July 1, 2015 NYP Community Programs, Inc. became the active parent of NewYork-Presbyterian/Queens and its subsidiaries. NYP Community Programs, Inc. (Community Programs) is the sole corporate member and active parent of Queens. The NewYork and Presbyterian Hospital (NYPH) is the sole member of Community Programs. NYPH is a major academic medical center that provides a full range of inpatient and outpatient services, mainly to residents of the New York metropolitan area. The Board of Trustees of NYPH consists of persons who have first been elected as members of NewYork-Presbyterian Foundation, Inc. (Foundation, Inc.). Community Programs, NYPH and Foundation, Inc. are each New York State not-for-profit corporations. Foundation, Inc. is related to a number of organizations. Services and Programs: The Hospital offers diagnostic and therapeutic services on an inpatient and outpatient basis in approximately 64 medical specialties and sub-specialties. NYP/Q has advanced capabilities in areas such as cardiac, general, and minimally invasive surgery. It receives tertiary inpatient referrals in the following service lines: Cardiovascular diseases, orthopedics, digestive diseases, Neuroscience, Obstetrics & Gynecology, Cancer and Kidney diseases. It operates a Level III Neonatal Intensive Care Unit and a Level I Trauma Center (both designed to handle the most severe cases). The Hospital Center provides the following programs as Centers of Excellence: The Neuroscience Institute, Cancer Center, Breast Center, Eye Center, Pediatric Asthma Center, and Obesity Surgery. NYP/Q has been designated by the New York State Department of Health ("DOH") as an AIDS Designated Center. The Ambulatory Care division provides adult and pediatric primary/preventive care at 20 sites throughout Queens County. Cardiac Services: The Hospital offers a variety of cardiac services including diagnostic and interventional cardiology, cardiac surgery, congestive heart failure and rehabilitation centers and cardiac research, education and prevention programs. The Cardiac Catheterization Laboratory at NYP/Q is one of only two interventional cardiology programs in Queens County, New York. The three catheterization laboratories at NYP/Q performed a combined 1,324 procedures in 2018. Now, physicians in the Electrophysiology Laboratory can implant new MRI-safe pacemakers for our patients and do atrial fibrillation ablation procedures. NYP/Q, in collaboration with The NewYork and Presbyterian Hospital and Weill Cornell Medicine, operates a cardiac surgery program at the NYP/Q campus. The Cardiac Health Center directs the Hospitals education and prevention programs. Lectures and special events are regularly presented to the general public at the site and at community centers and health fairs throughout Queens County. The Cardiac Health Center provides primary and secondary cardiac rehabilitation programs, including nutrition services, monitored fitness exercise, and psycho-social counseling. Surgical Services: In 2018, NYP/Q performed 15,092 inpatient and ambulatory surgical cases. These operations ranged from minimally invasive surgical procedures such as laparoscopic cholecystectomy, robotic procedures and arthroscopic orthopedics to major open procedures such as aortic aneurysm repair and ablative and reconstructive surgery. NYP/Q's Department of Surgery has encouraged use of inpatient and outpatient minimally invasive surgery, which reduces hospital stays and recovery times. Primary care and community outreach: NYP/Q places particular emphasis on community health programs and outreach initiatives. The Hospital team reaches out to Queenss residents to support health education and awareness - offering free health fairs, health lectures, screening, support groups and blood drives. A current calendar of events can be found at https://www.nyp.org/queens on the website home page. This commitment is expressed in its continued strategic expansion of community-based sites to enhance access throughout the Hospitals service area, and includes: The Trude Weishaupt Memorial Satellite Dialysis Center is a CMS Five Star facility (the "Dialysis Center") The Dialysis Center provides hemodialysis, chronic ambulatory peritoneal dialysis, and patient and family education. NYP/Q's renal hypertension division also provides outpatient ambulatory consultative services including the diagnosis and prevention of kidney stones, hypertension treatment and evaluation, and the management of kidney diseases. The Dialysis Center provided 40,910 treatments in 2018. Medical and dental staff: As of December 31, 2018, there were approximately 1,712 physician and dentist members of the medical and dental staff of the Hospital holding appointments in two categories: attending staff and emeritus staff. The medical and dental staff is organized into thirteen clinical departments: anesthesiology, cardiothoracic surgery, emergency medicine, medicine, obstetrics and gynecology, neurology, orthopedics and rehabilitation, pathology, pediatrics, radiation oncology, radiology, urology and surgery. The clinical departments include approximately 15 major services, subdivided further into 78 medical specialties and sub-specialties. The Hospital serves as tertiary care medical center that provides Graduate Medical Education for residency programs in Medicine, Surgery, Emergency medicine and Dental Medicine. Medical Center also offers fellowship training in Cardiology, GI, Pulmonary and Critical Care, Palliative Care and Nephrology. In addition, the Hospital participates in training residents in obstetrics/gynecology, orthopedics, pediatrics, through rotating residencies affiliated with various facilities including NYPH (Cornell Medical Center), and The Hospital for Special Surgery. For The Year Ending December 31, 2018, NYP/Q reported the following statistics: Patient Discharges 35,189 Deliveries 3,759 Average Length of Stay 4.83 Private Ambulatory Laboratory 117,208 Private Ambulatory Radiology 34,930 Surgical Procedures 15,092 Cardiac Catheterizations 1,324 Ambulatory Surgery/Endoscopy 16,125 Emergency Encounters (Incl. Admits) 127,702 Ambulatory Care Visits 148,358 Full-Time Employees 4,256 The Hospital provides quality medical care regardless of race, creed, sex, sexual orientation, national origin, handicap, age, or ability to pay. Although reimbursement for services rendered is critical to the operations and stability of the Hospital, it is recognized that not all individuals possess the ability to pay for essential medical services and, furthermore, the Hospitals mission is to serve the community with respect to health care and health care education. Therefore, in keeping with the Hospitals commitment to serve members of the community, the Hospital provides the following: charity care to the indigent; care to persons covered by governmental programs at below cost; and health care activities and programs to support the community. These activities include wellness programs, community education programs, health screenings, and a broad variety of community support services.Desc

PART V, LINE 5NYP/QUEENS IS COMMITTED TO IMPROVING THE HEALTH OF THE COMMUNITY IT SERVES. NYP/QUEENS HAS MULTIPLE FORMAL COMMITTEES, SUCH AS THE COMMUNITY ADVISORY COUNCIL. THE NYP/QUEENS COMMUNITY ADVISORY COUNCIL (CAC) PROVIDES THE OPPORTUNITY FOR COMMUNITY LEADERS AND RESIDENTS TO DIRECTLY ENGAGE HOSPITAL SENIOR LEADERSHIP. THE FOCUS OF THE COUNCIL IS TO ENHANCE COMMUNICATION AND COLLABORATION BETWEEN THE HOSPITAL AND DIVERSE SECTORS OF THE COMMUNITY. THE CAC IS COMPRISED OF 20 COMMUNITY LEADERS AND RESIDENTS WHO MEET WITH SENIOR HOSPITAL LEADERSHIP FIVE TIMES A YEAR TO DISCUSS NEW PROGRAMS/SERVICES, AND ADDRESS RELEVANT HEALTH CARE ISSUES IMPACTING PATIENTS AND THE COMMUNITY. IN ADDITION TO THE COMMUNITY NEEDS ADDRESSED IN THE COMMUNITY SERVICE PLAN, NYP/QUEENS HAS SEVERAL ONGOING COMMUNITY PROGRAMS THAT HELP FOSTER COMMUNITY ENGAGEMENT AND ADDRESS GAPS IN CARE THAT ARE HIGHLY NEEDED.
PART V, LINE 7a & 10Ahttps://www.nyp.org/documents/queens/about-us/NYPQ-CSP-2016_submitted.pdf Part V, Line 11 The NYP/Queens 2016-2018 Community Service Plan is based on data collected from the NYP/Queens Delivery System Reform Incentive Payment (DSRIP) Programs Community Health Needs Assessment (CHNA), the NYC Department of Health and Mental Hygienes (NYCDOHMH) Community Health Profiles, and the NYCDOHMHs Epiquery Database. An analysis of this data along with feedback from a wide range of community stakeholders, including Community Health Initiatives and DSRIPs community partners, and the hospital Community AdvisoryCouncil, resulted in the selection of the following two new priority areas: Priority #1: Increase access to high-quality chronic disease preventative care and management in both clinical and community settings, with a focus on increasing screening rates for hypertension and heart disease. Priority #2: Prevent HIV and STDs, with a focus on increasing screening rates for Hepatitis C (HCV). In the community: Based on the Community Health Needs Assessment, heart disease is one of the two leading causes of death in Queens County. NewYork-Presbyterian Queens understands the importance the role of education plays in maintaining optimal health, and thus strives to provide community education through its Community Health Initiatives (CHI) department. CHI hosts or participates in more than 150 community events to provide health education and screening to community members in Queens. In order to address health disparities, many of the events are held in ethnic and culturally diverse communities. In 2018, Internists and Internal Medicine residents of the Ambulatory Care clinics convened eight community events where they provided hypertension education and blood pressure screenings to 313 Queenss residents. All the screening participants receive on-site education about heart disease, understanding importance of diet, medication management, and appropriate recommendations for follow-up care. Combining these events with the residents is key in ensuring that the internists entering the work force are aware of community needs and health disparities. At primary care clinics: Elevated blood pressure for an extended period of time, or undiagnosed hypertension increases the work to the circulatory system, while decreasing its efficiencies. As a result, high blood pressure puts patients at risk for the development of life changing and potentially life threatening conditions. In 2018, 5,165 adult patients who were seen at the NYP/Queens primary care clinics (Fresh Meadows and Jackson Heights) received blood pressure screening at their visits, along with an agreed upon, individualized plan of care. In collaboration with the NewYork-Presbyterain Queens PPS, the treatment protocol for hypertension management, and management of undiagnosed elevated blood pressure was implemented at the two primary care clinics and adopted by all NYPQ Performing Provider Systems (PPS) members. The protocol includes screening, medication management, self-management goals, and follow-up recommendations. Patients who are at risk of hypertension are identified according to the following criteria: Stage 1 =2 elevated BP readings (=140 SBP or =90 DBP) at two separate medical visits, past 12 months A Stage 2 reading (=160 SBP or =100 DBP) at any medical visit in the past 12 months Category #1: Satge 1 141 patients had 2 readings in the past 12 months with SBP over 140 and DBP over 90 Category #2: Stage 2 2590 patients had 1 reading in the past 12 months with SBP over 160 and DBP over 100 The care coordinator at the clinic calls the patients to to schedule a follow-up appointment. 110 of the 141 patients and 14 out of the 25 patients completed their follow-up appointments Priority #2: Prevent HIV, STDs, Vaccine Preventable Diseases, and Health-Care Associated Infections by increasing HCV testing at ED and primary care clinics From April 2016 to Oct. 2017, a NYP/Queens viral testing task force designed and implemented routinized workflow at the emergency department (ED) and two primary care centers to identify eligible patients to receive Hepatitis C virus (HCV) test. Following the testing, the patient navigator conducted follow-up calls to patients with positive results, and assisted them with scheduling an appointment with NYP/Queens specialists. Under this grant funded project, which was completed in Oct. 2017, nine patients with active HCV infection were connected to care at NYP/Queens, community clinics, or community-based primary care physicians. In 2018, CHI has been actively partnering with several organizations in the LGBTQ community to promote HepC screening, as this population is most at-risk for HIV/AIDS and Hepatitis C infection, but have less access to the health care system and can easily be lost due to lack of follow-up care. Co-occurrence of HIV/AIDS and Hepatitis increases the risk of death from liver and non-liver related diseases. In 2018, CHI participated in 5 community events organized by Queens Community House, Queens Center for Gay Seniors, Queens Pride and Pride Lion Club to distribute hepatitis C education materials to thousands of LGBTQ community members. At events, CHI staff encouraged event participants to receive HepC screening and offered assistances and resources if needed.

This Policy places requirements upon Hospital and those agencies and attorneys undertaking debt collection activities that are consistent with the core mission, values, and principles of Hospital including, but not limited to, Hospitals Charity Care Policy. This policy applies to hospital and any agency, lawyer, or law firm assisting hospital in the collection of an outstanding patient account debt. PROCEDURE: A. General guidelines: 1. Hospital, collection agencies (Agency), and lawyers and law firms (Outside Counsel) will comply with all applicable federal and state laws and accrediting agency requirements governing the collection of debts including, but not limited to, the Fair Debt Collection Practices Act (FDCPA), the Fair Credit Billing Act, the Consumer Credit Protection Acts, Public Health Law Section 2807-k-9-a, Internal Revenue Service Code 501(r), Article 52 of the New York Civil Practice Law and Rules, and the Health Insurance Portability and Accountability Act (HIPAA). Hospital, Agency and Outside Counsel will also comply with Hospitals Charity Care Policy. To the extent that there are any inconsistencies between Hospitals Collection Policy and Charity Care Policy, the Charity Care Policy shall supersede and control. 2. Hospital shall enter legally binding written agreements with any parties (including Agency or Outside Counsel) to which it refers an individuals debt related to care that are reasonably designed to prevent Extraordinary Collection Actions (ECAs) from being taken to obtain payment for the care, until reasonable efforts have been made to determine whether the individual is eligible for Charity Care.
Part VI, Line 2:NYP/Queens identified two Prevention Agenda Priorities as well as disparities in both priority areas. The plan of action will be working towards addressing these disparities in both prevention priorities. The selection was based on the data, input, and feedback from both internal and external stakeholders. The quantitative research indicates that a number of chronic diseases are prevalent in the NYP/Queens service area. In addition, the data points to health disparities including the increasing hepatitis rates in the Asian community. In consideration of the aforementioned quantitative and qualitative data, NYP/Queens has chosen the following two NYS Prevention Agenda Priority areas: 1. Prevent Chronic Diseases 2. Prevent HIV, STDs, Vaccine Preventable Diseases and Healthcare-Associated Infections Priority #1 - Chronic Disease: High blood pressure (hypertension) directly increases the risk of coronary heart disease and stroke. In Queens County, heart disease is a leading cause of death. A CHNA focus group discussed the physical health issues of the community and found that living in New York, chronic diseases are caused by a multitude of environmental stresses. The prevalence of chronic disease in the community within NYP/Queens service area is higher than NY State (NYS). This was a significant source of concern cited by both the key informant interview and the focus group regarding health conditions that affect people in the communities where they live and work. The groups highlighted, among other risk factors: hypertension and heart disease, and HIV were of critical concern in their community. NYP/Queens is committed to addressing the concern of hypertension and heart disease by aligning with the NYP/Queens DSRIP Performance Provider Site (PPS) Cardiovascular Disease Project, which provides hypertension screenings as prescribed by the U.S. Preventative Service Task Force (USPSTF). The USPSTF recommendations are preventive, evidence-based guidelines about clinical services such as screenings, counseling services, and preventive medications. Additionally, the NYP/Queens Community Health Initiatives Group has committed to provide resources for hypertension screening at several community health events throughout the year. NYP/Queens is also developing a relationship with the Queens Public Library to host permanent hypertension screening booths. NYP/Queens will monitor the success of these initiatives by tracking the number of screening events, the number of patients screened, and the number of patients connected to care. In order to mitigate socioeconomic and linguistic barriers as well as limitations to access of care, NYP/Queens will host free screenings in venues that are in easily accessible to the community. 2017, updated information for Priority #1 - Prevent Chronic Diseases In the community: Every year, NYP/Queens Community Health Infinitives (CHI) hosts or participates in more than 150 community events to provide health education and screening to all community members in Queens. Most of the events were held in ethnic and culturally diverse communities. In 2017, medical residents and attending physicians of the primary care clinics were invited to participate in six community events where they provided hypertension education, blood pressure screenings and BMI measurements to 502 Queenss residents. All the screening participants received on- site education about hypertension and appropriate recommendations for follow-up care. At Primary Care Clinics: 5,653 adult patients who were seen at the NYP/Queens primary care clinics (Fresh Meadows and Jackson Heights) received blood pressure screening at their visits and patients' systolic blood pressure (SBP) and diastolic blood pressure (DBP) were recorded and analyzed. The treatment protocol for hypertension management was implemented at two primary care clinics and adopted by all NYP/Queens Performing Provider Systems (PPS) members. The protocol includes screening, medication, self-management goals, and follow-up recommendations. Clinics contact patients to ensure they are under appropriate medical care and complying with their blood pressure management regimen. 2018, updated information for Priority #1 - Prevent Chronic Diseases In 2018, Internists and Internal Medicine residents of the Ambulatory Care clinics convened eight community events where they provided hypertension education and blood pressure screenings to 313 Queenss residents. All the screening participants receive on-site education about heart disease, understanding importance of diet, medication management, and appropriate recommendations for follow-up care. At primary care clinics: 5,165 adult patients who were seen at the NYP/Queens primary care clinics (Fresh Meadows and Jackson Heights) received blood pressure screening at their visits, along with an agreed upon, individualized plan of care. Priority #2 - HIV and STDs: HIV is a growing concern for the community. It is often a comorbity associated with Hepatitis C (HCV). Early detection and proper follow-up care can dramatically reduce the mortality of patients with HCV and HIV. Without the availability of screening programs and linkages to care, patient s with HCV and HIV commonly go untreated until they end up in the NYP/Queens emergency room with end-stage liver disease. NYP/Queens started a new initiative: HIV, HBV and HCV Testing Model in an Urban Hospital Setting, which screens individuals for Hepatitis B virus (HBV), HCV and HIV so people living with any of these diseases can become aware of their status and linked to care. The program offers testing for HBV, HCV and HIV to all eligible individuals, 21 years or older, at NYP/Queens Emergency Department and the outpatient Ambulatory Care Center. Patients who test positive will be linked to a primary care physician or the appropriate medical specialists for follow-up care. It is anticipated that patients screened for HIV, HBC, and HCV in the Emergency Department and Ambulatory Care Center will reflect the ethnic and sub-ethnic communities served by NYP/Queens. For instance, chronic HBV has steadily increased among foreign-born populations in Queens, particularly immigrants from China. The NYP/Queens Triple Viral Taskforce will track the number of patients screened for HCV, HBV, and HIV and the number of patients connected to care, e.g. NYP/Queens and local primary care physicians and local clinics. The information will be shared with the NYCDOHMH Hepatitis C Coalition members, who will utilize NYP/Queens data for best practices and dissemination to other sites across NYC. Instance, chronic HBV has steadily increased among foreign-born populations in Queens, particularly immigrants from China. The information will be shared with the NYCDOHMH Hepatitis C Coalition members, who will utilize NYP/Queens data for best practices and dissemination to other sites across NYC. 2017, updated information for Priority #2 - Prevent HIV, STDs, Vaccine Preventable Diseases and Healthcare-Associated Infections. In April 2016, a NYP/Queens Viral testing task force designed and implemented routinized workflow for all eligible patients presenting to the emergency department (ED) and two primary care centers to receive Hepatitis C virus (HCV) test. HCV lab orders have been auto-populated into the hospitals electronic medical record (EMR) as a hard stop. This prompt requires the health provider to offer HCV test to the patients at the ED. This has been found to dramatically increase the rate of screening at the ED. Following the testing, the patient navigator conducted follow-up calls to patients with positive results, and assisted them with scheduling an appointment with NYP/Queens specialists. The navigator also provides and connects patients with resources in the community. By the end of October 2017, nine patients with active HCV infection were connected to care at NYP/Queens, community clinics, or community-based primary care physicians. 2018, updated information for Priority #2 - Prevent HIV, STDs, Vaccine Preventable Diseases and Healthcare-Associated Infections.
Part VI, Line 3:Written materials, including the application, full Policy, and plain language summary (Summary), shall be available to patients in the Hospital's primary languages, upon request and without charge, from Admitting and Emergency Departments at the Hospital during the intake and registration process, at discharge and/or by mail. Additionally, those materials shall be available on the Hospital's website (www.NYHQ.org). Also, notification to patients regarding this Policy shall be made through conspicuous posting of languageappropriate information in Emergency Rooms and Admitting Departments of the Hospital, and inclusion of information on bills and statements sent to patients explaining that financial aid may be available to qualified patients and how to obtain further information.
Part VI Line 4:Community Information: The service area is derived using hospital patient origin; specifically zip codes that represent ~75% of outpatient clinic patients in the most recent year. Queens County is the largest of the five boroughs of New York City, with a population of approximately 2.31M people. It is often referred to as the most ethnically diverse urban area in the world. The NYP/Queens service area comprises 33 of 52 Queens County zip codes. The service area currently has 1.59M people. Assuming the expansion of NYP/Queens service area is consistent with the 0.5% growth rate of Queens County, the number of residents living in the service area will increase to 1.66M by 2020. Race and Ethnicity: Queens County and the NYP/Queens service area cover the two most diverse geographies in NYC. racial or ethnic minority groups account for up to 30% of Queens County and service area population. Having a significant population base of all major races and ethnicities impacts the provision of health services in the following ways: cultural issues, language and health literacy, disease prevalence, and disease manifestation. Language: The languages spoken at home by NYP/Queens patients mirrors the racial and ethnic diversity represented throughout the NYP/Queens service area. 38% of the service area population primarily speaks English, compared with 43.4% in the rest of Queens County, 51.4% of New York City, and 70.0% of New York State. The NYP/Queens catchment area has a much higher rate of Asian, Indo-European and Spanish speakers than does the city or state. Employment and Income: There are 80k unemployed persons in the area, for a rate of 9.6%. This rate is higher than NYS, but lower than NYC average. Queens County median income of approximately $62k is higher than the NYS median of $56k, and approximately equal to the New York City median of $63k. The NYP/Queens service area; however, has a much lower median income than Queens County, at $58k. The percentage of NYP/Queens service area households with a low income (less than $35k per household) is 44.1%. This rate is higher than the Queens County and NYS rates of 41.5% and 32.9%, respectively. Conversely, the percentage of households with high income is lower in the service area than in the County or State. Poverty: Poverty is an issue of concern for this population particularly the disparity in poverty rates across the service area. Some portions of the service area see as much as one fourth of the population living in poverty. There is great geographic disparity in the percent of families living in poverty. Queens County has a lower percentage of families in poverty (14.4%) than Bronx, Kings and New York Counties, but higher than Richmond. Within the service area alone, the percentage of families living in poverty ranges from 4.6% in Whitestone to 23.8% in one zip code within Jamaica. Medicaid Beneficiaries: There are approximately 916,000 Medicaid beneficiaries in Queens County, and approximately 692,000 Medicaid beneficiaries in the NYP/Queens service area. These numbers translate to 41% of the population in Queens County and 43% in the service area. The percent of population enrolled in Medicaid ranges from 18% in Whitestone to 69% in Flushing (which is, notably, NYP/Queens zip code). The service area appears to have two areas of high Medicaid concentration, separated by an area of low Medicaid population. Corona, Elmhurst and one of the two Flushing zip codes each have more than 50k Medicaid beneficiaries. The Jamaica zip codes have a high percentage of individuals receiving Managed Care Medicaid or Medicaid HMO. Uninsured: Estimates of the uninsured rate ranges from 9.9% in Richmond County to 24.2% in NYS, with Queens County at 19.2%. While lower than the State, Queens County has the highest uninsured rate of any NYC County, at 19.2% or 431k persons. Assuming NYP/Queens service area uninsured rate is similar to Queens County rate, the service area has approximately 306k uninsured. In aggregate, there are 1.9M persons in the service area who are either Medicaid beneficiaries or uninsured. The uninsured rate for Queens County is estimated at 19.2% with a total of 430,385 individuals compared to a NYS estimate of 24.2%, totaling 4.6M individuals.
Part VI: Line 5Promotion of Community Health: NYP/Queens is committed to improving the health of the community it serves. Each of the community health improvement focus areas will have a task force which will address the operational concerns required to successfully implement the strategies noted. These workgroups will be charged with tracking and monitoring the interventions committed. Their data submissions will be used to provide annual updates. Additionally, NYP/Queens has multiple formal committees, such as the Community Advisory Council. The NYP Queens Community Advisory Council (CAC) provides the opportunity for community leaders and residents to directly engage Hospital senior leadership. In addition to the community needs addressed in this years community service plan, NYP/Queens has several ongoing community programs that help foster community engagement and address gaps in care that are highly needed. Many of these activities and groups directly support components of the NYS Prevention Agenda, Take Care New York, and the US Healthy People 2020 goals.
Part VI Line 6:Affiliate health care system: NYP/Queens is part of the NewYork-Presbyterian Regional Hospital Network. NewYork-Presbyterian Hospital assists its regional hospitals identify available resources and coordinate efforts to promote community health in the community the regional hospital services.
Part VI, Line 7The community benefit report is filed in New York State.

PART VII & SCHEDULE J, SUPPLEMENTAL INFORMATION:A Highest Compensated Employee of the New York and Presbyterian Hospital that is identified in part VII as an officer and Director of the NewYork-Presbyterian/Queens is responsible for executing the mission and management of the New York and Presbyterian Hospital (NYP) and its affiliated entities. Compensation for 2018 of the upper level executive includes the payout of an annual incentive plan and a long-term incentive plan. This performance-oriented program conditions payments upon the achievement of multiple individual and group performance measures. Measures to monitor performance include: operational and financial strength, patient quality and safety, patient satisfaction, advancement of patient care, and people development and partnership. Incentive awards may only be granted if the organization achieves a financial surplus. Even if all relevant performance measurements are achieved, the NYP Board of Trustees retains full discretion to make or not make any incentive awards, or to reduce the amount of any incentive award. This initiative is critical to assuring that NYP has the requisite leadership to create and manage a highly motivated and engaged workforce, to drive superior performance throughout the organization and to achieve top tier medical center status. As a separate matter, due to restrictions imposed by the Internal Revenue Code, upper level executives are limited in the amount of benefits received under a tax-qualified retirement plan. Like many employers, NYP supplements these executives' pension benefits through a supplemental ("nonqualified") retirement plan. The supplemental executive retirement plan (SERP) is subject to a multi-year vesting requirement (commencing after five years of participation in the SERP, in prorated amounts through age 65) which places an executive's supplemental retirement benefit at risk of forfeiture if the vesting requirements are not satisfied. Once vested, however, provisions of the Internal Revenue Code require that the vested executive include in current income the value of his or her vested supplemental retirement benefit. Notwithstanding the legal requirement to recognize the vested value of the supplemental retirement benefit as current income, the supplemental retirement benefit will not be distributed to the executive until the executive actually retires from NYP (although, as permitted by the Internal Revenue Code, the supplemental retirement plan will effect a distribution of an amount necessary to satisfy the executive's tax liability resulting from the income recognition upon vesting). As noted, this supplemental retirement benefit will not be distributed to the executive until the executive actually retires from NYP. There are constantly changing legal, tax, accounting, and public disclosure rules for a SERP (supplemental executive retirement plan) in not-for-profit organizations. The executive Compensation Committee continuously monitors these changes and incorporates any changes into the overall SERP plan design. As in past years, the executive Compensation Committee of NYP requires a third party complete a review of the organization's compensation program to ensure its effectiveness in terms of government regulations, market conditions and the need to continually elevate organizational performance. The report also serves to meet the regulatory obligations to ensure that all elements of the executive compensation programs are reasonable. The individual listed in part VII that is compensated by New York Presbyterian Hospital devote an average of sixty hours per week to perform their responsibilities for the New York and Presbyterian Hospital and other related organizations in the aggregate.
Schedule J, Line 3:Compensation decisions for the President was determined by a related organization following that organization's compensation policy.
