Part v, Line 5Since 2016, NewYork-Presbyterian Hudson Valley Hospital worked as a member of the Westchester County Health and Hospital Planning Team (WCHHPT) under the guidance of the Westchester County Department of Health to plan and execute a county-wide Community Health Needs Assessment survey, and to collaborate on the identification of priority areas and planned health interventions for 2016-2018. Members of the WCHHPT included the Department of Health and representatives from 12 hospitals and healthcare providers who met seven times throughout the year. As a result of this collaboration, survey results, and internal planning based on our hospitals strengths, resources and local community needs, NewYork-Presbyterian Hudson Valley Hospital developed its 2016-2018 Community Service Plan. In our efforts to meet the Agendas requirements, the Westchester County Health & Hospital Planning Team (WCHHPT), including NewYork-Presbyterian Hudson Valley Hospital, conducted a Community Health Needs Assessment Survey to sample residents and providers in the County and each hospitals or agencies service areas to determine which Prevention Agenda initiative(s) it plans to address during this three-year period. Surveys yielded 1,318 responses and 326 were from within the subset of NewYork-Presbyterian Hudson Valley Hospitals primary and secondary service areas. Significant socioeconomic gaps exist among the communities served by NewYork-Presbyterian Hudson Valley Hospital. Over a series of meetings in 2016, the Westchester County Health & Hospitals Planning Team crafted and disseminated throughout Westchester County community assessment and provider surveys. In addition to surveying the community at large, the team sought to identify a wide and varying range of providers of health care services to ensure that as many areas of health care as possible could be included in the surveys. Surveys were distributed to community members, community agencies and providers both electronically via a Survey Monkey on-line form, and in hard copy paper format. Completed paper surveys were manually entered into the data sets prior to analysis. NewYork-Presbyterian Hudson Valley Hospital completed a stand-alone Community needs assessment report. Part V, Line 7a: www.nyp.org/documents/hudsonvalley/about/CSP/HVH-CSP-2016-2018.pdf Part V, Line 10a: www.nyp.org/documents/hudsonvalley/about/CSP/HVH-CSP-2016-2018.pdf
Part V, Line 11The hospital is addressing the significant needs identified in its most recently conducted CHNA, namely Overweight and Obesity by its Community Service Plan Priority Prevent Chronic Disease programming. The hospital also provides community outreach to address the other health factors that were raised as health concerns, namely, nutrition/eating habits and care for the elderly. The original 2014 Intervention Healthy Heart Cooking Classes expanded to become the Healthy Heart Program. In 2014, participants expressed their interest in learning more about how physical activity could benefit their overall heart health. The program broadened from cooking classes to include a membership to The Wellness Center, the hospitals in-house fitness facility. Local physician practices and Hudson River Health Care, a not-for-profit, New York State licensed, federally qualified health center, helped to identify participants. The patient profile was minorities, ages 45-65, at risk of heart disease due to a previous heart attack, other cardiac issues or predisposition due to heredity, high blood pressure, diabetes or being overweight. In 2017, the program included enrollment in 10 Healthy Heart cooking classes spanning five months at NewYork-Presbyterian Hudson Valley Hospital Chef Peter X. Kelly Teaching Kitchen and membership to the hospitals Wellness Center, an onsite exercise facility. The participants were from minority populations, with ten African-Americans and two Latinos. Cooking classes were held every month from March through July 2017 with 12 participants enrolled in the program. The cooking classes are led by the chef, whose focus is teaching participants how to better manage their health through diet and lifestyle changes. The program aims to provide long-term solutions to improving health. Long term, the program emphasizes an increase in consumption of fruits and vegetables, and increase in heart health food preparation techniques, and increase in exercise habits. Moreover, the program seeks to empower participants, increasing their confidence in preparing heart health foods, and promotes mindful food consumption. Most indicative of the success of this program is how participants report feeling after the program. In our starting survey, they felt a certain way. The goal of the Healthy Heart program is to have most participants undergoing change and remaining motivated, and this goal was realized. After the program, four out of five participants embraced significantly new attitudes and corresponding behaviors, while one out of five was just behind them, taking action and deepening a commitment to the next level. This marked difference in attitude and behavior demonstrates how empowering this program is, and transformative for participants. Two Year update: From 2016 to 2018, NewYork-Presbyterian Hudson Valley Hospital achieved the goals of Priority/Focus Area 1, addressing the health disparity of race, and increasing access to high quality chronic disease preventative care with its Healthy Heart Program. Seventy-eight individuals from minority backgrounds were served with heart health education and resources from 2016 to 2018. In 2016, the program was held twice, for five months at a time. In 2017, one five-month program was held before transitioning in September to a five-week program. Noteworthy in 2018 was the increase in referrals by NewYork-Presbyterian Medical Group Hudson Valley physicians of their own patients to the hospitals Healthy Heart Program. In previous years, the hospitals Community Outreach team collaborated mostly with the Peekskill-based Hudson River Health Care (HRHC), a New York State federally-qualified health center, for recommendations. While that collaboration continued, in 2018 the Outreach team increased its efforts to physicians associated with NewYork-Presbyterian Medical Group Hudson Valley, which resulted in expanded membership in the program. In 2018 specifically, this meant that in addition to community participation (HRHC referrals and other), the Healthy Heart class enrollment reflected 10 patients of our cardiologists and 14 patients of our internal medicine practitioners. Demonstrating the impact of the program, in terms of the results of all the Healthy Heart programs - regardless of their length - were the attitudinal shifts toward nutrition to support cardiovascular health and overall well-being. The participants in the program indicated that they now understand the importance of the following: Importance of Fruits & Vegetables, Consumption of Red Meat vs. Poultry, Fish & Vegetables, The Reliance on Sugary Beverages, Salt Intake, Nutritional Literacy, Culinary Skills Education and Impact on Community. Perhaps most indicative of the success of this program is how participants report feeling upon completion of the program. In our starting survey, nearly 50% recognized that they needed to make healthy lifestyle decisions, thus their interest in the Healthy Heart Program. The other half of participants disclosed at the start of the program that either they had not tried at all to change unhealthy habits or had tried a little, but were frustrated by a lack of results. The goal of the Healthy Heart program is to have most participants undergo change and remain motivated, and this goal was realized. After the program, not one person remained negative, frustrated or only mildly committed. Instead, 16% were actively seeking information and changing unhealthy habits and 83% had made changes, were seeing results and said they would remain committed to a plan to achieve optimal health. In other words, four out of five participants embraced significantly new attitudes and corresponding behaviors, like eating better and exercising more, while one out of five was just behind them, taking action and deepening a commitment to the next level. This marked difference in attitude and behavior demonstrates how empowering this program is, and transformative for participants. Biometrics Biometric measurements of blood pressure, weight, and body mass index were recorded for the first class and last class at completion of the 10 week series: 83% of participants (10 people) had a reduction in blood pressure, 58% (7 people) lost weight and 58% (7 people) reduced their body mass index. Two Year update: With the restructuring of the program to five weeks in September 2017, biometric results pertaining to weight loss and an improvement in blood pressure readings were supplied at the start and conclusion of each five-week program, administered under the care of a Magnet nurse and an exercise trainer from the hospitals Wellness Center. Overall, within five weeks, most participants experienced an improvement in their blood pressure readings. These measures were shared with each individual to in turn discuss with their physician to make longer-term goals. Exercise component Exercise is an essential part of the Healthy Heart Program. Included in the program from 2016 to 2018 was a six-week membership to the hospitals Wellness Center facility at no charge to our participants. The participants are required to have physician clearance to participate in the exercise program. Two Year update: The Wellness Center is the only medical fitness facility in the hospitals service area and has 300+active memberships through Q4 2018. At the start of the program, a certified exercise trainer from the Wellness Center gives an introduction and overview of the membership benefits which includes a one-on-one fitness assessment and a fitness program designed specifically for each person. Included in the membership is use of all cardio and weight machines, as well as fitness classes such as Zumba, Step, Yoga, and Tai Chi. Emphasis is also placed on measuring ones progress, whether that is accomplished as Wellness Center members through our Activelinxx technology, through their own Smart Phone, or with the use of a pedometer. Drawing from the information supplied by the individuals medical provider, our exercise trainers design an exercise routine reflective of their health concerns and also instruct which exercise to avoid. In addition to the Healthy Heart Program, community members have access to Hudson Valley Hospital Center DBA NYP/Hudson Valleys bi-monthly Farmers Market offered from May through November, making healthier food choices more readily available. Healthy Heart participants were encouraged to use the knowledge learned in the cooking classes about the nutritional value of fresh fruits and vegetables, ask questions directly of the farmers and purchase fresh produce. Two Year Update: Chef Emilie Berner also began giving guided tours of the markets frequently in 2018, sharing insights and recipes. Each market serves an average of 479 visitors. PART I, LINE 7 the following is a detail of the sources used for determining the amounts reported on schedule H: Line 7A - Adjusted Ratio of Patient Care Costs to Charges Line 7B - Cost accounting system Line 7E - Actual Expenses PART III, LINE 2 - BAD DEBT EXPENSE EXPLANATION: For patients who were determined by the Hospital to have the ability to pay but did not, the uncollectable amounts are bad debt expense (price concessions). PART III, LINE 3: The amount included represents patients who qualify for charity care/financial assistance and have a bad debt write-off. Bad debt expense (price concessions) associated with patients that received charity care/financial assistance is represented in this $218,178 figure. These patients went through our charity care/financial assistance process and were determined to have financial need. As a result, we provided them with a discount based on our sliding scale charity care policy. If they were unable to pay the reduced balances, they were written off as bad debt and included as a community benefit. PART III, LINE 4 - FOOTNOTE TO FINANCIAL STATEMENTS: Audited Financial Statements page 18. PART III, LINE 8: THE HOSPITAL UTILIZED THE AMOUNTS REPORTED ON THE MEDICARE COST REPORT TO DETERMINE THE MEDICARE ALLOWABLE COSTS. TOTAL INPATIENT AND OUTPATIENT COSTS. THESE DO NOT INCLUDE CERTAIN MEDICARE PROGRAM REVENUE AND COSTS AND THUS DO NOT REFLECT ALL OF THE ORGANIZATIONS REVENUES AND COSTS WITH THE PARTICIPATION IN MEDICARE PROGRAMS. THE REVENUE AND COSTS EXCLUDES MEDICARE MANAGED CARE ACTIVITY. Assessing our Community's health needs has been paramount for the Hospital. The needs of the community continue to be assessed through partnerships with patients, physicians, elected officials, organizations, area businesses and local Departments of Health. The 2016 Community Health Needs Assessment demonstrated need for services addressing Obesity, Nutrition Education, Eating Habits, Breastfeeding, and Care for the Elderly. PART VI, LINE 5 - Promotion of Community Health: Through its understanding of community needs, Hudson Valley Hospital Center DBA NewYork-Presbyterian Hudson Valley Hospital has expanded and implemented programs to serve both patients and residents of the community. Nutritional support continues to be the focus of many of our community outreach programs. In an effort to combat obesity and an increase in Type II Diabetes, the Hospital has a multi-faceted plan under the banner of Harvest for Health. This included the creation of an organic garden for healing, which provided an outdoor classroom for nutritional education with cancer patients and continues to be a venue for volunteerism, with community residents assisting with first plantings. The hospital continued its food service that focuses on preparing wholesome meals with fresh and local produce. The hospital also offered its Farmers Market twice monthly and year-round. The markets provide the opportunity for members of the community to have access to healthier foods and encourage a plant-based diet, proven to be beneficial to many diagnoses from cardiovascular issues to diabetes. In addition, the Hospitals teaching and demonstration kitchen continued to provide the community with cooking and nutrition classes to encourage the preparation of healthy foods at home. Plans were made to hold free classes for at risk youngsters 11-13 in conjunction with the City of Peekskill School District to combat childhood obesity and teach nutritional principles, as well as classes for cancer patients and patients with heart disease and diabetes. To reduce the number of hospital readmissions in elderly populations, the Hospital continues to expand its NICHE program. The Hospitals designation as a NICHE hospital (Nurses Improving Care for Health System Elders) has allowed it to address many issues facing its growing elderly population fueled by five nearby nursing homes. A Silver Lining Healthy Aging Fair was conducted on site to educate seniors and their caregivers on the importance of hydration and mobility in helping the elderly to stay healthy. This was held in addition to a Senior and Caregivers Health Fair, held off site at a community center in Cortlandt, which provided health screenings and information to more than 150 seniors and their caregivers.. ExplanationA Certain Key Employee of the New York and Presbyterian Hospital that is identified in part VII as a former officer of the Hudson Valley Hospital DBA NYP/Hudson Valley Hospital is responsible for executing the mission and management of the New York and Presbyterian Hospital (NYP) and its affiliated entities. Compensation for 2018 of these upper level executives includes the payout of an annual incentive plan and a long-term incentive plan. This performance-oriented program conditions payments upon the achievement of multiple individual and group performance measures. Measures to monitor performance include: operational and financial strength, patient quality and safety, patient satisfaction, advancement of patient care, and people development and partnership. Incentive awards may only be granted if the organization achieves a financial surplus. Even if all relevant performance measurements are achieved, the NYP Board of Trustees retains full discretion to make or not make any incentive awards, or to reduce the amount of any incentive award. This initiative is critical to assuring that NYP has the requisite leadership to create and manage a highly motivated and engaged workforce, to drive superior performance throughout the organization and to achieve top tier medical center status. As a separate matter, due to restrictions imposed by the Internal Revenue Code, upper level executives are limited in the amount of benefits received under a tax-qualified retirement plan. Like many employers, NYP supplements these executives' pension benefits through a supplemental ("nonqualified") retirement plan. The supplemental executive retirement plan (SERP) is subject to a multi-year vesting requirement (commencing after five years of participation in the SERP, in prorated amounts through age 65) which places an executive's supplemental retirement benefit at risk of forfeiture if the vesting requirements are not satisfied. Once vested, however, provisions of the Internal Revenue Code require that the vested executive include in current income the value of his or her vested supplemental retirement benefit. Notwithstanding the legal requirement to recognize the vested value of the supplemental retirement benefit as current income, the supplemental retirement benefit will not be distributed to the executive until the executive actually retires from NYP (although, as permitted by the Internal Revenue Code, the supplemental retirement plan will effect a distribution of an amount necessary to satisfy the executive's tax liability resulting from the income recognition upon vesting). As noted, this supplemental retirement benefit will not be distributed to the executive until the executive actually retires from NYP. There are constantly changing legal, tax, accounting, and public disclosure rules for a SERP (supplemental executive retirement plan) in not-for-profit organizations. The executive Compensation Committee continuously monitors these changes and incorporates any changes into the overall SERP plan design. As in past years, the executive Compensation Committee of NYP requires a third party complete a review of the organization's compensation program to ensure its effectiveness in terms of government regulations, market conditions and the need to continually elevate organizational performance. The report also serves to meet the regulatory obligations to ensure that all elements of the executive compensation programs are reasonable. The individuals listed in part VII that are compensated by New York Presbyterian Hospital devote an average of sixty hours per week to perform their responsibilities for the New York Presbyterian Hospital and other related organizations in the aggregate. Schedule J, Line 3: Compensation decisions for the President was determined by a related organization following that organization's compensation policy. Part I, Line 4A: Severance Pay: John Federspiel received 750,009 in severance Pay. Kevin Murphy received 149,465 in severance pay.
