Part I, Line 7g - Subsidized Health Services ExplanationSubsidized health services include School Based Health Centers where LincolnHealth Group (LHG) and its subsidiary LincolnHealth (LH) provide physicians or nurse practitioners to consult and provide services within the school. Clinics are also offered throughout the year to provide subsidized services such as mammography, blood pressure screenings, etc. to encourage its residents to obtain these health services. Physician practices offering services such as Family Medicine, Internal Medicine and Pediatrics operate at a loss in order to ensure healthcare for its community members regardless of their ability to pay. $3,206,879 of expenses related to outpatient clinics are included in the subsidized health services total in Part I, Line 7g.
Part I, Line 7, Column (f) - Exclusions from Percent of Total ExpenseBad Debt is a deduction from revenue and is not included in total expenses.
Part I, Line 7 - Costing Methodology ExplanationCharity Care and Means-Tested Government Programs utilize a cost to charge ratio in determining the cost of the actual Charity Care Write-Off. The cost to charge ratio is calculated using Worksheet 2: Ratio of Patient Care Cost to Charges, located in the Schedule H instructions.
Part II - Community Building ActivitiesLincolnHealth Group and its subsidiary hospital organization, LincolnHealth, partner with various community groups such as Healthy Lincoln County and Healthy Maine Partnership to improve and protect the health of its community members. Meals are prepared by its staff as part of the local Meals on Wheels programs which delivers meals to the elderly population to assist with their nutritional needs. LHG participates in the community disaster training held on a regular basis throughout the county.
Part III, Line 2 - Bad Debt Expense MethodologySee Part III, Line 4 - Bad Debt Expense Footnote to Financial Statements
Part III, Line 4 - Bad Debt Expense Footnote to Financial StatementsFrom the LHG Audited Financial Statements: Charity Care: LH (the Hospital) accepts all patients regardless of their ability to pay. A patient is classified as a charity patient by reference to certain established policies. Essentially, these policies define charity care as those services for which limited or no payment is anticipated. In assessing a patient's ability to pay, the Hospital utilizes generally recognized poverty income levels. Charity Care is measured based on the Hospital's established rates. The Hospital utilizes annual federal guidelines and adjusts accounts for those whose income is 175% of the poverty level, taking family size into consideration. The costs and expenses incurred in providing these services are included in operating expenses. Revenue for services rendered to individuals from whom payment is expected and ultimately not received is written off and included as part of the allowance for bad debts. The costing methodology used in determining the amounts reported on Part III, Line 2 and 3 follow the Worksheets provided in the Schedule H Instructions and utilizes costs to charge information derived directly off the annual cost reports.LincolnHealth does not have a specific footnote in the financial statements that describes "Bad Debt Expense". LH reports Accounts Receivable are reduced by an allowance for doubtful accounts. In evaluating the collectability of patient accounts receivable, LHG analyzes its past history and identifies trends for each of its major payor sources of revenue to estimate the appropriate allowance for doubtful accounts and provision for bad debts. Management regularly reviews data about these major payor sources of revenue in evaluating the sufficiency of the allowance for doubtful accounts. For receivables associated with services provided to patients who have third-party coverage, LHG analyzes contractually due amounts and provides an allowance for doubtful accounts and a provision for bad debts, if necessary. For receivables associated with self-pay patients, LHG records a significant provision for bad debts in the period of service on the basis of its past experience, which indicates that many patients are unable or unwilling to pay the portion of their bill for which they are financially responsible. The difference between the standard rates and the amounts actually collected after all reasonable collection efforts have been exhausted is charged off against the allowance for doubtful accounts. LH believes that bad debt expense should be included as a community benefit.
Part III, Line 8 - Medicare ExplanationMedicare allowable costs were calculated using a cost to charge ratio. LH believes that the Medicare shortfall should be included as a community benefit because LH has a clear mission commitment to serving elderly patients and adults with disabilities through the provision of specific subsidized programs developed to help improve the health status of these patients. If these critical subsidized programs were not provided by LH, they would become the obligation of the Federal Government.
Part III, Line 9b - Collection Practices ExplanationPatients who qualify for free care have their account balance adjusted accordingly once free care has been approved. As a tax-exempt hospital, LH provides necessary patient care regardless of the patient's ability to pay for the services.
Part VI, Line 2 - Needs AssessmentThe organization prepares a Community Health Needs Assessment (CHNA) every three years.See https://mainehealth.org/healthy-communities/community-health-needs-assessment for the completed CHNA. The health needs of the community are assessed by: utilizing the Maine State Health Plan and Assessment, community forums, involvement in community based health status improvement programs and in conjunction with Maine Healthcare (our healthcare system.)Maine Healthcare also participates in various initiatives to help support and provide updates to community needs assessment planning. Some of these initiatives include: Clinical Strategic Planning, Financial Strategic Planning, Facility Planning, Manpower Planning, Physician Recruitment Strategic Planning, Emergency Preparedness Planning. Along with the internal assessments, member organizations also review and act on recommendations provided by local and state partners, such as the Maine Center for Disease Control and Prevention (MCDC). All hospitals within the Maine Healthcare system have conducted Community Health Needs Assessments (CHNA) to meet federal regulations that were created as part of the Affordable Care Act. In late 2013, CHNA and Implementation Plan Reports were produced by each Maine Healthcare member hospital, in collaboration with Maine Healthcare. These detailed reports covered the time period of 2013-2015 and were formally adopted by each member organization Board for submission with their respective FY13 990 filings. In 2015, a collaborative called the Maine Shared Health Needs Assessment was launched to create a framework and approach for a Shared CHNA that can: i) address community benefit reporting needs of hospitals, ii) support state and local public health accreditation efforts, and iii)provide valuable population health assessment data for various organizations concerned with the health of Maine's communities and citizens. Maine Healthcare, Eastern Maine Healthcare System, Central Maine Healthcare and the Maine Center for Disease Control and Prevention are contributing financials support to the study and reports. In 2016, Maine Shared CHNA produced county-level CHNA reports and helped coordinate community engagement processes that ultimately resulted in the prioritization of the health issues by hospitals,state/local governments and other partners. In collaboration with Maine Healthcare, each member hospital produced a detailed CHNA report and implementation plan covering the time period of 2016-2018. These reports were formally adopted by each member organization Board for submission with their respective FY16 990 filings.
Part VI, Line 3 - Patient Education of Eligibility for AssistanceFree Care information is provided in the Admitting and Emergency Registration locations in the following manner:- Postings including Free Care and Monthly Payment Plan- Handouts- InterviewsIf the patient is self-pay, under insured or can't afford to pay their hospital bill, they may receive a Financial Counseling Packet and/or referred to financial counseling from the registration staff or Change Healthcare, an outside vendor who helps manage the self- pay accounts. The packet includes:- Information on Maine Healthcare's financial policies- Information on Maine Healthcare's Free Care Program, Monthly Payment Plan Program, Care Partners, MedAccess and other community resource needs.- Program applications and instructions for Maine Healthcare's Free Care Program, and Monthly Payment Plan Application- Contact information for assistance with applications, bills or financial concernsSelf-pay or underinsured patients registering in person or via a phone interview receive financial counseling including information on our financial assistance programs. Registration staff or Change Healthcare provide forms and assist with completing financial assistance applications and providing follow up contact information. Maine Healthcare's Web site includes on line Registration and Patient Billing information:- Billing Process- Free Care- Monthly Payment Plan- Patient Statement- Price Information- Contact Us and QuestionsPrimary language, deaf and hard of hearing and interpreter needs are assessed during the registration interview and services are provided as needed. If a patient does not respond at pre-registration, registration, or while receiving care, all of these programs are explained again by the Single Billing Office staff. The intent of these efforts is to ensure that the patient is fully informed of and able to take advantage of these assistance programs.
Part VI, Line 4 - Community InformationThe Community Health Needs Assessment describes the geographic area and demographic constituents it services. See https://mainehealth.org/healthy-communities/community-health-needs-assessment for the completed CHNA.
Part VI, Line 5 - Promotion of Community HealthThe tax exempt purpose of the organization to improve the health of the community is promoted and enhanced in the following ways:* The majority of the governing body is composed of persons living within the hospital service area that are neither employed by, nor have contractual relationships with, the hospital or its affiliates.* Medical staff privileges are available to all qualified physicians in the community.* Solicitation of community input in the strategic planning process.* Common governance of the hospitals, long-term care, home health, Hospice and physician services provided in the county to promote common health care and improvement planning and control costs.* Voluntary cap on Income from Operations to restrain prices and to fund health care and health improvement activities.* Surplus funds are reinvested in capital equipment and improvements to ensure high quality care.* Each year a portion of Retained Earnings are distributed to our parent, Maine Healthcare, for the specific purpose of health status improvement programs throughout the Maine Healthcare service area.
Part VI, Line 6 - Affiliated Health Care SystemMaine Healthcare is a not-for-profit family of leading high-quality providers and other healthcare organizations working together so their communities are the healthiest in America. Ranked among the nation's top 100 integrated healthcare delivery networks, Maine Healthcare is governed by a board of trustees consisting of community and business leaders from its southern, central and western Maine regional service areas. The collaboration of Maine Healthcare members makes it possible to offer an extensive range of clinical integration and community health programs, many aimed at improving access to preventive and primary care services. Maine Healthcare includes the following member organizations: LincolnHealth Group, MaineHealth, Maine Behavioral Healthcare (Spring Harbor Hospital), Coastal Healthcare Alliance (Pen Bay Medical Center and Waldo County General Hospital), Southern Maine Health Care (Southern Maine Medical Center and Goodall campuses), Western Maine Health Care (Stephens Memorial Hospital), The Memorial Hospital at North Conway, N.H., MaineHealth Care at Home, NorDx, Synernet and MaineHealth Accountable Care Organization.
