DescClinical Care: The SRAlab was founded in 1951 as the Rehabilitation Institute of Chicago, an Illinois not-for-profit corporation. In March 2017, the Rehabilitation Institute of Chicago housing 182 beds moved to a new building with 242 licensed beds, and became the SRAlab. This move brought not only a physical change of address but a whole new model to the field of rehabilitation. Upon opening in March 2017, the 1.2-million-square-foot SRAlab became the first-ever "translational" research hospital in which clinicians, scientists, innovators and technologists work together in the same space, surrounding patients, discovering new approaches and applying (or "translating") research real time. SRAlab's vision is to advance human ability through science and research. The organization has earned the distinction of being named the nation's #1-ranked provider of comprehensive physical medicine and rehabilitation care to patients from around the world for 28 years by U.S. News & World Report - a record for any U.S. hospital. SRAlab also is the nation's largest rehabilitation research enterprise, advancing the field globally through innovation of the most advanced treatments and technologies. SRAlab is the only rehabilitation hospital in the nation to hold a record eight federally designated research programs in the areas of computers and robots in therapy, mobility technologies, health and function in disabilities, neurologic rehabilitation, spinal cord injury, rehabilitation engineering, patient outcomes, and pediatric orthopedics. SRAlab's Care Quality and Delivery Highlights:As of March 2017 SRAlab's system of care included a 242 bed hospital and outpatient facility, located at 355 East Erie Street in Chicago, Illinois, as well as a number of other facilities and joint ventures. The Corporation offers a comprehensive range of inpatient, day rehabilitation and outpatient services throughout the System. Inpatient rehabilitation services are provided using a multidisciplinary approach. Day treatment rehabilitation consists of a multidisciplinary approach for individuals no longer requiring the 24-hour nursing services provided in an inpatient setting. Outpatient care is typically a singular discipline (e.g., physical therapy) and offers various levels of therapeutic intensity, depending on the patient's needs and goals. In FY 2018 SRAlab treated more than 6,800 inpatients throughout its system of care and delivered approximately 1.9 million outpatient therapy units. SRAlab's facilities are staffed by approximately 2,400 medical professionals and support personnel, a dedicated team who in FY 2018 treated patients from 48 states as well as the District of Columbia and handled referrals from 52 countries. Each patient at SRAlab is assigned a personalized (coordinated interprofessional) care team consisting of the field's most well-trained rehabilitation doctors, nurses, therapists, care managers clinical specialists and researchers. No other specialty hospital in the nation has a more experienced track record of being rated #1 for twenty-eight consecutive years. SRAlab is one of the 3.7% of the nation's nearly 6,000 hospitals to have achieved three or more consecutive Magnet designations for nursing excellence from the American Nurses Credentialing Center (ANCC) and the first free-standing acute inpatient rehabilitation hospital to have this distinction.Desc

DescResearch Program: SRALab's Research Program is the world's largest physical rehabilitation research enterprise in pursuit of clinical advancement through scientific discovery. The Research Program has at any given time approximately 150 research projects underway, funded by more than $100 million in donations from both private and public sources. Its experts regularly publish research in the world's foremost medical journals, including the Journal of the American Medical Association (JAMA) and its European equivalent, The Lancet, and it is home to five federal research designations to develop cutting-edge solutions for patients.Computers and Robots in Therapy: An internationally acclaimed NIDILRR-funded center especially for patients with hemispheric stroke - the most debilitating kind. Seven studies address two elemental questions: When is the optimal time to use robotic and computer-based systems during therapy, and how do therapists best interact with robotic devices during therapy, often with minimal hands-on treatment coming from the clinician? Training of undergraduate rehabilitation engineers is included.Manipulation and Mobility Technologies: A NIDILRR-funded Rehabilitation Engineering Research Center award to fund six new projects researching innovations that are directly useful for patients, especially those who have experienced limb-loss or stroke. Projects include body-powered hand prostheses, use of powered exoskeleton for after-stroke patients, and a new wheelchair to allow both sitting and standing. Research will include both technology development and clinical testing.Health and Function in Physical Disability: This Rehabilitation and Training Center, funded by NIDILRR, was created to develop, evaluate, and implement methods to promote optimal health and function of people with physical disabilities. Through a variety of research, training and dissemination activities, we expect to gain a more comprehensive understanding of the efficacy and value of various interventions for achieving and maintaining health and function for people with a disability.Spinal Cord Injury Research: SRAlab operates the Midwest Regional Spinal Cord Injury Care System as designated by NIDILRR. One of only 14 SCI Model Systems nationwide, SRAlab advances spinal cord care through clinical research that identifies new standards of care for the industry. It leads multiple independent and collaborative research projects exploring walking therapy technologies that deliver the most effective treatment to advance ability.Engineering Research: This project establishes a new NIDILLR-funded Rehabilitation Engineering Research Center, Collaborative Machines Enhancing Therapies (COMET), focusing on advancing technologies that work in partnership with patients and clinicians to enhance gait and dexterity outcomes for individuals with stroke and spinal cord injury (SCI) and other people with disabilities in a real-life rehabilitation setting. This project is based at SRAlab and leads four other major research universities around the United States.Desc

DescMedical Education: The Department of Physical Medicine and Rehabilitation at Northwestern University's Feinberg School of Medicine (Dept of PM&R) is housed within SRAlab, which has one of the largest and most highly respected residency programs in physical medicine and rehabilitation in the country. Resident physicians spend three to four years rotating through the Dept of PM&R, taught by SRAlab physicians who are experts in this specialty. In FY 2018, SRAlab trained 53 residents. The SRAlab medical education program also provides opportunities for first-year medical students to meet regularly with patients from SRAlab's spinal cord injury, general rehabilitation, or pediatric program to gain patients' insights on recovery and needs for independent living.Academy: The Academy has offered continuing education courses aimed at helping healthcare professionals stay at the forefront of their field for 59 years. In FY 2018, over four thousand students participated in academy programs which are taught by interdisciplinary teams of recognized specialists from SRAlab and visiting experts from across the country and around the world. These courses were offered in a variety of formats including webinars, on-site courses, outreach programs and our first-ever online series and podcast.Fellowships: SRAlab offers physician fellowships, funded primarily by donations, in pediatric rehabilitation, musculoskeletal sports and spine medicine, spinal cord injury, pain management, and traumatic brain injury. In FY 2018, 7 physician fellows were named at SRAlab, 1 in spinal cord medicine, 2 in sports medicine, 2 in pediatric rehabilitation, 1 in Brain injury and 1 in pain medicine. SRAlab also supports fellowships for therapists, nurses and other non-physicians. Prosthetics and Orthotics (P&O) Residency: Each year, SRAlab's Prosthetics and Orthotics Clinical Center (POCC) selects three prosthetist and three orthotist residents for these highly sought after positions. The orthotics residency is a 12-month program and the prosthetics residency is an 18-month program to allow additional hands-on experience in research through our Center for Bionic Medicine. Through these training programs, P&O residents are mentored by SRAlab's certified, licensed professionals and get exposure to a variety of clinical presentations and pathologies, clinical evaluation, prescription recommendation, documentation, the actual fabrication of devices, and formal research.Desc

Shirley Ryan AbilityLabPart V, Section B, Line 5: SRAlab intentionally sought input from a wide variety of stakeholders who serve and represent the interests of medically underserved, low-income, and minority populations in the community. In May 2016, SRAlab hosted a focus group with leaders of the health community, as part of its effort developing the Assessment. Focus group participants included persons with special knowledge or expertise in public health, represented federal, state or local health departments, or agencies with current data or other information relevant to the health needs of the community. Participants also included leaders, representatives, or members of medically underserved, low-income, and minority populations and populations with chronic disease needs. Participants came from the following organizations:- Chicago Mayor's Office for Persons with Disability- Illinois DHS - Division of Rehabilitation Services- EOA Consulting, LLC- Chicago Transit Authority- Chicago Park District- Extended Home Living Services- Brain Injury Association of Illinois- Spinal Cord Injury Association of Illinois- Parkinson's Disease & Movement Disorder Center at Northwestern University- Gridiron Alliance- SRAlab Peer Mentor Program The external focus group was held at SRAlab and lasted over two hours. Group discussion focused on rehabilitation health needs of people with physical disabilities, access to information and care, research and clinical trials, education and training, and support programs. SRAlab also received comments from other community members who were unable to attend the focus group. Moreover, representatives from SRAlab spoke with representatives of the Illinois Department of Public Health (IDPH) in April 2016 for IDPH's input on the significant health needs of the community.
Shirley Ryan AbilityLabPart V, Section B, Line 11: SRAlab conducted and adopted its second Community Health Needs Assessment during tax year 2015 and identified the following significant health needs: Rehabilitation Health, Research and Rehabilitation Engineering, Improved Access to Information, Education and Training, and Support Programs. All significant health needs identified in SRAlab's most recently conducted CHNA are being addressed. SRAlab did take the following actions during tax year 2017 with respect to its most recently conducted CHNA:In the area of Rehabilitation Health, SRAlab continued to serve an array of patients with rehabilitation needs. These included patients with amputations and other limb deficiencies, neuro-musculoskeletal injuries, brain injuries, spinal cord injuries, stroke rehabilitation, and cancer rehabilitation, both from an adult and pediatric perspective. SRAlab treated patients to improve and eliminate the effects of injury, disease, and debilitating health conditions, continuing its work toward the vision of its new flagship hospital, opened in March 2017, of advancing patient outcomes through the integration of research and clinical care. In addition to this work at the flagship hospital, FY 2018 brought significant improvement to the facilities and access for the DayRehab programs located throughout the Chicagoland area. DayRehab is a community-based program ranging from three hours of therapy per day, two or three days per week, to six hours of therapy per day, five days per week. SRAlab's existing sites were at capacity. In FY 2018, SRAlab opened three new expanded programs serving the city of Chicago, the western suburbs and the northern suburbs, improving access and extending the new standards of care emanating from the new flagship hospital to the day rehabilitation setting. In the area of Research and Rehabilitation Engineering, SRAlab completed construction of its innovative new Research Hospital and began operations in the new facility in March of 2017. The innovative design of the Research Hospital integrates research and rehabilitation medicine to truly engage in cutting edge translational research. SRAlab continues its multiple programs of rehabilitation-related research. In tax year 2017, researchers at SRAlab demonstrated that the stem cell population in muscles from children with cerebral palsy is severely depleted, which may be a cause of contractures. Based on that study, they identified several potential drugs that may be useable in treating cerebral palsy that have never been used before. Additionally, our investigators have used small sensor devices to measure the tremor of patients with Parkinson's disease and can now both predict needed changes in medication and even notify patients when their walking pattern is becoming unstable, and they are at risk of falling. Another project incorporates the technology of self-parking cars into a power wheelchair. The individual operating the chair need not have quick or complete responses in order to safely and effectively navigate the wheelchair. In the area of Improved Access to Information, the Henry B. Betts LIFE Center, a virtual and multimedia education center providing patient education and consumer health information for patients, families, staff and the community, increased its users from an average of 187 visitors per day in September 2017 to more than 452 visitors per day in August 2018. The Center's resources are organized according to eight topic areas: Medical Information and Care, Caregiving and Equipment, Housing and Transportation, Education and Employment, Support and Wellness, Recreation and Leisure, Finance and Law and Inspiration and Hope. LIFE web traffic in FY 2018 averaged 19,093 page views per month with 229,127 page views annually from more than 190 countries. The 9,666 unique users each month were reflective of a steady increase in this area with 86% new to the site. In FY 2018, total outreach and service efforts increased 12% over FY 2017, and represented 10,168 person contacts served through programs, classes, special events, resource requests, teaching activities and tours for patients, families, staff and the community. In the area of Education and Training, SRAlab as the Northwestern Feinberg School of Medicine's Department of Physical Medicine and Rehabilitation physiatry residency program, continued to support the education needs of PM&R physicians. SRAlab also continued its robust educational programming for allied health, patients, and their families in the community. SRAlab held nearly fifty grand rounds educational presentations for physicians on a number of topics, including care for stroke patients, spinal cord injury patients, pain, research, chronic pain, and others. In the area of Support Programs, SRAlab provides high-quality supportive programs to the community. This includes its Adaptive Sports Program, comprised of Transitions Planning Program for Adolescents, Caring for Kids, Sports & Fitness Center programming, and the SRAlab Military Sports Camp. Other programs include vocational rehabilitation and a community aphasia program. In FY 2018, SRAlab provided 401,080 hours of participation to more than 1,200 participants in the Adaptive Sports Program. As part of its vocational rehabilitation program, for the first time SRAlab offered no-cost and accessible GED classes at its downtown Chicago location (541 N. Fairbanks). This program is offered to any individual with a spinal cord injury (SCI) who is looking to earn his or her GED. Unlike other GED programs, the SRAlab vocational facility is an accessible location, and the computer lab is equipped with assistive technology.

Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Part I, Line 7:The best available data was used to calculate the cost amounts reported. The Medicare cost report was the source used where a program category was separately identifiable. In other cases the internal cost allocation model was used as the source of the cost. The Medicare cost report cost to charge ratio was used to determine the cost of charity and bad debt.
Part III, Line 2:The bad debt expense was estimated by multiplying the cost to charge ratio from Worksheet C of the Medicare cost report times the provision for bad debt for the year.
Part III, Line 3:Amounts have been reported on Schedule H, Part III, Line 3 for SRAlab joint ventures to estimate the amount of bad debt expense attributable to patients eligible under these organizations' financial assistance policies. Southern Illinois Hospital (SIH) provided the SRAlab share amounts for bad debt at cost and bad debt at cost attributable to patients eligible under the SIH financial assistance policy. Alexian Brothers Medical Center (ABMC) and Illinois Masonic Medical Center (IMMC) provided financial statements for SRAlab to determine the amounts for bad debt. Bad debt at cost attributable to patients eligible under the ABMC and IMMC financial assistance policies was estimated by 1) using the ABMC and IMMC Schedule H's to calculate a ratio of bad debt at cost attributable to patients eligible under their respective financial assistance policies over their bad debt at cost, and 2) applying this ratio to the bad debt that SRAlab has determined from the financial statements of ABMC and IMMC.On Schedule H, Part III, Line 3 no amount was included for SRAlab for bad debt expense attributable to patients eligible under the SRAlab financial assistance policy. SRAlab's Presumptive Eligibility Policy sets forth the presumptive eligibility criteria by which a patient's financial need is determined and used by SRAlab to deem a patient eligible for hospital financial assistance without further scrutiny by SRAlab. Charity care may be provided to patients who are either uninsured or underinsured who do not have the financial resources to pay their SRAlab bill and who meet income and other guidelines specified by SRAlab. As soon as the need for financial assistance is identified, SRAlab provides the patient or Guarantor with a Program application. Patient Financial Services will review all patient accounts with self pay balances and determine presumptive eligibility after review of the patient's Charity Application and prior to sending the patient a bill requesting payment. Patient Financial Services may use third-party reporting to determine presumptive eligibility of a patient if the patient has not timely returned a completed Charity Application. If a patient is presumed eligible, their balances will not be written off to bad debt.
Part III, Line 4:The organization is part of the Rehabilitation Institute, Inc. and Affiliates' consolidated audit in which the footnote that discusses the bad debt expense is located on page 10.
Part III, Line 8:The portion of our Medicare shortfall reported on Part III, Line 7 which relates to patients that are dual eligible for both Medicare and Medicaid should be treated as community benefit consistent with our State of Illinois Annual Non Profit Hospital Community Benefits Plan Report. The amount on Line 7 related to these dual eligible patients is $2,182,752.The cost to charge ratio from Worksheet C of the Medicare cost report times the Medicare gross charges from the cost report less GME payments was used to determine Medicare allowable cost.
Part III, Line 9b:SRAlab's Collection policy requires that, before any extraordinary collection activity is taken, at least 120 days have passed since the patient's first post-discharge billing statement and that the patient has been given adequate notice regarding and opportunity to apply for financial assistance. Moreover, the policy requires that the Executive Director of Revenue Cycle/designee verify that these and all other statutory pre-conditions to collection activity have been met before any collection activity is initiated. The policy further requires that if SRAlab receives a Financial Assistance Application while collection activity is ongoing, it will suspend such activity (and require any retained collection agencies acting on its behalf to suspend such activity) so that a determination of Financial Assistance eligibility can be made; if it is determined that the patient is eligible for Financial Assistance, collection activity will be ceased and any applicable refunds/reversals will be made.
Part VI, Line 2:The needs of SRAlab's community are assessed on an ongoing basis as part of our routine operations and in accordance with IRS regulations relating to the Community Health Needs Assessment. A formal assessment is performed periodically related to the community health needs assessment. SRAlab receives input from our community through patient feedback programs, community support groups and through our clinicians and physicians who are interacting with patients every day. The SRAlab Henry B. Betts LIFE Center provides education and support for the disabled community regardless if they receive services from SRAlab or not. LIFE web traffic in FY 2018 averaged 19,093 page views per month with 229,127 page views annually. Total outreach and service efforts represented 9,666 person contacts. This included individualized teaching sessions with patients, families, staff, and visitors, as well as a wide variety of education programs, support groups, special events, and tours with prospective patients and families. Through this connection we are able to collect and track the resource needs of our community. We also have a number of disabled members on our governing and auxiliary boards. These individuals help guide the mission, vision and services at SRAlab. In 2016, SRAlab undertook a formal process which included review of medical literature relating to significant rehabilitation needs, a community focus group, and a review of historical patient characteristics in order to assess and develop its FY 2016 Community Health Needs Assessment. SRAlab is in the process of conducting its FY 2019 Community Health Needs Assessment.
Part VI, Line 3:SRAlab ensures that each registration/admission location contains signage (in English and other predominant languages of the SRAlab patient population) notifying patients that financial assistance is available to qualified individuals. This notice is also posted on SRAlab's website (www.sralab.org), along with SRAlab's Financial Assistance Program process and application. Information regarding the Financial Assistance Program and the application is available in each SRAlab registration/admission location. The information regarding financial assistance also appears on every self pay statement sent to our patients. In addition, when SRAlab care managers are involved in the patient's care, the care manager informs and assists the patients in enrolling in appropriate entitlement programs.
Part VI, Line 4:As a specialty hospital, SRAlab defines the patients it serves broadly as the disabled community throughout the state of Illinois, the United States and around the world. Due to SRAlab's unique services and its reputation as the leader in rehabilitation medicine, many patients will travel long distances for care. Approximately 79% of SRAlab inpatients come from Chicago and its 7 surrounding counties. This represents a primary reach of approximately 50 miles. In addition to those Chicago area patients, SRAlab also cared for patients from 48 states and handled referrals from 57 countries. SRAlab continues to increase its outreach and has impacted the lives of individuals in North, Central and South America; Europe; Africa; Asia (predominately the Middle East); and Australia. The largest users of rehabilitation medicine are those 45-64 years of age and older. However, SRAlab continues to serve this group of people and conducts strategic planning around patients of varying ages.
Part VI, Line 5:SRAlab provides a number of services that generally do not generate revenue, solely for the purposes of improving the health and quality of life for our community. These services include: research to improve patient outcomes; educational resources on living with a disability available to anyone through the Internet; vocational training and placement services; support groups to assist in psychological adjustment; fitness and sports training program to promote physical activity and case management for high risk patients discharged to the community. Any surplus funds are used to support these non-revenue patient centered activities as well as develop programs, sponsor research and improve facilities.
