Hospice Hospice is a philosophy of care that accepts death as the final stage of life. The goal of hospice is to enable patients to continue an alert, pain-free life and to manage other symptoms so that their last days may be spent with dignity and quality, surrounded by their loved ones. Hospice is not a place, but rather a special type of care that can be provided in a variety of locations, frequently right in the home. Hospice is designed to offer comfort and care, helping relieve issues that occur in the final phases of an end stage disease process, and is designed for people whose illnesses no longer respond to curative treatment but focuses on treating symptoms while increasing quality of life. Hospice care includes not only support for the patient, but also the family and loved ones. Northwestern Medicine Home Health & Hospice provides a caring hospice team that works with you to develop a care plan with goals to: improve quality of life; relieve pain, control symptoms and offer comfort and dignity; and, provide physical, emotional, social and spiritual support. Levels of hospice care Northwestern Medicine Hospice provides needs-based care in a variety of locations and may involve different levels of caregiving. Hospice care may be provided in your home, a hospice facility, hospital or nursing home, as appropriate. The Medicare Hospice levels of care include: * Routine care: the most common level of hospice care. The interdisciplinary hospice team, which includes skilled nurses, social workers, chaplains, home health aides, and volunteers, provides services in your home for assessment, medication management, personal care, emotional support and education. * General Inpatient care: Inpatient level of care is provided for pain control or other acute symptom management that cannot feasibly be provided in any other setting and begins when other efforts to manage symptoms have been ineffective. * Inpatient Respite care: a hospice patient may temporarily stay in a medical facility with 24-hour care so that caregivers can rest and recharge. * Continuous care: Care may begin only during periods of crisis to manage acute symptoms, requiring ongoing nursing care to help stabilize the symptoms and can be provided at home or in a hospital setting. The Northwestern Medicine hospice care team draws talents from a range of medical and professional areas, to offer a complete circle of care to meet your family's needs. Your care team includes: Medical director of hospice; Hospice nurses are educators and advocates who strive to provide comfort and grace throughout the final days; Hospice chaplains help your loved one and family focus on healing and forgiveness, and find a sense of meaning; Hospice social workers offer emotional support, care planning, problem solving, access to resources and assistance with financial issues; and, Hospice volunteers are carefully selected and trained to provide compassionate assistance, support and friendship. Northwestern Medicine hospice care staff are trained to meet the unique needs of seriously ill cancer patients, those suffering from advanced stages of Alzheimer's and other forms of dementia, and those with advanced heart disease. If someone you love is at the end-stage of life and is no longer receiving treatment, the compassionate skilled Northwestern Medicine Home Health & Hospice team can provide the care needed to control pain symptoms and make the final days as comfortable and meaningful as possible. Many times, your loved one can remain at home surrounded by family in a soothing, familiar environment. ExplanationEach of the NMHC not-for-profit entities is qualified under the Internal Revenue Code (the Code) as a tax-exempt organization and is exempt from tax on income related to its tax-exempt purposes under Section 501(a) of the Code. Accordingly, no income taxes are provided for the majority of the income in the accompanying consolidated financial statements for these corporations. Certain corporations had unrelated business income (UBI) generated primarily from the sale of certain services that are not directly related to patient care and through limited partnerships within the investment portfolio. Certain corporations have unused net operating loss carryforwards available to offset the UBI tax. The net operating loss carryforwards expire through 2037. The deferred tax assets associated with these net operating loss carryforwards of $10,844,000 and $6,802,000 at August 31, 2018 and 2017, respectively, are offset by valuation allowances on the accompanying consolidated balance sheets of $10,844,000 and $6,802,000, respectively. The total net operating loss carryforwards at August 31, 2018 and 2017, were $33,113,000 and $16,938,000, respectively. NMHC calculates income taxes for its taxable subsidiaries. Taxable income differs from pretax book income principally due to certain income and deductions for tax purposes being recorded in the consolidated financial statements in different periods. Deferred income tax assets and liabilities are recorded for the tax effect of these differences using enacted tax rates for the years in which the differences are expected to reverse. In assessing the realizability of deferred tax assets, management considers whether it is more likely than not that some portion or all of the deferred tax assets will not be realized. The ultimate realization of deferred tax assets is dependent on the generation of future taxable income during the periods in which those temporary differences become deductible. The Cayman Islands government does not impose any tax on income or capital gains. However, such corporations are subject to U.S. federal corporate taxation to the extent that they generate net income that is effectively connected with a U.S. trade or business. These corporations were not engaged in any such trade or business in the U.S. during fiscal year 2018 or 2017. Therefore, no income tax provision has been recorded related to these corporations and their operations. Provisions for federal and state income taxes of $6,028,000 and $13,010,000 for the years ended August 31, 2018 and 2017, respectively, are included within Other in Nonoperating gains (losses) in the accompanying consolidated statements of operations and changes in net assets.ExplanationTxt

Schedule J, Part II, Column (B)(ii) BONUS & INCENTIVE COMPENSATIONThe bonus and incentive compensation amounts listed in Column b(ii) for all listed individuals were determined using a specified formula. This formula and the calculation of their annual bonus is based on two components: the employee's title/position (staff, manager, director, VP, etc.) and the degree to which established performance goals were achieved. Incentive compensation amounts are at risk and are not paid unless there is exceptional individual and organizational performance in accordance with substantial pre-approved goals.
Schedule J, Part I, Line 3 Arrangement used to establish the top management official's compensationThe filing organization relied on Northwestern Memorial HealthCare, a related tax exempt organization, which used a compensation committee, comparability data, independent compensation consultant, and approval by the board or compensation committee, to determine the compensation of the President of Northwestern Medicine West Region, who is an employee of Northwestern Memorial HealthCare and who also serves as the Chairperson of the filing organization's Board of Directors. See the narrative for Form 990, Part VI, line 15a in Schedule O for more information.
