Group St. Josephs Hospital of Buckhannon Line Part V, Section B, Line 5Input for the completion of the initial Community Health Needs Assessment CHNA was obtained from public health officials, local health department and agencies, publicly elected officials, religious officials, long-term care executives, and representatives of or members of chronic disease groups and organizations. In early 2016 St. Josephs Hospital SJH conducted a CHNA centered around community engagement and input. For the 2016 CHNA update SJH contracted with the WVU School of Public Health to help with the process. The resulting report and implementation plan can be found online at https//wvumedicine.org/st-josephs-hospital/wp-content/uploads/sites/15/2016/12/SJHCHNA2016Final.pdf
Group St. Josephs Hospital of Buckhannon Line Part V, Section B, Line 11Based on the CHNA results, the leadership team chose two priority areas which they felt SJH could adequately target and address through an implementation plan. These areas were 1. Access to Wellness Opportunities and 2. Obesity/Diabetes. Additionally, it was a goal of the SJH Leadership Team to incorporate sensitivity to issues of limited transportation and poverty, especially children in poverty, into the overall CHNA. Complete details of the prioritization process and findings are available within the CHNA report at https//wvumedicine.org/st-josephs-hospital/wp-content/uploads/sites/15/2016/12/SJHCHIP2016Final.pdf
Group St. Josephs Hospital of BuckhannonPatients with no insurance coverage are considered self-pay patients. Those patients are automatically given a 50 uninsured discount from gross charges, based on commercial insurance average.
Group St. Josephs Hospital of BuckhannonAs of 10/01/2015, St. Josephs Hospital SJH transferred sponsorship from Pallottine Health Systems, Inc. to United Hospital Center, a founding member of West Virginia United Health System, Inc.. With this transfer, SJH changed from a fiscal year-end to a calendar year-end. The previous CHNA was completed during the 2013 calendar year. With the sponsorship transfer and new year-end, SJH completed its most recent CHNA in March 2016.
Group St. Josephs Hospital of Buckhannon Line Part V, Section B, Line 11During the 2012-2013 CHNA the top five priorities identified through public input were 1. Drug abuse by adults, 2. Drug abuse by youths, 3. Obesity, 4. Diabetes, and 5. Cancer. These issues were grouped together as 1. Drug abuse, 2. Obesity/Diabetes, and 3. Cancer. Leadership Teams were divided into two groups for additional discussion. It was decided that St. Josephs would focus on Access to Wellness Opportunities and Obestiy/Diabetes. Leadership Team members felt that the health issues of diabetes and obesity were intertwined and could very likely be addressed using complementary interventions.
Group St. Josephs Hospital of Buckhannon Line Part V, Section B, Line 24Access to wellness opportunities was identified as a highly ranked issue that leadership team members felt they had the capacity to address through the implementation planning process. Substance abuse was recognized to be a top community health priority by all three data prioritization activities. At the same time, there were limited secondary data to describe or further investigate the issue in a county wide fashion. The Leadership Team felt St. Josephs may have minimal control over the issue and thus would not choose to prioritize it for purposes of this report.
Group St. Josephs Hospital of Buckhannon Line Part V, Section B, Line 24All patients are charged an amount equal to gross charges regardless of payment method. Once FAP eligibility is confirmed, charge amounts are moved to Charity Care and no longer charged to the patient.

Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Part I Line 3cSt. Josephs uses 200 Federal Poverty Guideline FPG to determine free care eligibility. However, St. Joesphs does not offer discounted care to individuals who fail the 200 FPG test.
Part I Line 6aSt. Josephs community benefit numbers are reported in total with the other hospitals within West Virginia United Health System. The 2018 amounts can be found at the following web address https//wvumedicine.org/about/community-benefit/.
Part I Line 7Total community benefit expense for 2018 is 14,718,899 and is 29.34 of total net expenses. To calculate net expense, bad debt of 2,983,166 was deducted from total expenses of 53,151,586 as shown in Part IX line 25 of the core Form 990, for a net expense of 50,168,420.
Part I Line 7Worksheet 2 from the IRS Schedule H instructions was used to derive the Cost-to-Charge ratio, which was used to calculate Charity Care, Unreimbursed Medicaid and other means-tested government programs at cost.
Part IISt. Josephs reported Community Building activities of 28,201 for 2018. For 2019 St. Josephs will begin utilizing a tracking program to gather both Community Benefit and Community Building activites.
Part III Line 2Bad Debt Expense at cost was calculated by multiplying bad debt expense of 2,983,166 by our cost-to-charge ratio of 42.28 derived from Worksheet 2 in the IRS Schedule H instructions for a total of 1,261,283.
Part III Line 3Estimated bad debt attributable to patients eligible for charity care was calculated by running a report within our patient revenue software of all bad debt account balances greater than 15,000. The total of that report was 87,669 which we then multiplied by our cost to charge ratio of 42.28 for a total of 37,066.
Part III Line 3The estimated bad debt attributable to patients eligible for charity care of 37,066 should be considered community benefit due to the fact that anyone with outstanding balances of 15,000 or greater usually qualifies as catastrophic if the patient completes the application process.
Part III Line 3In our charity care policy, we define catastrophic care as any illness or injury that will likely require continuous or frequent treatment for more than one year. Regardless, that bad debt should be considered community benefit as we provide services to those in need regardless of their ability to pay.
Part III Line 4St. Josephs Hospitals financial statements are prepared on a consolidated basis as a member of the WV United Health System. The footnote for Accounts Receivable is as follows Patient accounts receivable are reported at net realizable value. For receivables associated with services provided to patients who have third-party coverage, the System estimates net realizable value based on the estimated contractual reimbursement percentage, which in turn is based on current contract provisions and historical paid claims by payor. For self-pay accounts, including uninsured and patient responsibility accounts, the net realizable value is determined using historical collection experience, adjusted for estimated conversions of patient responsibility portions, expected recoveries and changes in trends to estimate implicit price concessions in 2018 or the provision for bad debts in 2017. Management continually reviews the estimated net realizable value of accounts receivable by monitoring cash collections, economic conditions and trends, changes in payor mix, changes in federal or state healthcare coverage and other matters. The System grants credit without collateral to its patients, most of whom are local residents and are insured under third-party payor agreements, primarily with Medicare, Medicaid, and various commercial insurance companies. The System records accounts receivable net of estimated price concessions in 2018 and allowances for bad debts in 2017 and such amounts have historically been within managements expectations. The mix of accounts receivable at December 31, 2018 and 2017, from patients and third-party payors is as follows. For 2018, Medicare 29, Medicaid 18, Blue Cross 24, Commercial/Managed Care/Other 26, Patients 3, Total 100. For 2017, Medicare 29, Medicaid 14, Blue Cross 21, Commercial/Managed Care/Other 32, Patients 4, Total 100.
Part III Line 4For receivables associated with self-pay patients, which includes both patients without insurance and insured patients with deductible and copayment balances, the System records a significant provision for bad debts in the period of service on the basis of its past experience, which indicates that many patients are unable or unwilling to pay the portion of their bill for which they are financially responsible. The difference between the billed rates and the amounts actually collected after all reasonable collection efforts have been exhausted is charged off against the allowance for doubtful accounts.
Part III Line 8The amount reported in Part III Line 6 19,318,750 was calculated using the total Medicare allowable cost from the Medicare cost report.
Part III Line 9bSt. Josephs does have a debt collection policy. When a patient has been approved for financial assistance under our charity care policy, they will not be sent to bad debt. Additionally, if a patient is being evaluated for charity, the patient will not be sent to bad debt agency pending charity guarantor status until the pending status has been finalized approved/denied. All other patients with outstanding balances will be processed through billing and collections pursuant to our Financial Policy.
Part VI Line 2St. Josephs Hospital has contracted with the West Virginia University School of Public Health to perform the community wide needs assessment survey and has translated this information into the Community Health Needs Assessment Report which outlines actions taken. In addition, an outside company performs post admission surveys for all inpatients. This survey includes questions regarding whether the patients needs were met during the stay, and the hospital reviews this HCAHPS data to determine what services may need to be added based on that data.
Part VI Line 3St. Josephs Hospital of Buckhannon will advise patients and their families of financial assistance through the following means direct patient contact, in person or over the phone notice of availability of financial assistance will be posted in each registration area and emergency department and other waiting areas notice of availability of financial assistance will be printed with each patient initial bill availability of financial assistance will be printed on applicable letters and statements notice of availability will be posted on the hospitals website published in additional languages if needed by more than 10 of the population outreach to community most likely to utilize the FAP to include outreach to public agencies and other non-for-profit agencies and availability of financial assistance will be posted annually in local paper.
Part VI Line 4St. Josephs Hospital is located in Buckhannon, a small rural town in a mountainous region of Upshur County, West Virginia. The hospital serves its home county, with a population of approximately 24,415 residents in 2018, as well as parts of five surrounding counties. In Upshur County, 21.2 of the population is below the poverty level. 9.4 of the Upshur population is uninsured, while 3.6 of the children under 19 years of age have no insurance coverage. The unemployment rate stands at 8.1. The following hospitals provide a full range of acute care, ancillary and case management services and many also provide long-term care. The hospitals are located in the immediate service area or the contiguous service area Stonewall Jackson Memorial Hospital, United Hospital Center, Davis Memorial Hospital, Louis A. The hospital has also been actively recruiting both primary care providers and specialists to meet the healthcare needs of our service area.
Part VI Line 6St. Josephs Hospital of Buckhannon, Inc. is a not-for-profit health care facility wholly owned and sponsored by United Hospital Center, Inc. UHC. United Hospital Center is a subsidiary of the West Virginia United Health System. The West Virginia United Health System is comprised of eight hospitals in 2018 including Camden Clark Medical Center, Berkeley Medical Center, Jefferson Medical Center, Potomac Valley Hospital, Reynolds Memorial Hospital, St. Josephs Hospital, United Hospital Center and West Virginia University Hospitals. St. Josephs Hospital is committed to the core values of compassion, hospitality, reverence, interdependence, stewardship, and trust.
Part VI Line 6St. Josephs is a part of the WV United Health System WVUHS. WVUHS is a not-for-profit corporation formed to serve as part of an integrated health science and healthcare delivery team. WVUHS serves as the parent corporation to an affiliated group of healthcare providing entities.
