| Primary Exempt Purpose | The Hepatitis C Mentor and Support Group (HCMSG) was founded to address the lack of education awareness, support, and services for people living with Hepatitis C, including patients Co-infected with other conditions such as HCV/HIV, and patients in need of or living with liver transplants. To address these needs, we provide resources and services to foster the development and operation of successful support groups for Hepatitis C and Co-infected patients. These services are provided to prospective and current support group facilitators free of charge. We also provide one-on-one mentoring services to Hepatitis C and liver transplant patients. Right now, a revolution in the treatment of Hepatitis C is underway. New generation (DAA's) direct-acting antivirals have recently become available, and an even more promising new class of drugs are in development and testing. These drugs offer the promise of a cure for individual patients, and the ability to stem an emerging healthcare crisis for | PrimaryExemptPurposeTxt |

| Description Program Srvc Accom | In 2018, HCMSG facilitated trainings for hundreds of individuals in dozens of cities across the United States. Collaborations were established with reputable institutions including Bronx Lebanon Hospital, Evergreen Health and Erie County Medical Center in Buffalo NY, John Hopkins, PCORI site, Baltimore MD., University of Miami IDEA SE project, Miami FL,ekiM for change Syringe Exchange-Greenville NC Pitt County, St. Anns- SACHR, Washington Corner Project in NY., HCMSG and Street Works collaborated -Nashville, TN to implement trainings with other community organizations and the DOH. Additionally, HCMSG facilitated Healthcare Provider training on HCV at the Collaboration of Care Conference in Greenville, NC, which addressed integrated healthcare and featured various organizations in attendance. Testing and education events were undertaken with Positively Living in Knoxville TN, Scott County and Campbell County drug court using HCMSGs materials. HCMSGs Healthcare provider audio is now on AETCs website and was presented to doctors and providers at Montefiore telehealth Project Inspire program. Importantly, we have seen an uptick in participation not only in our in-person trainings and information sessions, but also in our Tele-Support resources, which enable individuals to access services electronically.Gaining the trust of high risk communities such as, former and current injection drug users, LGBTQ, co-infected with HIV, young adults and women of child bearing age, by showing them empathy, understanding, support and education. We must do this in a positive light or we will never gain their trust and therefor never be able to gain a handle on the disease of substance use disorder and Hepatitis C which go hand in hand. By gaining their trust and honestly caring about their health, we are better able to address the injection drug user on their immediate health issues, and then treatment appropriate for their substance use disorder. This is harm reduction, reduce the harm by supporting and empowering people who use drugs to reach their full potential. We have to recognize that this is a process and a difficult goal to reach. Any positive change helps reduce the persons risk of getting Hepatitis C or HIV.HCMSG modifies its programming to address each stage of the treatment care cascade continuum through frequent trainings, information dissemination and strategic partnerships. We attend community task force meetings and work with the Department of Health in each state to bring together the community. Our target audiences have been: healthcare providers, primary doctors, infectious disease doctors, GIs, residents, medical students, nurses, social workers, counselors, patient peers and staffs of organizations.HCMSG has made a strong effort to evaluate programming so as to best understand the needs of its service population. Evaluations are collected at the end of all sessions that HCMSG conducts, which in turn aids us in maintaining a certain level of programming. Attendees are asked to provide feedback as to which parts of the training were most helpful and least helpful, thereby allowing HCMSG to better understand its strengths and weaknesses. Furthermore attendees are provided a space in which they can voice suggestions concerning how training sessions can be modified or what issues and topics should be added or omitted to the manual and group discussion. This ensures that attendees are equipped with the best possible tools when managing their respective communities. We encourage active participation and allow time for Q&A. HCMSG stays in contact with attendees and conducts tele support when needed. Pictures of some of these sessions are available. For the past seven years, HCMSG has brought much needed education and supportive services to one of the countrys most diverse patient populations, including Baby Boomers, those co-infected with HIV, substance use communities, the LGBTQ community, youth, and women of child-bearing age. To date, HCMSG has trained over 1,000 facilitators, who have in turn touched tens of thousands of lives across the United States. HCMSG has filled a niche and therefore continues to expand our programs. We stay in contact with our attendees and in some cases follow up through the use of our tele-support program. As we traveled the country, we learned to tailor our trainings to fit each organization and individual as needed. However, what we found in common, whether it be in urban or rural settings, is that if people are treated with respect and given the power through education and support to make healthy decisions, in many cases it can lead to the eventual stabilization of their lives. | DescriptionProgramSrvcAccomTxt |

| Facts And Circumstances Test | Current Year Facts and Circumstances: The Hepatitis C Mentor and Support Group (HCMSG) meets the facts and circumstances test as follows:The organization provides services directly for the benefit of the general public on a continuous basis:The Hepatitis C Mentor and Support Group (HCMSG), founded in 2011 by a former patient and advocate, in order to fill gaps in services for individuals living with Hepatitis C and Co-infection HCV/HIV. Through our flagship program, the Hepatitis C Education and Support Group Assistance Program, HCMSG conducts a diverse array of programming geared towards enhancing the degree to which patients and healthcare providers across the United States can manage and treat Hepatitis C and Co-infection HCV/HIV. We are one of the foremost organizations providing services encompassing education, information about testing, linkage to care, adherence, and post-treatment care for both patients and healthcare providers. The Hepatitis C Education and Support Group Assistance Program has two overarching objectives:1.Deliver a treatment care cascade continuum to patients, in which education, regionally-specific testing, linkages to care, adherence, and post-treatment care facilitate better national health outcomes and create a better quality of life for patient populations affected by HCV and Co-infection HCV/HIV.HCMSG modifies its programming to address each stage of the treatment care cascade continuum through frequent trainings, information dissemination and strategic partnerships geared towards enabling patients to overcome barriers to care, education, and support. HCMSGs trainings cover education and supportive services at all stages for patients, including pre-diagnosis, diagnosis, pre-treatment, the moment a treatment decision is made, the wait for treatment, during treatment and post treatment.Broadening our outreach capabilities has been a vital component of the realization of this goal, as our services are increasingly targeted towards diverse communities such as youth, Baby Boomers, women of child-bearing age, those affected by substance use, and the LGBTQ community. This requires not only a high volume of trainings conducted across the United States, but also a leveraging of local and national partnerships to collaborate and fill gaps in services. 2.Educate healthcare providers on the best practices when working with diverse patient populations affected by HCV and Co-infection HCV/HIV. HCMSG has been asked to add trainings geared towards healthcare providers, and not simply patients. Recognizing best practices in patient relations is a vital part of the management and treatment of Hepatitis C. The Hepatitis C Education and Support Group Assistance Program has therefore adopted an approach directed towards training and educating providers who administer treatment. This includes training, not only on how to go about pursuing different avenues for treatment, but also on how to strengthen doctor-patient relations between providers and diverse patient populations. 1.Deliver a treatment care cascade continuum to patients, in which education, regionally-specific testing, linkages to care, adherence and post-treatment care facilitate better national health outcomes and create a better quality of life for patient populations affected by HCV and Co-infection HCV/HIV.Aspects of the Hepatitis C Education and Support Group Assistance Program are core program activities that fulfill this objective. Recent program activities include:The organization and establishment of trainings across the United States through in-person instruction or through Tele-Support training.Increased communication and partnership with experts in the field, which allows HCMSG to leverage their expertise for national trainings and create a tight network through which best practices, information and resources can be shared.Delivering information digitally through our social media platforms and website about how to obtain accurate information about the illness and navigate insurance issues. The addition of a Harm Reduction Consultant, who is planning to facilitate programming in locations like Florida and North Carolina, which will offer testing, linkages to care and educational support groups under HCMSGs umbrella and using HCMSG materials. She will also be engaging in broader health and non-medical activities, such as, in North Carolina in collaboration with the North Carolina Harm Reduction Coalition, being provided with naloxone and clean syringes.Increased partnerships with local, state, and national organizations that work with HIV positive populations, in order to integrate HCV-related programming into their work.Issuing the HCMSG Patient Support Newsletter, which prepares patients by providing them with types of questions to ask their medical providers, definitions of medical terms, information on how to read lab reports, and resources such as patient assistance programs, and up-to-date information on new and upcoming treatments delivered by medical providers from leading New York hospitals.2.Educate healthcare providers on the best practices when working with diverse populations affected by HCV and Co-infection HCV/HIV. Other aspects of the Hepatitis C Education and Support Group Assistance Program are geared towards healthcare providers. These services include:Meetings and conference calls with medical providers and support group facilitators to share information, resources and best practices.Educational training for healthcare providers on best practices for working with our service population, including diverse groups such as communities affected by substance use, the LGBTQ community, youth, and women of child-bearing age.The development of partnerships with local, state and national organizations, as well as prominent professionals in the field in order to disseminate best practices for medical providers. The organizations governing body is comprised of individuals with specialized knowledge and expertise in the field in which the organization operates and who represent the broad interests of the public:The board of directors consists of:Carl Deddens:He is Managing Partner, Maplewood Advisors. Management and financial consultants specializing in performance enhancement and growth strategies. He is a former Hepatitis C patient. From 2000-2006 Mr. Deddens served as president of the Board of the Greater New York Chapter of the American Liver Foundation, a non-profit organization dedicated to facilitating, advocating and promoting education, support and research for the prevention, treatment and cure of liver disease.
