Group Camden-Clark Memorial Hospital Line Part V, Section B, Line 5The CHNA process began with a thorough review of the previous cycles needs assessment priorities and included review of publicly available secondary data related to counties within Camden-Clark Memorial Hospital Corporations service area. This data included US Census data and County Health Rankings Data, as well as 2021 MATCH data. Using these reports as a springboard, the leadership team started the initial discussion around health needs, changes in the hospital community demographics over the previous few years, and lessons learned from the last CHNA cycles health topic prioritization and development of implementation activities. Primary data collection was comprised of surveying community members in a broad, combined service area to gather perceptions of health issues, quality of life, and other related topics.
Group Camden-Clark Memorial Hospital Line Part V, Section B, Line 5Two primary sources of data were utilized to inform the CHNA 1 a public input survey and 2 a data presentation to leadership and community partners. The public input survey was developed by WVU Health Affairs with the Camden Clark, Jackson General, and Wetzel County Hospitals CHNA leadership team following an iterative process. Survey topics included questions about specific health issues, thoughts on overall health of the community, quality of life, access to care, and medical needs including specialist care, risky behaviors, and demographic information including geographic location and income. To hear public feedback, data was collected anonymously from residents aged 18 and older. The survey was disseminated via a web-based platform called Qualtrics. Collection efforts included dissemination via MyChart, and reached local residents via social media, other local information channels, and with assistance from hospital partners and civic organizations. The survey was not intended to be a representative, scientific sample of residents of these twelve counties, but rather a mechanism to solicit the communitys perception of health needs. A total of 3,217 surveys were completed by community members from the area. The following organizations were represented on Camden Clark, Jackson General, and Wetzel County Hospitals CHNA leadership team and provided detailed input throughout the process of developing the CHNA. These individuals informed the public input survey and were instrumental in dissemination. Additionally, they identified and invited attendees to the community meeting, and were charged as a group with collaborative discussion of primary and secondary data to determine health priorities. These organizations included Camden Clark Medical Center, Jackson General Hospital, Wetzel County Hospital, Westbrook Health Services, Coplin Health System, Minnie Hamilton Health System, Integrated Health Centers of West Virginia, Mid-Ohio Valley Chamber of Commerce, Mid-Ohio Valley Health Department, United Way Alliance of the Mid-Ohio Valley, and Mid-Ohio Valley Rural Health Alliance.
Group Camden-Clark Memorial Hospital Line Part V, Section B, Line 11Following the community meeting, the leadership team discussed all input together and identified priority areas to address through implementation strategies. Leadership reviewed closely the top concerns raised across all categories including health and disease, quality of life, environment, and risky behaviors. The existing secondary data, new survey data, data presentation input, and lessons learned from the last CHNA cycle all factored into Camden Clark, Jackson General, and Wetzel County Hospitals leadership prioritization decisions. The group considered the degree to which the hospital can affect health outcomes for each topic, projects, or programs already underway for each, as well as level of importance placed on each by the community relative to the reach of Camden Clark, Jackson General, and Wetzel County Hospitals impact. Leadership paid particular attention to the demographics of respondents compared to the community SUD response by county where hospitals sit, feedback gathered about access to health care, substance use treatment feedback, as well as other areas and demographics they know to be of particular concern in their community. Within this discussion of where and how the hospital can realistically affect community wellbeing and health outcomes, what they could accomplish in collaboration with new and existing partners, and what topics to monitor and revisit later, the list of top health concerns was organized into areas that will be the focus moving forward with implementation planning. Potential community resources were identified such as affordable exercise programming, dietitian services, healthy eating program, low-cost labs and screening, weight loss clinic, contributions to youth-centric community or school-based activities, contributions or participation in community events, contributions to or support of other organizations efforts, community education such as breastfeeding and childbirth classes and dinner with a doctor, and lastly prevention methods such as health and wellness days and vascular lab screenings.
Group Camden-Clark Memorial Hospital Line Part V, Section B, Line 13bCamden-Clark Medical Center offers discounted care only to individuals who qualify for free care. Camden Clark Medical Center offers a Prompt Pay Discount Program. Patients who have no third party coverage uninsured or uninsurable may be eligible for a 50 self-pay discount while patients who have a guarantor balance may be eligible for a 20 discount. To be eligible for either prompt pay discount, the patient/guarantor must meet certain requirements.
Group Camden-Clark Memorial Hospital Line Part V, Section B, Line 15eA state Medicaid representative was on site to help determine eligibility and help patients through the eligibility process.
Group Camden-Clark Memorial Hospital Line Part V, Section B, Line 24All patients are charged an amount equal to gross charges regardless of payment method. Once FAP eligibility is confirmed, charge amounts are moved to Charity Care and no longer charged to the patient.

Part I Line 3cCamden-Clark Medical Center CCMC uses 200 Federal Poverty Guideline FPG to determine free care eligibility. However, CCMC does not offer discounted care to individuals who fail the 200 FPG test.
Part I Line 6aCamden-Clark Memorial Hospital Corporation, community benefit numbers are reported in total with other hospitals within the West Virginia United Health System. The most recent amounts can be found on WVU Medicines website here https//wvumedicine.org/about/leadership-and-more/community-benefit/
Part I Line 7Net community benefit expense for 2024 is 55,206,602 and is 11.49 of total net expenses. To calculate net expense, provision for doubtful accounts - patient accounts of 23,173,484 was deducted from total expenses of 503,369,528 as shown in Part IX line 25 of the core Form 990, for a net expense of 480,196,044.
Part I Line 7Worksheet 2 from the IRS Schedule H instructions was used to derive the Cost-to-Charge ratio, which was used to calculate Charity Care, Unreimbursed Medicaid and other means-tested government programs at cost.
Part IICamden-Clark Medical Center CCMC started a leadership program in 2007. Employees who are selected to participate in this leadership program are required to volunteer in the community or at hospital activities which benefit the broader community as a whole. The activities these individuals participated in during the current year included various health fairs, church community events, Wood County Telethon, National Healthcare Decision Day, Try-Again Homes work project and various local school activities and events. These individuals also worked with and supported various community organizations such as United Way of America, The Salvation Army, Boys and Girls Club of America, and various community boards.
Part III Line 2Bad Debt Expense at cost was calculated by multiplying provision for doubtful accounts - patient accounts of 23,173,484 by our cost to charge ratio of 29.18 derived from Worksheet 2 in the IRS Schedule H instructions for a total of 6,762,023.
Part III Line 3Estimated bad debt attributable to patients eligible for charity care was calculated by running a report within our patient revenue software of all bad debt account balances greater than 25,000. The total of that report was 2,108,961 which we then multipled by our cost to charge ratio of 29.18 for a total of 615,395.
Part III Line 3We feel that, at a minimum, the estimated bad debt attributable to patients eligible for charity care of 615,395 should be considered community benefit due to the fact that a patient with an outstanding balance of 25,000 or greater usually qualifies as catastrophic if the patient completes the application process. In our Charity Care policy, we define catastrophic care as any illness or injury that will likely require continuous or frequent treatment for more than one year.
Part III Line 4Patient accounts receivables are primarily paid by federal and state governmental authorities under the Medicare and Medicaid programs, managed health plans, commercial insurance companies, workers compensation programs, employers and patients. Patient accounts receivable are reported at net realizable value. For accounts receivable associated with services provided to patients who have third-party coverage, the System estimates net realizable value based on the estimated contractual reimbursement percentage, which in turn is based on current contract provisions and historical paid claims by payor. For self-pay accounts, including uninsured and patient responsibility accounts, the net realizable value is determined using historical collection experience, adjusted for estimated conversions of patient responsibility portions, expected recoveries and changes in trends to estimate implicit price concessions. The System does not believe there are any significant concentrations of revenues from any particular payor that would subject the System to any significant credit risks in the collection of patient accounts receivable. Management continually reviews the estimated net realizable value of accounts receivable by monitoring cash collections, economic conditions and trends, changes in payor mix, changes in federal or state healthcare coverage and other matters. Changes in general economic conditions, patient accounting service center operations, payor mix, payor claim processing could affect collections of accounts receivable, cash flows and results of operations.
Part III Line 4The System performs periodic assessments to determine if an allowance for expected credit losses is necessary. The System considers its incurred loss experience and adjusts for known and expected events and other circumstances. In estimating its expected credit losses, the System may consider changes in the length of time its receivables have been outstanding, changes in credit ratings for payors, requests from payors to alter payment terms due to financial difficulty, and notices of payor bankruptcies or payors entering receivership. Because the Systems accounts receivable is typically paid for by highly solvent, creditworthy payors, such as Medicare, Medicaid, other governmental programs, and highly regulated commercial insurers on behalf of the patient, the Systems credit losses are immaterial to the consolidated financial statements.
Part III Line 4The mix of accounts receivable at December 31, 2024 and 2023, from patients and third-party payors is as follows For the year-end December 31, 2024 Medicare - 22, Medicaid - 11, Blue Cross - 24, Commercial, managed care and other - 40, Patients - 3, totaling 100. For the year-end December 31, 2023 Medicare - 25, Medicaid - 13, Blue Cross - 21, Commercial, managed care and other - 39, Patients - 2, totaling 100.
Part III Line 8The amount reported in Part III Line 6 261,421,367 was calculated using the total Medicare allowable cost from the Medicare cost report, less Medicare reimbursement of direct GME.
Part III Line 8CCMCs shortfall, 108,783,739 of Medicare program reimbursement should be considered a community benefit because we are relieving a government burden by providing care in excess of our costs to these patients.
Part III Line 9bCCMC does have a debt collection policy. When a patient has been approved for financial assistance under our charity care policy, they will not be sent to bad debt. Additionally, if a patient is being evaluated for charity, the patient will not be sent to a bad debt agency pending charity guarantor status until the pending status has been finalized approved/denied. All other patients with outstanding balances will be processed through billing and collections pursuant to our Financial Policy.
Part VI Line 2Camden Clark Medical Center considers several components in assessing how the organization determines the need of the communities it serves. The CHNA utilized a robust community based process designed to engage a wide range of community members. CCMC conducted a Community Health Needs Assessment CHNA in 2024. The CHNA can be found here https//wvumedicine.org/wp-content/uploads/2025/01/ccmcjghwch2024communityhealthneedsassessmentbodapproved12032024.pdf.

CCMC also has representatives that are available to discuss options with inpatients and outpatients on a daily basis. WV State Medicaid workers are onsite to help determine patient eligibility and to assist patients through the eligibility process.
Part VI Line 4CCMCs leadership defined the community served as a 7 county area comprised of Calhoun, Pleasants, Ritchie, Roane, Tyler, Wirt, and Wood in WV. CCMC, located in Parkersburg, WV is a part of Wood County. Wood County has a population of 83,052, median household income of 54,350 and an employment rate of 55.1. 18.7 of Wood County residents are living in poverty and 7.8 of residents reported they are without health care coverage. The remaining 6 counties considered to be in CCMCs defined service area had a combined total population of 48,216 and approximately 7.3 of residents reported they are without health care coverage. Population and demographic information was obtained from the United States Census Bureau https//data.census.gov/cedsci/.
Part VI Line 5A majority of the organizations governing body is comprised of persons who reside in the primary service area and who are neither employees nor independent contractors of the hospital, not family members thereof. The organization extends medical staff privileges to all qualified physicians in the community to many of its departments.
