DescBraxton County Memorial Hospital offers primary care services from qualified physicians located in our rural health clinic. Our convenient location enabled us to provide and service the healthcare needs of 14,380 men, women, and children in our county and surrounding areas in 2024. Primary Care Service assists in keeping patients up to date on preventive care but also if specialists are needed then BCMH offers many services that allow the patient to stay within the community. This is often a plus for a rural community with limited public transportation.Desc

Group Braxton County Memorial Hospital, Inc Line Part V, Section B, Line 5The 2024 CHNA process began with a review of neighboring Davis Health Systems 2022 CHNA to determine health priorities in the area for these communities. It also included a review of publicly available secondary data related to Nicholas and Braxton counties including US Census data, County Health Rankings Data, and select Mountain State Assessment of Trends in Community Health data from Nicholas and Braxton counties. Using this information as a springboard, the leadership team discussed critical health needs, changes in the community over the previous few years and with the recent administrative changes to area hospitals, and lessons learned from the last CHNA cycles health topic prioritization and development of implementation activities. The team discussed this data alongside known health issues in the community, experiences at their own clinical and other professional settings, resources already in place to address some of the top issues, and initial thoughts about future addressing the most prevalent community concerns. The following organizations were represented on the CHNA leadership team and provided input throughout the process of developing the CHNA Braxton County Memorial Hospital, Summersville Regional Medical Center, Braxton County Health Department, and Nicholas County Health Department.
Group Braxton County Memorial Hospital, Inc Line Part V, Section B, Line 6aBraxton County Memorial Hospital BCMH and Summersville Regional Medical Center SRMC have chosen to complete the 2024 CHNA portion of this process in tandem. Adjacent service areas and shared hospital administration informed the decision to complete the needs assessment portion of this requirement as a single joint effort. Each hospitals Board of Directors conducted independent reviews of the CHNA. In addition to the requirement to conduct a CHNA, leadership of both hospitals continually expressed the desire to go beyond regulatory requirements in serving patients and the community at large. To facilitate this goal, the pair partnered with West Virginia Universitys Office of Health Affairs WVU OHA to complete this CHNA using a robust community based process designed to engage a broad representation of community members. A CHNA leadership team including hospital and community representation was convened by BCMH SRMC to inform and guide the process.
Group Braxton County Memorial Hospital, Inc Line Part V, Section B, Line 11The 2022 CHNA identified the following needs within the BCMH service area obesity and chronic disease, substance use and abuse, poverty, and tobacco and vaping. In 2024, BCMH participated in the following activities targeted at providing education, prevention, and support. Health Initiatives Hosting Rotary Blood Screenings twice a year, taking part in National Wear Red Day for heart disease awareness, Breast Cancer Awareness Month, Colorectal Cancer Awareness Day, and the Donate Life Hospital Challenge for organ donation awareness. They also hold an Annual Health Fair and Drive Through Flu Shot Clinic, offer low-cost lab work in Calhoun and Gilmer counties, and host Diabetes Support Group meetings in these areas. Safety and Youth Hosting a bicycle safety event for children, donating to the BCHS Football Team for safety equipment, sponsoring local school sporting events, and co-sponsoring the Winter Classic basketball tournament. Community Support Participating in the Elk River Summer Float with refreshments, sponsoring Christmas Spirit activities and Blood Drives, and coordinating community blood drives.
Group Braxton County Memorial Hospital, Inc Line Part V, Section B, Line 13bBraxton County Memorial Hospital BCMH offers discounted care only to individuals who qualify for free care. BCMH offers a Prompt-Pay Discount Program - patients who have no third party coverage uninsured or uninsurable may be eligible for a 50 self-pay discount while patients who have a guarantor balance may be eligible for a 20 discount. To be eligible for either prompt pay discount, the patient/guarantor must meet certain requirements.

Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Part I Line 3cBraxton County Memorial Hospital BCMH uses the 200 Federal Poverty Guideline to determine free care eligibility. BCMH offers a Prompt - Pay Discount Program - patients who have no third party coverage uninsured or uninsurable may be eligible for a 50 self-pay discount while patients who have a guarantor balance may be eligible for a 20 discount. To be eligible for either discount the patient/guarantor must meet certain requirements.
Part I Line 6aBraxton County Memorial Hospital community benefit numbers are reported in total with the other hospitals within West Virginia United Health System. The most recent report can be found at the following web address http//wvumedicine.org/about/leadership-and-more/community-benefit/.
Part I Line 7Column f - Net community benefit expense for 2024 is 158,066 and is 0.54 of total net expenses. To calculate net expense, patient bad debt of 1,732,162 was deducted from total expenses of 31,307,331 as shown in Part IX line 25 of the core Form 990.
Part I Line 7Worksheet 2 from the IRS Schedule H instructions was used to derive the cost-to-charge ratio, which was used to calculate Charity Care, Unreimbursed Medicaid and other means-tested government programs at cost. To calculate the Medicare allowable cost shown in Part III line 6, we used the allowable costs from the Medicare Cost report.
Part IINo Community Building Activies were tracked for 2024.
Part III Line 2Bad Debt Expense at Cost on Line 2 was calculated by multiplying bad debt expense of 1,732,162 by our cost-to-charge ratio of 50.90 for a total of 881,670
Part III Line 3Estimated bad debt attributable to patients eligible for charity care on Line 3 was calculated by pulling all outstanding patient balances of 5,000 or greater from our patient accounting system. That report totaled 42,304, which was then multiplied by the cost-to-charge ratio of 50.90 for a total of 21,533
Part III Line 3We feel that at a minimum, the estimated bad debt attributable to patients eligible for charity care of 21,533 should be considered community benefit due to the fact that anyone with outstanding balances of 5,000 or greater in our demographic usually qualifies as catastrophic if the patient completes the application process. In our Charity Care Policy, we define catastrophic care as any illness or injury that will likely require continuous or frequent treatment for more than one year and would have qualified for charity care if paperwork was completed.
Part III Line 4Patient accounts receivables are primarily paid by federal and state governmental authorities under the Medicare and Medicaid programs, managed health plans, commercial insurance companies, workers compensation programs, employers and patients. Patient accounts receivable are reported at net realizable value. For accounts receivable associated with services provided to patients who have third-party coverage, the System estimates net realizable value based on the estimated contractual reimbursement percentage, which in turn is based on current contract provisions and historical paid claims by payor. For self-pay accounts, including uninsured and patient responsibility accounts, the net realizable value is determined using historical collection experience, adjusted for estimated conversions of patient responsibility portions, expected recoveries and changes in trends to estimate implicit price concessions. The System does not believe there are any significant concentrations of revenues from any particular payor that would subject the System to any significant credit risks in the collection of patient accounts receivable. Management continually reviews the estimated net realizable value of accounts receivable by monitoring cash collections, economic conditions and trends, changes in payor mix, changes in federal or state healthcare coverage and other matters. Changes in general economic conditions, patient accounting service center operations, payor mix, payor claim processing could affect collections of accounts receivable, cash flows and results of operations.
Part III Line 4The System performs periodic assessments to determine if an allowance for expected credit losses is necessary. The System considers its incurred loss experience and adjusts for known and expected events and other circumstances. In estimating its expected credit losses, the System may consider changes in the length of time its receivables have been outstanding, changes in credit ratings for payors, requests from payors to alter payment terms due to financial difficulty, and notices of payor bankruptcies or payors entering receivership. Because the Systems accounts receivable is typically paid for by highly solvent, creditworthy payors, such as Medicare, Medicaid, other governmental programs, and highly regulated commercial insurers on behalf of the patient, the Systems credit losses are immaterial to the consolidated financial statements.
Part III Line 4The mix of accounts receivable at December 31, 2024 and 2023, from patients and third-party payors is as follows For the year-end December 31, 2024 Medicare - 22, Medicaid - 11, Blue Cross - 24, Commercial, managed care and other - 40, Patients - 3, totaling 100. For the year-end December 31, 2023 Medicare - 25, Medicaid - 13, Blue Cross - 21, Commercial, managed care and other - 39, Patients - 2, totaling 100.
Part III Line 8The amount reported in Part III Line 6 3,831,698 was calculated using the total Medicare allowable cost from the Medicare cost report.
Part III Line 8Braxton County Memorial Hospital is reimbursed 100 by Medicare reimbursement includes DSH and IME for services provided to Medicare patients.
Part III Line 9bBraxton County Memorial Hospital does have a debt collection policy. When a patient has been approved for financial assistance under our charity care policy, they will not be sent to bad debt. Additionally, if a patient is being evaluated for charity care, the patient will not be sent to a bad debt agency pending charity guarantor status until the pending status has been finalized approved/denied. All other patients with outstanding balances will be processed through billing and collections pursuant to our Financial Policy.
Part VI Line 2Braxton County Memorial Hospital considers several components in assessing how the organization determines the need of the communities it serves. Through feedback from stakeholders, outside consultants and collaborating with various departments, a needs assessment takes into account several factors including physicians to population ratios, physician availability in the entire service area, and general health risks that are perceived to be the highest priority by the stakeholders and hospital leadership. The service area for BCMH includes Braxton, Clay, Gilmer, Lewis, Nicholas, and Webster counties.
Part VI Line 3The West Virginia United Health System employs financial analysts to meet with patients at its hospitals to discuss financial eligibility for our charity care program. Braxton County Memorial Hospital has qualification requirements, as well as, applications for charity care available at registration areas. Patients that do not qualify for state or federally funded programs can apply for the charity care program at Braxton County Memorial Hospital.
