SCHEDULE D, PART V, LINE 4TEMPORARILY AND PERMANENTLY RESTRICTED ASSETS ARE INTENDED FOR FUNDING NEEDS BASED INSTITUTIONAL SCHOLARSHIPS.
SCHEDULE D, PART X, LINE 2The University is exempt from federal income taxes under section 501(c)(3) of the U.S. Internal Revenue Code (the "Code"). Any unrelated business income, as defined by Section 512(a)(1) of the Code is subject to income taxes. U.S. generally accepted accounting principles require the Universitys management to evaluate tax positions taken by the University and recognize a tax liability if the University has taken an uncertain position that more likely than not would not be sustained upon examination by the Internal Revenue Service. Management has analyzed the tax positions taken by the University, and has concluded that as of June 30, 2018, there are no uncertain positions taken or expected to be taken that would require recognition of a liability or disclosure in the financial statements. The University recognizes interest accrued related to unrecognized tax benefits in interest expense and penalties in other institutional expenses. There were no Interest or penalties recognized in either of the years ended June 30, 2018 or 2017. The University established the LAH subsidiary as a C-Corporation to ensure the University's tax exempt status under 501(c)(3) is not jeopardized. The Universitys federal Return of Organization Exempt from Income Tax (Form 990) for the years ended June 30, 2018, 2017 and 2016 are subject to examination by the IRS.

Form And Line ReferencePART I, LINE 2FormAndLineReferenceDesc
ExplanationPROCEDURES FOR MONITORING THE USE OF GRANT FUNDS Annually tuition discount expenditures are projected and budgeted for the upcomiNg budget cycle. Projections are prepared by the Director of Student Administrative Services, delivered to the Director of Budget and submitted for approval to the Executive Vice President of Finance and the Board of Trustees. Various constituencies across campus are "allocated" a certain amount of funding to award to students, within the budgeted amounts. These awards are submitted to the Executive Vice President of Finance for approval and then routed to the Director of Student Administrative Services who then, through the Director of Financial Aid, oversees the packaging of these awards and integration with other financial aid awards to insure that no students with federal financial aid are over-awarded. Since these awards are tuition-based, they are posted to students' accounts each quarter once schedules are locked and total tuition amounts can be verified by the Director of Student Accounts and staff. Quarterly, the Director of Student Administrative Services compares actual expenses to budgeted amounts and alerts the budget manager of any potential overages. The budget is reviewed in each case to make sure overages will not cause fiscal issues. If it appears that they will, adjustments are recommended by the Director of Budget to the Executive Vice President of Finance.ExplanationTxt

FORM 990 PART III LINE 4Athe college of chiropractic The Mission of the Life University College of Chiropractic, centered on the Vertebral Subluxation Complex, is to educate, mentor and graduate skilled and compassionate Doctors of Chiropractic to be primary care clinicians, physicians, teachers, and professionals, using the Universitys core life proficiencies as their foundation. Program Activities DCP Curricular Revision Step 1: Adopt a curricular framework (completed) Step 2: Use the backward design model of curricular planning (ongoing throughout) Step 3: Review and validate competency definitions and levels of mastery Workshops with DCP Curriculum Committee and administration Step 4: Plan and implement assessment strategies Step 5: Create the student learning journey Current Status: Step 3 of the plan was completed on October 5, 2018. A group that included the members of the DCP Curriculum Committee, Faculty Senate President, Clinic Directors, Division Chairs, Director of Clinical Education, and Associate Deans convened to review and validate competency definitions and levels of mastery that were created by the faculty curricular sub-groups. Next step is to review the information that was presented by the groups, edit for any gaps or omissions, and have the DCP Curriculum Committee to present to the DCP faculty for review and feedback (Winter 2019). The dean and VPAA will review timelines and reestablish contact with the educational consultants to determine what is needed to move into Step 4 of planning and implementing assessment strategies. DCP Completion Project The number of students identified for the Completion Project has been consistent between 30 and 61 students per quarter with 54.2% of the students registering for the recommended number of credit hours the following quarter. Students who are not able to continue the recommended schedule will be included in the project on an ongoing basis since they would continue to fall below the threshold of credit hours to complete the program in 21 quarters. There were 221 students identified in the project from Fall Quarter 2017 through Fall Quarter 2018. Of those 221 students, 120 (54.2%) are adhering to the academic plan provided to them by taking the number of credits the subsequent quarter to return to above 16 average credit hours and remain on track to graduate within 21 quarters. 56 (25%) of the 221 students complied with the plan by taking a similar credit load as outline in the academic planner and still have the possibility of graduating within 21 quarters. There were 42 (19%) students who were not able to follow the academic plan provided to graduate in 21 quarters but can still graduate from the program. Some of these students were not able to return to a normal load for personal or financial reasons, and three (1.3%) were no longer enrolled in the program. After four quarters of the project, a re-evaluation of the 46 students identified towards the beginning of the project, in Fall 2017, was conducted to determine their progression status as of Fall 2018. Of the 46 students identified, 22 (48%) were on track to graduate within the 21 quarters. 20 (43%) students will not be able to graduate in 21 quarters and 2 (4.3%) have accommodations for a reduced load; however, they can still graduate from the program. Two (4.3%) students were no longer in the program. Pre-Requisites Update: The DCP Pre-Requisites implementation was changed from Fall 2018 to Winter 2019 due to some course conflicts with student learning outcomes. All pre-requisites have been uploaded in Colleague, Student Planning, and Acalog for the Life Student and Community to view and utilize. The DCP prerequisite task force will monitor the changes effects on the students progression through the program. ChiroMissions Course: The DCP has created a course in collaboration with Peter Morgan to allow students to participate in mission trips for elective academic credit. This course will be an experience providing chiropractic services in an underserved community outside of the United States by interns in conjunction with licensed faculty/extension faculty doctors. The first group of students and faculty are scheduled to attend a mission trip in Haiti in March 2019. DCP Minority Student Groups: The DCP continues to meet with minority student organizations to determine how we can better serve them with resources (both academic and personal) to help support them in completing the program. The DCP administration meets with the student president of SABCA and the Hispanic Club quarterly. The DCP administration also attends SABCA meetings and Hispanic club A focus group was conducted with the African American Students in Summer 2018 to collect information on their experience in the program and determine if they were experiencing any challenges that were contributing to them not completing the program on time. 110 African American students attended and the DCP was able to extrapolate 8 themes based on the information collected. A task group has been created that meets weekly to create projects to address the themes. An Academic Support Person has been hired to also address the wants of the African American students and focus on first-year retention and academic support needs. Electronic Health Records Search for New Platform: The DCP Clinics created an RFP and sent it out to several different companies to let them determine if they would be considered a good EHR system for us to use. This was initiated based on the current platform (ChiroTouch) making major changes in their company and the changes are affecting how they do business with the chiropractic colleges. Four companies responded to the RFP and they were EZ Biz, ChiroTouch, NextGen, and PayDC. They have all been to the campus to demonstrate their EHR system for faculty, students, and administration. A recommendation will be coming forward from the Associate Dean of Clinics Winter 2019 with the pros and cons of each group to the senior administration to determine which company will best serve the needs of the clinics. College of Chiropractic Clinic System The mission of the Life University Clinic System is to improve the health and wellbeing of people in the communities we serve by providing chiropractic care and other health services, centered on the Vertebral Subluxation Complex. We will maintain excellence in the educational standards of our teaching clinics and encourage all constituents of our clinics to actively participate in their own health and wellness care, and to actively participate in health and wellness programs that reach out to others in our communities. Clinical operations incorporate a variety of chiropractic services provided to the general public inclusive of complementary services provided to a range of special populations. The clinic system is composed of several types of clinical experiences. The clinical education and patient care delivery are stratified into three levels. The educational goals and objectives are clearly spelled out for each level, and assessment tools developed to ensure that appropriate competencies are measured and outcomes utilized to improve the clinical program. The Life University Clinic System prepared and executed the implementation of the Electronic Health Records throughout the clinic system. The Electronic Health System was fully implemented throughout the clinic system in 2015. This implementation created a greater consistency for patient care and intern education. It also creates an almost paperless environment due to the limited use of paper forms. Level I Experience: The Level I clinic experience includes quarters one through nine and is where basic competencies are developing in an environment where there is close faculty supervision. The majority of patients in a Level I clinic are not expected to be complex cases. Level I Clinic: The Campus Center for Health and Optimum Performance provides health care services for the Life University student community and provided over 27,000 patient visits at no charge.
Cont'dThe Campus Center for Health and Optimum Performance 1269 Barclay Circle Marietta, GA 30060 Level II Experience: The Level II clinic experience includes quarters ten through twelve. Students continue to develop competency and critical thinking skills, and also focus on developing competency in patient management. Although competency development continues, the student now has access to a larger volume and variety of patients with a range of conditions. The outpatient facility provided for over 52,000 patient visits with 50% of the care provided at no charge. Level II Clinic: The Center for Health and Optimum Performance provides health care services to the general public in a fee-for-service environment. The Center for Health and Optimum Performance 1415 Barclay Circle Marietta, GA 30060 Level III Experiences: Level III clinic encompasses the final quarters of the clinical education program. In Level III the student experiences integration into broader health care environments. These experiences are provided through a variety of programs; these programs may include field doctors office experiences, international satellite clinics, and/or sporting events. All off campus clinical education experiences take place at sites that have agreements and/or signed contracts with the University. Life University also owns and/or operates two community outreach clinics in the metro Atlanta area that serve special needs populations. Level III Clinics: There are currently three programs operating under the Level III programs; they are Outreach, field practice, and International Clinics. Outreach: The outreach clinics provide health care services to special needs populations in the metro Atlanta area and provided over 12,000 patient visits at no charge. Due to the high volume of patient care within the Outreach clinic, it has outgrown its current facility and the college opened a new larger facility to handle the growth in order to continue to serve the public consistent with the overall mission and goals. Outreach Clinic Program i. Community Outreach Clinic 1323 Roswell Road Suite 300W Marietta, GA 30062 ii. Community Outreach Clinic - The Extension 1507 Church Street Marietta, GA 30060 The University continues to fulfill its initiative to expand its clinical experiences for students to both domestic and international locations through its Level III clinic programs. Qualified upper-quarter students have the opportunity to participate in qualified field experiences located in doctors offices in several domestic and international locations. Additional community service provided by clinic The clinical system continues to provide complimentary public screenings and/or educational programs each month. These programs are intended to provide the community with a greater understanding of chiropractic health care and to screen the community for the presence of vertebral subluxation. Positive feedback from participants provides direction for future locations and services.
