ExplanationHSHS and the Foundation are Illinois not-for-profit organizations as described in Section 501(c) (3) of the Internal Revenue Code (the Code) and are exempt from federal income taxes on related income pursuant to Section 501(a) of the Code. Kiara, Inc. is an Illinois for-profit corporation that recognizes income taxes under the asset-and-liability method. Deferred tax assets and liabilities are recognized for the future tax consequences attributable to differences between the consolidated financial statement carrying amounts of existing assets and liabilities and their respective tax basis and operating loss and tax credit carryforwards. Deferred tax assets and liabilities are measured using the enacted tax rates expected to apply to taxable income in the years in which those temporary differences are expected to be recovered or settled. The effect on deferred tax assets and liabilities of a change in tax rates is recognized in income in the period that includes the enactment date. In assessing the realizability of deferred tax assets, management considers whether it is more likely than not that some portion or all of the deferred tax assets will not be realized. The ultimate realization of deferred tax assets is dependent upon the generation of future taxable income during the periods in which those temporary differences become deductible. Management considers projected future taxable income and tax planning strategies in making this assessment. Based upon the level of historical taxable losses and projections for future taxable losses over the periods for which the deferred tax assets are deductible, management believes it is more likely than not Kiara, Inc. will not realize the majority of the benefits of these deductible differences. Full valuation allowances have been applied against the deferred tax assets attributable to the net operating loss carryforwards not realized as of June 30, 2018 and 2017 in the accompanying consolidated financial statements due to the uncertainty of realization. HSHS recognizes the tax benefit from an uncertain tax position only if it is more likely than not the tax position will be sustained on examination by the taxing authorities, based on the technical merits of the position. As of June 30, 2018 and 2017, HSHS does not have any liabilities for unrecognized tax benefits.ExplanationTxt

Schedule H, Part V, Section B, Line 3EYes, the significant health needs are a prioritized description of the significant health needs of the community and identified through the CHNA process.
Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - HSHS St. Joseph's Hospital Highland. In FY2018 (July 1, 2017 through June 30, 2018), HSHS St. Joseph's Hospital in Highland conducted its CHNA in partnership with representatives from the community. Upon completion of the CHNA, the hospital developed a set of implementation strategies and adopted an Implementation Plan to address priority community health needs. The population assessed was Madison County, Illinois. Data collected throughout the assessment process was supplemented with 2017 County Health Rankings & Roadmaps, University of Wisconsin's Population Health Institute, the Centers for Disease Control and Prevention (CDC), Behavioral Risk Factor Surveillance System Survey (BRFSS), and focus groups with key stakeholders in the community. Identification and Prioritization of Needs: The following health needs were identified based on scope, severity and urgency of the health need, the health disparities associated with the health need, the importance the community places on addressing the health need, and local community input. * Obesity * Mental health * Alcohol, tobacco, and other drug use * Access to care St. Joseph's Hospital led the planning, implementation and completion of the CHNA in partnership with Madison County Health Department, and Survey Research Office of University of Illinois Springfield. Internal St. Joseph's Hospital undertook a nine (9) month planning and implementation effort to develop the CHNA, identify and prioritize community health needs for its service area and formulate an implementation plan to guide ongoing population health initiatives with like-missioned partners and collaborators. These planning and development activities included the following internal and external steps: * The St. Joseph's Hospital senior leadership team (SLT) served as the executive steering committee for this CHNA * A review of the St. Joseph's Hospital 2015 CHNA report and implementation strategy documents (and related data) * The St. Joseph's Hospital Board of Directors, Foundation Leadership Council, and Advisory Council, while comprised of external community members, was engaged to assist with input on data collection and identification and prioritization of needs * A review of the hospital's existing community benefit activities * A review of the hospital's current and future strategic operations plans and goals * Development and implementation of stakeholder focus groups consisting of both internal and external constituencies that represented the broad interests of the hospital's service area to solicit input on data and identification and prioritization of needs External St. Joseph's Hospital also leveraged existing relationships that provided diverse input for a comprehensive review and analysis of community health needs in the hospital's service area. A primary component in this assessment was engaging the Madison County Health Department in soliciting their assistance in development of the public engagement survey and assistance in acquiring additional public health data through distribution of the hospital's public engagement survey to additional agencies/organizations that represented the broad public health interests located within the hospital's service area. In addition, St. Joseph's Hospital worked with the University of Illinois Springfield Survey Research Office to compile secondary data of the primary service area. Defining the Purpose and Scope The purpose of the CHNA was to 1) evaluate current health needs of the hospital's service area, 2) identify resources and assets available to support initiatives to address the health priorities identified, 3) develop an Implementation Plan to organize and help coordinate collaborative efforts impacting the identified health priorities, and 4) establish a system to track, report and evaluate efforts that will impact identified population health issues on an ongoing basis. Data Sources The CHNA process utilizes both primary data including hospital data, focus groups and key stakeholder meetings as well secondary data. Secondary data sources include Behavioral Risk Factor Surveillance System (BRFSS), the US Census Bureau, and Centers for Disease Control and Prevention (CDC) data sources. In addition, this data was supplemented with data from: * Behavioral Risk Factor Surveillance System * CDC Wonder * Center for Medicare and Medicaid Services * County Health Rankings * Dartmouth Atlas of Health Care * Diabetes Interactive Atlas * Feeding America * Illinois DCFS * Illinois Department of Public Health IQUERY * Illinois Gaming Board * Illinois Youth Survey * Institute for Health Metrics and Evaluation * National Center for Educational Statistics * National Center for Health Statistics * National Center for HIV/AIDS, Viral Hepatitis, STD and TB Prevention * Pediatric Nutrition Surveillance * Safe Drinking Water Information System (EPA) * Uniform Crime Reporting (FBI) * US Census * USDA Food Environment Atlas. Input from Community Stakeholders To gain community input for the FY2018-2021 Community Health Needs Assessment, a variety of stakeholders were asked to participate in engagement sessions. These individuals included HSHS St. Joseph's partners, individuals from other health care organizations, local school districts, community organizations, business members, public administrators, and local religious organizations. Possible participants for the 2018 CHNA were identified by researchers at the UIS Survey Research Office and approved by HSHS St. Joseph's Hospital. Community Stakeholders who participated in the CHNA process represented the following area organizations and institutions: * HSHS St. Joseph's Foundation Leadership Council * HSHS St. Joseph's Hospital Board of Directors * Community Physicians * Highland Area Christian Service Ministry Food Pantry* * Highland Area Ministerial Alliance* * Highland Chamber of Commerce * Highland Community Unit School District #5* * Highland High School* * Highland Home * Highland Middle School* * Highland News Leader * Madison County Health Department* * Special Olympics Illinois. *Denotes group representing medically underserved, low-income, and minority populations. Individuals were recruited to participate in an engagement session by researchers at the University of Illinois Springfield. Each individual received a letter or email introducing the project on October 18, 2017. Individuals were informed that the engagement sessions were organized and would be moderated by researchers from the University of Illinois Springfield Survey Research Office (SRO). The SRO is an independent research organization with a reputation for quality health care research. And while the stakeholders were informed that their participation in this process is significant to the success of the 2018 Community Health Needs Assessment, their participation was voluntary. They were also informed that anything they say during the discussion is confidential and the SRO will not release any information that can be linked to them. Finally, they were informed that a member of the SRO research team will be contacting them by telephone to give them more details. Follow-up phone calls encouraging their attendance to the event were conducted by SRO researchers beginning on October 30, 2017. Individuals were informed that this process is a crucial component for identifying the ongoing community health priorities in the region and that, as an important stakeholder, they have been identified by hospital leadership to participate in an engagement session to identify important health priority areas in the community. Engagement sessions were held on the following dates, locations, and times: * November 14 @ 3p.m., Sullivan conference room, St. Joseph Hospital, Highland, IL * November 14 @ 5p.m., Sullivan conference room, St. Joseph Hospital, Highland, IL * November 16 @ 5p.m., Sullivan conference room, St. Joseph Hospital, Highland, IL In total, 17 stakeholders participated in the engagement sessions (7 attended the 3 p.m. group on November 14, 7 attended the 5 p.m. group on November 14, and 3 attended on November 16).
Schedule H, Part V, Section B, Line 6b Facility , 1Facility , 1 - HSHS St. Joseph's Hospital Highland. Yes, the hospital conducted its FY2018 CHNA with HSHS St. Joseph's Foundation Leadership Council, HSHS St. Joseph's Hospital Board of Directors, Community Physicians, Highland Area Christian Service Ministry Food Pantry, Highland Area Ministerial Alliance, Highland Chamber of Commerce, Highland Community Unit School District #5, Highland High School, Highland Home, Highland Middle School, Highland News Leader, Madison County Health Department, and Special Olympics Illinois.
Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - HSHS St. Joseph's Hospital Highland. The organization's most recently conducted CHNA was completed for the fiscal year ended 6/30/18. The related implementation strategy that was adopted is for the period July 1, 2018 through June 30, 2021. The information below pertains to the 3rd year cycle of the implementation strategy from the CHNA conducted in the fiscal year 2015. As part of the identification and prioritization of health needs, the St. Joseph's Hospital Senior Leadership Team considered the estimated feasibility and effectiveness of possible interventions by the hospital to impact health priorities. Prioritization was based on scope, severity, and burden; health disparities associated with the need; the importance the community places on addressing the health need; the hospital and community assets and resources available to address the health need; and local expertise and input. Based on the CHNA planning and development process described, the following priority community health needs were identified access to care, heart health, diabetes, and nutrition and obesity, and preventative education (screenings). Objectives established to address these needs through specific initiatives included: Access to Care: Fifty-four percent of respondents to the 2015 HSHS St. Joseph's Hospital Public Engagement Survey stated that "access to primary health care services" and "lack of affordable health care services" were of great concern to them. Outcomes * Offered free transportation service courtesy of the Friends Van for individuals to medical, dental and other personal appointments that encompass a service area within a 20-mile radius of Highland. In total, the Friends Van was driven 27,020 miles in FY2018 to make more than 3,800 trips to increase access to care. * Offered two health fairs including free tests and screenings for blood pressure, pulse oximetry, hearing, dental health, glaucoma, vision, and nutrition serving 886 community members. The fairs also included internal and external services including hospice, cardiac rehab, rehab services, oncology, wound care, vision, nutrition, oral health, and more. * Participated in many community health fairs offering screenings and lab work for those in need including health fairs for Local Union 250, Basler Electric, Trouw Nutrition, Curves, and St. Paul Church. Through these community health fairs, the hospital served almost 400 community members. * Connected Emergency Department patients who have no primary care physician (PCP) with a physician to promote management of care. Upon discharge from the Emergency Department, patients who have indicated they do not have a PCP are scheduled for a follow up visit with a HSHS Medical Group PCP. Heart Health: More than 50% of respondents to the 2015 HSHS St. Joseph's Hospital Public Engagement Survey responded to the survey question: "Have you ever been told that you have a heart-related condition?" * Offered numerous community health education programs to area residents on a variety of topics including: enhancing strength and reconditioning; reducing cholesterol and triglycerides; altering risk factors to prevent the progression of heart disease; improving muscle strength; improving self-confidence; and lowering blood pressure. * In recognition of National Heart Month, the hospital hosted several events that increased access to heart health education and screenings including "Glo Bingo," which offered free educational activities and materials to the community, a Heartsaver CPR and AED class, and a heart health fair hosted by the five hospitals of the HSHS Southern Illinois Division. Diabetes (Disease Management): More than 20% of respondents to the 2015 HSHS St. Joseph's Hospital Public Engagement Survey responded to the question of "Have you ever been told that you have...diabetes." Outcomes: * Celebrated Diabetes Awareness Month by hosting an open house event to educate participants about diabetes and the hospital's Diabetes Self-Management Education (DSME) program. The DSME Program is an outpatient interactive workshop led by diabetes educators (including registered nurses, dietitian, and pharmacist) for people with diabetes or pre-diabetes who are interested in learning more about how to manage diabetes and reduce complications. * In partnership with the American Diabetes Association, encouraged area residents to visit HSHS St. Joseph's Hospital and take the Diabetes Risk Assessment Test annually in March in recognition of Diabetes Alert Day. Participants were encouraged to share the test results with their family and primary care providers. In addition, free blood sugar and blood pressure screenings were available. * Supported Healthy Pathways, a program of the hospital's Cardiac Rehab department that focuses on total health of the patient and educates those with diabetes and heart disease on proper diet and exercise to improve health and wellness. Nutrition & Obesity: More than 33% of respondents to the 2015 HSHS St. Joseph's Hospital Public Engagement Survey responded to the survey question: "Have you ever been told that you have a need for healthier nutrition?" * Partnered with the Madison County Health Department through participation in the Madison County Partnership for Community Health's three (3) work groups to develop and implement actions plans, help provide and promote access to resources, and work with various constituencies to address obesity. * Developed a new implementation strategy using U.S. Department of Agriculture (USDA) resources including MyPlate, MyPyramid, and Food Guide Pyramid programs to distribute nutrition education. The hospital's registered dietitian developed educational resources that were distributed through both education sessions and online channels. * Offered a free, quarterly nutrition program offered in the community. The four-week or six-week series were led by registered dietitian and more than 200 community members attended in FY18. * For more than 39 years, HSHS St. Joseph's Hospital has been a lead collaborator in the Highland Area Meals on Wheels Program. The hospital partners with the civic organization Highland Area Meals on Wheels Program to prepare and deliver nutritious meals to the elderly, those who are recuperating from illness or hospital stays, and anyone wishing to participate in the program. The Highland Area Meals on Wheels Program serves residents who live in communities within a 15-mile radius of the hospital. Meals for program participants are prepared and delivered Monday through Friday including holidays. On average, 80 meals are delivered by volunteer drivers covering 12 routes each week. An estimated 16,896 meals are delivered annually. Each meal is prepared and packaged by the hospital's food and nutrition services (FANS) department, or 5,200 hours annually. * Hosted a second health fair, which included nutritional screening by a registered dietician. Hosted a nutritional series facilitated by a certified dietitian to introduce better eating habits and weight management; 271 community members participated. Prevention Services (Screenings and Education): More than 31% of respondents to the 2015 HSHS St. Joseph's Hospital Public Engagement Survey responded to the question of "To what extent are you concerned about the following issues in your community..." - lack of healthcare education and prevention services. * Sponsored an annual health fair offering both free and low-cost health screenings for residents to gain baseline knowledge regarding certain aspects of their personal health. In addition, several internal hospital resources and external community-based health care and social service agencies provide information booths distributing information and offering on-site counseling to health fair attendees. * Offered a second annual health fair to area residents in addition to the existing spring health fair. Existing screenings were provided with the possibility for additional screenings and information booths to be provided. In addition, the hospital evaluated additional in-hospital or community-based screenings for services such as (but not limited to) sleep disorders (emergency department & pre-operative patients) and depression prevention (through the hospital's senior renewal department). * Pledged to increase colorectal cancer screening rates by supporting the 80% by 2018 initiative, led by the American Cancer Society (ACS), the Centers for Disease Control and Prevention (CDC) and the National Colorectal Cancer Roundtable (an organization co-founded by ACS and CDC). HSHS St. Joseph's Hospital recently introduced a take home stool test called a FIT kit (fecal immunochemical test) which can be performed in the privacy of one's home on an annual basis.
Schedule H, Part V, Section B, Line 11 Facility , 2Facility , 2 - HSHS St. Joseph's Hospital Highland. * Hosted a Girls Night Out event where women could get on the spot mammograms with instant radiology readings or schedule an appointment. Attendees received education on breast health and the importance of preventative self-breast assessment to detect breast cancer. * Offered wellness screening lab tests on a walk-in basis at the hospital one day a week. Community members can receive lab tests at a price that was previously only available at the hospital's two annual health fairs, without having to schedule an appointment. Screenings available include PSA, vitamin D, TSH, Hemoglobin A1C, and lipid panel. All tests are $15 or less. As an outcome of the prioritization process, unemployment was also identified and will not be addressed directly by the hospital. Being a provider of health care services through inpatient and outpatient services, the hospital does not have the expertise or resources to address unemployment in the region. The hospital is, however, one of the largest employers in the community and is committed to continued growth to support the community.

THE HOSPITAL BEGAN TO FOLLOW THAT REVISED POLICY DURING 2015. IN OCTOBER 2017, THE HOSPITAL'S BOARD OF DIRECTORS ADOPTED THE POLICY. TO ENSURE FULL COMPLIANCE WITH SECTION 501(R) AND THE TREASURY REGULATIONS ISSUED THEREUNDER, IN NOVEMBER 2017 THE ORGANIZATION ENGAGED AN OUTSIDE CONSULTANT TO REVIEW ALL APPLICABLE POLICIES AND PRACTICES.
Schedule H, Part I, Line 7g Subsidized Health ServicesThe organization did not have any subsidized health services costs attributable to physician clinics.
Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceSJH ACCOUNTING LOOKED AT FY18 TOTAL EXPENSES (adjusted for the Medicaid tax assessment) AND DIVIDED BY TOTAL GROSS REVENUE TO CALCULATE A COST/CHARGE RATIO. ALL COSTS COME FROM THE SJH GENERAL LEDGER TRIAL BALANCE AND/OR INVOICES.
Schedule H, Part II Community Building ActivitiesFor more than 40 years, HSHS St. Joseph's Hospital has been a lead collaborator in the Highland Area Meals on Wheels Program. In April 1977, the hospital partnered with the now defunct Highland Area Council of Churches to introduce the Meals on Wheels program. Today, HSHS St. Joseph's partners with Highland Area Meals on Wheels Program to prepare and deliver nutritious meals to the elderly, those who are recuperating from illness or hospital stays, and anyone wishing to participate in the program. Highland Area Meals on Wheels serves residents who live in communities within a 15-mile radius of HSHS St. Joseph's Hospital (cumulative population 20,000+) including: Highland, Grantfork, Pierron, New Douglas, St. Jacob, St. Rose, Marine and Pocahontas. Meals for program participants are prepared and delivered Monday through Friday every week - including holidays falling on weekdays. On average, 263 meals are delivered by volunteer drivers covering 12 routes each week. An estimated 14,220 meals are delivered annually. Each meal is prepared and packaged in the HSHS St. Joseph's Hospital's Food and Nutrition Services (FANS) Department. Every weekday (totaling 260 days each year), five hospital FANS colleagues devote five hours to prepare and pack meals for the program. The total hours donated to support this program equals 1,300 hours annually. In addition to the preparation and packaging of food for Meals on Wheels, the hospital's FANS management team assumes the responsibility of coordinating menus as well as ordering the ingredients and other foods that are prepared for the meals. HSHS St. Joseph's Hospital's leadership and involvement in Highland Area Meals on Wheels is vital to its continual operation because no other local area organization, agency or group is available or able to sponsor this program or it would not occur.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountSelf-pay patients are screened for verification of no insurance coverage. Once verification of no coverage has been completed, the guarantors will be screened for charity or uninsured discounts based on income levels. Those guarantors who do not qualify for charity through the screening process will be asked to provide proof of income in the form of a recent pay stub and/or recent tax return to determine the level of uninsured discounting. If no proof of income is provided or a guarantor's income is greater than 201% of the federal poverty level, the appropriate discount according to the established criteria up to 600% of the federal poverty level will be applied. The amount of money not collected from those self-pay patients who are not eligible for charity care is reported as bad debt. Allowances for Doubtful Accounts have been recorded based on an historical matrix, using collectability percentages by payor and aging category in accordance with HSHS Executive Policy #F-23, Valuation of Net Accounts Receivable. Percentages are based on historical experience at the hospital and adjusted for known limitations. The Hospital reports bad debt in accordance with Generally Accepted Accounting Principles (GAAP).
Schedule H, Part III, Line 3 Bad Debt Expense MethodologySt. Joseph's Hospital strongly believes that its charity care, and the related community benefit obtained from such care, is understated because of those patients who potentially qualify for charity care but do not wish to apply for it. In addition, some care is not classified as charity because of missing documentation on patient resources. Thus, the hospital's bad debt includes a portion that could be classified as charity care if application for such care was sought and/or completed. While the hospital feels their bad debt expense includes amounts attributable to patients who would qualify for financial assistance under the hospital's financial assistance policy, the hospital is unable to estimate this amount.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteThe text of the footnote to the organization's financial statements that describes bad debt expense can be found on page 20 of the HSHS Consolidated Audit Report.
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsThe hospital continually strives to provide excellent patient care in the most cost effective fashion. Nonetheless, the Medicare program, in many cases, does not provide payment that covers the full cost of the care provided. Since it is the mission of the hospital to respond to community need, hospital leadership continually advocates for improved Medicare payment so that the cost of quality care to those patients who are not able to afford it is not compromised and is fairly subsidized by all payers. The IRS does not classify this shortfall in Medicare payments as Community Benefit. However, we believe it is an important contribution made by the hospital to the health and well-being of the community. If the Medicare program did not exist, many Medicare patients would be eligible for financial assistance or other means-tested government programs. Further, by absorbing this shortfall and providing care below cost to these individuals, St. Joseph's Hospital is relieving the burden of government. Accordingly this shortfall should be treated as community benefit.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistanceEvery effort is made prior to, during and after provision of medical services to determine whether a patient is eligible for charity/community care and to assist the patient in completing the application and provide adequate documentation. If the patient qualified for charity/community care for the full balance of their account, the entire amount is written off to charity/community care and hence no debt collection is pursued. If the patient qualified for charity/community care for a portion of their account balance, that portion is written off to charity/community care, with the patient being responsible for the remainder of the balance. Additionally, reasonable effort will be made to obtain third-party or government payer reimbursement on behalf of the patient. If those efforts are not fruitful, an offer will be extended to the patient to make installment payments on their balance. Only at such point that the patient defaults on installment payments or refuses to cooperate with the hospital's efforts to be reimbursed will the account be sent to collections. The hospital will make reasonable efforts to determine whether a patient is eligible under the financial assistance policy before it engages in an ECA. To the extent a patient is determined to be eligible for financial assistance under the FAP, the hospital will take all reasonably available measures to reverse any ECA taken against the patient to obtain payment for the care.

Schedule H, Part VI, Line 2 Needs assessmentIn addition to our triennial CHNA process, HSHS St. Joseph's Hospital maintains an ongoing communication with the Illinois Department of Public Health (IDPH) through the Madison County Public Health Department. Through the Madison County Public Health Department, HSHS St. Joseph's Hospital senior leaders and managers participate in focus groups and planning sessions to evaluate plans to address community health needs. One instrument through the health department that the hospital used was the IDPH IPLAN. St. Joseph's Hospital also collaborated with non-profit agencies such as the Highland Relevant Care Pregnancy Center, Highland Area Community Foundation, other civic organizations, as well as public/civic entities to evaluate the health care needs of the constituents within the hospital's service area. This past year, members of the St. Joseph's Hospital leadership team joined the Madison County Community for a Drug Free Partnership Committee. This group focuses on drug and alcohol related problems in the county and ways to help those who are addicted and prevent addiction from occurring in the future. Members of this group include people from police and fire departments, county sheriff's department, health department, every hospital in the community, school officials, and many more. The group meets monthly and is focused on many education programs for school aged kids, drug give back programs, and ways to help those with mental health concerns. To stay connected to what the members of the community identify as top priorities, St. Joseph's Hospital formed a community engagement council and patient and family engagement council this year. The community engagement council is made up of 10 community members who provide feedback on what health needs they see in the community and how they perceive the hospital is meeting or not meeting those needs. The patient and family engagement council is made up of previous patients and/or family members of patients from HSHS St. Joseph's Hospitals in Highland and Breese and HSHS Holy Family Hospital in Greenville. The focus of this group is to evaluate services received by the patients and how those compare with the overall health care needs of the patient. In addition, the foundation leadership council and hospital board of directors are asked for input throughout the year. Leaders at the hospital also review patient satisfaction surveys.
Schedule H, Part VI, Line 3 Patient education of eligibility for assistanceThe Registration Department provides brochures to patients upon registration regarding the financial assistance program available to them at HSHS St. Joseph's Hospital. The brochures are also supplied in waiting and ancillary areas for easy access by the patient. Plus, additional contact information (separate from any hospital brochures or literature) for several community agencies that help patients to determine their eligibility for assistance under public and private programs is available. To better serve our patient population, HSHS St. Joseph's Hospital offers information on our financial assistance program in English and Spanish. This publication allows the hospital to serve populations surrounding the Highland area that may speak Spanish as their primary language. For patients who need further assistance, translation services are available within the HSHS Southern Illinois Division, of which HSHS St. Joseph's Hospital is a member. The hospital has a representative on site to assist patients who may be eligible for Medicaid assistance through the Illinois Department of Healthcare and Family Services. They work with patients to determine if they may be eligible, and if so, assist in completing the necessary application for submission. Patients are also provided a Fair Billing Act notification at the point of registration regarding separate physician billing they can expect to receive, plus information about the patient's financial responsibility if their insurance plan is out of network. One-on-one conversations to educate patients about financial assistance often stem from patients receiving a statement or a pre-collection letter. At that point, the customer service representative or cashier, or other qualified staff, will inform/educate the caller about financial assistance options. The first pre-collection letter reminds the patient their account is in delinquent status. It also informs them they may be eligible for financial assistance through the organization's charity care program. The letter provides direct contact information to begin the application process for charity care and lists the internet address where the policy can be obtained. After failed attempts made by the hospital and our law firm to work out payment arrangements, the organization has processes in place for next steps. The system accesses credit and other information that allows the hospital to discern the probability of a patient being able to pay their bill. After assessing that information, the hospital determines whether to grant charity care or continue collection efforts.
Schedule H, Part VI, Line 4 Community informationHSHS St. Joseph's Hospital is a critical access, tertiary hospital located in Madison County, Illinois. For more than 140 years, the hospital has been the leader in health and wellness in Madison County. St. Joseph's Hospital provides a wide range of specialties, including inpatient, surgical services, rehab services, emergency care, and outpatient services such as medical imaging and laboratory. St. Joseph's Hospital's service area is comprised of approximately 400.314 square miles with a population of 31,052 and a population density of 657 per square mile. The service area consists of eight (8) suburban and rural communities. HSHS St. Joseph's Hospital does not exclude Medically Underserved Areas (MUAs), low income or minority populations from its service area. The average income for the hospital's service area is $55,305 with 14.09% of service area community residents below the federal poverty guideline. Up to 10 % of the hospital's and community's patients are uninsured and 12% are Medicaid recipients. There are three (3) other hospitals (Anderson Hospital, Maryville; HSHS Holy Family Hospital, Greenville; HSHS St. Joseph's Hospital, Breese) serving the community or communities that St. Joseph's also serves. Within Madison County, 13.31% of the population lives below the poverty level. There is also a population that is uninsured for health care totaling 8% of the total population. St. Joseph's Hospital partners with other area organizations to address the health needs of the community, living its mission to reveal and embody Christ's healing love for all people through our high quality Franciscan health care ministry, with a preference for the poor and vulnerable. The hospital is part of Hospital Sisters Health System (HSHS), a highly-integrated health care delivery system serving more than 2.6 million people in rural and midsized communities in Illinois and Wisconsin. HSHS generates approximately $2 billion in operating revenue with 15 hospitals and more than 200 physician practice sites. Our mission is carried out by 14,000 colleagues and 2,100 physicians who care for patients and their families in both states.
Schedule H, Part VI, Line 5 Promotion of community healthAs a healing ministry of the Catholic Church and an affiliate of Hospital Sisters Health System HSHS St. Joseph's Hospital Highland is committed to delivering high quality, compassionate, and cost-effective health care services to all. Founded in 1878, the hospital brings a healing presence and improves the health of our community, especially those persons who are sick, poor, and disadvantaged. Because of the hospital's mission and heritage, the hospital is dedicated to promoting the health of Highland and surrounding areas. The hospital is governed by a board of directors, most of whom reside in the hospital's primary service area and who are neither employees nor independent contractors of the hospital (nor family members thereof). The board ensures that St. Joseph's Hospital is responding to community need. In FY2018, for example, board members received reports on community benefit at every meeting. The board of directors also were given a presentation on the CHNA results for FY18 and approved the report and implementation plan at a following board meeting. In addition, St. Joseph's Hospital receives community support and guidance through the patient family advisory council, community engagement council, and HSHS St. Joseph's Foundation Leadership Council. The family advisory council and community engagement council meet quarterly, while the foundation leadership council meets every other month. These groups provide feedback from the community on additional services or improvements needed, as well as help the hospital understand the community's perception of those services and what is missing. St. Joseph's Hospital is blessed with a strong volunteer program. Volunteers are active in many hospital departments and donate many more hours throughout the year to hospital-sponsored community programs. Consistent with its exempt purpose, St. Joseph's Hospital has an open medical staff with privileges available to all qualified physicians in the area. The hospital also operates an emergency department that is open 24 hours to all persons regardless of their ability to pay. As a not-for-profit hospital, St. Joseph's Hospital reinvests surplus funds to support the mission of the organization and promote the health of the community rather than distributing surplus funds as profits to shareholders or individuals. Funds not committed to ongoing operations are generally used to upgrade facilities, secure new technologies, improve patient care, and support initiatives designed to promote health and ensure access for all. In FY18, St. Joseph's Hospital invested in the community's health with significant equipment purchases including ERBE, EKG, bladder scanner, VitalStim, MAS mini, portable vital signs machines, and adult CPR manikins. The ERBE is an important piece of equipment for the GI surgeon for colonoscopy screenings. The previous equipment was outdated and needed replaced. This purchase was more than $50,000. The emergency department needed an EKG machine to free up the lone machine that was being borrowed from another department. The new machine allows for faster treatment. The bladder scanner, VitalStim, Mas mini, and portable vital signs machines are all pieces of equipment that are needed for improved, faster care of patients. The hospital also purchased new Voice Activated Manikins (VAM) to help train nurses. These advanced manikins give real time feedback on process and procedures taken by the nurses. In addition to equipment, the hospital invested in the facility with improvements to the women and children's center. Additional walls and a door were added in front of the 3D mammography suite to increase privacy. In addition, a space in the center was closed off and improved for a permanent breast-feeding area for colleagues, patients, and visitors. St. Joseph's Hospital also devotes significant resources to access for patients who cannot afford care, along with other Community Benefits. In FY2018, St. Joseph's Hospital provided more than $3.3 million in Community Benefit, including financial assistance at cost, unpaid costs of Medicaid and other public programs, and a range of diverse programs designed to enhance access and improve community health. Additionally, in FY2018, St. Joseph's Hospital provided $778,660 million (at cost) in uncompensated care to patients who did not qualify for charity care or public assistance and nearly $200,000 (at cost) in excess of Medicare payment for health care services. St. Joseph's Hospital provides a range of Community Benefit initiatives and programs that further our mission and long-standing commitment to our community. In many cases, these Community Benefits would not exist without the leadership role played by St. Joseph's Hospital and they often relieve a burden that would otherwise be carried by government.
Schedule H, Part VI, Line 6 Affiliated health care systemHeadquartered in Springfield, Illinois, Hospital Sisters Health System (HSHS) is a highly integrated, multi-institutional health care system comprised of 15 hospitals and more than 200 physician practice sites in Illinois and Wisconsin. HSHS is sponsored by Hospital Sisters Ministries (HSM), which is a public juridic person of the Roman Catholic Church. HSHS continues the health care ministries begun by its founders, the Hospital Sisters of St. Francis, more than 140 years ago, in rural and midsized communities throughout Illinois and Wisconsin. HSHS provides compassionate, holistic, high quality, and cost-effective health care through its acute care hospitals, physician practices, home health programs, palliative and hospice programs, and community outreach services. In addition, HSHS affiliated hospitals include critical access facilities that offer essential health services that otherwise would not be available in many communities. Our mission calls us to reveal and embody Christ's healing love for all people, and we extend our high quality Franciscan health care ministry beyond the walls of our hospitals by providing community health improvement services with special emphasis on the poor and vulnerable. In addition to providing community health improvement services to the broader community, HSHS cares for everyone who comes to its facilities, regardless of their ability to pay. In tandem with a variety of cross sector community partners, HSHS's Community Benefit initiatives are strategically planned and professionally implemented to improve access to health care services; enhance the health of the community; advance medical or general health care knowledge; and relieve or reduce the burden of government to improve health. In FY2018, HSHS collectively provided $198.4 million in Community Benefit (9.1% of total hospital expenses). This amount included $25.7 million provided for Financial Assistance (a.k.a. Charity Care) and $133.7 million for unreimbursed care provided as part of the Medicaid program. In addition, HSHS hospitals committed significant resources to treat Medicare patients. The cost of providing services to primarily elderly beneficiaries of the Medicare program - in excess of governmental and managed care contract payments - was $221.5 million. HSHS hospitals also recorded $45.9 million in uncollectible accounts. While HSHS does not count the latter two amounts as Community Benefit, they nonetheless reflect our commitment to serving all persons in need of care. In addition to the dollars invested in our Community Benefit programs, HSHS continues to reinvest any surplus revenue from operations and investments into new medical technology, facility infrastructure and health care services in our communities. By doing so, we ensure our ability to meet the ongoing demand for high quality, efficient and easily accessible health care.
