INPUT FROM PERSONS WHEN CONDUCTING CHNASchedule H, Part V, Section B, Line 5 Input from persons representing the broad interests of the community was taken into account through various means including online surveys, targeted focus groups, and an advisory committee group. The advisory committee group consists of members from Harford County Government, Harford County Health Department, the interfaith-based community, and representatives from Upper Chesapeake Health. Key members of the advisory group are listed as follows: Patsy Astaria - UM UCH Kaufman Cancer Center Vickie Bands UM UCH Community Health Mallory Canami Harford County Health Department (HCHD) Health Policy Leslie Clark UM UCH Comprehensive Care Center Karen Hensley UM UCH Women & Children Gary Hicks UM UCH Education Bari Klein Healthy Harford Sharon Lipford Healthy Harford Dr. Russ Moy HCHD Health Officer Debbie Ostrowksi UM UCH Diabetes & Endocrine Christina Pedini UM UCH Rehab Services Amber Shrodes Harford County Government Community Services Julie Siejack UM UCH Community Outreach Kimberly Theis UM UCH Community Benefit Rev. Byron Young Harford County Faith Based Community Additionally, qualitative data was collected from the Local Health Improvement Coalition (LHIC) community forum meeting. Data collected from the surveys, focus groups, and advisory groups were used to assess health status, health risk and behaviors, preventative practices, and healthcare access related to chronic disease and injury. The advisory panel used all information to discuss current priorities and reach a consensus on what those priorities should be going forward.
CHNA CONDUCTED WITH HOSPITAL FACILITIESSchedule H, PART V, SECTION B, QUESTION 6A HARFORD MEMORIAL HOSPITAL'S CHNA WAS CONDUCTED IN CONJUNCTION WITH ITS RELATED AFFILIATE HOSPITAL, UPPER CHESAPEAKE MEDICAL CENTER. CHNA CONDUCTED WITH ORGANIZATIONS OTHER THAN HOSPITALS SCHEDULE H, Part V, Section B, Question 6b THE HOSPITAL'S CHNA WAS CONDUCTED WITH ORGANIZATIONS OTHER THAN HOSPITAL FACILITIES AS FOLLOWS: HEALTHY HARFORD (A 501(C)(3) ORGANIZATION) AND HARFORD COUNTY HEALTH DEPARTMENT. HOSPITAL FACILITY WEBSITE Schedule H, Part V, Section B, Line 7a For the community health needs assessment 2018 report, visit: https://www.umms.org/uch/community/outreach/health-benefits HOSPITAL FACILITY WEBSITE Schedule H, Part V, Section B, Line 10a See page 7 of the 2016 community health improvement report for HMHs implementation strategy: https://www.umms.org/uch/community/outreach/health-benefits
ADDRESSING SIGNIFICANT NEEDS IDENTIFIED IN THE CHNASCHEDULE H, PART V, SECTION B, QUESTION 11 There are several ways in which UMUCH addresses the needs identified in the CHNA. The first way in which UMUCH addresses the identified needs is through the development of a system-wide (across both hospitals, UCH and HMH) implementation plan. The implementation plan is a comprehensive plan that is specific to the identified needs of the County. The plan is developed and compiled through extensive input from all areas that have planned community benefit activities directly related to the identified needs in the CHNA. The areas that provide community benefit develop programs, education and screenings that directly address the community needs as well as collect data and document outcomes for each of their activities. The implementation plan is a moving document with frequent reviews and updating through the hospital based committee (Community Benefit Advisory Council) as the need requires. The second way in which UMUCH addresses the needs of the County is through County partnerships. These partnerships are created to work on one or several of the needs identified, for example The Drug Exposed Newborn Taskforce. Newborn substance exposure is identified as identified problem in the County. UMUCH is not solely working to address this issue. UMUCH is part of group of community entities that are working collaboratively to address the issue. Another example are the LHICs (Local Health Improvement Coalition). These coalitions address the identified needs centered on the behavioral health and chronic disease issues. The Harford Crisis Center is a direct positive outcome of the Behavioral Health LHIC work and partnership. UMUCH personnel chair both the Behavioral Health and Chronic Disease LHICs. These coalitions are reworked every three years to be in line with the most recent CHNA to continually address the most pressing identified needs of the County. Transportation is a need identified that is not being addressed by UMUCH. This is a countywide issue identified as a major issue by all County partners. Funding for a comprehensive countywide public or private transportation system is not available at this time. Many County partners meet to discuss alternatives, with UMUCH at the table; it continues to be an unmet need. An additional issue that was identified through County police data is an increase in spousal abuse. This is not a priority that was chosen to be a UMUCH priority due to lack of expertise, services, and funding. However UMUCHs focus on the behavioral health priority may impact the data over the next couple of years. Eligibility Criteria for Financial Assistance Schedule H, Part V, Line 13b THE FINANCIAL ASSISTANCE POLICY EXPLAINS SEVERAL ELIGIBILITY CRITERIA, INCLUDING PARTICIPATION IN MEDICAID/MEDICARE PROGRAMS AS WELL AS ELIGIBILITY UNDER VARIOUS STATE REGULATIONS. In addition to FPG, the income levels defined by the Maryland State Department of Health and Mental Hygiene (MD DHMH) are used to determine eligibility for financial assistance. The MD DHMH income levels are more generous than the FPG income levels.
Financial Assistance Policy Available to PublicSchedule H, Part V, Section B, Lines 16a, 16b, and 16c The filing organizations financial assistance policy, application, and plain language summary are widely available on a website at the following address: https://www.umms.org/uch/patients-visitors/for-patients/financial-assistan ce Hospital Efforts Before Initiating Action Schedule H, Part V, Section B, Line 20c In the case of an incomplete FAP application, patients are not automatically denied. A letter is mailed to the patient requesting the missing documentation, and FAP applications are approved once the missing documentation is provided.

Eligibility Determined by Factors Other Than FPGCriteria for free or discounted care Schedule H, Part I, Line 3c HMH is committed to providing financial assistance to persons who have health care needs and are uninsured, underinsured, ineligible for a government program, or otherwise unable to pay, for medically necessary care based on their individual financial situation. In addition to FPG, the income levels defined by the Maryland State Department of Health and Mental Hygiene (MD DHMH) are used to determine eligibility for financial assistance. The MD DHMH income levels are more generous than the FPG income levels. The FAP also uses a financial hardship threshold when determining eligibility. A patient with medical debt exceeding 25% of family annual household income may be eligible for financial assistance. Community Benefit Report Schedule H, Part I, Line 6b The organization annually files a community benefit report as required by the Maryland HSCRC. The report can be found at: https://hscrc.state.md.us/pages/init_CB.aspx. Costing Methodology Schedule H, Part I, line 7 Maryland's regulatory system creates a unique process for hospital payment that differs from the rest of the nation. The Health Services Cost Review Commission, (HSCRC) determines payment through a rate setting process and all payors, including governmental payors, pay the same amount for the same services delivered at the same hospital. Maryland's unique all payor system includes a method for referencing Uncompensated Care in each payors' rates, which does not enable Maryland hospitals to breakout any offsetting revenue related to Uncompensated Care. Community benefit expenses are equal to Medicaid revenues in Maryland, as such, the net effect is zero. Additionally, net revenues for Medicaid should reflect the full impact on the hospital of its share of the Medicaid assessment. COMMUNITY BUILDING ACTIVITIES SCHEDULE H, PART II THE HOSPITAL'S COMMUNITY BUILDING ACTIVITIES PROMOTED THE HEALTH OF THE COMMUNITIES IT SERVES. Bad Debt Expense Reporting In Maryland Schedule H, Part III, lines 2 and 3 The Health Services Cost Review Commission (HSCRC) started setting hospital rates in 1974. At that time, the HSCRC approved rates applied only to commercial insurers. In 1977, the HSCRC negotiated a waiver from Medicare hospital payment rules for Maryland hospitals to bring the federal Medicare payments under HSCRC control. In 2014, Maryland's waiver with Medicare was renegotiated and updated to reflect the current healthcare environment. Under this new waiver, several criteria were established to monitor the success of the system in controlling healthcare costs and the continuance of the waiver itself: 1. Revenue growth per capita 2. Medicare hospital revenue per beneficiary 3. Medicare all provider revenue growth per beneficiary 4. Medicare readmission rates 5. Hospital acquired condition rate Because of this system, the hospital is unable to estimate how much of the amount reported in Line 2 is attributed to patients who would apply under the FAP. Bad Debt Expense Schedule H, Part III, line 4 The corporation records revenues and accounts receivable from patients and third-party payors at their estimated net realizable value. Revenue is reduced for anticipated discounts under contractual arrangements and for charity care. An estimated provision for bad debts is recorded in the period the related services are provided based upon anticipated uncompensated care, and is adjusted as additional information becomes available. The provision for bad debts is based upon management's assessment of historical and expected net collections considering historical business and economic conditions, trends in healthcare coverage, and other collection indicators. Periodically throughout the year, management assesses the adequacy of the allowance for uncollectible accounts based upon historical write-off experience by payor category. The results of this review are then used to make modifications to the provision for bad debts and to establish an allowance for uncollectible receivables. After collection of amounts due from insurers, the Corporation follows internal guidelines for placing certain past due balances with collection agencies. For receivables associated with services provided to patients who have third-party coverage, the Corporation analyzes contractually due amounts and provides an allowance for bad debts, allowance for contractual adjustments, provision for bad debts, and contractual adjustments on accounts for which the third-party payor has not yet paid or for payors who are known to be having financial difficulties that make the realization of amounts due unlikely. For receivables associated with self-pay patients or with balances remaining after the third-party coverage had already paid, the Corporation records a significant provision for bad debts in the period of service on the basis of its histoical collections, which indicates that many patients ultimately do not pay the portion of their bill for which they are financially responsible. The difference between the discounted rates and the amounts collected after all reasonable collection efforts have been exhausted is charged against the allowance for doubtful accounts. Costs Cove
COLLECTION PRACTICESSchedule H, Part III, line 9b The organization expects payment at the time the service is provided. Our policy is to comply with all state and federal law and third party regulations and to perform all credit and collection functions in a dignified and respectful manner. Emergency services will be provided to all patients regardless of ability to pay. Financial assistance is available for patients based on financial need as defined in the Financial Assistance Policy. The organization does not discriminate on the basis of age, race, creed, sex or ability to pay. Patients who are unable to pay may request a Financial Assistance application at any time prior to service or during the billing and collection process, even in excess of 240 days following the first post-discharge billing statement. The organization may request the patient to apply for Medical Assistance prior to applying for financial assistance. The account will not be forwarded for collection during the Medical Assistance application process or the Financial Assistance application process. No extraordinary collection actions (ECAs) will occur earlier than 120 days from submission of first bill to the patient and will be preceded by notice 30 days prior to commencement of the action. Availability of financial assistance will be communicated to the patient and a presumptive eligibility review will occur prior to any action being taken. If a patient is determined to be eligible for Financial Assistance after an ECA is initiated, the organization will take reasonable measures to reverse the ECAs against the patient account.
Community Health Needs AssessmentSchedule H, Part VI, line 2 THE HOSPITAL'S SECOND COMMUNITY HEALTH NEEDS ASSESSMENT (CHNA) WAS CONDUCTED IN 2015. THE PURPOSE OF THE ASSESSMENT WAS TO GATHER INFORMATION ABOUT LOCAL HEALTH NEEDS AND HEALTH BEHAVIORS. IN MARCH OF 2015, STAKEHOLDERS FROM UNIVERSITY OF MARYLAND UPPER CHESAPEAKE HEALTH SYSTEM (UMUCHS - PARENT COMPANY OF HARFORD MEMORIAL HOSPITAL AND UPPER CHESAPEAKE MEDICAL CENTER), THE HARFORD COUNTY HEALTH DEPARTMENT, HARFORD COUNTY GOVERNMENT, AND A REPRESENTATIVE FROM THE INTERFAITH COMMUNITY MET TO REVIEW THE DATA AND DISCUSS THE HEALTH PRIORITIES THAT UMUCH WOULD FOCUS ON FOR THE NEXT THREE YEARS. THE BOARD OF DIRECTORS OF UMUCH VOTES TO ADOPT THE FINAL SUMMARY REPORT AND IMPLEMENTATION STRATEGY AND PROVIDE THE NECESSARY RESOURCES AND SUPPORT TO CARRY OUT THE INITIATIVES THEREIN. UMUCHS MAINTAINS A KEY LEADERSHIP ROLE IN HEALTHY HARFORD (AN IRC SECTION 501(C)(3) ORGANIZATION LABELED - THE HEALTHY COMMUNITIES INITIATIVE OF HARFORD COUNTY), ESTABLISHED IN 1995. IN 1996, HEALTHY HARFORD BEGAN COLLECTING COMMUNITY DATA VIA A COMPREHENSIVE COMMUNITY HEALTH ASSESSMENT PROJECT (CHAP) SURVEY THAT MEASURED THE INCIDENCE OF DISEASE, PREVENTIVE BEHAVIORS, AND LIFESTYLE BEHAVIORS OF HARFORD COUNTY RESIDENTS WITH AN EYE TOWARDS ASSESSING COMMUNITY HEALTH AND ESTABLISHING HEALTH PRIORITIES IN THE COMMUNITY. CHAP DATA WAS SUBSEQUENTLY COLLECTED IN 2000, 2005, AND 2010. THE GOALS OF THE CHAP SURVEY WERE MULTIPLE: ASSESSING THE OVERALL HEALTH OF HARFORD COUNTY ADULT RESIDENTS, INSURING THAT HEALTH EDUCATION AND PROGRAMMING EFFORTS IN HARFORD COUNTY MATCH ACTUAL NEEDS, ESTABLISHING A BASELINE OF HEALTH INDICATORS SO THAT PROGRESS CAN BE MEASURED OVER TIME, AND ALIGNING COMMUNITY STAKEHOLDERS AROUND THE COMMON GOAL OF IMPROVING HEALTH IN OUR COMMUNITY. TO MEET THE NEW AFFORDABLE CARE ACT REQUIREMENTS, IN 2014 THE CHAP SURVEY WAS REPLACED WITH THE MORE UNIVERSAL COMMUNITY HEALTH NEEDS ASSESSMENT (CHNA) SURVEY WHICH IS TIED NATIONALLY TO HOSPITAL-BASED COMMUNITY BENEFIT REPORTING. IN ADDITION TO THE CHNA SURVEY, A STATISTICAL SECONDARY DATA PROFILE IS USED THAT DEPICTS POPULATION AND HOUSEHOLD STATISTICS, EDUCATION AND ECONOMIC MEASURES, MORBIDITY AND MORTALITY RATES, INCIDENCE RATES AND OTHER HEALTH STATISTICS FOR HARFORD COUNTY. THE DATA WAS GATHERED AND INTEGRATED INTO A GRAPHICAL REPORT TO PORTRAY THE CURRENT HEALTH AND SOCIO-ECONOMIC STATUS OF RESIDENTS OF HARFORD COUNTY. SECONDARY DATA WAS COLLECTED FROM REPUTABLE SOURCES INCLUDING THE U.S. CENSUS BUREAU, CENTERS FOR DISEASE CONTROL AND PREVENTION, AND THE MARYLAND DEPARTMENT OF HEALTH AND MENTAL HYGIENE STATE HEALTH IMPROVEMENT PLAN. KEY COMMUNITY PARTNERS AND INFLUENCERS IN CREATING THE SECONDARY DATA PROFILE WERE UMUCH, HEALTHY HARFORD, THE COUNTY HEALTH RANKINGS AND ROADMAPS PROJECT, THE HARFORD COUNTY HEALTH DEPARTMENT, AND THE OBESITY TASK FORCE OF THE HEALTHY COMMUNITY PLANNING BOARD. AN ONLINE COMMUNITY SURVEY WAS CONDUCTED WITH HARFORD COUNTY RESIDENTS BETWEEN OCTOBER 2014 AND JANUARY 2015. THE SURVEY WAS DESIGNED TO ASSESS HEALTH STATUS, HEALTH RISK BEHAVIORS, PREVENTIVE HEALTH PRACTICES AND HEALTH CARE ACCESS PRIMARILY RELATED TO CHRONIC DISEASE AND INJURY. A TOTAL OF 1,549 SURVEYS WERE COMPLETED THROUGHOUT THE COUNTY TO PROMOTE GEOGRAPHICAL AND ETHNIC DIVERSITY AMONG RESPONDENTS. ADDITIONALLY, KEY INFORMANT INTERVIEWS WITH 54 REPRESENTATIVES FROM PUBLIC HEALTH AND MEDICAL SERVICES DEPARTMENTS, NON-PROFIT AND SOCIAL ORGANIZATIONS, CHILD AND YOUTH AGENCIES AND THE BUSINESS COMMUNITY WERE CONDUCTED BETWEEN OCTOBER AND NOVEMBER 2014. THE HEALTH AREAS THAT WERE DEEMED THE MOST PRESSING FOR OUR COMMUNITY WERE (1) CHRONIC DISEASE, (2) TOBACCO USE, (3) MENTAL HEALTH/ADDICTIONS, (4) ACCESS TO CARE, (5) MATERNAL AND CHILD HEALTH, AND (6) INJURY AND ILLNESS PREVENTION.
