The facilities are adequate for treatment or care of individuals suffering from mental or physical illness, disease, injury or deformity in an acute care, skilled nursing care, outpatient or emergency room setting. LCH cares for residents of the Smoky Valley with the operation of a 24/7 emergency room. 1,966 emergency room patients were seen in the 2018 fiscal year. 562 urgent care visits, 565 privately paid home health visits were made, and 263 cardiac rehab visits improved the health of community members. 12,485 visits were made in the Family Health Care Clinic to provide acute treatment, manage chronic conditions, or provide preventive care. 3,156 meals on wheels were supplied to community members. Education was provided to the Smoky Valley Community through Stepping On, Safe Sitter, and Chronic Disease Self-Management Program education. Employees of the LCH partner with various community groups to provide assistance to the Midsummer's Committee, Hyllningsfest Committee, Chamber of Commerce, and Strategic Alliance to name a few. Part X, Line 2:Lindsborg Community Hospital Association is organized as a Kansas not-for-profit corporation and has been recognized by the Internal Revenue Service (IRS) as exempt from federal income taxes under Internal Revenue Code Section 501(c)(3). Lindsborg Community Hosptial Association is annually required to file a Return of Organization Exempt from Income Tax (Form 990) with the IRS. In addition, the organization is subject to income tax on net income that is derived from business activites that are unrelated to its exempt purpose. In this case, the organization would file an Exempt Organization Business Income Tax Return (990T) with the IRS to report unrelated business taxable income. Lindsborg Community Hospital Assoication believes that it has appropriate support for any tax positions taken affecting its annual filing requirements, and as such, does not have any uncertain tax positions that are material to the financial statements. The organization would recognize future accrued interest and penalties related to unrecognized tax benefits and liabilities in income tax expense if such interest and penalities are incurred.

Lindsborg Community HospitalPart V, Section B, Line 6a: McPherson Hospital, McPherson, Kansas Mercy Hospital, Moundridge, Kansas
Lindsborg Community HospitalPart V, Section B, Line 6b: McPherson County Health Department
Lindsborg Community HospitalPart V, Section B, Line 11: The Implementation Strategy was completed in July 2016. All identified needs are being addressed.Health Issues - The CHNA identified a need for services that target obesity, cancer, and mental health issues. To address obesity a Medically Supervised Weight Management program has been expanded with the addition of Jade Banning, PA. Jade provides services one afternoon per week. Renee Riedel, RDLD, CDE also joined the LCH healthcare team. She provides inpatient and outpatient consultation. We are targeting cancer by utilizing our EMR to identify patients who use tobacco. We offer smoking cessation programs and early cancer detection through mammography and colonoscopy. The percentage of patients receiving appropriate screenings are being measured and compared against national benchmarks. Mental health is a challenge throughout the state. LCH partnered with Prairie View Mental Health to provide emergency screening through telemedicine. We also continue our partnership with Veridian Behavioral Health to provide care. Central Kansas Foundation is utilized for those who've received a referral for alcohol or drug treatment. Education on many of these health issues has been performed since the inception of our implementation plan.Specific Populations with Barriers to Access and Risky Behaviors - The CHNA identified availability to medical specialists and hours of operation as the barriers to access. The risky behaviors were identified as drug abuse, texting/cell phone use while driving, and alcohol abuse. We hope to improve upon Access to specialists with the addition of orthopedics through our managing entity, Salina Regional Health Center. We are in search of further Orthopedic services, as a part of SRHC. With our patient centered remodel allowing for the expansion of urgent care we will have the ability to offer Urgent Care services seven days per week in the fall of 2019. This will greatly expand access to our service area and beyond. Drug and alcohol abuse typically go hand in hand. Part of the action plan will be to continue to perform alcohol and drug screening in the clinic and Emergency Department with referral to the Central Kansas Foundation or other facilities as needed. Standardized evaluation tools in the hospital and clinic EMRs improve the consistency of screening for these behaviors. Another current nationwide challenge is the abuse of opioid medications. The Family Health Care Clinic has adopted policies to monitor and reduce the amount of opioids, as per Center for Disease Control (CDC) guidelines. Improved standardization and policies are being developed in the Emergency Department as well. These actions will be ongoing in development, education and enforcement. Texting and cell phone use while driving was also identified as a risky behavior. Facebook and newspaper will be used to provide public safety announcements around texting and cell phone use while driving. Partnerships with the local schools, as well as local law enforcement will also be developed to enhance the outreach. Action plan for Strengthening Families: Circles of McPherson County is part of a national program to assist families. Taken from its website, "Circles connects people in relationships across income lines and helps develop clear goals for education and stable income. Circles also focuses on supporting families to help themselves completely out of poverty." LCH Administrator Larry Van Der Wege began serving on the board of the local chapter, Circles of Smoky Valley, in the fall of 2017. The LCH Employee Committee has also designated three months' worth of employee donations to this entity. Although not specific for just "strengthening families", LCH took part in training local lay people and two non-clinical employees in teaching management of chronic conditions. The program - developed by Stanford University - is designed to teach people from the community on how to manage their chronic condition, through research, goal setting and partnership with their medical provider(s). LCH has already begun providing the session course at no cost to members of their service area. The initial training for the local teachers was provided through a partnership with the Kansas Department of Health and Environment and the Kansas Foundation for Medical Care.
Lindsborg Community HospitalPart V, Section B, Line 13h: The methodology used to determine eligibility for financial assistance takes into consideration income, net assets, family size and resources available to pay for care. In addition, presumptive charity care may be applied in situations where all other avenues have been exhausted.
Lindsborg Community HospitalPart V, Section B, Line 16j: The FAP application and plain language summary can be found at the URL listed on lines 16b and 16c by clicking on the link "Learn More - financial assistance program form."
Lindsborg Community HospitalPart V, Section B, Line 24: The hospital financial assistance policy does not cover elective procedures. The hospital may have charged FAP eligible patients gross charges for services that are not covered under the financial assistance policy.

In both meetings, issues concerning needs of the community are discussed and addressed or at least brought to the attention of administration for development of an action plan.LCHA has a Patient and Family Advisory Council (PFAC) which is made up primarily of community members who are patients or family members of patients. The group meets monthly to discuss quality and other projects that LCHA is performing. As a part of the regular agenda, community members are given an opportunity to voice comments and concerns about needs of the community.
Part VI, Line 3:The Hospital's charity care policy is posted in patient waiting rooms in a three-ring note book. Surplus funds are utilized to provide home health services, health and education through the Lindsborg News-Record and the Marquette Tribune, Stepping On classes, Safe Sitter classes and frozen meals for purchase. The organization's governing body is comprised primarily of persons who reside in the organization's primary service area who are neither employees nor contractors of the organization nor family members thereof. The organization extends medical staff privileges to qualified physicians in its community for some or all of its departments. Surplus funds of the organization are returned to the organization to improve patient care through updated equipment and technology and care team member education. The organization operates an emergency room that is available 24/7 regardless of ability to pay. Through the assistance of a donor, the organization has trained 825 CNA students over the last 16 years through a partnership with Bethany Home and The Cedars.
