Part V, Line 4: Intended uses of the endowment fund.Endowment funds are designated for purposes that align within the organization's exempt purpose.
Part X : FIN48 FootnoteEMHS, its hospitals, and certain other affiliates have been determined by the Internal Revenue Service to be tax-exempt charitable organizations as described in Section 501(c)(3) or 501(c)(2) of the Internal Revenue Code (the Code) and, accordingly, are exempt from federal income taxes on related income pursuant to Section 501(a) of the Code. Accordingly, no provision for federal income taxes has been recorded in the accompanying consolidated financial statements for these organizations.Tax-exempt charitable organizations could be required to record an obligation for income taxes as the result of a tax position they have historically taken on various tax exposure items including unrelated business income or tax status. Under guidance issued by the Financial Accounting Standards Board (FASB), assets and liabilities are established for uncertain tax positions taken or positions expected to be taken in income tax returns when such positions are judged to not meet the "more-likely-than-not" threshold, based upon the technical merits of the position. Estimated interest and penalties, if applicable, related to uncertain tax positions are included as a component of income tax expense. The System has evaluated its tax position taken or expected to be taken on income tax returns and concluded the impact to be not material.Certain of the System's affiliates are taxable entities. Deferred taxes related to these entities are based on the difference between the financial statement and tax basis of assets and liabilities using enacted tax rates in effect in the years the differences are expected to reverse. The deferred tax assets and liabilities for these entities are not material.

Part V, Line 5 - Account Input from Persons Who Represent the CommunityThe Maine Shared CHNA research team conducted a statewide survey among stakeholders to identify and prioritize significant health issues in communities across the state. The survey, coordinated with the Maine CDC, engaged public health expertise throughout the process. The survey was administered using a snowball approach where stakeholder agencies agreed to send the surveys to their members and stakeholders for participation. 1,639 people, representing more than 80 organizations and businesses in Maine, completed the survey. Respondents represented health care agencies, public health agencies, law enforcement, municipalities, schools, local businesses, social service agencies, and nongovernmental organizations. In addition to the stakeholder survey, each county conducted a number of community engagement opportunities (forums) to ensure broad interests of the local community were represented; Obtain stakeholder input on identifying significant health needs based on review of shared CHNA data; Solicit stakeholder feedback on prioritizing significant health needs, and; Identify local assets and resources that potentially may address local health priorities. The local forums were conducted in collaboration with the Maine CDC to solicit input from individuals representing populations with health disparities. The following organizations were in attendance at the December 10, 2015 DCC SHNAPP Waterville Shared CHNA Community Engagement Input Forum and provided valuable feedback on the Shared CHNA.Details about organizations: Sample of organizations KVCAP, Waterville Main Street, Kennebec Messalonskee Trails, Maine Children's Home, NAMI Maine, American Lung Assn., Healthy Northern Kennebec, Somerset Public Health, Healthy Sebasticook Valley, MaineGeneral (MG), Inland Hospital, Redington Fairview General Hospital, EMHS, HealthReach Community Health Centers, Kennebec Behavioral Health, MG Community Care, Assistance Plus, MSAD 59, Waterville/Vassalboro Before/After School Programs, MG Board of Directors, Maine CDC. Hospitals (MaineGeneral, Inland, EMHS, Redington Fairview), health care providers (HealthReach Community Health Centers), and behavioral health care providers (Kennebec Behavioral Health, MG Community Care) serve low income patients through Medicare/MaineCare and provide low-cost/no-cost services to medically underserved residents. Many public health programs and services (Healthy Northern Kennebec, Somerset Public Health, Healthy Sebasticook Valley), non-profit agencies (KVCAP, Maine Children's Home, NAMI Maine), and local/state government agencies (Maine CDC) specifically serve low-income and medically under-served residents.
Part V, Line 6a - List Other Hospital Facilities that Jointly Conducted Needs AssessmentThe Shared CHNA was conducted through the Maine Shared Health Needs Assessment Planning Process (SHNAPP) a collaborative effort among Maine's four largest health-care systems Central Maine Healthcare, Eastern Maine Healthcare Systems (EMHS), MaineGeneral Health, MaineHealth and the Maine Center for Disease Control and Prevention, an office of the Maine Department of Health and Human Services (DHHS). EMHS member organizations participating in the Shared CHNA included Acadia Hospital, Blue Hill Memorial Hospital, Charles A. Dean Memorial Hospital, Eastern Maine Medical Center, Inland Hospital, Maine Coast Memorial Hospital, Mercy Hospital, Sebasticook Valley Health, and TAMC.
Part V, Line 11 - Explanation of Needs Not Addressed and Reasons WhyInland Hospital non-selected priorities-Immunizations are provided in our medical practices and through a variety of community settings and programs.Behavioral/mental health and substance abuse - As a small hospital, we are unable to commit the financial resources to address the gaps at this time. Part I, Line 7, Column F - Explanation of Bad Debt Expense$4,824,735 of bad debt expense, $2,352,126 of charity care, $65,049,713 of contractual allowances is included on Form 990, Part IX, line 25, column (A).
Part III, Line 2 - Methodology Used To Estimate Bad Debt ExpenseThe costing methology used to determine the amount is cost to charge ratio.
Part III, Line 4 - Bad Debt ExpensePatient and trade accounts receivable are stated at the amount management expects to collect from outstanding balances. Management provides for probable uncollectible amounts through a charge to earnings and a valuation allowance based on its assessment of the current status of individual accounts. Balances that are still outstanding after management has used reasonable collection efforts are written off through a charge to the valuation allowance and the applicable patient accounts receivable. Credit is extended without collateral.
Part III, Line 8 - Explanation Of Shortfall As Community BenefitMedicare losses should be treated as a community benefit because the losses are incurred in performing an important public service, and Maine hospitals experience one of the lowest Medicare reimbursement rates in the country.
Part III, Line 9b - Provisions On Collection Practices For Qualified PatientsAll account guarantors who express an inability to pay inpatient and outpatient services will be screened for eligibility for charity care using an application and guidelines established by Inland Hospital. An account may be reconsidered for charity care at any time when new information is available about a patient's inability to pay.
Part VI, Line 3 - Patient Education of Eligibility for AssistanceAt Inland Hospital, Financial Counselors are available to assist and answer any questions our patients may have regarding Financial Assistance. Additional interactive strategies are in places that include billing statements that have contact information added for the patient's convenience, providing all uninsured patients with financial assistance packets at point of registration. When a patient states that they are under-insured, Financial Assistance Packet information is provided. For any other patient requests we receive (via emails and phone calls) packets are mailed. All patient accessible areas, such as hospital waiting areas, contain information to direct patients on how to receive financial assistance. Additionally, Inland Hospital provides online resources and tools to assist our patients with the necessary information regarding charity/financial assistance via www.northernlighthealth.org. Inland Financial Counselors are Certified Application Counselors for the Federal Market Place and will assist customers with the Healthcare Market Place as requested. It is the policy of Inland Hospital to treat all patients requiring care without regard for ability to pay and to assist patients in arranging for settlement of their financial obligation. Under this program, medically necessary services will be provided to Maine residents at no cost. If a patient does not qualify for financial assistance, he or she is entitled to ask for a fair hearing.
Part VI, Line 4 - Community InformationInland Hospital is a not-for-profit, community hospital located in Waterville, Maine with a mission to care for and serve our community. We serve residents in parts of Kennebec, Somerset, and Waldo Counties (mostly Kennebec). We are a 48-bed medical-surgical facility, with Lakewood, our 105-bed continuing care facility on the hospital campus, along with 14 primary and specialty care physician practices in Waterville and the surrounding communities of Unity, Oakland, and Madison. We also offer our primary care patients a Weekend Care service to help them avoid unnecessary, costly emergency room visits. Inland has two rural health centers in Unity and Madison serving medically underserved populations.Inland is the only inpatient hospital serving Waterville. The next nearest inpatient hospital is located 15 miles south of Waterville in Augusta. The nearest tertiary hospital is located 60 miles north of Waterville.Data from US Census Bureau, American Community Survey 2012-2016 shows that Kennebec County has 120,953 residents. Median household income is $48,570. 14.6 percent of individuals and 20.3 percent of children are living in poverty, according to US Bureau of Labor Statistics, 2015-2017. Somerset County has 51,363 residents. Median household income is $40,484. 18 percent of individuals and 26.2 percent of children are living in poverty.
Part VI, Line 4 - Community Building ActivitiesInland Hospital gets involved and gives back to our community to improve quality of life in Greater Waterville its how we help fulfill our community service mission to care for and serve our community. Whether its sponsoring scholarships to the local colleges, supporting physical activity and nutrition initiatives in our local schools, funding local public transportation, or supporting a cultural or economic development effort, Inland is proud to support and collaborate with groups that foster health and well-being. Our Community Benefit Grant Program contributed $60,000 in 2018 towards health and community organizations and initiatives. Inland donated $10,000 to the City of Waterville to support the RiverWalk at Head of Falls, a community building project which is part of the continued revitalization of Watervilles downtown area. $5,000 was granted to the Central Maine Growth Council for economic and workforce development efforts in our community.
Part VI, Line 5 - Promotion of Community HealthAt Inland we strive to live up to our non-profit mission every day by caring for community members who otherwise would have limited access to quality healthcare. Besides providing charity care and financial assistance, we believe we can also affect the health of the communities we serve beyond the walls of our hospital and medical practices. We agree with the World Health Organization which states, Health is a state of complete physical, mental, and social well-being, and not merely the absence of disease or infirmity. We have a vision to be a responsible corporate citizen whose actions can impact our communitys overall well-being through commitments to priority issues. A hospital task force considered priority issues from the 2014 Community Health Needs Assessment and other data, and identified areas of significant needs to be addressed, as well as areas of need unable to be addressed by the hospital. An Implementation Strategy was developed and consisted of actions the hospital took to address the health need. Programs/resources the hospital committed to address the health need were identified along with planned collaborations with other area organizations. Inland Hospital focused on four key areas: Childhood obesity; Mental health; Substance abuse; Poverty/Economic Opportunity. In addition to efforts led by Inland and our collaborating partners on the issues above through our Community Health Strategy, resources such as in-kind staff time and community benefit funding were dedicated to the following secondary issues:-Domestic Violence-Homelessness-Economic development-Education-Cultural supportInland's 2018 Community Wellness initiatives included:a)"Womens Wellness Walk", an organized community walk held at established walking routes. The event shares with our community the safe, easy, local places to walk for good health.b)In an effort to combat the childhood obesity epidemic, in 2018 we continued our year-long program to reach families with children to encourage active lifestyles and healthier eating. Our "Let's Go! Family Fun Series" offers regular activities to engage families with children to be physically active and eat healthier through free and low cost resources and activities such as hiking, swimming, and cross-country skiing. We also provided $10,000 in mini-grants for Let's Go! in 17 local schools, out-of-school and childcare sites through our Community Benefit Grant Program. Lets Go! is a nationally recognized childhood obesity prevention program that focuses on healthy habits and changing environments where children and families live, learn, work, and play.c)We also utilized the Lets Go! program in the healthcare sector. Five of Inland's practices were recognized as Let's Go! Sites of Distinction in 2018 for their efforts to curb childhood obesity in our patient populations. Components of the program include displaying posters with the 5210 healthy habits messages, tracking weight classification for patients ages 2-18, and using the Healthy Habits survey at all well-child visits to inspire respectful conversations around weight status and lifestyle behaviors. d)We continued to dedicate staff time to provide Lets Go! technical support to local schools, after-schools, and childcare sites.e)We continued to maintain the Inland Woods Trail on the hospital campus (created in 2010) with support from several local organizations. The trails were developed to create a lasting way to impact health, and give residents an easy, free resource for regular activity. The trails are used regularly by residents, hospital staff, and local schools for walking, biking, and snowshoeing. The trailhead was enhanced by adding a family-friendly Little Free Library (in 2017).f)We provided ongoing support for the Quarry Road Recreation Area, investing $46,000+ since 2013 in annual support ($3,500 in 2018), event and youth program sponsorships and in-kind contributions of staff volunteer hours and board participation.g)Inland provided the following Community Benefit funding in 2018: $4,250 to the Greater Waterville Area Food Bank for a new industrial refrigerator; $5,000 to Kennebec Behavioral Health to support outreach to homeless youth; $2,500 to Unity Barn Raisers for food access programming; $1,500 to Maine Farmland Trust for its Veggies for All community garden and food bank program; $5,000 to Spectrum Generations Meals on Wheels program for homebound elderly; $2,500 to NAMI Maine for Mental Health First Aid training for youth and adults. h)We employ a full-time Community Health Navigator to help patients connect to local resources that help them overcome barriers to good health, such as housing, medication support, transportation, food support, and health care access. In 2018, our Navigator managed a pilot project with the Good Shepard Food Bank to provide 240 food bags to Inland patients who were identified during a primary care visit to face immediate food insecurity.A majority of Inland Hospitals board of trustees is comprised of people who reside in our primary service area who are neither employees nor independent contractors of the organization, nor family members thereof. Inland extends medical staff privileges to qualified physicians in its community for some or all of its departments or specialties. The Inland Hospital Foundation helps by raising funds for everything from capital expansion projects to residential activities to help offset normal operating expenses.Inland Hospital utilizes surplus funds to support a medical simulation center on the hospital campus that is used for training of clinical staff. Examples of training include Advanced Cardiac Life Support, Neonatal Resuscitation Program, and Pediatric Advanced Life Support. Training is not limited to employees at Inland Hospital - we also offer courses to local Emergency Medical Technicians and to clinicians at other hospitals in our region. Inland Hospital invites men, women, and young adults to volunteer their time to further the hospital's mission. In 2018, volunteers from our community donated 10,919 hours of their time to Inland and Lakewood, our continuing care center. Some volunteers work in areas with direct contact with patients and families; others are involved in behind the scene activities, which enhance the care provided by the hospital. In 2018, more than 100 Inland and Lakewood employees engaged in our community by volunteering on local boards, committees and projects that helped contribute to a vital community.
Part VI, Line 6 - Affilated Health Care SystemThe 2016 Shared Community Health Needs Assessment conducted by EMHS (Eastern Maine Healthcare Systems) in collaboration with several member/affiliated hospitals and the many public health and community organizations across the state. The Shared Community Health Needs Assessment informs initiatives to promote community health across the system as well as within each member hospitals local service area. Each member hospital adopted a local implementation strategy and community health improvement plan, tailored to meet local needs.Inland Hospital has been part of EMHS (Eastern Maine Healthcare Systems) since 1998. As part of EMHS, we conducted the 2016 Shared Community Health Needs Assessment in collaboration with several member/affiliated hospitals and many public health and community organizations across eight Maine counties.

It appears in our state motto Dirigo from the Latin word for I lead. We were the first state to elect a woman, Margaret Chase Smith, to both houses of Congress. We are home to the countrys first veterans hospital, and we are the first state to greet a new day as the rays of the sun touch our easternmost peaks and shores before any other place across the continental United States. The light of a new day symbolizes a rebirth or an awakening. And this renewed purpose to make healthcare work for people across our great state is what we are experiencing as we embrace our new name-Northern Light Health. In the pages of this years annual report, you will see examples of how we are rolling up our sleeves to raise quality through innovation, teamwork, and efficiency. We have fostered new clinical relationships with Massachusetts General Hospital, a world-class medical research hospital. We are using population health data in new ways to be smarter about how we deliver care. And we have created new programs that bring healthier locally-sourced foods to our hospitals while supporting the hard-working farmers and fishermen who make up the very core of our identity here in Maine. We are working to make healthcare more accessible and straightforward while also making great strides to reduce our costs and improve our operating margins. We are finding new ways to guide our patients and deliver care based on the needs of each person. Its a New Day in Maine. We invite you to join us on this new journey.M. Michelle Hood, FACHENorthern Light Health President and CEOBarry McCrumNorthern Light Health Board ChairA New LightWelcome to Northern Light HealthOpen up, give me a big ahhh, Sheena Whittaker, MD, a pediatrician at Northern Light Pediatrics in Ellsworth asks of her 12-year old patient, Ellis, as she shines a light in his mouth. They are in a pediatrics exam room where Ellis is getting a check up. The windows allow in a generous amount of light; the walls are warm yellow and decorated with Dr. Seuss decals like the Cat in the Hat. Dr. Whittaker wears many hats too. Not only is she a pediatrician, but she is also the senior physician executive for Northern Light Maine Coast Hospital in Ellsworth.Medicine is very controlled, and its very scientific and careful, and it should be; but kids bring a fun aspect to it, Dr. Whittaker explains. If we can fortify children to be strong mentally, physically, emotionally, and psychologically, were going to produce stronger, kinder, healthier children, a better adult population, and a stronger community.Dr. Whittaker may be based at Maine Coast Hospital, but she is part of Northern Light Health, formerly Eastern Maine Healthcare Systems. In addition to changing its name, Northern Light Health has re-identified its priorities and is becoming more integrated across the entire system which spans from Portland to Presque Isle and Greenville to Blue Hill.Dr. Whittaker sees the move to Northern Light Health as a step in the right direction that helps improve patient care. Being united medically means we have resources from the whole Northern Light Health system. Whether were at Northern Light Maine Coast, Northern Light Blue Hill, or Northern Light Eastern Maine Medical Center, we have access to all of the specialty services easily, consecutively, consistently. So, it doesnt matter which hospital a patient walks into, they are receiving the best medical care they possibly can get, explains Dr. Whittaker.Michelle Hood, FACHE, president and CEO of Northern Light Health, and Matt Weed, senior vice president and chief strategy officer have not only worked to implement this new brand identity but to spread the message of what it means beyond the name change. They also called on leaders and front line employees throughout the system to help.At Northern Light Health, we want to make healthcare work for everyone, whether thats an individual or a community. Were going to roll up our sleeves and bring a lot of energy and enthusiasm to our continuing work, explains Michelle. Were raising the bar and that is a simple but substantive way of saying we cant rest. Were always looking for a better way to do things. Our system is very broad not just in geography but in the types of services that we offer, and we want to meet people where they are.Northern Light Health is accomplishing this by making investments in telemedicine, converting to one unified electronic health record system that links all member hospitals together, and creating new clinical relationships with world-class organizations like Massachusetts General Hospital and Dana Farber Cancer Institute.A rebrand to Northern Light Health spans timeit celebrates how far we as a system have come, but it also creates a single shared starting line for where we need to go next, Matt explains. As chief strategy officer, Matt was instrumental in the rebranding effort which was a necessary evolution as healthcare in America is moving toward integrated systems. This in part is driven by economics and financial sustainabilityin all states including Maine, the healthcare industry is being required to coordinate expertise in a way that avoids unnecessary costs.Businesses, governments, and individual consumers are looking to contract with systems for their healthcare needs because this is how they will get the highest quality at the lowest cost. More importantly still, Matt feels that what people want is to know that when the need arises, they or their loved one will receive competent, compassionate care. This is the brand promise, concludes Matt. Its what our neighbors and our family members and a person were meeting for the first time can expect when they come to our physicians and to our hospitals, and when they interact with any other person or service in the system. If were not doing it perfectly today, were motivated and driven by the brand promise to improve; and if we are doing it perfectly today, the brand promise says we wake up and do it again tomorrow.A New FocusOncology Pharmacy Management ProgramLori Boynton is at her happiest at home in her backyard, a spacious spread of land in Lamoine with barns, a paddock, and animals. She cracks a smile and laughs as Clementine quickly scoffs down a fresh carrot. Im the beast of burden. They dont do any work at all, she laughs, I do all the work and thats my therapy which has been awesome. To look at Lori, with her thick wavy brown hair and healthy complexion, its hard to imagine that just months ago she was battling cancer and undergoing chemotherapy and radiation treatments. Its such a shocking thing when you hear you have cancer, and I just thought its going to be stage one, itll be no problem; it was actually stage three! It makes you think much differently about your time here, she says.Lori was diagnosed with stage III colon cancer in October 2017 and would undergo treatments through May 2018. She had to go to Northern Light Cancer Care for radiation treatments but was able to take oral chemotherapy medication at home. Having already started on chemotherapy treatments at the hospital, Lori knew how it made her feel.Youre kind of in the cloud. Its kind of foggy for lack of a better word. Before, I could multi-task and I could just keep things going in my brain and then it was gone! What Lori was experiencing is not uncommon, explains Sheila Pascual, MD, who was Loris oncologist, A lot of literature has been written about what they call chemo brain or brain fog and its a neural-cognitive dysfunction where theres a decline or deficit in memory, learning new things, attention.Lori might have been worried about trying to keep track of her medications, appointments, bills, and daily chores, except that she was an outpatient in Northern Light Pharmacys Oncology Pharmacy Management Program and she had help keeping track of it all.
