DescCabinet Peaks Medical Center operates a 25-bed critical access hospital Providing medical services to the residents of Lincoln County. Due to the remote location of Lincoln County, Cabinet Peaks serves a critical role in caring for the medical needs of its community, as the next closest emergency room is over 50 miles away and the nearest regional hospital is 90 miles away. As such Cabinet Peaks makes it a priority to provide an Intensive Care Unit, 24/7 Emergency Services, Birthing Services, and extensive Surgery Services. Cabinet Peaks also offers a complement of outpatient services, including primary care and OB/GYN services, a variety of rehab and therapy services, infusion and chemotherapy, imaging, and lab. Continued on Schedule O.Patients often do not have the luxury of time or money or the physical strength necessary to make the 90-mile trip to Kalispell to receive these services, which is why Cabinet Peaks recognizes the value of providing these services close to home. In fiscal year 2023, Cabinet Peaks Medical Center provided services to 10,750 unique patients amounting to a total of 19,609 adjusted patient days. 1,465 surgical cases were performed. Additionally, Cabinet Peaks Medical Center delivered 125 babies during fiscal year 2024. As Lincoln County consistently has one of the top unemployment rates in the State and is an economically depressed area, many of Cabinet Peaks' patientsare uninsured or underinsured, meaning Cabinet Peaks provides a significant amount of uncompensated care. The Hospital provides health care services to patients who meet certain criteria under its charity care policy without charge or at amounts less than established rates. Since the Hospital does not pursue collection of these amounts, they are not reported as patient service revenue. The amounts of charges foregone, based on established rates, were $827,737 and $934,249 during the years ended March 31, 2024, and 2023, respectively. The estimated cost of providing these services was approximately $513,000 and $556,000 for the years ended March 31, 2024and 2023, respectively, calculated by multiplying the ratio of cost to gross charges for the Hospital by the gross uncompensated charges associated with providing charity care to its patients.Desc

Cabinet Peaks Medical CenterPart V, Section B, Line 5: Cabinet Peaks Medical Center (CPMC), Northwest Community Health Center (NWCHC), and the Lincoln County Health Department (LCHD) collaborated on a joint community health needs assessment. The Hospital, Community Health Center and Health Department jointly participated in the Community Health Services Development (CHSD) Project administrated by the Montana Office of Rural Health (MORH). Community involvement in steering committee meetings, focus groups and key informant interviews enhance community engagement in the assessment process.CPMC's, NWCHC's, and LCHD's service area was surveyed about its healthcare system, local health issues, and the overall health of the community. Three focus group interviews were conducted to identify important local healthcare issues, how to improve the health of the community, and gaps in health services. It was intended that this research would help determine the awareness of local programs and services as well as the level of satisfaction with local services, providers, and facilities. The focus groups were designed to represent various consumer groups of healthcare including senior citizens, local community members, and community leaders.A steering committee was convened to assist Cabinet Peaks Medical Center, Northwest Community Center, and the Lincoln County Health Department in conducting the Community Heath Needs Assessment. A diverse group of community members representing various organizations and populations within the community (ex. public health, elderly, uninsured) came together. The Steering Committee met twice during the CHSD process: first to discuss health concerns in the community and offer their perspective in designing the survey instrument and again to review results of the survey and focus groups and to assist in the prioritization of health needs.
Cabinet Peaks Medical CenterPart V, Section B, Line 6b: Cabinet Peaks Medical Center (CPMC), Northwest Community Health Center (NWCHC), and the Lincoln County Health Department (LCHD) collaborated on a joint community health needs assessment.
Cabinet Peaks Medical CenterPart V, Section B, Line 11: 1. Access to care (i.e., mental health and specialty services)2. Social determinants of Health3. Healthy lifestyles1. 41.9% of survey respondents indicated "Asbestos related disease" is a top health concern for the community. This will not be addressed because CARD Clinic fulfills most of this need.2. 10.4% of survey respondents indicated that they had difficulty getting a prescription or taking their medication regularly in the last year. This as been addressed with the 340B Program which includes providers and retail pharmacies.3. 40.1% of survey respondents reported that they or a member of their household thought they needed healthcare services in the last three years, but had to delay getting them. The top reasons for delaying services included "It cost too much" (31.3%), "My insurance didn't cover it" (28.4%), and "Could not get an appointment" (19.4%). This need is addressed through publishing and promoting charity care policies(sliding fee scale). In the current year the facility has taken the following actions to address access to care: * Sought and hired an additional General Surgeon * Hired a Women's Health NP * Changed to a Physician based hospitalist model for inpatient care. * Pediatric PT became certified the Prechtl Assessment of General Movement Basic * Hired an additional Family Medicine PA-C * Continued to upgrade/replace equipment for enhanced patient care * Hired additional staff in Rehab to help with the patient loadIn the current year the facility has taken the following actions to address healthy lifestyles: * Health Fair * PREVENT Lifestyle & Health classes. * Offered Senior Life Solutions for aging population. * Fundraised for and offered free mammograms to the under/uninsured. * Sustained Safe to Sleep program. * Offered car seat safety for new parents. * Held prenatal/postnatal classes. * Labor and Delivery RNs are Certified Lactation Consultants * Started "breathe better" club for those dealing with pulmonary issuesSocial Determinates of Health* Actively seeking grants to provide healthy food options for patients leaving the hospital that do not have the means to purchase healthy, fresh food.* Researching the need and ability to provide behavioral health* Food Services and Nutrition are working to provide a healthy cooking class for our patients and their families with a focus on Congestive Heart Failure (CHD) * Helped with the Back to School Bash-community effort offering school supplies, shoes, haircuts, and available community resources- to name a few. * County website created listing available services and resources. * Working on ways to educate the community on prevalent morbidities such as COPD, CHD, HTN, Diabetes, Obesity, * Mental Health using different platforms to reach more people.
Cabinet Peaks Medical CenterPart V, Section B, Line 13h: The following information will be taken into account in determining eligibility for and amount of financial assistance:1. The amount of third-party financial resources including insurance and health plan benefit coverage, or government health plan coverage (such as Medicare or Medicaid), any recovery from a personal injury claim, Victims of Crime assistance, and non-hospital financial aid programs (including public assistance and private charity or foundation grant programs, for example), will be taken into account.2. Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Part I, Line 3c:CPMC (with the exception of Cabinet Peaks Clinics) also uses an asset test. A patient must meet both the income and asset criteria. Insurance status and medical indigency are also used to determine eligibility.
Part I, Line 6a:The Organization's community benefit report can be found on its website at https://www.cabinetpeaks.org/getpage.php?name=Community_Impact&sub=About%20Us
Part I, Line 7:Charity care expense was converted to cost using an overall cost-to-charge ratio addressing all patient segments. Community health improvement services on lin 7e are reported on based on actual expenses. The cost for subsidized health services on line 7g was determined using the Medicare cost report.
Part III, Line 2:The amount reported on line 2 represents implicit price concessions. CPMC determines its estimate of implicit prices concessions based on its historical collection experience with the class of patients.
Part III, Line 3:The Hospital provides health care services to patients who meet certain criteria under its charity care policy without charge or at amounts less than established rates. Since the Hospital does not pursue collection of these amounts, they are not reported as patient service revenue. The Hospital does not estimate a percentage of its implicit price concessions that may be related to patients eligible under CPMC's Financial Assistance Policy due to the expectation that all eligible patients would apply for financial assistance.
Part III, Line 4:The footnote to the Organization's financial statements that describes implicit price concessions is located on pages 13 and 14 of the attached financial statements.
Part III, Line 8:The expenses represent an overall shortfall for the Medicare patient population. The costs themselves are a community benefit as the community would not receive these services if we did not provide them.Total revenue received from Medicare is the gross reimbursement plus settlement. Both total revenue received from Medicare and the Medicare allowable costs are reported from the Medicare Cost Report. The Medicare Cost Report is completed based on the rules and regulations set forth by Centers for Medicare and Medicaid Services.
Part III, Line 9b:There are no collection efforts for patients that qualifed for SharePay. SharePay is a financial assistance program offered by Cabinet Peaks Medical Center. Under this program, some patients may qualify for discounts on the care received. The discount is based on a sliding scale, taking into account income level, debts, assets, and family size. Even those with health insurance or who participate in a Federal assistance program may qualify for SharePay. However, there may be collection actions on their personal portion not covered by SharePay. Our sliding scale financial assistance program is designed to meet the medical financial obligations of the uninsured and underinsured.
Part VI, Line 2:CPMC utilizes a third party to collect patient input and perspective. This company calls a sample of our patients who received services in Acute Care, Emergency Department and Surgery to see if we are meeting their expectations. CPMC reviews the compiled data and comments monthly. Additionally, CPMC holds a yearly Patient and Family Advisory Council (PFAC) Focus Group meeting to listen to what our patients have to say about the care and services they receive. This group participates by openly answering questions about their experience, what worked and what did not as well as an opportunity for them to make suggestion on ways we can improve in the future.Thirdly, annually we invite a patient who received less than excellent care to attend one of CPMC's Board of Director's meetings to speak about their experiences and offer insight into ways we can improve. In addition to these formal methods for feedback, we are continuously rounding with patients to ensure their needs are met and we regularly attend community events and meetings to stay abreast on our community's needs and initiative. A complete community health needs assessment was conducted and completed during the 2021 fiscal year. Cabinet Peaks Medical Center participated in the Community Health Services Development (CHSD) process, a Community Health Needs Assessment (CHNA), conducted by the Montana Office of Rural Health (MORH). Community involvement in steering committee meetings and focus groups enhanced the community's engagement in the assessment process.
Part VI, Line 3:CPMC publicizes the availability of financial assistance through the following methods:A. Postage of signage in the Patient Admitting and Registration areas as well as the Emergency Department and other areas throughout the Hospital's facilities offering meaningful visibility;B. Posting the Financial Assistance Policy, the Financial Assistance Application, and the Plain-Language Summary on CPMC's website;C. Including the Plain-Language Summary in patient registration materials and inpatient handbooks; andD. Including the Plain-Language Summary in materials offered to each patient as part of the intake or discharge process.CPMC also includes in each billing statement sent to the patient a statement regarding the availability of financial assistance, including a website address where the Financial Assistance Policy, Financial Assistance Application, and the Plain Language Summary can be found as well as a phone number for information about the Financial Assistance Policy and application process. In addition, the hospital employs self-pay account representatives to assist patients in determining if they qualify for free or discounted care. The account representatives also provide hands-on assistance to patients applying for other financial assistance such as CHIP's, heating assistance, Medicaid, and WIC. Applications for charity care are also discussed at registration and discharge and with patients who have bills that are past due.
Part VI, Line 4:Cabinet Peaks Medical Center is located in Libby, a very remote town in northwestern Montana. Libby is 72 miles south of the Canadian border, 89 miles northwest of Kalispell, Montana, and 35 miles east of the Idaho border. Libby is the county seat for Lincoln County. Due to its remote nature, Libby relies upon transport services, including helicopter and fixed wing transports, to ensure patients requiring specialized care can travel the longer distances to access larger facilities, usually in Spokane, Kalispell, or Missoula. The closest hospital is Bonner General Hospital about 47 miles from Libby in Sandpoint, ID, which is also of a somewhat limited scope of practice. The next largest hospital with more specialized and high acuity services is Logan Health 90 miles from Libby.CPMC has been designated a Medically Underserved Population and a Health Professional Shortage Area due to our economic challenges, lack of resources and remote location. We are a relatively poor county, with 22.8% of Libby population being at or below the poverty level, compared to a state average of 15.2%.
Part VI, Line 5:Cabinet Peaks Medical Center operates a 25 bed critical access hospital providing medical services to the residents of Lincoln County. Cabinet Peaks also offers a complement of outpatient services, including a variety of rehab and therapy services, infusion and chemotherapy, family medicine, imaging, and lab. Patients often do not have the luxury of time or money or the physical strength necessary to make the 90 mile trip to Kalispell to receive these services, which is why Cabinet Peaks recognizes the value of providing these services close to home. As Lincoln County consistently has one of the top unemployment rates in the State and is an economically depressed area, many of Cabinet Peaks' patients are uninsured or underinsured, meaning Cabinet Peaks provides a significant amount of uncompensated care. Emergency and other medically necessary services are provided to all persons, regardless of their ability to pay.In addition to the services provided within the Hospital campus, Cabinet Peaks also does a considerable amount of outreach to promote the health of the community. Organizing and funding a community health fair, providing discounted services for mammography and laboratory tests, providing speakers for health lectures, promoting hospital tours for the schools, and providing internship opportunities for the schools are just a few ways Cabinet Peaks partners to promote the health of the community. Cabinet Peaks' mission is to provide quality compassionate care to its community. A majority of Cabinet Peaks' Board of Directors is comprised of residents and/or business owners of Lincoln County. With the exception of the Medical Staff representative on the Board, none of the Board members are Cabinet Peak's contractors or employees, meaning they truly do serve the needs of the community. In order to ensure the appropriate medical staff and services are available for patients, Cabinet Peak's medical staff privileges are open to all qualified, licensed physicians in the community and are also extended to physicians who visit Lincoln County in order to offer additional services to our residents. Additionally, in order to ensure the future success of Cabinet Peaks, any and all surplus funds generated by the Hospital are reinvested into operations, capital equipment and infrastructure, including facility upkeep.
