DescThe New York and Presbyterian Hospital provides quality medical care regardless of race, creed, sex, sexual orientation, national origin, handicap, age, or ability to pay. Although reimbursement for services rendered is critical to the operations and stability of the Hospital, the Hospital recognizes that not all individuals possess the ability to pay for essential medical services and, furthermore, the Hospital's mission is to serve the community with respect to health care. Therefore, in keeping with the Hospital's commitment to serve all members of the community, the Hospital provides the following: free and reduced price medical care (financial assistance/charity care) to the indigent; care to persons covered by governmental programs at below-cost; subsidized health services; and health care activities, medical education and programs to support the community. Community benefit activities include wellness programs, community education programs, health screenings, and a broad variety of community support services, health professionals education, and subsidized health services. The Hospital had 126,179 discharges and provided 927,177 outpatient visits (clinic - 638,746 emergency room - 288,431) plus 103,115 ambulatory surgery procedures.Desc

Part V, Section B community health needs assessmentQuestion 5: A primary data source informing this Community Service Plan is the 2016 NYPH Comprehensive Needs Assessment. Additional sources included the NYS Prevention Agenda, NYCs Take Care New York, various Center for Disease Control & Prevention (CDC) datasets, and DOHMHs Community Health Surveys and EpiQuery database. NYPH fostered community participation and input through meetings with community leaders, community-based organizations, community board members, and physicians from the region. Many longstanding collaborators contributed to the assessments, including: the NYP/Lower Manhattan Hospital Community Advisory Board, the NYP/Columbia Leadership Council, the NYP/Weill Cornell Community Advisory Board, the NYP/Westchester Division Community Advisory Board and Community Physicians of NYP/Columbia. The input gathered through the DOHMH-led community consultations for the Take Care New York initiative also informed this report Question 7a: www.nyp.org/pdf/communityserviceplan2016.pdf Question 7b: www.health.ny.gov/prevention/prevention_agenda/2013-2017/implementation/ch ip_csp/nyc.htm#hosp79 Question 7D: We at NewYork-Presbyterian are committed to serving our community and ensuring that all of our state-of-the-art programs are within everyone's reach. The Community Service Plans feature a vast array of programs, initiatives and events that highlight NewYork-Presbyterian's continued commitment to our community. Any member of the public can get a copy of all Community Service Plans by visiting the following website: http://www.nyp.org/about-us/community-health/community-service-plans or by contacting one of the following offices: NewYork-Presbyterian Office of Government and Community Affairs: (212) 305-2114 NewYork-Presbyterian Office of Public Affairs: (212) 821-0575 In addition, Copies of the needs assessment and plan were disseminated to: elected officials, the NYS Department of Health, and the hospital community advisory boards. It was also posted on the NYP website. Question 11: NewYork-Presbyterian is committed to providing quality health care and support to the communities that we serve. Through our CHNA we identified three health issues that are most evident and of greatest concern to the community, specifically: Chronic Diseases and their risk factors Issues related to Behavioral Health HIV Disparities Tobacco use is a major contributor to all of the chronic diseases identified in NYPHs service area and has been targeted in the 2016 Community Service Plan with a multimodal evidence-based set of tobacco cessation interventions. These interventions include increasing assess to smoking cessation resources, increasing capacity to address and treat tobacco use amongst community residents, and fostering an interdisciplinary approach to treating tobacco use. To support mental health needs in our community, an innovative approach to community-based depression recognition and treatment, known as Mental Health First Aid, is being deployed. This intervention will provide information on mental health issues and tools to cope, train community members to identify early signs, and work toward onsite assessment of mental health issues when no psychiatrist is present. The effort includes work with area churches and ministries, and NYC DOHMH. This effort is part of the City of New Yorks Thrive NYC initiative. NYPH will also work with National Alliance of Mental Illness (NAMI) Westchester on educational activities in Westchester County. NYPHs community-based effort targeting racial and ethnic disparities in the prevention and treatment of HIV is led through the REACH Collaborative (Ready to End AIDS & Cure Hepatitis C). This intervention will focus on increasing access to HIV care, Hepatitis C, and STD screening and treatment. In the last Community Health Needs Assessment (CHNA), we identified community health needs in the areas of chronic disease, mental health and HIV. In consultation with our community collaborators and stakeholders, we chose to focus on the aforementioned areas given our ability to successfully design comprehensive programs and outreach efforts that would address them. In contrast, the CHNA also uncovered environmental risk factors (e.g. built environment, housing, toxic exposures) that are outside of our ability to directly influence. However, we will continue to monitor these risk factors and work towards identifying strategies to potentially address them in the future.
Part V, Section B Financial Aid PolicyQuestion 15e: NewYork-Presbyterian Hospital has a financial advocacy program staffed by representatives who reach out to patients to provide information regarding Medicaid, Exchange Plans, Charity Care and to assist those patients who need help to Apply. 16a, 16b and 16c: www.nyp.org/pay-my-bill-charity-care Question 16j: The Hospital follows two basic approaches to publicizing the availability of Charity Care. First, it makes the Charity Care Policy itself, a plain language summary(Summary), and the Charity Care application available at various Hospital patient access points, posts signs conspicuously in public areas of the Hospital, includes information on billing statements, posts information(including how to obtain the Policy, Summary and application) on the website, and responds to inquiries from patients and members of the community on Charity Care. Secondly, the Hospital is in the process of providing updates and information(including the Policy, the Summary and the application)on a regular basis to leaders of community advisory boards, local community boards, elected officials, the City Health Department as well as our DSRIP Project Advisory Committee.

Part III, Line 2:For patients who were determined by the Hospital to have the ability to pay but did not, the uncollectable amounts are bad debt expense. Part III, Line 3: The amount included represents patients who qualify for charity care/financial assistance and also have a bad debt writeoff. bad debt expense associated with patients that received charity care/financial assistance is represented in this $2,162,720 figure. These patients went through our charity care/financial assistance process and were determined to have financial need. As a result we provided them with a discount based on our sliding scale charity care policy. If they were unable to pay the reduced balances they were written off as bad debt and included as a community benefit. Part III, Line 4: Audited Financial Statements page 19.
Part III, Line 8:The required method of reporting in schedule h obfuscates the full losses associated with delivery of services to medicare beneficiaries; a loss which exceeds $118 million. As reported in part III, section b, line 7, medicare is calculated to result in a $125 million shortfall; this results because medicare losses of $172 million are instead reflected in Part I, lines 7f and 7g where losses identified with professional education and subsidized health services are calculated per the methodology mandated for completion of schedule h. furthermore, medicare managed care losses of $164 million are excluded altogether from all schedule H disclosures. If all these revenue and costs were included the medicare shortfall of $118m would be a medicare shortfall of $461m. Part III, Line 9b:POLICY AND PURPOSE: The purpose of the Collection Policy (Policy) is to promote patient access to quality health care while minimizing bad debt at NewYork-Presbyterian Hospital (Hospital). This Policy places requirements upon Hospital and those agencies and attorneys undertaking debt collection activities that are consistent with the core mission, values, and principles of Hospital including, but not limited to, Hospitals Charity Care Policy. This policy applies to hospital and any agency, lawyer, or law firm assisting hospital in the collection of an outstanding patient account debt. PROCEDURE: A. General guidelines 1. Hospital, collection agencies (Agency), and lawyers and law firms (Outside Counsel) will comply with all applicable federal and state laws and accrediting agency requirements governing the collection of debts including, but not limited to, the Fair Debt Collection Practices Act (FDCPA), the Fair Credit Billing Act, the Consumer Credit Protection Acts, Public Health Law Section 2807-k-9-a, Internal Revenue Service Code 501(r), Article 52 of the New York Civil Practice Law and Rules, and the Health Insurance Portability and Accountability Act (HIPAA). Hospital, Agency and Outside Counsel will also comply with Hospitals Charity Care Policy. To the extent that there are any inconsistencies between Hospitals Collection Policy and Charity Care Policy, the Charity Care Policy shall supersede and control. 2. Hospital shall enter legally binding written agreements with any parties (including Agency or Outside Counsel) to which it refers an individuals debt related to care that are reasonably designed to prevent Extraordinary Collection Actions (ECAs) from being taken to obtain payment for the care, until reasonable efforts have been made to determine whether the individual is eligible for Charity Care.
Needs Assessment:The NewYork-Presbyterian Hospital 2016 Comprehensive Three Year Community Service Plan lays out a plan of action that responds to the community health needs of the Hospitals service area, supports the New York State Prevention Agenda 2013-2018 and New York Citys Take Care New York and supports nonprofit hospital reporting requirements to the U.S. Internal Revenue Service. To develop this plan, NewYork-Presbyterian Hospital (NYPH) conducted a community health needs assessment that included a review of quantitative and qualitative data, as well as direct input from community representatives, including the New York City Department of Mental Health & Hygiene (DOHMH). These sources indicated that chronic diseases, mental health, and HIV are important priorities in the NYPH service areas. Key informants, focus group participants and public sessions repeatedly identified cancer, cardiovascular disease, asthma, depression, diabetes, and its frequent companion, obesity, as major concerns. In addition, the data points to disparities in the incidence, prevalence and treatment of HIV in the NYPH service areas, including disproportionately high rates of new HIV diagnoses among Black and Hispanic men who have sex with men. Prevention Agenda Priorities: Based on these findings, NYPHs 2016 Plan lays out a plan for action to address the following NYS Prevention Agenda Priorities: Prevent Chronic Diseases; Promote Mental Health & Prevent Substance Abuse; and Prevent HIV, STDs, Vaccine Preventable Diseases and Healthcare-Associated Infections. The 2016 Plan builds on the accomplishments of NYPHs 2013 Community Service Plan, which had identified chronic disease and promotion of mental health as its two Prevention Agenda Priorities. All of the 2013 Plan objectives were met, setting the stage operationally for the adoption of new interventions as well as the naming of a third priority, aimed at addressing racial and ethnic disparities in the prevention and treatment of HIV. As requested by the NYS Commissioner of Health, NYPHs activities in this third priority area particularly focus on closing a significant health disparity. Also as requested by the New York State Department of Health Commissioner Howard Zucker, MD, MPH in his state-wide letter, NYPHs targeted activities in HIV and tobacco align with the Domain IV activities of the NYPH Delivery System Reform Incentive Payment (DSRIP) Performing Provider System (PPS). Interventions and Community Engagement The following interventions to support the Prevention Agenda will be undertaken with the support of community partners: Prevent Chronic Disease by Increasing Access to Tobacco Cessation Resources: Tobacco use is a major contributor to all of the chronic diseases identified in NYPHs service area and has been targeted in the 2016 Plan with a multimodal evidence-based set of tobacco cessation interventions. It was a top concern for community members who participated in the DOHMHs Lower Manhattan Take Care New York community consultations, which is the area surrounding the NYP/Lower Manhattan Hospital campus. Tobacco cessation is also one of the NYPH PPS DSRIP Domain 4 areas. Groups collaborating on the intervention will include the NYCDOHMH, area schools, Harlem United, the Charles B. Wang Community Health Center and the Chinese Community Partnership for Health as well as members of the NYPH DSRIP PPS. The intervention will be tracked by the number of patients and providers that participate, number of prescriptions for treatment, number of community collaborators engaged, and community participation. Mental Health Promotion through Education & Training: An innovative approach to community-based depression recognition and treatment, known as Mental Health First Aid, will be advanced to support mental health promotion. The effort will include work with area churches and ministries, and NYC DOHMH. This effort is part of the City of New Yorks ThriveNYC initiative. NYPH will also work with National Alliance of Mental Illness (NAMI) Westchester on educational activities in Westchester County. Reducing HIV Disparities by Assuring Linkages to Care: NYPHs community-based effort targeting racial and ethnic disparities in the prevention and treatment of HIV is led through the REACH Collaborative (Ready to End AIDS & Cure Hepatitis C). The intervention will be tracked with a number of established measures including adherence measures, viral loads, and emergency department and inpatient utilization. 2017 Year 1 Update Summary: Since the 2016 Comprehensive Needs Assessment, NYPH has undertaken several key initiatives to improve the health needs of the communities we serve. A summary of initiatives is included summary included below: Prevention of Chronic Disease Tobacco Use: NYPH is on track with efforts to reduce illness, disability and death related to tobacco use and second hand smoke exposure. Patient educational materials related to smoking cessation have been identified or created, and distributed to patients through NYPH clinical sites and community-based events. Efforts are underway to increase linkages with local healthcare providers who can improve access to cessation programs. Finally, online training modules have been made available for healthcare providers with supplemental face-to-face sessions to improve cessation education. There have been challenges limiting the implementation of electronic referrals to the NYS Quitline but this continues to be a focus area. Promoting Mental Health and Prevention of Substance Use NYPH is working to strengthen the mental, emotional and behavioral health (MEB) infrastructure. Our plan is on track with a Speakers Bureau of NYPH clinicians providing education on mental health topics in community settings and a successful Mental Health Clergy Summit to promote awareness of the challenges of mental illness in our communities. The Summit was attended by New York Citys First Lady Chirlane McCray, Congressman Adriano Espaillat and Assemblywoman Carmen De La Rosa as well as over 100 clergy from the NYC area. Additionally, the Westchester campus has sponsored an Addictions Recovery Fair, Mental Health Fair, and a Community Lecture Series to provide information on a variety of mental health topics. NYPH continues to work on expanding availability of clinical assessments of MEB through the introduction of tele-psychiatry across its campuses. Prevention of HIV, STDs, and Vaccine Preventable Diseases: NYPH is on schedule with several key interventions to increase the number of individuals who receive two sequential anti-retroviral medication scripts and attend two office visits within the previous 12 months. These interventions are focused on increasing access to HIV care, engagement in Hepatitis care, and STD screening and treatment. Other interventions underway include the promotion of pre-exposure prophylaxis, or PrEP, an HIV prevention strategy in which HIV-negative individuals take anti-HIV medications to reduce their risk of infection. NYPH is focusing the promotion of PrEP in ambulatory centers; through community collaborators for at-risk individuals, ED navigation, and expanded testing at multiple community-based settings. NYPH continues to deploy multimodal interventions to impact this important health concern for the community we serve. NYPH will continue to support and monitor progress made on the Community Service Plan and its aim of addressing the community needs identified in the 2016 Comprehensive Needs Assessment. The DOH updated the methodology for submitting the update recently. The progress update is now comprised of two parts: updating the Prevention Agenda Work Plan table, and a brief online survey to gather feedback on any changes to priorities or other challenges identified.
Patient Education of eligibility for assistance:Written materials, including the application, full Policy, and plain language summary (Summary), shall be available to patients in the Hospital's primary languages, upon request and without charge, from Admitting and Emergency Departments at the Hospital during the intake and registration process, at discharge and/or by mail. Additionally, those materials shall be available on the Hospital's website (www.nyp.org). Also, notification to patients regarding this Policy shall be made through conspicuous posting of language appropriate information in Emergency Rooms and Admitting Departments of the Hospital, and inclusion of information on bills and statements sent to patients explaining that financial aid may be available to qualified patients and how to obtain further information.
Community Information:NYPH SERVICE AREA NYPH includes six campuses, two of which are major academic medical centers, and serves patients from the New York metropolitan area, across the nation, and around the globe. As a regional resource, NYPHs service area differs from that of a typical community hospital where the service area is defined by the residential profile of those zip codes where the largest number of the hospitals discharges originate. Given its multiple campuses and wide range of services NYPHs service area is more broadly defined and includes the counties of New York, Queens, Kings, Bronx and Westchester. The total service area includes approximately 3,600,434 households with a total population of approximately 8,845,686. The percentage of households with incomes less than $10,000 is 9.6% in New York County, 15.7% in Bronx County, 11.4% in Kings County, 7.1% in Queens County, and 4.7% in Westchester County. The population demographics each of the six campuses are briefly described below. Washington Heights/Inwood: Total Population = 195,302; % of Residents < 45 = 63%. New York-Presbyterian's Strategic Initiatives were updated in 2013 to support the ultimate goal: "We Put Patients First Always." This means that New York-Presbyterian must make patients the first priority and strive to provide them with the highest quality, safest, and most compassionate care and service always. New York-Presbyterian's six Strategic Initiatives are: 1. Culture - Our culture is defined by our core beliefs, which guide everything we do, both in our interactions with patients, and with each other. Our culture of respect, teamwork, excellence, empathy, innovation and responsibility help us continue to deliver the best care possible while meeting the challenges ahead. 2. Access - Improve and Expand Access: We will continue to work to improve and expand access to the Hospital and the Physician 0rganizations. Patients should be able to receive care promptly and not have long waits to schedule appointments. We will also work with our Healthcare System members to broaden our geographic reach and expand care delivery to the communities we serve. 3. Engagement - Engage Staff and Patients: Engaged staff are actively involved in the work they do and the care they provide to patients and their families. Engaged staff will help us deliver the highest quality, most compassionate care and service, and ultimately the best patient experience. At the same time, engaged patients actively participate in their own health and recovery. We will provide patients with tools and educational materials to help manage their own care, as well as enhance cultural competence among our staff. 4. Health & Wellbeing - Enhance Health and Wellbeing: The Hospital is committed to fostering health and wellbeing as part of our patient care and community service mission, and, as an integral part of our culture. In 2013, we successfully launched NYPBeHealthy as a new, comprehensive wellness and prevention initiative designed specifically for our staff. The program offers employees enhanced access to new and existing Hospital programs, healthier choices in our cafeterias, and targeted information to help our staff meet their individual health goals. 5. Value - Deliver and Demonstrate Value: We must deliver the highest quality care as efficiently and effectively as possible, as this is important for both our financial health and for our patients who contribute to the costs of their care. Our Making Care Better Initiative will help us reduce unnecessary clinical variability, promote quality and safety, and achieve efficiency. We will also continue to seek opportunities to streamline processes and reduce unnecessary costs through HERCULES and Operational Excellence initiatives. 6. High Reliability - Provide Highly Reliable, Innovative Care: We want to provide the highest quality and safest care to every single patient with every single interaction. To achieve this goal, we will focus on developing highly-reliable processes, enhancing our culture of safety, and reducing variability in care. These Strategic Initiatives support the ultimate goal: "We Put Patients First Always"

Part VII & Schedule J, Supplemental Information:The officers and key employees identified in Part VII are responsible for executing the mission and management of The New York and Presbyterian Hospital (NYP) and its affiliated entities. Compensation for 2017 of these upper level executives includes the payout of an annual incentive plan and a long-term incentive plan. This performance-oriented program conditions payments upon the achievement of multiple individual and group performance measures. Measures to monitor performance include: operational and financial strength, patient quality and safety, patient satisfaction, advancement of patient care, and people development and partnership. Incentive awards may only be granted if the organization achieves a financial surplus. Even if all relevant performance measurements are achieved, the NYP Board of Trustees retains full discretion to make or not make any incentive awards, or to reduce the amount of any incentive award. This initiative is critical to assuring that NYP has the requisite leadership to create and manage a highly motivated and engaged workforce, to drive superior performance throughout the organization and to achieve top tier medical center status. As a separate matter, due to restrictions imposed by the Internal Revenue Code, upper level executives are limited in the amount of benefits received under a tax-qualified retirement plan. Like many employers, NYP supplements these executives' pension benefits through a supplemental ("nonqualified") retirement plan. The supplemental executive retirement plan (SERP) is subject to a multi-year vesting requirement (commencing after five years of participation in the SERP, in prorated amounts through age 65) which places an executive's supplemental retirement benefit at risk of forfeiture if the vesting requirements are not satisfied. Once vested, however, provisions of the Internal Revenue Code require that the vested executive include in current income the value of his or her vested supplemental retirement benefit. Notwithstanding the legal requirement to recognize the vested value of the supplemental retirement benefit as current income, the supplemental retirement benefit will not be distributed to the executive until the executive actually retires from NYP (although, as permitted by the Internal Revenue Code, the supplemental retirement plan will effect a distribution of an amount necessary to satisfy the executive's tax liability resulting from the income recognition upon vesting). As noted, this supplemental retirement benefit will not be distributed to the executive until the executive actually retires from NYP. There are constantly changing legal, tax, accounting, and public disclosure rules for a SERP (supplemental executive retirement plan) in not-for-profit organizations. The executive Compensation Committee continuously monitors these changes and incorporates any changes into the overall SERP plan design. As in past years, the executive Compensation Committee of NYP requires a third party to complete a review of the organization's compensation program to ensure its effectiveness in terms of government regulations, market conditions and the need to continually elevate organizational performance. The report also serves to meet the regulatory obligations to ensure that all elements of the executive compensation programs are reasonable. Each of the officers and key employees listed devotes an average of sixty hours per week to perform his or her responsibilities for the reporting entity and other related organizations in the aggregate.
Part I, Line 1a:The travel policy states that coach class is required for trips less than 4 hours in duration. Business class for trips of greater duration. The CEO, President, and Executive Vice Presidents are authorized first class if business class is not available. For others, first class requires prior authorization. The New York and Presbyterian Hospital supplies monthly housing allowance to certain executives due to the extent and nature of their responsibilities. The New York and Presbyterian Hospital supplies transportation to certain executives due to the extent and nature of their responsibilities across various physical locations. In so far as necessary, The New York and Presbyterian Hospital records any applicable items as taxable compensation to the individual(s) as required by the Internal Revenue Code. by the Internal Revenue Code.

Part VI, Section B, Line 11A:Finance coordinated and completed all of the information required for Form 990, accessing various resources including, legal, human resources, development, and other departments as needed. Senior Finance executives complete a review of the return in conjunction with, Ernst & Young U.S. LLP, paid preparer, prior to submission to the Audit and Corporate Compliance Committee of the Board. A copy of the 990 is sent to the Committee for review and approval at the audit and corporate compliance committee meeting preceding the filing. The Audit and Corporate Compliance Committee recommends to the Executive Committee and/or the Full Board of Trustees for their approval. A copy of the Form 990 was made available to the governing body at the Board of Trustee's meeting preceding the filing. The Hospital files the 990 upon final approval.
Part VI, Section B, Line 12C:The Hospital adheres to a conflict of interest (COI) policy that was approved by the Audit and Corporate Compliance Committee of the Board of Trustees. The policy states in part: "Each Board Member, Officer or Key Person of a New York-Presbyterian Organization shall complete a conflict of interest questionnaire prior to becoming a Board Member, Officer or Key Person of the New York-Presbyterian Organization and annually thereafter." The policy also states that "each Board Member, Officer, or Key Person shall promptly advise the Chief Executive Officer of the New York and Presbyterian Hospital, or his or her designee, of any changes to the information provided in that individual's last completed conflict of interest questionnaire." The Chief Executive Officer of New York-Presbyterian Hospital, or his or her designee, shall review all completed questionnaires and all subsequent advice of changes and shall take such action as is deemed appropriate to eliminate potentials for conflicts of interest, including such steps as reassignment of responsibilities or establishment of protective arrangements. All disclosures of interests in completed questionnaires or subsequent advice, unless clearly irrelevant or immaterial, shall be compiled and reported by management to the Audit and Corporate Compliance Committee of the Board of New York- Presbyterian Hospital, together, in each case, with response or recommendation of management. The Audit and Corporate Compliance Committee shall determine whether the reported resolution of issues raised by the disclosures is satisfactory and, if not, shall require such further action as it deems appropriate.
Part VI, Section B, Line 15 A & 15B:The Executive Compensation process at The NewYork and Presbyterian Hospital (NYP) is administered by a committee of independent trustees. They follow a Board-approved charter and overall executive compensation philosophy. The charter empowers the NYP Board Compensation Committee to administer the executive compensation program and process on behalf of the full Board of Trustees of NYP. Overall, the philosophy is intended to reward a broad spectrum of high organizational and predetermined, measurable individual performance expectations, as well as to foster the retention of key management talent. NYP's executive compensation philosophy is focused on establishing a performance - oriented philosophy and pay strategy designed to attract, retain and reward top talent. To fulfill their responsibility, the Committee also reviews information from multiple sources of market data. One such market definition is a stable group of large health care systems of similar scale and circumstances. Additional information from not-for-profit systems, for-profits systems and comparably sized publicly-traded health care facilities is also used. They use this additional information to support their decisions regarding on-going administration of the program. The Compensation Committee is comprised of independent members of the Board. They meet three to four times per year and make all critical decisions in executive session. These decisions are documented in minutes which are approved in subsequent meetings. The Committee is empowered to, and does, engage outside counsel and consulting support. The above described Executive Compensation Process is an ongoing process, applied annually on a calendar year basis, to all vice presidents, senior vice presidents, group senior vice presidents, executive vice presidents, as well as the chief executive officer and President.
Part VI, Section C, Line 19:External requests for our governing documents, conflict of interest policy, and financial statements are reviewed for validity. These requests are then granted if deemed appropriate.
