Schedule H, Part V, Section B, Line 3EThe significant health needs of the community are included and prioritized within the joint CHNA conducted by Children's Health. The priority needs identified are health, economic security, safety, and education.
Schedule H, Part V, Section B, Line 5 Facility A, 1Facility A, 1 - CHILDREN'S MEDICAL CENTER DALLAS AND CHILDREN'S MEDICAL CENTER PLANO. Every other year, Children's Health recruits a citizen advisory council made up of key stakeholders serving children and knowledgeable in public health. These community leaders meet regularly to discuss children's health care issues and to contribute their experience, insights, and expert recommendations for the Beyond ABC report prepared by Children's Health. The citizen advisory boards for Beyond ABC identify recommendations for advocates and public officials in order to improve the lives and health of children. Recent advisory boards have included city and county officials, former legislators, members of law enforcement, school district administrators, nonprofit organizations' executives, child-advocacy representatives, chambers of commerce and senior staff from corporations and financial institutions. Additionally, Children's Health participates in the regional healthcare partnership (RHP) task force, which was created as part of the Texas healthcare transformation and quality improvement program. Schedule H, Part V, Section B, Line 6a Facility A, 1Facility A, 1 - CHILDREN'S MEDICAL CENTER DALLAS AND CHILDREN'S MEDICAL CENTER PLANO. As a part of Children's Health, Children's Health System of Texas collaborated on the 2022 Children's Health joint community health needs assessment (CHNA). The collaboration was conducted with Children's Medical Center Plano (CMCP). The Children's Health CHNA collaboration was conducted in compliance with the final federal regulations to section 501(r), specifically §1.501(r)-3(b)(6)(v) joint CHNA reports.
Schedule H, Part V, Section B, Line 11 Facility A, 1Facility A, 1 - CHILDREN'S MEDICAL CENTER DALLAS AND CHILDREN'S MEDICAL CENTER PLANO. Upon conducting the CHNA and developing the community health implementation strategy, Children's Health decided on priority areas of the identified health needs that were reviewed. The priority areas are those areas which Children's Health has decided to focus its resources and integrate into its strategic and operational plans. Multiple goals were developed for each priority area, along with strategies to accomplish those goals. Children's Health has implemented and/or is working towards implementation of the strategies for accomplishing the developed goals. Full details of priority areas, goals, and strategies are contained with the publicly available CHNA and community health implementation strategy. Actions taken during the tax year to address significant health needs identified through its most recently conducted CHNA include the following: HEALTH: Nearly two-thirds of the children we serve at Children's Health depend on Medicaid or CHIP for their health care coverage. Children's Health participates in the Children's Health Coverage Coalition and North Texas Healthcare Collaborative, two coalitions that work on strategies to promote CHIP and children's Medicaid and reduce the number of uninsured children in the state. Dedicated outreach representatives at Children's Health help eligible families with children enroll in CHIP and Medicaid, ensuring North Texas families have access to health care. In 2024, Children's Health directly served 578 unique families and 1,436 unique individuals by assisting with application for state benefits, including Medicaid and CHIP enrollment. The outreach team reported more than 2,253 family encounters in the process. The American Academy of Pediatrics reports that by age sixteen, nearly 40% of children have been diagnosed with a mental, emotional or behavioral health condition, yet research indicates a 2-4-year window between when symptoms or concerns first appear and when a full disorder develops. Children's Health is increasing access to services and early identification of disorders by integrating into over 30 primary care medical homes across the community, serving children while they are in school and educating providers on behavioral health issues. Children's Health is using telemedicine to provide more children access to behavioral health care and address youth mental health issues such as anxiety, bullying and depression. The TCHATT program partners with UT Southwestern and is fully funded by the Texas Child Mental Health Care Consortium. TCHATT offers short-term therapy as well as psychiatric consultation. Both programs include case management, psychoeducation and referrals to community resources. Both programs are of no cost to families. Our behavioral health services are now available in over 1,000 school campuses increasing access to care for more than 680,000 students across 9 counties in North Texas. ECONOMIC SECURITY: Children's Health participates in the Collin County Early Childhood Coalition and the Early Matters Dallas Coalition. These broad-based coalition groups are dedicated to working together to raise awareness about the importance of quality early education, coordinate advocacy efforts and increase funding for quality early learning to ultimately ensure a strong future workforce. Children's Health serves as a member of the Dallas Coalition for Hunger Solutions, which is focused on providing education and advocacy opportunities for different programs that impact food security for families such as the Supplemental Nutrition Assistance Program (SNAP), and the public charge. The Children's Health Community Relations team also helps eligible families and children enroll in other government assistance programs, such as SNAP and TANF, and provides referrals to additional community resources and services. SAFETY: The Rees-Jones Center for Foster Care Excellence at Children's Health is the only clinic in North Texas dedicated to exclusively providing integrated primary medical care for children in foster care. Our providers are experienced in treating victims of abuse and neglect and provide a wide range of support to caregivers and families. Through health services, research, policy analysis and medical education, the Center's experts promote policies and practices that advance care and outcomes for children and families in the child welfare system. EDUCATION: Children's Health Behavioral Health team meets with educators, parents, and others across the community to provide training on behavioral health topics to continue to increase awareness and end the stigma of mental illness. Specific topics provided this year continue to focus on drug and alcohol abuse as well as vaping. In Feb. 2022, Children's Health launched, the Behavioral Health Integration and Guidance (BHIG), in conjunction with the Meadows Mental Health Policy Institute to train pediatric primary care providers to identify, diagnose and treat mild to moderate behavioral health conditions in the scope of their primary care practice. Training cohorts begin quarterly and lasted for six months, providing education, support, and practice optimization. While most participants are based in Texas, others are located in fourteen additional states. BHIG has led to overall improvements in increased screenings, diagnostics, and treatment for pediatric patients. Its impact on the health trajectories of children in North Texas has been significant, and it is poised to serve as a model for other communities across the country. The program has increased access to care for approximately 450,000 families since its inception and training is now also available in a fully self-paced on-line format. In 2023, Children's Health completed a project of embedding behavioral health screening tools into our electronic health record. Over 50 tools were added so that PCPs and medical specialists throughout Children's Health can easily screen their patients for behavioral health concerns. In the first 6 months of 2025, over 5,000 patients have been screened for behavioral health concerns to identify those in need of treatment. These tools are recommended to providers who participate in the BHIG training as they allow ongoing monitoring of symptom resolution once treatment interventions are initiated. Providers report increased use of behavioral health screening tools and interventions upon completion of the BHIG training program. BHIG also includes a best in practice integrated medical home in southern Dallas where the clinical team provides Collaborative Care Management to support the pediatrician, assuring most of the patient's medical and behavioral health treatment needs can be met in their medical home. The child psychiatrist also treats more complex patients referred by community-based providers who completed the BHIG training. Once stabilized, patients are returned to their PCP for ongoing treatment. Additional locations were added in Plano in 2024 and Rockwall in 2025.
Schedule H, Part V, Section B, Line 13 Facility A, 1Facility A, 1 - CHILDREN'S MEDICAL CENTER DALLAS AND CHILDREN'S MEDICAL CENTER PLANO. Children's Health uses the federal poverty guidelines to determine eligibility for low-income individuals. 200% of the federal poverty guidelines is used for 100% charity adjustment, and 201% to 400% of the federal poverty guidelines is used for a sliding scale adjustment. Children's Health uses various documents and forms to verify income and expenses when determining the eligibility for financial assistance of a patient and their family. Schedule H, Part VI, Line 7 STATE FILING OF COMMUNITY BENEFIT REPORTCHILDREN'S MEDICAL CENTER OF DALLAS AND CHILDREN'S MEDICAL CENTER PLANO ARE REQUIRED TO FILE A COMMUNITY BENEFIT PLAN AND REPORT WITH THE STATE OF TEXAS AS REQUIRED BY SENATE BILL 427.
Schedule H, Part V, Section B, Line 16a FAP AVAILABILE WEBSITEhttps://www.childrens.com/patient-families/billing-and-insurance/financial-assistance-and-support/full-financial-assistance-policy
Schedule H, Part I, Line 6a Community benefit report prepared by related organizationThe Children's Health community benefit report is included as part of the organization's "BEYOND ABC" report, which is prepared by the Marketing & Communications department in collaboration with the Government & Community Relations department.
Schedule H, Part I, Line 7g Subsidized Health ServicesNO PHYSICIAN CLINIC COSTS ARE INCLUDED IN THE SUBSIDIZED HEALTH SERVICES COMMUNITY BENEFIT EXPENSES.
Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceChildren's Health uses Worksheet 2 from the Form 990, Schedule H instructions to calculate the costs for Part I, Line 7 and ratio of patient care cost to charges attributable to community benefits.
Schedule H, Part II Community Building ActivitiesChildren's Health is passionate about maintaining a vocal presence in legislative affairs on the local, state, and federal levels, and is at the forefront of advocacy efforts that impact the community at large, from booster seat and car safety measures to critical funding for health care providers. Additionally, Children's Health collaborates extensively with stakeholders in coalitions like the Children's Hospital Association of Texas and The Texas Hospital Association in Austin, Texas, as well as the Children's Hospital Association in Washington, D.C. Through these affiliations, Children's Health is better able to advocate for the development of an effective, comprehensive and appropriately funded health care system in Texas and across the country. Our behavioral health leaders are also active in state and local committees within the Children's Hospital Association focused on legislation to improve funding and services.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountChildren's Health uses its cost accounting system and the actual costs that are written off by patient accounts to calculate the bad debt expense attributable to patients eligible under the organization's charity care policy. Children's rationale for including a portion of bad debt as community benefit is that bad debt expense should be treated as a community benefit as it is similar to other unreimbursed financial assistance, as reported in part i, line 7a, but for a different population (namely, those patients who would qualify for charity care if they completed the financial assistance application).
Schedule H, Part III, Line 3 Bad Debt Expense MethodologyCHILDREN'S HEALTH USES ITS COST ACCOUNTING SYSTEM AND THE ACTUAL COSTS THAT ARE WRITTEN OFF BY PATIENT ACCOUNTS TO CALCULATE THE BAD DEBT EXPENSE ATTRIBUTABLE TO PATIENTS ELIGIBLE UNDER THE ORGANIZATION'S CHARITY CARE POLICY. CHILDREN'S Health's RATIONALE FOR INCLUDING PORTION OF BAD DEBT AS COMMUNITY BENEFIT IS THAT BAD DEBT EXPENSE SHOULD BE TREATED AS A COMMUNITY BENEFIT AS IT IS SIMILAR TO OTHER UNREIMBURSED FINANCIAL ASSISTANCE, AS REPORTED IN PART I, LINE 7A, BUT FOR A DIFFERENT POPULATION (NAMELY, THOSE PATIENTS WHO WOULD QUALIFY FOR CHARITY CARE IF THEY COMPLETED THE FINANCIAL ASSISTANCE APPLICATION).
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteTHE ORGANIZATION'S FINANCIAL STATEMENTS DO NOT CONTAIN A BAD DEBT FOOTNOTE. CHILDREN's HEALTH HAS ADOPTED ASU 2014-09 WHICH TREATS THE PROVISION FOR DOUBTFUL ACCOUNTS AS AN IMPLICIT PRICE CONCESSION WITHIN NET PATIENT SERVICE REVENUE. THE PROVISION FOR DOUBTFULACCOUNTS IS NO LONGER SEPARATELY STATED AS A REDUCTION TO NET PATIENT SERVICE REVENUE.
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsWorksheet A from the as filed Medicare cost report and the IRS Schedule H instructions to determine Medicare allowable costs. SHORTFALLS ARISE FROM PAYMENTS THAT ARE LESS THAN THE COSTS TO PROVIDE THE SERVICES. THE SHORTFALLS SHOULD BE CONSIDERED COMMUNITY BENEFIT AS THEY MUST BE ABSORBED IN ORDER TO CONTINUE PROVIDING CARE TO OUR COMMUNITY. IT IS ALSO IMPLIED IN INTERNAL REVENUE SERVICE RULING 69-545 WHICH ESTABLISHED THE COMMUNITY BENEFIT STANDARD FOR TAX-EXEMPT HOSPITALS AND INDICATES THAT PARTICIPATION IN PUBLICLY-FINANCED PROGRAMS, SUCH AS MEDICARE, IS EVIDENCE THAT A HOSPITAL MEETS THE COMMUNITY BENEFITS STANDARD.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistanceAs a part of Children's Health System of Texas (Children's Health), Children's Medical Center Dallas and Children's Medical Center Plano adheres to and follows Children's Health collection policy and practices. If a patient qualifies or is attempting to qualify for assistance under the Children's Health Financial Assistance Policy and is attempting in good faith to settle an outstanding bill by negotiating a reasonable payment plan or by making regular partial payments of a reasonable amount, the unpaid bill will not be sent to any collection agency or other assignee. Children's Health does not engage in extraordinary collection actions against patients to obtain payment of care.

Schedule H, Part VI, Line 2 Needs assessmentChildren's Health has conducted a Community Health Needs Assessment (CHNA), with primary and secondary data included in the 2023-2024 Beyond ABC assessing the Well-Being of Children in North Texas counties of Dallas, Collin, Cooke, Denton, Fannin, Grayson and Tarrant (2023-2024 Beyond ABC), to ensure community benefit programs and resources are focused on significant health needs as perceived by the community at large, as well as alignment with the Children's Health mission, services, and strategic priorities. Since 1996, Children's Health has published Beyond ABC, a comprehensive report on the quality of life of children in North Texas. The report tracks not only health and safety factors, but also economic and education data. The Beyond ABC report is guided by a regional Advisory Board representing leaders in health care, education, philanthropy and social services across North Texas. In collaboration with the University of Texas at Dallas Institute for Urban Policy Research, the board ensures the report reflects both data and lived experience to inspire meaningful community action.
Schedule H, Part VI, Line 3 Patient education of eligibility for assistanceChildren's Health makes extensive efforts to inform and educate patients and patient's families about eligibility for financial assistance. The Children's Health Financial Assistance Policy and financial assistance application are made widely available online and in print, along with a plain language summary of the financial assistance policy. The plain language summary of the financial assistance policy is provided at patient discharge. All three documents are available without charge and available in the primary languages spoken by Limited English Proficiency (LEP) populations. In addition, Children's Health financial counselors or customer service representatives work with patients' guardians/guarantors to ensure that all public and voluntary assistance programs are fully explored. Children's Health has a financial program for patients who are considered indigent and do not qualify for a federal or state program. The eligibility criteria for financial assistance is based upon federal poverty guidelines published annually. Patients eligible for financial assistance will have charges reduced to the lowest level charged to individuals who have insurance covering such care. Gross charges will not be used to calculate patient's bills.
Schedule H, Part VI, Line 4 Community informationCommunity Served by Children's Health System of Texas Children's Medical Center Dallas is located at 1935 Medical District Drive in Dallas, Texas, and is an academic medical center campus anchored by a 488-bed full-service hospital, including the only, free-standing pediatric hospital in North Texas to be designated a Level I Trauma Center. The city of Dallas is the county seat of Dallas County. The city of Dallas is one of the most populous cities in Texas, as well as the United States - expected to grow by nearly 700,000 children by 2050. Children's Medical Center Plano is a 212-bed full-service hospital with two specialty centers, including the Children's Health Andrews Institute for Orthopedics & Sports Medicine. Additionally, in Dec. 2024, Children's Health opened a 395,000-square-foot patient tower at Children's Medical Center Plano, adding 140 inpatient beds, nearly doubling ER capacity and establishing a Level III trauma center. The Plano ER is now the largest pediatric ER in the county. Identification and Description of Geographical Community The city of Dallas is the third largest city in Texas. In 2024, Children's Health saw 174,077 unique patients from Dallas County. The system saw 337,253 unique patients and had 1,037,034 total patient encounters from the seven-county region that includes Dallas, Collin, Cooke, Denton, Fannin, Grayson and Tarrant counties. Community Population and Demographics In the past 10 years, from 2010 to 2020, the U.S. pediatric population fell by more than one million children. In that same timeframe, Texas' pediatric population increased by more than half a million. And more than 145,000 or about 29% of those additional Texas children are in the Dallas-Fort Worth metroplex. Currently, the pediatric population in the Dallas-Fort Worth region is almost 2.5 million, expected to grow by 700,000 children by 2050. Today, 25% of the population of Texas is age 17 or younger, meaning more than 1 out of every 4 people in our state is a child. Comparatively, 22% of the U.S. population is pediatric. Additionally, Collin County is one of the fastest-growing cities in the countries, with the population expected to double by 2060. More than 1.76 million children live in the six North Texas counties featured in the 2023 Beyond ABC report, including Dallas, Collin, Cooke, Denton, Fannin and Grayson counties. The child population in Dallas County decreased by almost 2,500 from 2017-2021. The percentage of non-Hispanic white children fell one percentage point, while the percentage of children of other or multiple races rose 10 points from 2017 to 2021. The percentage of Hispanic and/or Latino children remained constant at 53% of the child population. While about 1 in 5 Dallas County children live below the poverty line, stark differences are seen across racial and ethnic groups. Black and Hispanic and/ or Latino children had higher poverty rates, 27% and 24.8% respectively, while white non-Hispanic children had a rate below 10%. Service to The Community Children's Health has a longstanding commitment to the community - one that reaches beyond hospital walls and into schools, social-service organizations, corporate boardrooms, city halls and legislative chambers. A large part of the Children's Health mission to make life better for children is a commitment to serving as a voice for the voiceless - an advocate for the children who have no vote and no lobbying group to speak on their behalf. Children's Health devotes itself solely to caring for the complex medical needs of children. Children's Health provides patient care ranging from simple eye exams to specialized treatment in areas such as heart disease, hematology-oncology and cystic fibrosis. In addition, Children's Health is a major pediatric kidney, liver, intestine and heart transplant center. Through its established injury prevention, advocacy program and disease management programs, the hospital works diligently to educate the community so that no child has to ever come through our doors.

With oversight from a board of directors comprised of individuals who are representative of the community served, Children's Health promotes health and benefits the community through its two licensed hospitals - Children's Medical Center Dallas and Children's Medical Center Plano. Children's Health medical staff is open to physicians in the community who meet all requirements outlined in the children's health medical and dental staff bylaws. Children's Health considers it a privilege and responsibility to serve as advocates for kids in Texas and across the country. Through programmatic initiatives, organizational partnerships and community events, Children's Health spreads its influence throughout the region and provides area children with much-needed access to a better quality of life. Whether that means establishing programs to increase services to the area's underserved children or fostering partnerships with existing organizations, Children's Health is committed to connecting families to the services they need. COMMUNITY OUTREACH Many families have reported that without in-school telehealth, they would not know where to go for minor health care needs, resorting to unnecessary Emergency Room visits for minor ailments such as runny noses, sore throats and coughs. In 2024 alone, the program facilitated over 1,500 telemedicine visits. This has a significant impact not only on the care provided, but also in reducing the cost and time associated with hospital visits; and reducing the number of patients in an Emergency Room that do not require emergency services. Safeguarding access to CHIP and Medicaid/increase the number of insured Texas children: Nearly two-thirds of the children we serve at children's health depend on Medicaid or CHIP for their health care coverage. children's health participates in the children's health coverage coalition that works on strategies to promote CHIP and children's Medicaid and reduce the number of uninsured children in the state. Dedicated outreach representatives at children's health help eligible families with children enroll in CHIP and Medicaid. In 2024, Children's Health directly served 578 unique families and 1,436 unique individuals by assisting with applications for state benefits, including CHIP /Medicaid enrollment. The outreach team reported more than 2,253 family encounters in the process. ensuring all working parents, including low-income families, have access to affordable, safe and quality childcare: children's health participates in the Collin County early childhood coalition and the Early matters Dallas coalition. these broad-based coalition groups are dedicated to raising awareness about the importance of quality early education, coordinating advocacy efforts and increasing funding for quality early learning. Combating Child Poverty: Children's Health serves as a member of the Dallas Coalition for Hunger Solutions, which is focused on providing education and advocacy opportunities for programs that impact food security for families such as the Supplemental Nutrition Assistance Program (SNAP), and the public charge. The Children's Health Community Relations team also helps eligible families and children enroll in other government assistance programs, such as SNAP and TANF, and provides referrals to additional community resources and services. Expanding trauma-informed care education, training and intervention throughout the child welfare system: The Rees-Jones Center for Foster Care Excellence at Children's Health provides integrated primary care for children in foster care, many of whom have experienced abuse and neglect. As a regional leader in trauma-informed care, the center collaborates with school districts, child welfare organizations and other community partners to facilitate trauma-informed trainings and curriculums for educators, providers, volunteers and caregivers. COMMUNITY-BASED PROGRAMMING In recent years, Children's Health has strengthened and expanded our community programs and services to catalyze wellness from the ground up, ultimately creating a healthier community. By working with community leaders and organizations to meet families where they are, we connect health care providers across the community to better integrate care for children. Children's Health encourages organizations to provide wellness programs and primary-care options in non-traditional locations such as neighborhood churches and community centers. These programs include school-based health care, Asthma Management, Get Up and Go Weight Management, Injury Prevention, Children's Health Andrews Institute for Orthopaedics & Sports Medicine, and CHIP/Medicaid Outreach and Community Forums. Asthma Management Program: The nationally certified Asthma Management Program at Children's Health has proven to reduce asthma-related Emergency Room visits, as well as school and work absences. This three- to six-month program helps children aged 18 and younger better manage their condition for more symptom-free sleep, learning and play. Children's Health Andrews Institute for Orthopedics & Sports Medicine: As the only institute of its kind in the region, Children's Health Andrews Institute is here to get athletes back on the field. Developed under the direction of nationally renowned orthopedic surgeon James Andrews, M.D., this state-of-the-art facility offers a full spectrum of services from orthopedic surgery, a same-day fracture clinic and spinal care to rehabilitation, performance training, nutrition plans, advanced imaging and diagnostics. This community-based initiative provides a wide range of free programs and services at several YMCA and community locations in Dallas, Denton and Collin counties to empower families in making lasting healthy lifestyle changes. The program also offers virtual options. Get Up & Go is a free, 10-week program that takes a team approach to achieving a healthy weight, encouraging both children and their families to set goals and learn together. Injury Prevention: Injury is the number one cause of death for kids, but the good news is injuries are preventable. With evidence-based education tools, both in the hospital and community, the Injury Prevention Service at Children's Health helps keep children safe from traumatic injuries. From car seat safety to water safety and firearm safety, our program provides educational materials and interactive events in both English and Spanish. School-Based Telehealth: School nurses can now connect students with health care providers right from school using School-Based Telehealth by Children's Health Virtual Care. As one of the fastest-growing telemedicine programs in the country, our School-Based Telehealth program has serves over 250 schools throughout across and has conducted nearly 20,000 visits since its inception. It has trained over 1,000 school nurses and maintains a 94% parent satisfaction rate. One early adopter school district reported an improved absenteeism rate by 2%, resulting in $500,000 in additional state funding.
