DescPlease see the following excerpt from the EMHS Annual Report to the Community for details of community benefit projects at EMHS members:Coming together is a beginning; keeping together is progress; working together is success. The man who spoke these words understood the value of teamwork. He embraced technology in a way that nobody else had before him. His vision, passion, and innovation revolutionized the world. He made it possible for families to feel more connected despite living long distances apart. In many ways, EMHS shares the same goals championed 100 years ago by industrialist and automaker, Henry Ford.Like Ford, we know that we are succeeding by working together. Our new integrated, system wide electronic health record (EHR) will allow all our hospitals, primary care practices, and other members within the EMHS family to share information seamlessly. Using this system, we can collaborate more effectively and improve patient care.By mass-producing the automobile, Ford helped rural Americans feel connected. We are connecting people through technology, too. People who live in rural communities can talk one-on-one through a computer screen with doctors who may be several states away, thanks to our investments in telemedicine. Patients have access to specialists that would have been out of reach without this technology.Ford was an innovator. We too are embracing innovation in the way we deliver care. Our partnership with Dana-Farber Cancer Institute affords opportunities for cancer patients to access clinical trials closer to home at our Lafayette Family Cancer Center in Brewer. We are using paramedics to make routine house calls in our rural communities to perform wellness checks. This service is helping patients avoid trips to the hospital or doctors office while getting compassionate care in the comfort of their own homes.Of course, all of these initiatives require dedicated people who are committed to teamwork. Its the very reason why we take extraordinary measures to recruit and retain our employees. We have added a new nurse residency program, which is already showing signs of success in addressing a critical statewide nursing shortage and providing our nurses the tools they need to thrive. By working together, we can continue to do great things. To borrow another line from Henry Ford, If everyone is moving forward together, then success takes care of itself. In the pages that follow are numerous testaments to the innovative spirit underscoring many successes this past year throughout our system. While the hard work will continue, we are pleased to share with you the progress we are making towards achieving our vision to be a nationally recognized model of excellence in healthcare.M. Michelle Hood, FACHEEMHS President and CEOBarry McCrumEMHS Board ChairCONNECTING WORLD CLASS CARETerry White longs for the day that she can return to the sandy shores of Myrtle Beach to feel the warm Carolina sunshine on her face and the ocean waves splashing upon her feet. She has plans to go back to visit family and wont let cancer slow her down. My journey is not over just because I have this disease, Terry said. It doesnt mean life doesnt go on. I definitely have a lot of life to live.Terry was diagnosed with breast cancer a decade ago. I was 38 when I found a lump in my breast in the shower one morning and immediately called my primary care physician. Unfortunately, the cancer became metastatic, meaning it spread to other parts of her body. I went from being a survivor to a lifer because I live with cancer every day, and Ill live with it every day until the end, she said. Terry has gone through several treatments of radiation and chemotherapy over the years.During a recent visit to EMMC Cancer Care at the Lafayette Family Cancer Center in Brewer, she was comfortably dressed in jeans and a maroon blouse, and wore a hat on her head. Because the cancer spread to the bone marrow in her leg, she wore a knee brace to alleviate pain. Whats remarkable about Terry is how she projects a positive attitude. A powerful storm that day was toppling trees and knocking down power lines outsideit wasnt enough to stop her from making the 30-minute ride to the treatment center from her home in Pittsfield or to diminish her optimism. Research today has given us treatments that are light years ahead of what we used to have, she said.Terry is hopeful because she is taking part in a clinical trial thanks to a collaborative partnership between Dana-Farber Cancer Institute in Boston and EMMC Cancer Care. She is taking an experimental drug under the coordinated care of her medical team in Brewer and the Dana-Farber specialists in Boston. The treatment I come for is administered by a syringe. I dont have to sit there for four hours waiting for this to be over and waiting for the side effects, she said.Eric Winer, MD, is chief clinical strategy officer at Dana-Farber and director of the Breast Oncology Center. He and other members of the Dana-Farber team visited EMMC Cancer Care last fall for a tour. Dana-Farber only partners with cancer centers that already deliver excellent care, Dr. Winer said. The goal of the collaborative for Dana-Farber is to extend its reach and bring clinical trials to people living in rural areas. When people participate in clinical trials, the hope is that they will do better than they might with standard care, but no matter what, they are contributing to the future of cancer care, he said. The staff at EMMC Cancer Care also benefits by being in the collaborative. They have more educational opportunities and access to medical specialists at Dana-Farber for rapid consultation on complex cases. Dana-Farber clinical care is provided in Boston and this collaborative brings their expertise to the heart of Maine, explained Thomas Openshaw, MD, medical director, Oncology Research at EMMC Cancer Care. The staff at EMMC and Dana-Farber have a firm resolve to help not only current patients, but also future patients to live better lives. Our whole goal is to try to cure whatever patients can be cured, and thats a large proportion of patients, and for the others to give them the best and the longest life that they can have, Dr. Winer said. That seems to be working for Terry White, whos already making travel plans to Myrtle Beach. It gives you a reason to go through what youre going through. Weve got to learn from it for the future, she said, adding, I want to be healthy for my family, I want to participate in life with my family and my friends, and continue to enjoy life.CONNECTING WITH TECHNOLOGYCharles A. Dean Memorial Hospital (CA Dean) in Greenville sits along the scenic shores of Moosehead Lake, one of Maines most treasured natural resources. Its a peaceful, idyllic spot that draws tourists from far and away who are eager to get away from the hustle and bustle of their daily grind. But beneath the hum of motorboats cruising along Moosehead Lake in the summer, or the snowmobiles skirting across its icy surface in winter; beneath the laughter of kids splashing in the water on a hot August day or pulling a trophy trout through the ice on a frigid February morning, there is a rural health problem that those visitors do not see. To be fair, it affects communities throughout Maine but has been more problematic in those towns and hamlets tucked far away from urban centers. There is very little rural mental health period, explains Joseph Babbitt, MD, an emergency room physician and hospitalist at CA Dean. Historically people in rural communities have had to travel hours to access psychiatric services. If they showed up at any rural hospital emergency room with a mental health crisis, they had to wait hours for an evaluation. Youve got an emotionally unstable patient who is in a very disruptive emergency waiting room environment, waiting 18 hours to have a mental health professional come assess them. That doesnt help anyone, Dr. Babbitt said. But technology is bridging the gap and allowing those who live in rural communities to have the same access to psychiatric services as their urban counterparts. Telemedicine is a medium that connects a patient with a practitioner via a screen, explained Gavin Ducker, MD, chief medical officer at Inland Hospital in Waterville.Inland Hospital and CA Dean use telemedicine to provide psychiatric services to patients through Acadia Hospital, which offers telepsychiatry services to 15 rural emergency departments and 12 rural primary care practices across the state. CA Dean uses telemedicine for behavioral health in its emergency department. Inland does, too, but also offers the service through its primary care practices. Our goal has been to deliver the care on site, in a place theyre familiar with, comfortable with, and would feel more relaxed, and hopefully in a more timely manner. Telemedicine has allowed us to do just that, Dr. Ducker said.In addition to providing patients with access to services closer to home, the teDesc

DescMichael Workman, PMH-NP, is one of those providers who works for Acadia but lives in Indiana. He is a psychiatric mental health nurse practitioner who sees patients in Maine via telemedicine. He works from a laptop in his home office. Its actually no different than you sitting across the table from me in my office. Were just using the technology available to help people living in areas where we dont have as many psychiatric providers, Workman explained. He says initially he thought the technology would make it harder to do his job and make a personal connection with a patientto pat them on the shoulder reassuringly as theyre leaving his office or to shake their hand as they arrive. He says sometimes hell shake their hand through the screen, which he demonstrated. It lends some humor to the visit and makes a connection with the patient, he said, adding that even some reluctant patients came around quickly. I had one gentleman who came into the office and said Im not doing this and by the end of the session, I had a hard time getting him out of the room because he was so comfortable talking to me.Mike is eminently qualified and is great with patients, Dr. Ducker noted, adding that he is pleased with the telemedicine services and the benefit it has offered to patients. And even though Mike is several hundred miles away, hes a full-time employee at Acadia. He attends staff meetings, has supervision, and help is just a phone call or computer screen away. They really are a part of our team at Acadia and Inland, whether they live in Indiana, Arizona, or anywhere. The result of this technology is that people living in rural Maine, who choose to live far from the noise or chaos of a larger community, can still have access to quality care and enjoy their beautiful corner of the world that tourists only see on short visits.CONNECTING YOUR CARE TEAMColbey Bowen noticed that Norma Craft was clenching her hands and seemed a little uneasy at one point when he was explaining her medical care. So, he decided to stop talking and give his patient some time to process what he was saying. Colbey is a medical student on a home health visit as part of a program of Beacon Health called Interprofessional Practice and Education. That program teams up medical students, pharmacy students, and social work students to go on home visits to provide care for patients who have recently returned from the hospital or may be at high risk to end up in a hospital. I think one of the greatest things it does is it exposes us to different viewpoints from other professions because in medicine we all speak a slightly different language, Colbey said. And, it also brings the patient into a more confident, more comfortable setting which really helps us to bring them in as a part of a team experience.Pharmacy student, Manpreet Badwal, went with Colbey on this visit with Norma. She let Norma show her all her prescriptions and carefully categorized and explained what each one was. This is helping me to be a better pharmacist because it helps me practice the skills we learn in the classroom and use them in the real world, said Manpreet.Felix Hernandez, MD, is a retired cardiothoracic surgeon who runs the interprofessional practice program. He helped debrief Colbey and Manpreet after their home visit. Hes a staunch supporter of this approach to patient care. When you combine many of these skill sets and many of these knowledge sets in the same room with the patient, you get what I call a synergy where the sum of the parts is bigger than just adding them up, he said.Dr. Hernandez gives Colbey high marks for reading his patients nonverbal communication, and he says Manpreet did a great job preparing, asking good questions, and seeking feedback. When you are with a team thats firing on all cylinders, learn that, know that, see what that feels like. So, next time when youre with another team not functioning on all cylinders, you recognize it and you can troubleshoot it, said Dr. Hernandez.Megan Foss, LMSW-CC, is a member of the Beacon Health Community Care Team that goes out on home visits and provides the same services that these students are learning through the interprofessional practice and education program.Ultimately, this means better doctors, better pharmacists, better nurses, better social workers, better healthcare, she said.Laurie Chadbourne, RN, a Community Care Team nurse, echoes those sentiments. You can meet the patients where they are living. Sometimes, they cant get out of their homes; so, I think its a very good benefit. We can make sure they have the resources they need, so they dont get readmitted after they leave the hospital, she said. This way of delivering care to patients is a different approach than even some medical professionals were trained to provide. James Jarvis, MD, FAAFP, interim senior vice president and chief medical officer at Eastern Maine Medical Center, says the old way of doing things no longer makes sense. Doctors were always trained by doctors, nurses by nurses, pharmacists by pharmacists, and social workers by social workers, but in practice, we work better as a team, he said, adding that the changes that are happening are for the better.Weve already seen that this generation of resident physicians have no trouble calling up a pharmacist or a nurse on the floor to say, I dont really know what Im supposed to be doing here. Can you help me? That never happened before. In my generation, a doctor could only talk to a doctor, and that is just wrong. And so this is much, much better care, Dr. Jarvis continued. And that is the ultimate goal of the programto not only prepare the next generation of healthcare workers to work collaboratively, but to provide quality care to people where they are most comfortablein their homes.CONNECTING BEYOND MEDICINERhonda Jordan remembers exactly what went through her mind when she learned her stage four breast cancer had spread to her lungs, spine, liver, and hip. I was scared to death that I was not going to see my son graduate because I was sick. I was very sick, and I was ready to give up, she said.Nancy ONeill, MD, TAMC physician at Mars Hill Health Center, is all too familiar with patients dealing with those difficult thoughts, feelings, and emotions. When I look into my patients eyes, I can see fear; I see anxiety; I see pain, confusion, all of those emotions, she said. That is on the first visit. As a palliative care specialist for TAMC, Dr. ONeill works to help her patients process their diagnosis and manage their quality of life while they are getting treatment for serious, potentially life-threatening illnesses. Im not focusing on whether their diabetes is well managed; Im focusing on whether theyre doing great in the midst of their treatment, their turmoil, their decision-making. That is fulfilling, although also challenging, explained Dr. ONeill. She spends a lot of time teaching and educating her patients about the care that is available. Many are unfamiliar with palliative care and what it entails. Dr. ONeill says that people often confuse palliative care with hospice care. Both palliative care and hospice care provide comfort. But palliative care occurs at the same time as treatment. Hospice care begins after treatment of the disease is stopped and when it is clear that the person is not going to survive the illness. Its really family medicine at its purest point to work with patients and families, she said.TAMC is just one member organization offering palliative care services. In fact, EMHS recently launched a community-based palliative care service line systemwide. VNA Home Health Hospice (VNA HHH) is the operational home of the EMHS palliative care service line. Jim VanKirk, MD, is the medical director for this service line, and Robin Hirsh-Wright, LCSW, is the director.I think sometimes theres a negative connotation about being vulnerable and making connections, but at the end of the day, its human connection that makes things better when medicine cant, Robin said.As part of this service line, VNA HHH assigns nurse practitioners and social workers to seriously ill, homebound patients to focus on defining what is most important to patients who live with chronic illness. Those nurse practitioners and social workers collaborate with designated primary and specialty care providers to help guide patients through their care. Were not pushing patients, were not pulling patients; we are trying to walk alongside patients in their journey of illness, Robin said. They are certainly walking alongside patients like Rhonda Jordan. Since starting her care regimen, Rhonda has a new outlook on life, and she gives Dr. ONeill a lot of the credit for that. She is standing beside me by reinforcing that I am the driver of the bus, Rhonda said. She has found the will to fight her disease; she has found the strength to live. My strength comes from having a nine-year-old stepson that I know I have to be around for to teach hiDesc

Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Part IV, Section A, Line 1: Description Of How Supported Organizations Are DesigatedThe supported organizations are organizations for which we raise and manage funds.
Part IV, Section C, Line 1: Control Or Management Of Supported Orgs. The EMHS Foundation and the supported organizations each have Eastern Maine Healthcare Systems as their corporate parent. Restated Articles of Incorporation and Bylaws of Eastern Maine Healthcare Systems, the supported organizations, and EMHS Foundation have tightly integrated the supported organization and EMHS board governance structure into a unified and cohesive governance system in which the EMHS board has ultimate authority over EMHS Foundation and the supported organizations with respect to nearly all governance domains. Thus, Eastern Maine Healthcare Systems board authority goes far beyond traditional powers of appointment and reserved powers of approval typical of many healthcare system governance models and actually vests authority in the Eastern Maine Healthcare Systems board to initiate and direct action on the part of EMHS Foundation and any one or more supported organizations, in essence acting itself as the supported organization board, thus establishing the presence of common supervision or control among the governing bodies of all organizations involved. Type II supporting organization status for Eastern Maine Healthcare Systems was confirmed by the IRS on March 8, 2016, in response to a request filed on form 8940 on September 28, 2015.

Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Part V, Line 4: Intended uses of the endowment fund.Endowment funds are designated for purposes that align within this organization's exempt purpose.
Part X : FIN48 FootnoteIncome TaxesEMHS, its hospitals, and certain other affiliates have been determined by the Internal Revenue Service to be tax-exempt charitable organizations as described in Section 501(c)(3) or 501(c)(2) of the Internal Revenue Code (the Code) and, accordingly, are exempt from federal income taxes on related income pursuant to Section 501(a) of the Code. Accordingly, no provision for federal income taxes has been recorded in the accompanying consolidated financial statements for these organizations.Tax-exempt charitable organizations could be required to record an obligation for income taxes as the result of a tax position they have historically taken on various tax exposure items including unrelated business income or tax status. Under guidance issued by the Financial Accounting Standards Board (FASB), assets and liabilities are established for uncertain tax positions taken or positions expected to be taken in income tax returns when such positions are judged to not meet the more-likely-than-not threshold, based upon the technical merits of the position. Estimated interest and penalties, if applicable, related to uncertain tax positions are included as a component of income tax expense. The System has evaluated its tax position taken or expected to be taken on income tax returns and concluded the impact to be not material.Certain of the System's affiliates are taxable entities. Deferred taxes related to these entities are based on the difference between the financial statement and tax basis of assets and liabilities using enacted tax rates in effect in the years the differences are expected to reverse. The deferred tax assets and liabilities for these entities are not material.

Form 990, Part III, Line 4d: Other Program Services DescriptionOTHER PROGRAM SERVICES 4: It was a humbling privilege to provide palliative care to such a selfless woman such as Rhonda Jordan and to her family. She was driven by a tangible bravery in her quest for a cure. May her life continue in the memories made by family, spouse, son, coworkers, and friends. She has touched so many as I witnessed in her final days. - Nancy ONeill, MD.CONNECTING ALL OF MAINEFOOD INSECURITYOn a trip to a public housing complex in Presque Isle last summer, Sherry Locke, executive director of the United Way of Aroostook, and Jamie Guerrette, a community health specialist for The Aroostook Medical Center (TAMC), spotted a seven-year old boy eating a cattail plant he pulled out of the marsh behind the housing authority building. When they questioned him, the boy replied very matter-of-factly that cattails were safe to eat. They soon realized he was eating that plant because he was hungry. When we left, we knew we had to do something. This is a problem, and its going to be a problem every summer when theres a split harvest, Sherry said.During a split harvest, Presque Isle High School students return to school for three weeks in August so they can take time off in late September to early October to help harvest potatoes. When theyre in school in August, the MSAD 1 school district can no longer offer the federally funded summer school lunch program to its elementary and junior high school students. To meet the need, TAMC and the United Way partnered with the city of Presque Isle and the Presque Isle Housing Authority to create the Summer Gap meals program. That program provides healthy lunches to 250 MSAD 1 students. Im thankful for TAMC and this program because it helps make sure that every kid gets the food they need, explained Heather Fickel, one of the mothers whose child takes advantage of the program.Rural Community services like the Summer Gap meals program are helping people live healthier lives in communities throughout our system. At Sebasticook Valley Health (SVH) in Pittsfield, they are also helping people battle food insecurity thanks to a Partnership to Improve Community Health (PICH) grant that helped SVH and other member organizations set up screen and intervene initiatives. Under screen and intervene, the staff at primary care practices ask patients two questions at their regular checkups that evaluate if a person has run out of food or has struggled to purchase food in the last year. If the answer to either question is yes, then patients receive a resource guide about available programs. Thanks to a partnership with Good Shepherd Food Bank, the practices can also provide bags filled with a three-day food supply to hold people over until they can get to a local food pantry. All of us within EMHS are very aware of the struggle with food insecurity across the state of Maine and in the communities we serve. We are equally committed to finding a solution to that problem. - Terri Vieira President of Sebasticook Valley Health and Charles A. DeanCOMMUNITY PARAMEDICINEWhen paramedic Kevin Springer pulls up in front of the home of Virginia Simko in Greenville, his lights arent flashing; theres no emergency. Hes at her home for a routine visit as part of Charles A. Dean Memorial Hospitals (CA Dean) Community Paramedicine program. With Virginia and the other patients I see, I have to make a phone call so they can prepare with coffee and donuts before I get there, Kevin said with a slight laugh.CA Deans service was one of a dozen approved by the Maine Legislature in 2012 as part of a pilot program to improve healthcare access in rural communities. Community paramedics are paramedics who have an expanded role to provide care at home to patients who may not qualify for traditional home healthcare visits. Some of the services they provide free of charge include: routine wellness checks, medication management assistance, wound care, blood and specimen collection, posthospital discharge follow-up, home fall and safety assessments, and chronic disease management. The idea of the program is to keep patients out of the hospital. We see patients in the comfort of their own homes and we work closely with their physicians, Kevin said.In addition to providing quality care close to home, the service is also designed to reduce unnecessary emergency room visits. Patients can get a referral for the service from their primary care physician, hospital discharge planners, care transition teams, emergency room staff, or nurse case managers. Kevin says he enjoys visiting people for nonemergency care and definitely sees a value in helping patients stay healthy. He says the fringe benefits arent bad either. Getting out to see patients like Virginia puts a smile on my face every day, and sometimes I even get coffee and donuts.CONNECTING THE FUTURELike most people who decide to go into nursing, Brianne Ecker has a passion for helping people, especially children. She graduated from the nursing program at the University of Maine (UMaine) in December of 2012 and went on to become a pediatric nurse at Eastern Maine Medical Center. It was extremely gratifying to be a new grad and be on the pediatric floor because it doesnt happen often, Brianne said.In Briannes case, just graduating from the nursing program was a feat the majority of her classmates did not achieve. She enrolled with roughly 120 other students, and says only about 40 graduated with her. National studies suggest that those who do graduate wont last very long in their careers. According to a study funded by the Robert Wood Johnson Foundation, nearly one in five nurses will leave the profession after the first year because the transition from school to work will be too difficult. One in three will leave after the first three years. People get into it, but they dont stay, Brianne said, but she is bucking that trend and credits part of that to her supportive family. Her sister, Rebecca (Becca) Cowan, was there for her during nursing school. In fact, her sister was there alongside her in the classroom. I was actually in all of my nursing programs with my sister Brianne, which made it easier. When we began, we actually lived together for part of it, and that made it a lot easier, explained Becca. After graduation, Brianne and Becca both took their nursing exams together. Brianne started working on Eastern Maine Medical Centers pediatric floor in January 2013. Becca went to work with an older population at an assisted living facility. My love is with the elderly. I really started loving their stories and everything that they teach you, she said. These two sisters arent only setting an example for other nursing students, but for yet another member of the familytheir younger sister Kara Cowan, whos also a graduate of UMaines nursing program. You never really know whats coming through the door and thats what really enticed me, said Kara. She and her two older sisters agree theres a lot of pressure on nursing students and new nurses. Its a profession thats held to really high standards. So, I think the pressure of going through nursing school knowing that you need to be the best is and can be really difficult for nurses and students coming out of school, Kara explained.Thats why Eastern Maine Medical Center launched a nurse residency program in 2016. The program consists of a four-hour workshop once a month, an evidence-based practice project, and additional simulation sessions throughout the year at the Husson University Simulation Lab. The monthly workshops include topics focused on leadership, patient outcomes, and professional role. Workshops also include small group discussions. This allows residents an opportunity to discuss the clinical application of the topic as it relates to their own experiences to gain support, insight, and confidence.Deb Sanford, MBA, MSN, RN, EMMC vice president of Nursing and Patient Care Services, says Eastern Maine Medical Center is hoping to do what Duke University did with its nurse residency program, which was to reduce its dropout rate from 28 percent to five percent. This is not going to be the only thing that solves our nursing shortage in the state; what it is going to do is keep nurses, who are really good and really caring for patients, in the profession longer and help them really develop a maturity so that they feel confident in their practice, Deb said.Catharine MacLaren, PhD, vice president of Talent and Diversity at EMHS, says the system has been preparing for this nursing shortage for the past decade. She says the biggest problem is the changing demographics in our state. Maines nursing shortage is not likely to go away, but we are making strides to keep up with demand, Catharine said. EMHS has built a strong network with the nursing schools and the nursing associations across Maine. We are recruiting across the US and even into Canada. We are also relying more on social media. Eastern Maine Medical Centers residency program is another key r
