DescRESEARCH: THE RESEARCH DIVISION OF HOSPITAL FOR SPECIAL SURGERY CONTINUED ITS ACTIVITIES IN 2024 TOWARD ENHANCING AND EXPANDING OUR RESEARCH IN MUSCULOSKELETAL DISEASES. RESEARCH AT HOSPITAL FOR SPECIAL SURGERY WORKS TO ADVANCE THE UNDERSTANDING AND TREATMENT OF MUSCULOSKELETAL DISORDERS THROUGH BASIC, TRANSLATIONAL AND CLINICAL RESEARCH AND EDUCATION. THE GOALS ARE TO IDENTIFY GENETIC, ENVIRONMENTAL, CELLULAR, STRUCTURAL AND MOLECULAR MECHANISMS UNDERLYING DISORDERS IMPORTANT IN ORTHOPEDICS, RHEUMATOLOGY AND RELATED DISCIPLINES AND TO DISCOVER EFFECTIVE APPROACHES FOR DIAGNOSIS PREVENTION, AND TREATMENT OF THESE DISORDERS. HSS Research Institute (HSSRI) is home to a uniquely collaborative culture in which basic scientists work together with clinician-investigators to accelerate discoveries that will result in new treatments and improved outcomes for musculoskeletal patients. HSS Research Institute is composed of basic, clinical, and translational scientists to accelerate discoveries that will result in new treatments and improved outcomes for musculoskeletal patients. The key programmatic areas that have allowed us to make major contributions to musculoskeletal health include (1) Bone Health and Implant Integration (2) Inflammation & Autoimmunity (3) Joint Preservation and Regeneration (4) Musculoskeletal Pain Research. These programs are supported by an enabling infrastructure including a Genomics Center, Precision Medicine Laboratory, Center for Regenerative Medicine and Center for Biostatistics. HSSRI has active collaborations with the Applied Researchers within the Imaging and Radiology and Biomechanics Departments, many clinical departments including Orthopedic Services and Rheumatology/Medicine and is expanding collaborations with Physiatry, Anesthesia, Pathology and Neurology. Combining HSSRI expertise with the volume, breadth, and depth of patients seen at HSS to develop unparalleled clinical research and populations health outcomes programs, can further the national and international visibility of our institution. AS A RESULT OF STRONG INTRA - AND INTER - PROGRAMMATIC INTERACTIONS, AND THE USE OF SHARED CORE RESOURCES, OUR PORTFOLIO OF INNOVATIVE DISCOVERY AND DEVELOPMENT HAS EXPANDED EXPONENTIALLY. TO PROVIDE THE FINAL COMPONENT NEEDED TO ACHIEVE BENCH-TO BEDSIDE RESEARCH, THE BASIC RESEARCH INSTITUTE, IN COLLABORATION WITH CLINICIAN RESEARCHERS HAS CREATED A DISTINGUISHED CLINICAL RESEARCH PROGRAM. PART V, LINE 5A 32-question survey was developed through the collective efforts of a seven-member HSS CHNA steering committee, internal stakeholders, community partners, and the public. Collaboration with these groups was crucial to the success of this survey, with valuable feedback provided on survey construction and length. To reach a culturally diverse community, the survey was translated into Spanish, Chinese and Russian, using a culturally sensitive back translation approach by certified translators. HSS UTILIZED ALCHEMER, A WEB-BASED SURVEY PLATFORM FOR THE DEVELOPMENT AND DISTRIBUTION OF THE ELECTIONIC FORMAT OF THE SURVEY. THE SURVEY INCLUDED VALIDATED MEASURES FROM SERVERAL NATIONAL AND STATE HEALTH QUESTIONNAIRES. Below outlines the list of community partners involved in survey construction: . Good Neighbors of Park Slope . Presbyterian Hospital/Weill Cornell Medical Center . Y - YW/YMHA . Over 60 Senior Center . Creaky Joints Espanol (GHLF) . Chelsea Piers . Lupus Research Alliance . Ridgewood YMCA . Carter Burden Network . Mount Sinai Hospital . New York Presbyterian-Columbia University Hospital . Spondylitis Association of America . NYC DOHMH, Bureau of Bronx Neighborhood Health . Norwalk Senior Center . DOROT Inc . Self-Help Innovative Senior Center . Friedman Health Center . Clinical and Translational Science Center . Greenwich Alliance for Education . Americares Free Clinics . Lupus Foundation of America . New York City Department of Health . Touro College Graduate School of Social Work . Westport Library . COHME . Stamford Senior Center . Clinical and Translational Science Center . Ivy Rehabilitation Network . Gray Panthers . 305 West End Assisted Living . Arthritis Foundation . Integrity Senior Services . The Bronx Health Link . Lupus Foundation of America . Visiting Nurse Services . Primecare Before survey implementation, HSS piloted the survey with community members to garner feedback and ensure survey items were relevant and easily understood. HSS also facilitated feedback from internal key stakeholders and community partners throughout the process of developing the survey. Recruitment and Sampling: A convenience sampling strategy was used to recruit individuals 18 years and older living within HSS' primary and secondary service areas to complete the community survey. An oversampling approach was used to reach medically underserved and diverse populations, through the following recruitment strategies: 1. Alchemer panel service: This was used to administer the survey online and obtain community input from underserved and diverse populations by oversampling. 2. HSS patient targeting approach: A list of over 140k patients aged 18 years and older who received medical services from all HSS locations between 2019 and 2021 were pulled from our electronic medical record (EMR). This allowed us to segment, target, and send the online survey to patients with no medical insurance, public insurance (Medicaid), living in medically underserved areas and who preferred to receive medical information in Spanish, Chinese or Russian. 3. Text Messaging: To engage and obtain input from HSS patients that are underserved and may not have access to a PC to complete the online survey, we used text message reminders to complete the survey. 4. Social Media Geo-Targeting: To further garner input from the public, especially from medically underserved communities, we used social media such as Facebook and Patch.com to geo-target individuals aged 18 years and older living in specific zip codes identified as Medically Underserved Areas (MUAs) within HSS' primary and secondary service areas. The MUAs were derived from the U.S. Department of Health and Human Services (https://bhw.hrsa.gov/workforce-shortage-areas/shortage-designation). In addition to our recruitment efforts described above in completing the online survey, other varied approaches such as paper, and QR codes were used to increase accessibility and survey response from our community. BOARD COMMUNITY INPUT: HSS facilitated systematic feedback from its various constituents (i.e., internal stakeholders, community partners including the local public health department, and the public) based on the CHNA results to guide the selection of the health needs and services to address in its community programming. Our approach to engaging our constituents to obtain their input is described below. a. CHNA Steering Committee and Internal Stakeholders: We involved various representatives from HSS departments in developing the community survey and interview guide. A seven-member HSS CHNA steering committee was formed to guide the development and implementation of the CHNA process, and ensure alignment with HSS values and strategic priorities, and the NYS prevention health agenda. The CHNA steering committee identified research questions to be addressed, drafted the individual survey questions, and identified community partners and internal stakeholders to be involved in the CHNA process. The steering committee also facilitated interviews with community partners. Key internal stakeholders from various departments (i.e., Education Institute, Nursing, Social Work, Nutrition, Rehabilitation, Ambulatory Care Centers, Service Excellence, and Language Services, Regional Markets, Patient Experience, Medical Staff and Attendings, Development, Public Relations, and Marketing, Digital Communications, and Quality) were integral in the community health assessment process by providing relevant feedback to the survey construction and the use of validated instruments. b. Public Health Departments and Other Experts: HSS collaborated with the New York City Department of Health and Mental Hygiene and the Greater New York Hospital Association (GNYHA). Through these collaborations, we adopted a community-engaged approach that involved feedback in the development of our community survey, collection of qualitative data to broadly identify the health status of our communities, and guidance on incorporating best practices for our CHNA. c. General public including medically underserved and low-income population: To facilitate input from the public including medically underserved and low income population on the construction of the community survey, we piloted the survey in English, Spanish, Chinese, and Russian from October 22-29, 2021, among 70 community members. This allowed us to obtain meaningful feedback about the length of the survey and ensure cultural relevancy and health literacy. d. Community partners: To facilitate active engagement among our community partners, we contacted 74 organizations requesting their involvement in the CHNA process. We received interest to review and provide feedback on the community survey from 36 community partners. Feedback included the length of the survey, use of validated instruments, cultural relevance, and literacy level of the community survey. In addition, 22 community partners provided their input by participating in key informant interviews as described above. e. PAST COMMUNITY HEALTH NEEDS ASSESSMENT AND IMPLEMENTATION PLANS: THE POSTED THE 2019-2021 CHNA IMPLEMENTATION PLAN ON ITS WEBSITE (HTTPS://WWW.HSS.EDU/COMMUNITY.ASP), ALLOWING THE PUBLIC TO REVIEW AND PROVIDE FEEDBACK. AN EMAIL ADDRESS (COMMUNITY.ED@HSS.EDU) WAS PROVIDED ON THE WEBSITE TO RECEIVE QUESTIONS OR COMMENTS. HSS HAS NOT RECEIVED ANY COMMENTS REGARDING ITS 2019 - 2021 COMMUNITY HEALTH NEEDS ASSESSMENT. Prioritization and selection of health needs: To further HSS' commitment to developing programs that improve the health of our culturally diverse communities, obtaining feedback from the public and medically underserved, low-income, and minority populations were instrumental in driving the Hospital's selection of significant health needs to improve the health of communities where dramatic health disparities exist. Our approach to prioritizing health needs was to solicit input from the public through virtual community forums. Eight community forums were held to allow community members an opportunity to identify and prioritize health needs that provided HSS with the appropriate direction in selecting its public health priorities. Specific dates, locations, and attendance for the community forums were as follows: - June 02, 2022 at Community members (4 people present) - June 06, 2022 at Community members (1 person presents) - June 14, 2022 at Community members (29 people present) - June 16, 2022 at Community members (10 people present) - June 16, 2022 at Charla de Lupus (Teen, Young Adult, and Parent Support Group) (29 people present) - June 20, 2022 at Community members (18 people present) - June 22, 2022 at Community members (32 people present) - June 24, 2022 at Community partners (8 people present) A total of 131 community members participated in the community forums where participants were asked to rank seven health indicators, from a list of 20, identified in the CHNA according to the order of personal importance (wher
PART V, LINE 7DTHE HOSPITAL SUMMARIZED THE RESULTS OF ITS COMMUNITY HEALTH NEEDS ASSESSMENT (CHNA) IN ITS COMMUNITY BENEFIT REPORT. THE COMMUNITY BENEFIT REPORT AND CHNA ARE POSTED ON THE HOSPITALS WEBSITE, AT https://www.hss.edu/education-institute/community-education/community-bene fit, AND BOOKLETS ARE ALSO DISTRIBUTED WITHIN THE HOSPITAL. THE COMMUNITY BENEFIT REPORT IS ALSO E-MAIL BLASTED TO OUR COMMUNITY PARTNERS, SELECTED DONORS AND PUBLIC OFFICIALS.
PART V, LINE 11A detailed description of how HSS' hospital-based, outreach, and support initiatives will address health needs, the anticipated impact of the implementation strategy, and planned collaboration with organizations are provided below. HSS is addressing all significant health need identified in the most recently conducted CHNA. 1. Asian Bone Community Health Initiative: This initiative is comprised of culturally relevant evidence-based interventions (i.e. yoga, low-impact chair exercises and Self-management education workshops) designed to improve Asian older adults' management of their chronic musculoskeletal conditions (such as osteoarthritis and osteoporosis) and its symptoms (e.g. stiffness, fatigue, chronic pain) while also increasing access to care in this medically underserved community. The anticipated program impact includes increased access to linguistically-and culturally, increased competent musculoskeletal health programs, increased knowledge of self-management techniques, increased self-management skills of at-risk Asian older adult, and improved musculoskeletal health outcomes by decreasing musculoskeletal pain, stiffness, fatigue, and falls, and increasing the frequency of physical activity and self-efficacy. This program is a community-based initiative and will partner with Visiting Nurse Services of NY, Carter Burden Network, Self Help Community Center-Flushing, and Queens Community House in addressing the health need. 2. Musculoskeletal Health Wellness Initiative (MHI): This initiative is comprised of education and exercise programs to raise awareness, educate and reduce the impact of musculoskeletal conditions (such as osteoarthritis, osteoporosis, rheumatoid arthritis, gout, fibromyalgia) and its symptoms (e.g. stiffness, fatigue, chronic pain) in the community. Programs are held throughout the tri-state area specifically in New York City, Long Island, Westchester, NY, Stamford, CT, and Paramus, NJ as well as virtually via zoom and teleconference. The initiative offers evidence-based interventions such as Yoga, Tai Chi, Pilates, and SME workshops. The anticipated program impact includes increased access to musculoskeletal health programs among ethnically diverse populations, increased self-management skills and confidence to manage chronic conditions and orthopedic care, increased knowledge of musculoskeletal conditions and self-management techniques, and improved musculoskeletal health outcomes by decreasing musculoskeletal pain, stiffness, and fatigue, and physical health of the at-risk population. This program is a community-based initiative and will partner with YMCAs, Libraries, Senior Centers, and Mediflix. 3. VOICES 60+ Senior Advocacy Program: The program provides resources and support for patients and community members over 60 years old with arthritis and related rheumatologic diseases. The program was developed to assess and address the specific and unique needs of culturally and linguistically diverse older adults who are frequently marginalized in the healthcare setting by providing advocacy services, referrals to community resources, enhancing patient-provider communication, and supporting patients and their families in understanding and coping with their illnesses. Anticipated program impact includes increased knowledge of cognitive behavioral therapy (CBT) and mindfulness techniques, increased self-efficacy in using CBT and mindfulness techniques to reduce pain, self-reported improvement in communication between participant and provider, and self-efficacy in managing patient-provider communication. This program is a community-based initiative and will partner with Carter Burden Network, Dorot, Inc., Lenox Hill Neighborhood House, Service Program for Older People (SPOP), Selfhelp Community Services, and Stanley Isaacs Neighborhood Center. 4. Pain and Stress Management (PSM) Program: This program is comprised of educational, and mindfulness-based coping techniques to raise awareness, educate and improve the ability to cope with pain and stress. It offers mind/body workshops, exercises such as Yoga, and expert-guided meditation to help reduce physical and mental stressors. PART I, LINE 7, COLUMN F2024 BAD DEBT EXPENSE PRESENTED ON THE 2024 AUDITED FINANCIAL STATEMENTS WAS $10,874,058. IN ADDITION, TOTAL OPERATING EXPENSE WAS ADJUSTED ACCORDINGLY TO REFLECT THE .35% ($5,100,499) HEALTH FACILITY TAX ASSESSMENT AND 1% ($4,271,030) STATEWIDE ASSESSMENT IMPOSED BY NEW YORK STATE. AS SUCH, THESE FEES WERE ADJUSTED ACCORDINGLY IN LINE 7 AS WELL AS THE TOTAL OPERATING EXPENSE. PART I, LINE 7 THE HOSPITAL USED THE FOLLOWING METHODOLOGIES TO CALCULATE THE AMOUNTS REPORTED FOR EACH LINE ON SCHEDULE H, LINE 7: (1)LINE 7A "FINANCIAL ASSISTANCE AT COST" - UTILIZED GROSS CHARGES WRITTEN OFF REFLECTED IN THE 2024 MEDICARE COST REPORT AND REDUCED TO COST BY APPLYING THE RATIO OF PATIENT CARE COST-TO-CHARGES. THE PATIENT COST TO CHARGE RATIO IS CALCULATED IN WORKSHEET 2 WITH INFORMATION FROM THE 2024 MEDICARE COST REPORT AND HSS' 2024 GENERAL LEDGER TRIAL BALANCE. THIS COST WAS OFFSET BY THE NEW YORK STATE UNCOMPENSATED CARE POOL REVENUE. (2)LINE 7B "UNREIMBURSED MEDICAID" - UTILIZED THE DECISION SUPPORT ACCOUNTING SYSTEM TO IDENTIFY TOTAL COST (DIRECT AND INDIRECT) IN PROVIDING PATIENT SERVICE TO MEDICAID AND MEDICAID MANAGED CARE PATIENTS. IN ADDITION, ASSESSMENTS TO NYS FOR INDIGENT CARE POOLS ARE INCLUDED. (3)LINE 7E "COMMUNITY HEALTH IMPROVEMENT SERVICES" - THE HOSPITAL APPLIED ACTUAL EXPENSES TO DERIVE DIRECT COST AND UTILIZE THE COST REPORT TO DERIVE INDIRECT EXPENSES, APPLICABLE TO SUCH PROGRAMS. (4)LINE 7F "HEALTH PROFESSION EDUCATION" - THE MEDICAL EDUCATION COST WAS OBTAINED FROM THE 2024 MEDICARE COST REPORT FOR DIRECT AND INDIRECT EXPENSE. THE COST WAS OFFSET BY MEDICARE AND MEDICAID REVENUE RELATED TO DIRECT GME AS WELL AS OTHER DIRECT OFFSETTING REVENUE RELATED TO "HEALTH PROFESSION EDUCATION". (5)LINE 7H "RESEARCH" - THE 2024 MEDICARE COST REPORT WAS UTILIZED.
PART III, LINES 2, 3, 4A COST TO CHARGE RATIO OF 37.18% AS CALCULATED PER THE APPLICABLE FORM 990 INSTRUCTIONS WAS APPLIED TO THE BAD DEBT EXPENSE (AT CHARGES) OF $10,874,058 TO CALCULATE THE BAD DEBT EXPENSE OF $4,042,975 AT COST. IN 2024, THE HOSPITAL SCREENED BOTH INPATIENT AND OUTPATIENT ACCOUNTS PRIOR TO BEING CLASSIFIED AS BAD DEBT FOR "PRESUMPTIVE ELIGIBILITY" UTILIZING A COMMERCIALLY AVAILABLE INCOME PREDICTOR SOFTWARE. THE UNCOMPENSATED CARE TEXT OF THE FOOTNOTE TO THE ORGANIZATION'S FINANCIAL STATEMENT STATES, "AS A MATTER OF POLICY, THE HOSPITAL PROVIDES SIGNIFICANT AMOUNTS OF PARTIALLY OR TOTALLY UNCOMPENSATED PATIENT CARE. FOR ACCOUNTING PURPOSES, SUCH UNCOMPENSATED CARE IS TREATED EITHER AS CHARITY CARE UNDER THE HOSPITAL'S FINANCIAL ASSISTANCE POLICY OR AS BAD DEBT EXPENSE. THE HOSPITAL'S FINANCIAL ASSISTANCE POLICY ENSURES THE PROVISION OF QUALITY HEALTH CARE TO THE COMMUNITY SERVED WHILE CAREFULLY CONSIDERING AND TAKING INTO ACCOUNT THE ABILITY OF THE PATIENT TO PAY. THE HOSPITAL'S FINANCIAL ASSISTANCE PROGRAM'S ELIGIBILITY THRESHOLD IS 700% OF THE FEDERAL POVERTY GUIDELINES, WHICH IS IN EXCESS OF THE NEW YORK STATE MINIMUM REQUIREMENTS OF 400%, updated from 300% in October of 2024. THE HOSPITAL HAS DEFINED CHARITY CARE FOR ACCOUNTING AND DISCLOSURE PURPOSES AS THE DIFFERENCE BETWEEN ITS CUSTOMARY CHARGES AND THE DISCOUNTED RATES GIVEN TO PATIENTS IN NEED OF FINANCIAL ASSISTANCE. AS THE COLLECTION OF AMOUNTS DETERMINED TO QUALIFY AS CHARITY IS NOT PURSUED, SUCH AMOUNTS ARE NOT REPORTED AS REVENUE. PATIENTS WHO DO NOT QUALIFY FOR DISCOUNTED FEES ARE BILLED AT THE HOSPITAL'S ESTABLISHED CHARGES. UNCOLLECTED BALANCES FOR THESE PATIENTS ARE CATEGORIZED AS BAD DEBTS. TOTAL UNCOMPENSATED CARE IN 2024 AND 2023 FOR ALL PATIENT SERVICES AGGREGATED APPROXIMATELY $39.9 MILLION AND $33.1 MILLION RESPECTIVELY. IN ADDITION, THE HOSPITAL OPERATES ITS CLINICS AT A LOSS, TO HELP MEET THE NEEDS OF LOW INCOME AND UNINSURED INDIVIDUALS, AND ALSO OPERATES NUMEROUS OUTREACH AND EDUCATION PROGRAMS WHICH BENEFIT THE COMMUNITIES IT SERVES."
PART III, LINE 8OVER 56.6% OF THE HOSPITAL'S INPATIENT POPULATION IS REPRESENTED BY MEDICARE and Medicare Managed Care. THE MEDICARE SHORTFALL OF $86,173,697 AS REPORTED IN PART III, LINE 7 WAS CALCULATED USING MEDICARE COST REPORTING PRINCIPLES AND A COST TO CHARGE RATIO METHODOLOGY. THE MEDICARE SHORTFALL SHOULD BE CONSIDERED AS A COMMUNITY BENEFIT BECAUSE THE HOSPITAL IS PROVIDING CARE TO THESE PATIENTS WITH FULL KNOWLEDGE THAT THE MEDICARE REIMBURSEMENT WILL NOT COVER THE COST OF PROVIDING CARE TO THESE RESIDENTS OF OUR COMMUNITY.
PART III, LINE 9BSELF PAY BALANCES, INCLUDING RESIDUAL BALANCES DUE AFTER PAYMENT FROM AN INSURANCE CARRIER, AS WELL AS BALANCES DUE AFTER FINANCIAL ASSISTANCE DISCOUNTS HAVE BEEN APPLIED, ARE HANDLED ACCORDING TO THE FOLLOWING POLICY: ALL PATIENTS ARE BILLED USING STANDARD CYCLES INCLUDING THE ISSUANCE OF A DETAILED BILL, COLLECTION LETTERS AND/OR TELEPHONE CALLS, AND THEN TRANSFERRED TO A "PRE-COLLECTION STATUS." THESE COLLECTION EFFORTS ARE DOCUMENTED ELECTRONICALLY IN THE PATIENT'S FILE. DURING THE PRE-COLLECTION PHASE, ADDITIONAL TELEPHONE CALLS AND/OR LETTERS ARE MADE TO ATTEMPT TO COLLECT PAYMENT IN FULL. IF UNSUCCESSFUL, THE ACCOUNT IS TRANSFERRED TO A PROFESSIONAL COLLECTION AGENCY, NO LESS THAN 120 DAYS AFTER THE ORIGINAL SELF PAY BILLING DATE. THESE REASONABLE COLLECTION EFFORTS INCLUDE ALL PATIENT ACCOUNTS, FOR BOTH MEDICARE AND NON-MEDICARE PATIENTS. MEDICARE AND NON-MEDICARE ACCOUNTS ARE RECORDED SEPARATELY. ANY PATIENTS EXPRESSING INTEREST IN APPLYING FOR FINANCIAL ASSISTANCE WILL HAVE THEIR BILLS HELD PENDING ELIGIBILITY DETERMINATION. IF PATIENT IS ELIGIBLE, THE ACCOUNT WILL BE REDUCED BASED ON THE APPROPRIATE SLIDING SCALE DISCOUNT AND THE RESULTING BALANCE WILL BE RE-BILLED ACCORDINGLY. PAYMENTS RECEIVED BY THE COLLECTION AGENCY REDUCE THE AMOUNT RECORDED AS BAD DEBT. IT IS THE HOSPITAL'S PRACTICE NOT TO FILE SUIT, ATTACH ASSETS, OR PURSUE LIENS FOR ALL PATIENTS INCLUSIVE OF FAP ELIGIBLE PATIENTS. IN CERTAIN INSTANCES, IN COORDINATION WITH THE HOSPITAL'S INTERNAL LEGAL COUNSEL, THE HOSPITAL MAY TAKE SUCH MEASURES IF DEEMED WARRANTED. PAYMENT ARRANGEMENTS: IF PATIENTS ARE UNABLE TO PAY THE ENTIRE BALANCE IN FULL, PAYMENT ARRANGEMENTS OF MONTHLY PAYMENT WILL BE OFFERED. UNDER SUCH ARRANGEMENTS, ACCOUNTS ARE TYPICALLY PAID IN FULL WITHIN SIX TO TWELVE MONTHS UNLESS OTHERWISE EXTENDED AS PART OF THE FINANCIAL ASSISTANCE PROGRAM. A CONFIRMATION LETTER WILL BE SENT WITHIN 5 DAYS, EXPLAINING THE TERMS OF THE AGREEMENT. IF A MONTHLY PAYMENT IS MISSED, THE ACCOUNT WILL PROGRESS TO THE NEXT STAGE IN THE CYCLE. UNDER CERTAIN CIRCUMSTANCES, HSS MAY PROVIDE FINANCIAL ASSISTANCE PRIOR TO, OR WITHOUT, ANY APPLICATION BEING MADE FOR SUCH ASSISTANCE. AT HSS, THIS WILL BE PERFORMED THROUGH A SCREENING PROCESS. UNPAID ACCOUNTS WILL BE SCREENED USING COMMERCIALLY AVAILABLE INCOME PREDICTOR SOFTWARE TO DETERMINE WHETHER INDIVIDUALS MAY QUALIFY FOR FINANCIAL ASSISTANCE (PRESUMPTIVE ELIGIBILITY). HSS WILL ALSO CONDUCT PRESUMPTIVE ELIGIBILITY SCREENINGS ON ACCOUNTS PRIOR TO REFERRAL FOR ANY EXTRAORDINARY COLLECTION ACTIONS (ECA) AS DEFINED BELOW IN "COLLECTION ACTIVITIES". PRESUMPTIVE ELIGIBILITY DETERMINATIONS WILL APPLY ONLY TO THE UNPAID BALANCE(S) THAT TRIGGERED THE SCREENING PROCESS. IF THE INDIVIDUAL QUALIFIES UNDER THE PRESUMPTIVE METHODOLOGY WITH RESPECT TO THE UNPAID BALANCE(S), THEN HSS WILL GRANT THE MAXIMUM LEVEL OF ASSISTANCE OTHERWISE PERMITTED WITH RESPECT TO THE UNPAID AMOUNT. INDIVIDUALS WHO ARE GRANTED FINANCIAL ASSISTANCE UNDER PRESUMPTIVE ELIGIBILITY WILL BE PROVIDED NOTICE OF THEIR ABILITY TO APPLY FOR ADDITIONAL ASSISTANCE UNDER THE POLICY. IF PRESUMPTIVE ELIGIBILITY IS AWARDED BASED ON A SCREENING OF UNPAID BALANCES OVER 240 DAYS, INDIVIDUALS MAY, WITHIN THIRTY (30) DAYS FROM THE DATE OF THE GRANTING OF PRESUMPTIVE ELIGIBILITY, APPLY FOR ADDITIONAL FINANCIAL ASSISTANCE FOR THE SERVICES TO WHICH THE PRESUMPTIVE ELIGIBILITY DETERMINATION HAS BEEN APPLIED. IN 2024, THE HOSPITAL CONDUCTED NO EXTRAORDINARY COLLECTION ACTIONS.
PART VI, LINE 2 - NEEDS ASSESSMENTHOSPITAL FOR SPECIAL SURGERY'S NEEDS ASSESSMENT PROCESS INCORPORATES RELEVANT NATIONAL, STATE, AND CITY HEALTH DATA, GOALS, AND PRIORITIES. THE HOSPITAL UTILIZES HEALTHY PEOPLE 2025, WHICH SETS BROAD-BASED GOALS AND OBJECTIVES TO EXPAND AMERICANS' ACCESS TO CARE, AND TO ELIMINATE HEALTH DISPARITIES BY AGE, GENDER, RACE, OR DISABILITIES, AND PAYS SPECIAL ATTENTION TO INFORMATION REGARDING THE DIVERSE NEEDS OF ALL NEW YORKERS. THE HOSPITAL ROUTINELY CONDUCTS NEEDS ASSESSMENTS AMONG KEY GROUPS IN ITS COMMUNITY, ON WHICH TO BASE MULTI-YEAR PROGRAMS. OTHER NEEDS ASSESSMENT DATA IS GATHERED THROUGH A RIGOROUS EVALUATION PROCESS OF PUBLIC PROGRAMS. IN 2022, HSS CONDUCTED A COMMUNITY HEALTH NEEDS ASSESSMENT (CHNA) TO PROVIDE INSIGHT INTO OUR COMMUNITY'S: (1) HEALTH STATUS AND QUALITY OF LIFE; (2) HEALTH BEHAVIOR AND LIFESTYLE; (3) USE OF AND ACCESS TO CARE; AND (4) SOCIO-DEMOGRAPHIC CHARACTERISTICS, garnering a response from more than 18,000 patients and community members. IN ADDITION, key informant interviews were held with 22 community partners TO ALLOW for additional insight into community health needs. A series of 8 virtual forums reaching 131 community members were also held, where they engaged in an open discussion about the CHNA results and were then asked to rank 7 health indicators in order of importance. This provided HSS WITH DIRECTION IN FOCUSING ITS PUBLIC HEALTH PRIORITIES. ALL HOSPITAL Community Education & Outreach PROGRAMS, AND DIAGNOSIS-BASED SUPPORT AND EDUCATION GROUPS INCLUDE PARTICIPANT SURVEYS, AND THE FEEDBACK FROM THESE ARE USED IN FORMULATING NEW OR REFINING EXISTING EDUCATION AND SUPPORT-BASED OFFERINGS FOR PATIENTS and the public. FURTHER, THE HOSPITAL'S PATIENT EXPERIENCE COUNCIL REVIEWS PRESS GANEY PATIENT COMMENTS AND QUARTERLY REPORTS, AND ALONG WITH DEPARTMENTAL MANAGERS AND MULTIDISCIPLINARY TEAMS DEVELOPS IMPROVEMENTS IN SERVICES BASED ON THIS FEEDBACK OF PATIENTS' NEEDS. FINALLY, TO ENSURE THE RANGE OF NEEDS IS INTERPRETED AT THE HIGHEST LEVEL OF MANAGEMENT, THE QUALITY BETTER COMMITTEE OF THE HOSPITAL'S BOARD OF TRUSTEES RECEIVES AND REVIEWS ONGOING REPORTS THAT INCLUDE RESULTS OF PATIENTS' SATISFACTION TRENDS AND A SUMMARY OF THE HOSPITAL-WIDE QUALITY ASSESSMENT AND PERFORMANCE IMPROVEMENT PROGRAM.
PART VI, LINE 3 - PATIENT EDUCATION OF ELIGIBILITY FOR ASSISTANCEINFORMATION ABOUT THE AVAILABILITY OF THE FINANCIAL ASSISTANCE PROGRAM IS POSTED IN SIGNAGE IN ALL AREAS OF THE HOSPITAL AND ANCILLARY SERVICE LOCATIONS WHERE A PATIENT IS REGISTERED FOR SERVICES. ANY HOSPITAL BILL RECEIVED BY THE PATIENT FOR SERVICE CONTAINS INFORMATION ON THE AVAILABILITY OF THE FINANCIAL ASSISTANCE PROGRAM. ADDITIONALLY, ANY BILL SENT TO A PATIENT BY THE HOSPITAL OR THE HOSPITAL'S COLLECTION SERVICE CONTAINS INFORMATION ON THE AVAILABILITY OF THE FINANCIAL ASSISTANCE PROGRAM. ALL REGISTRATION, CUSTOMER SERVICE, AND PATIENT ACCOUNTING STAFF ARE TRAINED ON THE AVAILABILITY OF THE PROGRAM AND INSTRUCTED ON HOW AND WHEN TO COMMUNICATE THIS INFORMATION. THE FINANCIAL ASSISTANCE ASSOCIATES EDUCATE, SCREEN, AND MAKE THE ELIGIBILITY DETERMINATION FOR THE HSS FINANCIAL ASSISTANCE PROGRAM. THE ASSOCIATES ARE ALSO AVAILABLE TO ASSIST THE PATIENT IN FILLING OUT ALL FINANCIAL ASSISTANCE RELATED FORMS.
PART VI, LINE 4 - COMMUNITY INFORMATIONThe Hospital for Special Surgery's primary service area consists of the five boroughs of New York City, while its secondary service area is comprised of Hudson Valley, New York; Northern and Central New Jersey; Connecticut; Long Island, New York; Palm Beach and Collier County, Florida. HSS patients come from many communities locally, regionally, nationally, as well as internationally.
PART VI, LINE 5 - PROMOTION OF COMMUNITY HEALTHHospital For Special Surgery's (HSS) community outreach brings information to patients, professionals and the public through collaboration with educational, multi-service social service agencies, faith-based organizations, support and education groups and programs, community-based organizations, and academic institutions. In addition, HSS provides educational opportunities to the community through health fairs, educational lectures and workshops through partnerships with public schools, health clinics, senior centers and other community based organizations. In 2024, we collaborated with an array of community organizations to meet educational and support needs for underserved communities in areas such as musculoskeletal health, nutrition and physical activity, lifestyle medicine, health literacy, social support and disease self-management. Each year HSS sponsors various community education, support and outreach programs including educational workshops on musculoskeletal health and wellness, Leon Root, MD Pediatric Outreach Program, HSS Asian Community Bone Health Initiative, Greenberg Academy for Successful Aging's Aging With Dignity Initiative, Sneaker (super nutrition for all kids to eat right), HSS Youth athlete injury prevention programs, Pain & Stress Management Program, Lupusline, LANtern (Lupus Asian Network) The SLE Workshop, Early Rheumatoid Arthritis (RA), Living with RA Support And Education Groups and Latinx RA Community Support and Education Group VOICES 60+ Senior Advocacy Program, the Myositis Support and Education Group, Charla de Lupus (Lupus Chat), and the VOICES Medicaid Education Program. HSS offers a hybrid approach to our education & outreach programs, offering a mix of in-person and virtual programming. Depending on the audience, there were various formats used including zoom and conference call, ensuring our education, support and coaching programs were able to reach our diverse community. This hybrid approach allows us to reach a national and global audience of patients and the public. Many HSS programs featured HSS medical/clinical staff in helping patients and the public navigate their mobility, health and musculoskeletal issues. HSS places a high value on the impact that our community benefit programs have on the community. In 2024, we reached more than 11 million people with our live and on-demand programs, through over 1,300 programs, and distributed health education materials to nearly 6.5 million people (which includes over 900 thousand web page views and 5.5 million video views). Program evaluation surveys given to program participants assessing self-management metrics indicated that 93% of respondents gained knowledge from attending programs, 90% learned how to manage their condition and 95% were satisfied with the program. Qualitative assessments indicated that due to participating in our programs, participants felt a sense of connection with others, learned something new, felt less isolated and learned the most current, up-to-date information. In 2024, the Department of Social Work Programs reached over 5,376 patients and community members through a range of community support and education initiatives. These included in-person and virtual support programs, community education sessions, and disease-specific education for individuals living with rheumatic conditions. Our department's YouTube channel-available in English, Spanish, and Chinese-has garnered over 33,000 views in 2024. These videos reflect our commitment to patient-centered, disease-specific education and are culturally and linguistically tailored to reduce barriers and improve access for the diverse communities we serve. As part of its commitment to the community, in 2024 HSS continued to provide our community with valuable information on developed the Feel Better From Home platform, designed to make HSS expertise, insights and care available to anyone who needed it, wherever they needed it. The content provides information to help people stay active and healthy and are focused on injury prevention, fitness, health, mind/body and kids/sport. In addition, Social Work Programs and Community Education & Outreach worked with rheumatology clinicians and other HSS faculty to provide their expertise for a better understanding of musculoskeletal disease and continue to offer patients support around feelings of anxiety, depression, isolation, the loss of loved ones and coping with a chronic illness. A committee of the Board of Trustees focusing on the hospital's community benefit activities continues to provide oversight on HSS community education, outreach and support programs. HSS continues to have wide representation of our Community Service Plan (CSP) initiatives through ongoing programs specifically tailored for diverse communities offered in English, Spanish & Chinese. This included addressing the management of musculoskeletal issues and pain in older adults, lupus and mental health, lifestyle medicine and conducting key informant interviews to understand cultural themes regarding healthcare to enhance our interventions with our diverse communities. In addition, HSS has created webinars, blogs and educational videos for wide range of community benefit topics. HSS continues to engage with national audiences to elevate awareness of musculoskeletal health and best practices in disease self-management. As part of these efforts, staff from the HSS Community Education & Outreach team were invited to deliver two podium presentations at the 2024 American Public Health Association (APHA) Annual Meeting. The first presentation, "Bridging the Trust Gap: Implementing Innovative Email Strategies to Improve Public Health Communication in a Pandemic Era," highlighted digital engagement approaches during public health crises. The second, "Ensuring Cultural Competency in Evaluating Chinese Older Adults," focused on culturally responsive evaluation practices. In addition, HSS Education Institute staff presented a poster at the American Evaluation Association (AEA) Annual Conference titled "Building a Data Culture: Our Journey at the HSS Education Institute and Lessons Learned." This presentation explored institutional strategies for fostering a culture of continuous improvement through data-driven decision-making. The Social Work Programs Department was invited to the 2024 annual meeting of the American College of Rheumatology (ACR). Various social work leaders within our department serve in volunteer roles that highlight and reinforce the value of rheumatology professionals' contributions to patient care and research, advancing care for individuals impacted by rheumatic conditions. Our Assistant Director for Lupus Programs and Community Engagement served as Chair of the ACR Annual Meeting Planning Committee, leading a national, interdisciplinary team of health professionals. Together, they curated over 25 scientific sessions aimed at enhancing the education and knowledge of the rheumatology community, while promoting best practices in engaging diverse populations and providing culturally responsive support and educational services. In addition, our department contributed several scientific presentations, including a poster titled: "Empowering Lupus Communities: Evaluating a National Peer Support Program for Lupus Teen and Young Adult Patients and Their Families." This presentation highlighted the impact of the Charla de Lupus support group in strengthening participants' support networks and increasing knowledge around lupus treatment issues. Parent participants also reported improved coping with their child or loved one's condition, as well as a greater understanding of lupus and its treatment options. Our department also led a significant scientific session titled "Hidden in Plain Sight: The Impact of Ableism on Persons with RMD." The session aimed to raise awareness of ableism-one of the most pervasive forms of prejudice and discrimination faced by patients-and to emphasize that ableism exists across society, within professional environments, and at the individual level. The session encouraged clinicians to recognize and challenge ableist attitudes and to adopt more inclusive perspectives and practices, ultimately working to reduce barriers and disparities in patient care. For the past ten years the hospital has been designated as a leader in LGBTQ health by the Human Rights Campaign's Healthcare Equity Index devoted to promoting equitable and inclusive care for LGBTQ patients and their families. HSS is committed to equity and inclusion for all and as such created the Diversity, Equity and Inclusion (DEI) Leadership Council which is working to further enhance diversity initiatives; develop mentorship and leadership efforts and policies, procedures and processes that cultivate a sense of inclusion for everyone at HSS; and utilize metrics and goals to hold the institution accountable to new programmatic initiatives. Consistent with these initiatives, th
