Schedule H, Part V, Section B, Line 3ETHE SIGNIFICANT HEALTH NEEDS ARE A PRIORITIZED DESCRIPTION OF THE SIGNIFICANT HEALTH NEEDS OF THE COMMUNITY AND WERE IDENTIFIED THROUGH THE CHNA.
Schedule H, Part V, Section B, Line 5 Facility A, 1Facility A, 1 - GARNET HEALTH MEDICAL CENTER - CATSKILLS. Community input for the Community Health Needs Assessment (CHNA) in Sullivan County was collected between March 14, 2022, and May 22, 2022, through a regional survey conducted by the Siena College Research Institute (SCRI) in partnership with Garnet Health and the Local Health Department Prevention Agenda Collaborative. This collaborative includes seven local health departments and sixteen hospital facilities, representing a broad cross-section of public health expertise and community stakeholders. The survey engaged 5,699 adult residents aged 18 and older across the region, including a representative subset from Sullivan County. Responses were weighted by age, gender, race/ethnicity, and socioeconomic status to ensure accurate reflection of the population. According to the U.S. Census, Sullivan County's 2024 demographic projections include a population estimate of 80,450 with approximately 2.35% Asian & Pacific Islander Non-Hispanic, 7.60% Black Non-Hispanic, 19.94% Hispanic, 67.22% White Non-Hispanic, and 5.23% identifying as other or mixed ethnicities. Age distribution includes 20.42% aged 0-17 years, 33.14% aged 18-44, 25.41% aged 45-64, and 21.03% aged 65 and older, indicating a sizable older adult population with potential access and chronic care needs. Socioeconomic indicators show that 8.42% of Sullivan County households earn less than $15,000 annually, and 21.98% earn between $25,000 and $50,000. Educational attainment data reveal that 12.77% of residents have not completed high school, including 4.71% with less than a 9th-grade education and 8.06% with some high school but no diploma. These indicators reflect potential challenges related to health literacy, economic vulnerability, and access to services. To gather qualitative input from medically underserved and vulnerable populations, the Sullivan County Department of Health through the leadership of the Orange County Department of Health as part of the regional collaborative conducted focus groups with service providers who represent low-income individuals, veterans, seniors, people with disabilities, and members of the LGBTQ+ community. These focus groups provided firsthand insight into disparities in health outcomes and barriers to care across Sullivan County. Organizations that actively provided input during the CHNA process included Bon Secours Charity Health System (Good Samaritan Hospital), Bon Secours Community Hospital, St. Anthony Community Hospital, Garnet Health Medical Center (formerly Orange Regional Medical Center), Garnet Health Medical Center - Catskills, HealthAlliance Hospital, Nuvance Health, Montefiore Hudson Valley Collaborative, Montefiore Nyack Hospital, Montefiore St. Luke's Cornwall, Putnam Hospital Center, Vassar Brothers Medical Center, and St. Joseph's Medical Center. Additional community input came from local health departments, and community partners including offices of aging, veterans' affairs, school districts, food banks, and other key stakeholders. Additional collaborators that provided input or were invited to participate include Siena College Research Institute, The Foundation for Community Health, Dutchess County Department of Behavioral & Community Health, Orange County Department of Health, Putnam County Department of Health, Rockland County Department of Health, Sullivan County Public Health Services, Ulster County Department of Health and Mental Health, and Westchester County Department of Health. These organizations represent the interests of Sullivan County's medically underserved and vulnerable populations. Local business partners and public health departments contributed perspectives on healthcare barriers for low-income and uninsured populations, including the 8.42% of households earning less than $15,000 annually and those facing low health literacy. Veterans' services organizations provided insight into the unique needs of local veterans. Offices for the aging highlighted access challenges and chronic disease concerns among the 21.03% of residents aged 65 and older. Disability service providers represented those with mobility, sensory, or cognitive challenges. LGBTQ+ advocacy groups shared input on barriers to care for sexual and gender minorities. Food banks and social service agencies described the effects of food insecurity and poverty, which continue to impact significant portions of the population. This diverse and inclusive input process ensured that the CHNA accurately reflects the health needs, disparities, and service gaps experienced by medically underserved, low-income, minority, senior, disabled, veteran, and LGBTQ populations across Sullivan County

Schedule H, Part V, Section B, Line 11 Facility A, 1Facility A, 1 - GARNET HEALTH MEDICAL CENTER - CATSKILLS. Garnet Health Medical Center - Catskills (GHMCC) is actively addressing the significant health needs identified in its most recent Community Health Needs Assessment (CHNA) through a collaborative and multi-faceted approach. GHMCC played a key leadership role in partnering with the County Health Department and other community-based organizations to develop the Community Health Improvement Plan (CHIP), which outlines priority areas aligned with the New York State Prevention Agenda. The hospital and its partners selected evidence-based strategies to address the following priority areas: prevent chronic disease, improve mental health, and prevent substance use. In 2024, GHMCC reported $10.7 million in net community benefits, representing 10.48% of total hospital expenses. This included financial assistance and means-tested programs (0.06%) and $10.6 million (10.42%) in other community benefit initiatives. These efforts supported a wide range of programs and services addressing the identified health needs. To promote healthy eating and food security, GHMC-C continued to provide resources for local food pantries at discharge, offered shelf-stable meals through its Food Farmacy (screening over 4,000 patients and distributing 500+ food bags in 2024), and referred patients to the FreshRx nutrition incentive program in collaboration with Cornell Cooperative Extension. To support early detection of cardiovascular disease, diabetes, and obesity, the hospital enhanced referrals to primary care for patients presenting in emergency or urgent care settings, provided community-based prevention opportunities, relaunched the Healthy Heart program at local events and businesses, and delivered the Diabetes Prevention Program through multiple platforms to increase accessibility. GHMCC also partnered with employers to reduce barriers to cancer screening, including promotion of paid leave or flexible scheduling for preventive care. GHMCC expanded access to cancer services with targeted breast and prostate cancer screenings for uninsured and underinsured individuals, and implemented systems to connect patients to insurance navigators while awaiting care. Behavioral health and substance use were also key areas of focus. GHMCC supported the expansion of youth mental health training, group therapy, and community outreach. The hospital implemented policies and training for the initiation of medication-assisted treatment (MAT) in emergency settings, funded MAT training for prescribers, and, in collaboration with Catholic Charities, supported the PeerRx application to enhance peer referrals for individuals in need of substance use treatment. New or expanded programs in 2024 included "Just Breathtaking," a support group for individuals with chronic lung disease; "Warrior Kids," which engaged 379 children in nutrition and physical activity education; a Speakers Bureau offering community education on topics such as mental health, nutrition, and substance use; and a major expansion of cardiovascular care, including the launch of an open-heart surgery program and the introduction of a hybrid surgical suite at the Middletown campus to increase access to advanced cardiac care. In addition to the CHIP's primary focus areas, other identified health needs-such as access to medical providers, cancer care, obesity, preventive care, and women's and children's health were not specifically prioritized in the Implementation Strategy. However, many of these needs are being addressed through the hospital's delivery of comprehensive services and will continue to be integrated into strategic efforts over the next three years. Finally, certain community concerns such as affordable housing, domestic violence, and lack of affordable or reliable transportation fall outside the hospital's direct scope of clinical services. While these issues are not addressed through the Implementation Strategy, Garnet Health remains committed to exploring opportunities to support local organizations that are actively working to improve these important social determinants of health.

Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Schedule H, Part I, Line 5a Budgeted amounts for free or discounted careGarnet Health Medical Center - Catskills (GHMCC) does not maintain a separate, formal budget line specifically designated for financial assistance under its financial assistance policy. Although a discrete budget line is not maintained, Garnet Health forecasts and tracks financial assistance expenditures to ensure adequate resources are available throughout the year, demonstrating that it remains fully committed to providing significant financial assistance and charity care to patients in need. Financial assistance is incorporated within the broader operational and community benefit planning processes, enabling flexibility to respond to patient needs as they arise. In 2024, GHMCC delivered substantial financial assistance through a variety of means-tested programs, uncompensated care, and sliding-scale services. This approach ensures that patients who qualify for assistance receive appropriate care without the constraints of a rigid budget. This operational model supports the hospital's mission of serving the community and maintaining compliance with regulatory requirements. By embedding financial assistance into overall community benefit efforts rather than isolating it as a separate budget item, the hospital efficiently addresses patient needs while demonstrating transparency and accountability.
Schedule H, Part I, Line 7g Subsidized Health ServicesLINE 7G DOES NOT INCLUDE ANY COSTS ATTRIBUTABLE TO A PHYSICIAN CLINIC.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountFOR FINANCIAL STATEMENT PURPOSES, GARNET HEALTH MEDICAL CENTER - CATSKILLS (GHMCC) HAS ADOPTED ACCOUNTING STANDARDS UPDATE NO. 2014-09 (TOPIC 606). IMPLICIT PRICE CONCESSIONS INCLUDES BAD DEBTS. THEREFORE, BAD DEBTS ARE INCLUDED IN NET PATIENT REVENUE IN ACCORDANCE WITH HEALTHCARE FINANCIAL MANAGEMENT ASSOCIATION STATEMENT NO. 15 AND BAD DEBT EXPENSE IS NOT SEPARATELY REPORTED AS AN EXPENSE.
Schedule H, Part III, Line 3 Bad Debt Expense MethodologyGHMCC follows FASB ASC Topic 606 for revenue recognition, which requires that implicit price concessions be recorded as a reduction of patient service revenue rather than as bad debt expense. As a result, GHMCC does not separately report bad debt expense in its financial statements. Therefore, the estimated amount of bad debt attributable to patients who may have been eligible under the financial assistance policy is considered to be zero.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnotePer Note 2 of the audited financial statements: Subsequent changes to the estimate of the transaction price (determined on a portfolio basis when applicable) are generally recorded as adjustments to net patient service revenue in the period of the change. Portfolio collection estimates are updated monthly based on collection trends. Subsequent changes that are determined to be the result of an adverse change in the patient's ability to pay (determined on a portfolio basis when applicable) are recorded as bad debt expense. Bad debt expense for the periods ended December 31, 2024 and 2023, was not significant.

Schedule H, Part VI, Line 4 Community informationGarnet Health Medical Center - Catskills (GHMCC) defines its community as its broader service area, which includes portions of Orange, Sullivan, and Ulster Counties in New York State. Approximately 87% of GHMCC's patients reside within this service area, and approximately 20% of patients reside specifically in Sullivan County. For purposes of this Community Health Needs Assessment (CHNA), GHMCC has placed emphasis on Sullivan County to more effectively focus its resources on addressing significant health needs, targeting areas of greatest need and health disparities within its primary patient population. Comparative data from Orange and Ulster Counties is included in the CHNA report when available, to provide regional context. As of 2024, the population of Sullivan County is projected to be approximately 80,450 residents. with a racial and ethnic composition of approximately 2.35% Asian & Pacific Islander Non-Hispanic, 7.60% Black Non-Hispanic, 19.94% Hispanic, 67.22% White Non-Hispanic, and 5.23% identifying as other or mixed ethnicities. The age distribution includes 20.42% of residents aged 0 to 17 years, 33.14% aged 18 to 44, 25.41% aged 45 to 64, and 21.03% aged 65 and older. This demographic profile underscores the importance of a diverse range of health and social services, from pediatric and preventive care to chronic disease management and support for the older adult population. Economically, Sullivan County reflects both opportunity and ongoing challenges. According to the U.S. Census Bureau. The median household income is approximately $69,826, with a county-wide poverty rate near 14.5%. Gender disparities exist in earnings, with men earning a median income of $51,418 compared to $40,762 for women. The county's labor force numbers about 35,500 employed individuals, with the largest employment sectors being Health Care and Social Assistance (6,234 workers), Educational Services (4,100 workers), and Retail Trade (3,521 workers). The highest-paying industries include Utilities ($85,867 median earnings), Professional, Scientific, and Technical Services ($71,447), and Public Administration ($65,943). This economic diversity reflects a workforce engaged in both professional and service-based sectors, many sensitive to seasonal and market fluctuations typical of rural regions. Household income data highlights economic challenges within the county. Approximately 8.4% of Sullivan County households earn less than $15,000 annually, and 22.0% earn between $25,000 and $50,000. The median household income in Sullivan County is approximately $69,826. Approximately 14.5% of residents live below the Federal Poverty Guidelines, with economic vulnerability concentrated in specific rural and underserved communities. Healthcare access capacity in Sullivan County remains limited. Primary care physicians see an average of 3,069 patients annually-a 13.4% increase from the previous year-highlighting growing demand on an already stretched system. Dentists serve approximately 2,414 patients annually, and mental health providers see about 450 patients per year. These figures point to provider shortages and systemic challenges in attracting and retaining health professionals in rural areas. Educational attainment further highlights structural barriers to health and economic equity. Among adults aged 25 and older, approximately 12.35% have not completed high school, 29.81% are high school graduates without college, 28.16% have some college or an associate degree, 16.99% hold a bachelor's degree, and 12.69% have graduate or professional degrees. Males are more likely than females to lack a high school diploma and less likely to hold advanced degrees, trends that intersect with employment and insurance disparities. This comprehensive demographic, economic, healthcare, and educational profile informs Garnet Health - Catskills' efforts to identify service gaps, reduce health disparities, and align healthcare resources with Sullivan County residents' evolving needs. Emphasis is placed on targeting communities facing significant barriers-including high poverty, low educational attainment, inadequate insurance, and poor access to primary and behavioral health providers-to enhance health equity and outcomes across the region.
Schedule H, Part VI, Line 5 Promotion of community healthGarnet health medical center-Catskills (GHMCC) has demonstrated its ongoing commitment to improving the health status of the Sullivan county community through strategic reinvestment of surplus funds directly aligned with priorities identified in its most recent community health needs assessment (CHNA). The majority of CHMCC governing body consist of residents from the primary service area who are neither employees nor contractors of the hospital, nor family members thereof. These local health consumers provide essential insights into the healthcare needs and priorities of the communities served. In addition, GHMCC conducts and participates in extensive community outreach programs. Offering residents opportunities to engage directly with hospital representatives and voice their concerns. GHMCC maintains an open medical staff model, encouraging all qualified physicians to apply for membership, which expands specialty care access within the community. During the 2024 tax year, GHMCC allocated approximately $1.67 million in surplus funds towards targeted capital improvements and program expansions that address specific health needs identified through the CHNA. These investments have been purposefully directed to improve accessibility, affordability and quality of care for vulnerable populations including low-income residents, older adults and rural communities. More specifically, approximately $1.2 million was used to upgrade outpatient services and diagnostic capabilities, including advanced imaging equipment and laboratory technology. These upgrades enable earlier and more accurate diagnosis of chronic conditions such as diabetes, cardiovascular disease, and respiratory illnesses priority areas in the CHNA reducing the need for emergency care and hospital readmissions. As a result, wait times for crucial testing have decreased by 15% enhancing timely intervention for patients with limited transportation options or financial barriers. An additional $470,000 was invested in facility renovations and technology upgrades to improve patient flow and expand capacity in emergency and specialty care departments. These targeted improvements have led to a 10% increase in patient volume capacity and shorter waiting times, particularly benefiting seniors and individuals with disabilities who face transportation and mobility challenges. GHMCC also strengthened its telehealth infrastructure with funds allocated to expand virtual care series. This expansion has increased telehealth visits by 25% during the tax year, providing rural and homebound patients with greater access to primary and specialty care without the burden of travel. Furthermore, GHMCC partnered with local organizations to deliver community education and preventative services focused on social determinants of health such as housing, nutrition and behavioral health. These programs have engaged over 1,000 residents in 2024, targeting high-risk groups and improving health literacy, which is critical for managing chronic diseases and reducing health disparities. Through these strategic investments, CHMCC reinvest surplus funds to make a meaningful impact on access, affordability and quality of care, directly responding identified community health needs and supporting the well-being of the most vulnerable populations in Sullivan county.
