Schedule J, Part I, Line 4b Supplemental nonqualified retirement planMCCG HAD A SUPPLEMENTAL EXECUTIVE BENEFIT PROGRAM ("SEBP") FOR CERTAIN EXECUTIVES THAT WAS DESIGNED AS A LOAN REGIME SPLIT DOLLAR LIFE INSURANCE PROGRAM. THIS PROGRAM WAS EXPECTED TO PROVIDE DEATH AND OTHER BENEFITS TO EXECUTIVES AND TO PROVIDE REPAYMENT OF LIFE INSURANCE PREMIUMS TO THE ORGANIZATION. AS A RESULT OF ECONOMIC CONDITIONS, THE LIFE INSURANCE POLICIES DID NOT PERFORM AS ANTICIPATED. DURING 2009 MCCG SUSPENDED ALL PREMIUM PAYMENTS AND REEVALUATED THE EFFECTIVENESS OF THE PROGRAM FOR ALL CURRENT PARTICIPANTS. DURING CALENDAR YEAR 2009 THE MCCG BOARD OF DIRECTORS, AFTER CONSULTATION WITH COMPENSATION AND LEGAL ADVISERS, ADOPTED A RESOLUTION TO MAKE PAYMENTS TO THE RETIRED PARTICIPANTS IN THE SEBP. IN CONSIDERATION FOR THE RECEIPT OF SUCH PAYMENT, THE RETIRED EXECUTIVES SURRENDERED SUBSTANTIALLY ALL RIGHTS AND BENEFITS (OTHER THAN A SMALL DEATH BENEFIT) UNDER THE SEBP TO THE ORGANIZATION. SUBSEQUENTLY, A SIMILAR DECISION WAS MADE DURING FISCAL YEAR ENDED SEPTEMBER 30, 2011 FOR THE REMAINING (EMPLOYED) PARTICIPANTS WITH ANY PAYMENTS TO BE MADE DEPENDENT ON THE INDIVIDUAL CONTINUING TO PROVIDE SUBSTANTIAL SERVICES TO A SPECIFIED FUTURE DATE. IN ADDITION, MCCG AND ITS AFFILIATED ORGANIZATIONS ADOPTED A SERP PROGRAM FOR CERTAIN EXECUTIVES EFFECTIVE FOR SERVICES RENDERED ON OR AFTER JANUARY 1, 2010. THIS PROGRAM PROVIDES FOR ANNUAL ACCOUNT VESTING IF THE PARTICIPANT IS EMPLOYED ON DECEMBER 31 OF THE THIRD YEAR AFTER THE ACCOUNT IS CREATED. THE BENEFIT EQUALS THE ANNUAL INCREASE IN THE PRESENT VALUE OF A LIFETIME ANNUITY PAYABLE COMMENCING AT A SPECIFIED TARGETED FUTURE DATE. THE ANNUTIY IS EQUAL TO A SPECIFIC PERCENTAGE OF FINAL AVERAGE EARNINGS (GENERALLY 60%) LESS (1) THE EXPECTED ANNUAL SEBP PAYMENT USED IN CALCULATING THE SEBP LUMP SUM PAYMENT, (2) THE ANNUAL BENEFIT PROVIDED UNDER THE DEFINED BENEFIT PLAN, AND (3) 100% OF THE PARTICIPANT'S SOCIAL SECURITY BENEFIT. DURING THE YEAR, THE FOLLOWING BENEFITS WERE ACCRUED: KEN BANKS $118,780 ELBERT MCQUEEN $148,523 IN ADDITION, NAVICENT ADOPTED A RETENTION PAYMENT PLAN EFFECTIVE OCTOBER 1, 2011 DESIGNED TO ENCOURAGE DESIGNATED EMPLOYEES TO CONTINUE THEIR EMPLOYMENT. UNDER THE PLAN, CGHS MAY SELECT A RETENTION CREDIT EQUAL TO A PERCENTAGE OF THE EXECUTIVE'S BASE SALARY. THE CREDIT IS REDUCED BY THE VALUE OF LIFE INSURANCE COVERAGE PROVIDED TO THE EXECUTIVE. IN GENERAL, THE PLAN IS SUBJECT TO VESTING AT THE END OF THE THIRD PLAN YEAR AFTER THE ACCOUNT WAS CREATED OR AGE 65 IF EARLIER, AND IS SUBJECT TO FORFEITURE IF THE EXECUTIVE VOLUNTARILY SEPARATES FROM SERVICE. DURING THE YEAR THE FOLLOWING BENEFITS ACCRUED UNDER THIS PLAN: RHONDA PERRY $38,386 KEN BANKS $19,353 ELBERT MCQUEEN $11,650 Ninfa Saunders $150,000 THE FOLLOWING NONQUALIFIED RETIREMENT PLAN BENEFITS WERE REPORTED AS TAXABLE INCOME TO VESTED INDIVIDUALS: RHONDA PERRY $397,150 KEN BANKS $144,078

NUMBER OF UNITS: 26 CARLYLE PLACE AT HOME - CARLYLE PLACE AT HOME IS A PLAN SPECIFICALLY DESIGNED FOR SENIORS WHO WANT TO PREPARE FOR THE LONG-TERM, YET REMAIN IN THE COMFORT OF THEIR OWN HOMES. The impact of non-quantifiable contributions Carlyle Place has on the greater Middle Georgia Area can be subdivided into the following areas: Advocacy: Carlyle Place offers volunteer hours to the local chapter of the Alzheimer's Association and hosts events to offer educational opportunities for the community such as AARP and various Health-Driven topics. The events focus on and create an atmosphere of learning about other community services to improve mental, physical, and emotional health for the aging population. Community Involvement: Carlyle Place employees are active members of other community boards and committees. Some examples of these civic agencies include: Leading Age, Macon Little Theatre, Macon Symphony Orchestra, Meals on Wheels, Rainbow House, and Uptown Rotary. Conduit for Using Donations: Carlyle Place offers opportunities for the residents, staff members and others to participate in quarterly projects. Each of the projects has a list of items for donation to the following agencies: Daybreak, Family Advancement Ministries, Hephzibah Children's Home, Macon Outreach, Macon Rescue Mission, Loaves & Fishes, and Sonny Carter Elementary. Through numerous recruitment opportunities and social media, the community is invited to participate in our projects. In addition, fundraisers are held through bake sales and special meals to raise money for the American Heart Association and The Alzheimer's Association. Economic Impact: Carlyle Place attracts seniors from all over the country to relocate to the middle Georgia area. The residents purchase locally and utilize the healthcare services available to central Georgia. The community encourages local purchasing and investing in entertainment by offering transportation to community events, retail facilities, and healthcare agencies. Good Neighbors: The facilities at Carlyle Place are used by some community groups to facilitate monthly or quarterly meetings. Carlyle Place management encourages residents to recycle various items by offering pickup services at each resident's home on a weekly basis. The residents have organized a recycling service which picks up recycled items door to door and places them in the single stream recycling container acquired through Advanced Disposal. In addition, residents participate in an annual call for larger recyclables (electronics, TVs, Cell Phones, etc) which are collected and distributed to businesses and organizations to be disposed of properly. Donations can be made by residents to the local Goodwill Industries through our facility services department; this encourages the recycling of household goods rather than the items ending up in the landfill. During the refurbishment process, items in working condition are salvaged from some of the residences and donated to local housing service providers such as Macon Rescue Mission. Intergenerational engagement opportunities were offered to students of the following local schools and agencies: First Presbyterian Dayschool, Hephzibah Children's Home, North Macon Presbyterian Dayschool, Northway Church Preschool, Sonny Carter, and Stratford Academy. Recognition: Carlyle Place is Macon Telegraph's 2017 Best of the Best - Retirement Facility. Responding to Needs: The Middle Georgia area has a significant number of individuals who are homeless. In response to these needs, residents partnered with Mount DeSales for a day of service; creating hats and laundry soap to be sent to St. Peter Clavier Food Bank. Residents and staff members assisted the Backpack Ministry of Forest Hills UMC in creating bags for distribution and offered time serving lunches with Macon Outreach. 2017 created a unique opportunity with two Hurricanes making landfall within the State of Georgia. Carlyle Place responded with offering housing to Marshes Edge residents from St. Simon's Island. Residents opened their homes to allow some of the residents a place to stay. Carlyle Place served as a command center for the North Macon Area, partnering with several agencies to ensure safety and providing overflow housing for several families who were unable to be accommodated through Ronald McDonald House. Social Impact: The residents of Carlyle Place are avid servers within their community. Many residents have spent a lifetime investing time and talents into the communities in which they lived and worked. Now retired, many seek to give to the local community. For some this is no easy task due to physical challenges. As a result, staff members and leadership team have worked together to create opportunities for the residents to give back to the community while never leaving the facility. Some of the opportunities include: Sorting jewelry for Goodwill Industries, serving time with families who are staying at Ronald McDonald House, Cards for veterans through Red Cross, assisting with teach projects for Northway Preschool, pen pals for students at Mount DeSales, and making and delivering blankets for older children at The Children's Hospital, Navicent Health. The nurse in the wellness clinic has partnered with local colleges and universities to offer opportunities for practicing interviewing skills for students who will be entering the field of medicine. Mercer University sends medical students to learn how to assess and complete a poly-pharmacy review. The nursing students from Middle Georgia State University come to observe and interview the residents who visit the wellness clinic. The Resident Life and Facility Services departments combine forces to offer the residents an opportunity to make sandwiches 2 - 3 times each month, which are delivered to Loaves and Fishes for distribution the 2nd, 3rd, and 5th Tuesday of the month. Service learning opportunities have been created at Carlyle Place for students in high school and college in the surrounding areas. Students volunteer assisting our activity staff members in providing programming for the residents in our personal care homes and skilled nursing facility. In addition, we offer a University Affiliated Internship for music therapy students of Georgia College in Milledgeville. Students can complete their required 1200 clinical hours with residents in all levels of care. Volunteer Activities: Carlyle Place staff members have volunteered time, talents, and made donations (both monetary and in-kind) with various agencies in the middle Georgia Community. Volunteer hours totaled over 2500, monetary and in-kind donations totaled over $4500.
