DescWe continue to serve as a clinical affiliation site for therapy and nursing students in the surrounding counties as well as the southeastern states. In addition, we provide opportunities for high school students to shadow at our facility and continue to participate in community activities such as stroke screenings, oversight of support groups, and community organized events to raise money for community programs. We enhanced our volunteer program offering more recreational activities for our patients and administrative support for our staff. We continue to offer specialized programs to our patients such as vestibular therapy, lymphedema therapy, wheelchair seating, and pool therapy.Desc

DescHome Health: As western North Carolina continues to have one of the fastest growing elderly populations in the country, our Home Health program expanded to meet the demand for in-home services. During this year, the program provided over 163,000 vhome health visits. Our Home Health program serves a wide spectrum of our community needs, ranging from neonatal abstinence to joint replacement to strokes to complicated surgical wounds to our chronic disease population. The unique needs of our patient population require a staff of generalists and specialists, and this year staff continued to receive a wide variety of clinical training to ensure these needs are met. Working with our regional tertiary hospital, Mission Hospital, Mission clinical specialist and our home care educators we trained our home based staff and increased our competencies to work with our diabetic patients. We used consistent tools in educating patients from inpatient setting to home. The most significant work focused on a smooth transition of our Transylvania and Angel home care programs to CarePartners. Our initial efforts were to move to Joint Commission. This change centered our efforts on standardizing our policies and practice. We began standardizing clinical work, moving to common EMR, and common post-acute policy and procedures. We replicated our joint replacement home care pathways to these two programs. The area orthopedist supported the change; all nurses and therapist were on-board to new practice. We consolidated our performance improvement efforts. Staff from all sites were trained in LEAN principles by Mission engineer. We selected common patient outcomes to attend and shared agency best practices. The entire CarePartners clinical and administrative team remains patient focused, as demonstrated by the continued improvement in our patient outcome scores and patient satisfaction scores. Adult Day: In 2016, Adult Day Services provided over 29,700 days of care compared with over 7,300 in the prior year due to expansion of the programPrivate Duty: During FY16, our private duty program provided services throughout Buncombe and Henderson County to clients ranging from 3 hours to 24 hours care, seven days a week. Over 98,000 hours of private duty care were provided.Desc

COMMUNITY CAREPARTNERS, INC.Part V, Section B, Line 5: Including input from the community is an important element of the community health assessment process. The following primary data (collected directly from community members via surveys or focus groups) was reviewed. You will note icons for each data type. The same icons appear throughout this document as each data type is referenced. Results from Primary Survey of 300 Buncombe County residents done by Professional Research Consultants, Inc. (PRC) as part of the WNC Healthy Impact Partnership. The same survey was done of 200 residents of partnering counties for comparison.Electronic survey of 43 local community leaders across Buncombe County who have leadership roles in community health, businesses, social service, mental health and healthcare organizations. This survey was also done by PRC and so was done in many other western NC counties.Locally compiled electronic surveys collected from 60 community health partners currently working within agencies in Buncombe County to understand what they see as their clients greatest health concerns and challenges.Survey data from 400 older adults completed through The Area Council on Aging, Aging Planning Consortium to gather information on healthy lifestyles.Women and Children s Safety Coalition s Intimate Partner Violence Victim Focus Group results that gathered input from women experiencing intimate partner violence to help improve the system.Survey data from residents of a local public housing community asking about their biggest health concerns and challenges.Responses from pregnant and parenting women in our community who were asked, What are the main issues impacting your health and pregnancy? as part of the Community Centered Health Home Project through MAHEC. In addition, results from the Photovoice Project conducted by Positive Parenting Program and Buncombe Partnership for Children that captured the voice of pregnant and parenting women through their photos and stories to better understand the challenges and needs facing these women. In addition to the WNC Healthy Impact s Core Data Set, Buncombe County CHIP Data Team reviewed many additional secondary (already existing) data sources including:Smoky Mountain Local Management Entities 2015 Provider Capacity, Community Needs Assessment and Gap Analysis for 2013-2014,The Homelessness Count,Department of Social Services Regional Data Report,MAHEC's Community Centered Health Home Epidemiology Report,NC Center for Health Statistics Data on Adverse Childhood Experiences in NC.The CHIP Data Team (made up of representatives from BCHHS, Mission Hospital, MAHEC and NC Center for Health & Wellness) reviewed and prioritized key data concerns identified that were also community concerns voiced in the surveys and focus groups. Results of the data review were shared with the CHIP Advisory, which provides leadership for our CHIP. The Advisory Board's 30+ community leaders prioritized the health concerns and recommended the top four health priorities. Partners and stakeholders will come together around the priorities to gather input on what is currently happening, gaps and additional opportunities to better address the issues.
Asheville Specialty HospitalPart V, Section B, Line 5: Throughout the collaborative health needs assessment process in our community, input was obtained in a number of ways. Community input in Buncombe County is led by the Buncombe County Community Health Improvement Process (CHIP) Advisory Board, comprised of 30+ community leaders who collectively lead and guide the process, including prioritizing health concerns and recommending top priorities.Community input in Madison County is coordinated through the Madison Community Health Consortium, which has been working to improve health in Madison County since 1989. The Consortium works with community agencies and groups throughout the needs assessment process, and currently has more than 50 members representing 25 community organizations.Further information about community engagement can be found in the Local Community Health Assessments which can be found at each county's website. Our collaborative health needs assessment process solicited and took into account input from the following:Public Health DepartmentIn the collaborative assessment process for our community, the Buncombe County Department of Health and Human Services, and the Madison County Health Department are key partners. The Health Departments provided coordination for the local process that Mission Hospital and CarePartners help to support and implement.Medically underserved, low-income, and minority populationsOur process included input regarding the needs of medically underserved, low-income, and minority populations in two ways. (1) As part of our collaborative data collection effort, a community-wide telephone survey was conducted to better understand the specific health needs and status of all of the community, which includes these special populations. (2) In addition, a survey of key informants was conducted to gain input from the individuals and organizations in our community representing the interests of these populations in their local efforts.Written CommentsThrough our partners at Mission Hospital and CarePartners, our facility also considers any written comments received since the last CHNA and implementation strategy. Mission Hospital maintains public access to its CHNA and implementation strategy on its website, and accepts comments through the Community Investment office.

COMMUNITY CAREPARTNERS, INC.Part V, Section B, Line 11: Priority Health Issue #1: Obesity and Chronic Disease PreventionThe organization will provide health education opportunities and health services that help individuals manage/prevent chronic diseases and maintain a healthy weight. Description of Activities - As a post-acute care provider, CarePartners primarily serves individuals who are recovering from an acute episode related to a chronic condition. Our clinical expertise is in preventing chronic diseases from getting worse and preventing the development of a secondary or tertiary condition. The following programs are part of CarePartners: Mindful Living Education Series - Year-round educational classes on SMART Goals and chronic disease therapy Outpatient Clinic offers balance screenings and rehabilitation therapy. The Organization will also work through partner agencies promote increased physical activity in our community. Description of Activities: CarePartners PACE will partner with YMCA to offer low cost exercise classes to individuals with Parkinson's or any other movement disorders. In addition other movement programs will be offered to the community at various Organization locations. Priority Health Issue #2: Mental Health and Substance AbuseThe Organization will support vulnerable individuals and families through free and low cost bereavement and mental health counselling. Description of Activities - The Organization offers bereavement services to all who are experiencing mental health issues as related to the death of a loved one. Ability to pay is not a consideration to receive support. Priority Health Issue #3: Infant MortalityThe Organization will grow and strengthen the program for infants with neonatal withdrawal syndrome (NWS) and continue to increase the intervention and treatment for vulnerable babies suffering from NWS and families who are returning home. Needs Not Addressed in This PlanThough we recognize that each community health need identified by the community is important, for the reasons described below, our hospital facility will not have a specific role in the implementation of strategies to address the following issues:Intimate Partner ViolenceIntimate partner violence is not a clinical core competency of CarePartners and the hospital does not necessarily have the expertise or resources to address this need directly. As an affiliate of Mission Health, Mission Hospital has more resources and experts to address this community need.Substance AbuseSubstance abuse is not a clinical core competency of CarePartners and the hospital does not necessarily have the expertise or resources to address this need directly. As an affiliate ofMission Health, Mission Hospital has more resources and experts to address this community need.
Asheville Specialty HospitalPart V, Section B, Line 11: Asheville Specialty Hospital (ASH) is connected to community outreach beyond the limited scope of its clinical services through its affiliation with Mission Health and Carepartners. Typical of long-term, acute care facilities, ASH engages in minimal community outreach and education.Obesity & Chronic Disease Prevention and Healthy Weight needs are addressed through the Clinical Dietician on staff for routine patient care, and rounds with a multi-disciplinary team four days a week in addition to providing services seven days per week. Nutrition assessment administered to every patient within 72 hours of admission. Discharge Planners/Case Managers refer patients to community resources to ensure support at discharge or next level of care.Mental Health and Substance Abuse are addressed as nearly 15% of ASH patients at any given time are patients referred for medical treatment support due to identified IV drug use. ASH provides monitoring while patients receive IV antibiotics. ASH works closely with licensed clinical social workers to screen and support these patients through their treatment and identified appropriate services upon discharge. Patients are connected with local primary care providers as part of discharge planning to ensure their care continues.Due to ASH's limited scope of services, and the resources provided through affiliation with Mission Health and CarePartners, which are better equipped to support community efforts, ASH does not have a specific role in addressing Intimate Partner Violence or Infant Mortality. Intimate Partner Violence and Infant Mortality are addressed by community partners in Buncombe County, as well as hospital implementation strategies by Mission Hospital and CarePartners.
COMMUNITY CAREPARTNERS, INC.Part V, Section B, Line 15e: The Organization's staff will work in collaboration with the patient and appropriate community health and human services agencies and other organizations that assist people in need of health care services to determine available funding sources.
