Hebrew Home for Aged DisabledPart V, Section B, Line 5: The organization is a member of the San Francisco Health Improvement Plan (SFHIP), which consists of non-profit hospital partner members who collaborated together to complete the 2016 San Francisco Community Health Needs Assessment. The goals of the community engagement component of the CHNA were to: (i) identify San Franciscan's health priorities, especially those of vulnerable populations, (ii) obtain data on populations for which there was little quantitative data, (iii) build relationships between the community and SFHIP, and (iv) meet the regulatory requirements, including the IRS rules for charitable 501(c)(3) Hospitals, Public Health Accreditation Board requirements for San Francisco Health Department, and the San Francisco Planning Code requirements for a Health Care Services Master Plan.SFHIP worked with community partners to co-host community meetings with target populations. Such community partners include: Advancing Justice of the Asian Law Caucus, African American Art and Cultural Center, Asociacion Mayab, CARECEN, Filipino American Development Foundation, Instituto Familiar de la Raza, Larkin Street Youth, LGBT Center, Native American Health Center, On Lok 30th Street Senior Center, Swords to Plowshares, and Trasitions Clinic. Target populations were selected based on four factors: (1) the population has known health disparities, (2) little information describing the health of the population was available, (3) the population was not included in a recent health assessment, and (4) the population was reachable through an existing community group. When possible, SFHIP joined existing meetings in an effort to increase efficiency and facilitate participation by residents. The main question asked of participants in these meetings was "What actions can we take--including residents, community groups, and SFHIP--to improve health"? Participants were also asked about the assets and barriers which exist in their communities regarding health. In total, 127 participants attended 11 meetings between July 1st and October 2nd, 2015. Participants came from a wide variety of backgrounds. The ethnic group with the largest representation in the meetings were Latino (23%), Black/African American (15%), White (17%), and Asian (12%). Other self-reported ethnicities included Arab, Filipino, Jewish, Middle Eastern, and Native American. The majority of participants were female (59%).

Part I, Line 7:COST TO CHARGE RATIOTHE COST TO CHARGE RATIO AS DETERMINED BY WORKSHEET 2 WAS USED TO CALCULATE THE AMOUNTS REPORTED ON LINE 7A AND 7B. THE COSTING METHODOLOGY USED ON LINE 7H WAS CALCULATED BASED ON ACTUAL COSTS FROM THE ORGANIZATION'S GENERAL LEDGER.
Part I, Ln 7 Col(f):BAD DEBT EXPENSE IS REPORTED AS A NET ADJUSTMENT TO PATIENT CHARGES AND THEREFORE, IS NOT INCLUDED AS A PART OF TOTAL EXPENSES FROM PART IX, LINE 25, COLUMN A.
Part III, Line 2:UNCOLLECTED RESIDENT ACCOUNTS ARE ANALYZED BASED ON STANDARD PROCEDURES FOR ALL RESIDENT ACCOUNTS. THE RESULT OF THE ANALYSIS IS WHAT IS RECOGNIZED AS BAD DEBT EXPENSE.
Part III, Line 3:Cost-to-charge ratio method
Part III, Line 4:THE FOOTNOTES IN THE AUDITED FINANCIAL STATEMENTS THAT ADDRESS "PATIENT ACCOUNTS RECEIVABLE, NET", WHICH INCLUDES A DISCUSSION OF "ALLOWANCE FOR DOUBTFUL ACCOUNTS" CAN BE FOUND ON PAGES 9, 10, AND 23-24 OF THE AUDITED REPORT. THERE IS NO SEPARATE FOOTNOTE FOR BAD DEBT EXPENSE.
Part III, Line 8:MEDICARE ALLOWABLE COSTS WERE CALCULATED USING A COST-TO-CHARGE RATIO. THE AMOUNTS REPORTED IN LINES 5, 6, AND 7 COME FROM THE HOSPITAL'S FILED MEDICARE COST REPORT. THE TOTAL REVENUE EQUATES TO MEDICARE PAYMENTS AND THE MEDICARE ALLOWABLE COSTS ARE DETERMINED ON THE MEDICARE COST REPORT THAT UTILIZED COST TO CHARGE RATIOS APPLIED TO COVERED MEDICARE REVENUES.
Part III, Line 9b:Patients who are known to qualify for financial assistance may request and negotiate the terms of an extended payment plan. If the patient and SFCJL agree on a payment plan, the patient must make regular, timely payments. A patient who does not comply will be sent to collections, may have adverse information reported to a credit agency, and may be subject to a property lien, excluding the patient's primary residence. However, asset information obtained in the charity care eligibility determination process are not used for collection activities.
Part VI, Line 2:To identify the most significant health needs in San Francisco, the SFHIP steering committee, and SFHIP Community Health Needs Assessment Subcommittee (including representatives from SFCJL) met on October 8, and November 4th, 2015. Participants identified health needs through a multistep process. First participants reviewed data and information from the Community Health Status Assessment, the Assessment of Prior Assessments, and the Community Engagement, as well as the health priorities from the 2012 Community Health Improvement Plan. Then, using the Technology of Participation approach to consensus development, participants engaged in small group focused discussions about the data. Finally, participants developed consensus on the health needs. Throughout the process needs were screened using pre-established criteria. Through this process two foundational issues and seven health needs were identified. Foundational issues are needs which affect health at every level and must be addressed to improve health in San Francisco. The two foundational issues identified were: (i) economic barriers to health, and (ii) racial health inequities. The seven health needs identified were: (i) psychosocial health, (ii) healthy eating, (iii) safety and violence prevention, (iv) access to coordinated, culturally and linguistically appropriate services across the continuum, (v) housing stability/homelessness, (vi) substance abuse, and (vii) physical activity.
Part VI, Line 3:A RESIDENT SERVICES COORDINATOR STAFF IS AVAILABLE TO ASSIST RESIDENTS AND/OR THEIR FAMILY MEMBERS OR LEGAL REPRESENTATIVES THROUGH THE PROCESS OF DETERMINING THEIR ELIGIBILITY FOR ALL GOVERNMENT PROGRAMS AND ASSIST THEM IN THE APPLICATION PROCESS.
