DescThe Organization provides behavioral health services in the form of inpatient, outpatient, and clinical counseling settings with a focus on senior behavorial sciences at the "Senior Behavorial Sciences" (SBS) center. The primary patients served by this center are patients over the age of 55 and are primarily Medicare and Medicaid program beneficiares which often reimburse the organization at rates below the cost of treating these patients. However, the Organization feels that it is their mission in the community to provide these services to patients in need. The SBS is proud to offer these services in a modern facility at a time when many distinct part psychiatric units are closing due to poor reimbursement from third-party payors. Through a grant provided by the Helen Bader Foundation, SBS also offers a memory clinic to work with senior patients who are having difficulties with memory as they continue to work to live on their own as long as possible. The SBS center is staffed by a full-time pyschiatrist, a nurse practitioner, and professional nursing and social services staff. Patients of the SBS also have full access to all of the diagnostic, therapy, and emergency services of the Southwest Health Center hospital. The total SBS center patient days were 2,098 in 2017.Based on the 2017 Medicare Cost Report, additional expenses of approximately $278,229 were allocated to the SBS center that are included in the hospital's expenses on line 4a of Part III of the 990, and these expenses include allocations for employee benefits, administrative functions, building depreciation, dietary, and other shared overhead expenses.Desc

ExplanationThe organization pays annual association membership dues to the Wisconsin Hospital Association (WHA). These dues are paid primarily to access educational materials and for staff training and materials. The WHA has notified Southwest Health Center, Inc. that $2,545 of the annual dues were used in conjunction with lobbying activities with the goal of improving the overall health care environment. The organization is also a member of the Rural Wisconsin Health Cooperative (RWHC). Each year, the organization pays membership fees to RWHC. The RWHC provides support services for a number of its member hospitals throughout the state of Wisconsin. Some of the many services provided to member hospitals include providing assistance to organizations in finding grant funding for new programs, legal services, reimbursement review services, accounting assistance, contracting for therapist and emergency room patient care coverage, and administrative consulting services. As a part of these services, the RWHC also does provide analysis of current health care issues in an effort to promote and better health care for hospitals in rural communities throughout Wisconsin. One of these efforts also includes some lobbying on the part of the member organizations. The RWHC determined that $6,283 of the fees paid by Southwest Health Center, Inc. in 2016 related to lobbying activities with the goal of improving the health care environment in the State of Wisconsin.ExplanationTxt

Part V, Section BFacility Reporting Group A
Facility Reporting Group A consists of:- Facility 1: Southwest Health Center, Inc., - Facility 2: Southwest Health Center, Inc. - SBS
Southwest Health Center, Inc. Part V, Section B, line 3j:ACTION PLAN TO ADDRESS COMMUNITY HEALTH NEEDS.
Southwest Health Center, Inc. Part V, Section B, line 5:The Community Health Needs Assessment (CHNA) sought input from diverse stakeholders, including health care providers, community leaders, and the public. A key stakeholder survey was created online with a link disseminated via email. Responses as well as the survey tool itself are included in the report. A separate survey was created in a hard copy version and diverse members of the general public were asked to complete the survey at community events, such as fairs and large gatherings. The CHNA process also relied significantly on health data from various credible sources to provide actual comparative risks for morbidity and mortality on a wide range of health issues from cancer and heart disease, smoking, accidents, and the environment.
Southwest Health Center, Inc. Part V, Section B, line 11:IN ADDITION TO ONGOING DAILY SHARING OF VITAL HEALTH INFORMATION, CARING FOR INDIGENT OR LOW INCOME COMMUNITY MEMBERS, PROVIDING NON-REIMBURSED CARE, AND EDUCATING OUR PATIENTS, THEIR FAMILIES, AND OUR COMMUNITIES ON INDIVIDUAL AND COLLECTIVE HEALTH ISSUES, WE ADDRESS PRIORITY COMMUNITY HEALTH NEEDS BY ENGAGING OUR CONSTITUENCIES, EMPOWERING THEM, CONNECTING THEM TO SERVICES AND RESOURCES, AND BY WORKING TO CREATE A LOCAL CULTURE OF HEALTH AND WELLNESS. THAT INCLUDES REDUCING BARRIERS TO GAINING ACCESS TO CARE, CREATING OPPORTUNITIES FOR SCREENINGS AND EARLY INTERVENTION, IMPROVING PATIENT ADVOCACY AND INFORMATION, PROVIDING EDUCATION, AND ELIMINATING STIGMAS. WE ALSO WORK TO RAISE AWARENESS OF EXISTING CLINICAL AND AOTHER VITAL SERVICES, OFFER SERVICES LOCALLY FREE OF CHARGE OR AT LOW COST, OFFER PROGRAMS TARGETING POPULATIONS, AND RECRUIT AND RETAIN LOCAL PROVIDERS NECESSARY TO DELIVER COST EFFECTIVE CARE. FINALLY, WE'RE ALSO BOOSTING OUR EFFORTS TO MORE EFFECTIVELY PROMOTE HEALTH, HEALTHY EATING AND HEALTHY ACTIVITY, AND WE'RE FOSTERING COMMUNITY COLLABORATION TO INCREASE KNOWLEDGE ON HEALTH ISSUES, FROM HEALTHY EATING AND PHYSICAL ACTIVITY TO IMPACTING SUBSTANCE ABUSE AND CREATING OPPORTUNITIES FOR EDUCATION THAT WILL CHANGE THE UNDERLYING CAUSES OF DISEASE.

Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Part I, Line 3c:ALTHOUGH FEDERAL POVERTY GUIDELINES ARE USED FOR CALCULATING DISCOUNTED CARE TO BE PROVIDED UNDER THE ORGANIZATION'S CHARITY CARE POLICY, THESE GUIDELINES ARE NOT USED TO DETERMINE FREE CARE GIVEN. ALTHOUGH FREE CARE IS GENERALLY NOT GIVEN, A 95% DEDUCTION FROM BILLED CHARGES IS GRANTED TO PATIENTS UNDER 100% OF THE FEDERAL POVERTY GUIDELINES.
Part I, Line 7:THE COSTING METHOD USED ON FORM 990 IS BASED ON A COST TO CHARGE RATIO WHICH IS DEVELOPED BASED ON THE Organization's TOTAL OPERATING EXPENSES LESS THE PROVISION FOR BAD DEBTs DIVIDED BY GROSS PATIENT SERVICE REVENUE. THIS COST TO CHARGE RATIO IS APPLIED AGAINST VARIOUS REVENUE AND EXPENSE CATEGORIES TO COMPUTE THE ESTIMATED COMMUNITY BENEFIT EXPENSE UNDER IRS SUGGESTED COSTING METHODS FOR THE FORM 990.
Part I, Line 7g:The cost of subsidized services is based on the cost allocations from the Medicare cost report. Allowable costs are assigned directly to departments throughout the year and then the cost report uses the allocation methodology to assign all other costs to calculate service line costs. Subsidized health services include the operation of the hospital's inpatient medical/surgical nursing unit. These services are unavailable to members of the community other than through Southwest Health Center, Inc. It has been the goal of Southwest Health Center, Inc. to provide these services to the community regardless of the patient's ability to pay.
Part I, Line 7, Column (f):The Bad Debt expense included on Form 990, Part IX, Line 25, Column (A), but subtracted for purposes of calculating the percentage in this column is $ 944,109.
Part II, Community Building Activities:While there is a growing agreement in the United States about what constitutes a non-profit Hospital's "community benefit," these efforts continue to be a work in progress. Southwest Health Center, Inc. provides charity care and other community benefits as defined by the IRS and in addition, the organization believes that it provides a critically important benefit which is not quantified. Southwest Health Center, like most community hospitals, was created and is maintained to provide high quality care locally, without without which area residents would be required to travel great distances at significant personal cost and often in poor weather. Our organization's vision (Creating a Healthier Southwest Wisconsin) propels our organization forward, expanding access to ever higher quality care, more services, more professionals and practitioners, and greater contributions to the community in terms of outreach and education. Beyond inpatient hospitalizations, the organization provides local access to many services including; primary medical care, wide ranging diagnostic services, emergency and urgent care services, infusion services, nursing home services, swing bed services, clinical services, laboratory services, occupational health, rehabilitation services, specialty medicine, sleep medicine, speech pathology, a wide and increasing range of general surgical services as well as other specialty surgical services (ENT, OB/GYN, Urology, Orthopedics, etc), inpatient and outpatient psychiatric services, women's health services, nutrition and diabetes education/counseling, emergency medical and ambulance services optometry and advanced ophthalmology care, exercise physicology and sports medicine, orthopedics, to name some of the major services provided. We are conscientious stewards of health care for our region, and people turn to us in all states of need for health related issues.
Part III, Line 4:Patient and resident accounts receivable and credit policy: In evaluating the collectibility of patient and resident accounts receivable, Southwest Health Center, Inc. analyzes past results and identifies trends for each of its major payor sources of revenue to estimate the appropriate allowance for doubtful accounts and provision for bad debts. Management regularly reviews data about these major payor sources of revenue in evaluating the sufficiency of the allowance for doubtful accounts. Specifically, for receivables associated with services provided to patients or residents who have third-party coverage, Southwest Health Center, Inc. analyzes contractually due amounts and provides an allowance for doubtful accounts and a provision for bad debts for expected uncollectible deductibles and copayments on accounts for which the third-party payor has not yet paid, or for payors and patients or residents who are known to be having financial difficulties that make the realization of amounts due likely. For receivables associated with self-pay patients (which includes both patients without insurance and patients with deductible and copayment balances due for which third-party coverage exists for part of the bill), Southwest HEalth Center, Inc. records a significant provision for bad debts in the period of service on the basis of its past experience, which indicates that many patients are unable or unwilling to pay the portion of their bill for which they are financially responsible. The difference between the standard rates (or the discounted rates if negotiated) and the amounts contractually collected after all reasonable collection efforts have been exhausted is charged off against the allowance for doubtful accounts. The audited financial statements do not include a separate footnote regarding bad debt expense.
Part III, Line 8:The total Medicare revenue shown in schedule H to the Form 990 is based on the IRS 990 instructions and includes only a portion of the gross Medicare Revenue of the Organization, and also does not consider all contractual adjustments for services reimbursed by the Medicare Program. Amounts listed for Medicare revenues do not include physician services for the coverage of the emergency department at the hospital as well as AMBULANCE SERVICES, anesthesia professional services, psychiatric inpatient physician professional services, SURGICAL PROFESSIONAL SERVICES, revenues for any patients covered under Medicare Advantage plan programs, OR physical therapy services provided to nursing home residents. These physician professional services, AMBULANCE services, and nursing home therapy services are reimbursed primarily on fee schedule reimbursements at rates that are often below the costs of caring for patients. Emergency physician, PHYSICAL therapy, and AMBULANCE services provided to Medicare patients are vital to the well-being of the community and as such these costs and shortfalls should also be considered as an additional benefit that Southwest Health Center provides to the community and surrounding areas. The costing method used above for IRS 990 compliance reporting is also based on the filed Medicare Cost report for the year ended June 30, 2017, and does not consider Medicare non-allowable expenses, as it is based on total hospital patient service revenues (ignoring contractual adjustments on fee schedule reimbursed items and non-allowable Medicare expenses as noted above). Additionally, the amounts above do not include amounts paid for inpatient medical/surgical nursing unit services as these are included in the subsidized health services on page 1 of Schedule H. Whether there is a shortfall or surplus on services provided to Medicare beneficiaries, these people, who are typically elderly or disabled members of the community, are an underserved population who experience issues with access to health care services. Without tax-exempt hospitals providing Medicare patient services, the Centers for Medicare and Medicaid (CMS) would bear the burden of directly providing services to the elderly and disabled members of the community.
Part III, Line 9b:Under the organization's collection and charity care policies, Southwest Health Center, Inc. makes every attempt to identify and promote charity care to patients. Included in the Organization's charity care policy it is noted that patients may qualify for charity care either prior to admission or following discharge. All inpatient self-pay admissions are screened by the organization's patient financial counselors or social workers to allow these patients the ability to complete their application during their stay at the hospital. Depending upon the patient's condition, the patient's responsible party may be contacted to complete and return the forms at a later time when their care allows this completion. During the patient account collection process, self-pay patients are also informed of the organization's collection policies as well as the charity care program to allow patients the opportunity to complete the appropriate forms and qualify under the program.
Part VI, Line 2:Southwest Health Center, Inc. assesses the health care needs of the communities and geographic region we serve by reviewing local demographics and trends, by tracking patient service utilization to determine which services could and should be made available to the community in a cost-effective manner to be consistent with Southwest Health Center's mission in the community, and by monitoring state, county and other reports on the health status of our populations, Southwest Health Center also reviews statistics in the hospital and clinics as part of regular reviews of current and potential services to ensure resources are being appropriately allocated to areas of need by community members and patients.
