SCHEDULE H, PART V, SECTION B, LINE 5As part of the community health needs assessment (CHNA), Miami County Medical Center, Inc (MCMCI) conducted a town hall meeting on Thursday, Oct. 22, 2015. The community members invited to this town hall represents the broad interests of the community served by the hospital facility, including individuals with special knowledge and expertise in public health. This includes people representing the following groups: local hospital, public health community, mental health community, free clinics, community-based clinics, service providers, local residents, community leaders, opinion leaders, school leaders, business leaders, local government, faith-based organizations, people with chronic conditions, uninsured community members, low income residents, and minority groups. Section V of the CHNA provides a list of the town hall attendees and notes from the meeting. All priority-setting and scoring processes at the town hall meeting involved the input of key stakeholders in attendance. The meeting included discussion of community health data; reflection on size and seriousness of any health concerns; and discussion of current community health strengths. Participants were then asked to rank the community health concerns cited during the meeting and from the primary and secondary research already completed. A preliminary research survey was conducted by VVV Research LLC on behalf of MCMCI.
SCHEDULE H, PART V, SECTION B, LINE 11MCMCI developed a Community Health Improvement Plan in early 2016 to address the top health-need priorities that were identified through the assessment described above. The MCMCI Board of Trustees approved it in November 2016. This is a three-year plan, which will begin implementation in 2017. It speaks to each health need priority, outlining MCMCIs initiative(s) to address the need and the anticipated community impact. You can read the full Community Health Needs Assessment and Community Health Improvement Plan documents on our website at olathehealth.org/community.
SCHEDULE H, PART V, SECTION B, LINE 13bWhen annual household income is more than 300% of the federal poverty guidelines, individuals with extraordinary medical expenses may be eligible, on case by case basis, for financial assistance discount or extended payment arrangements.
SCHEDULE H, PART V, SECTION B, LINES 16A-CWWW.OLATHEHEALTH.ORG/PATIENTS-AND-VISITORS/FINANCIAL ASSISTANCE
SCHEDULE H, PART V, SECTION B, LINE 18Miami County Medical Center does not permit any of the actions listed without making a reasonable effort to determine the patients eligibility under the facilitys financial assistance program.
SCHEDULE H, PART V, SECTION B, LINE 22DThe hospital utilized the look back method for all insurance companies to determine the maximum about that could be charged.

SCHEDULE H, PART I, LINE 3CSEE PART VI, SECTION B, LINE 13 FOR THE LIST OF FACTORS USED IN THE ELIGIBILITY CRITERIA EXPLAINED IN THE FAP FOR PROVIDING FREE AND DISCOUNT CARE.
SCHEDULE H, PART I, LINE 7, COLUMN FThe bad debt expense is not considered a community benefit expense and is not included in Schedule H, Part I, Line 7. The amount excluded is $2,426,751.
SCHEDULE H, PART I, LINE 7The costing methodology used to calculate the amounts contained in the table of Part I, Line 7, of Schedule H, is a cost accounting system.
SCHEDULE H, PART I, LINE 7GNO PHYSICIAN CLINIC COSTS ARE INCLUDED IN THE COMMUNITY BENEFIT EXPENSES.
SCHEDULE H, PART IIMiami County Medical Center, Inc. (MCMCI) is committed to improving the community we serve. Our employees are involved in a variety of activities in our community that support the general economic improvement of our community; the health, well being and safety of our areas residents; as well as supporting programs and other organizations that reach out to our citizens in need. A few examples of MCMCIs involvement in 2016 include: 1) Involvement in the Miami County Economic Development Advisory Board and local civic organizations. 2) Our support of the Health Partnership clinic, serving the uninsured of our community. 3) Our support of Lakemary center, a nonprofit serving children with developmental and intellectual disabilities. 4) Participation in community organizations and events through donations and by providing health education and services, including the Miami County Cancer Foundation. 5) Leadership and participation on our areas emergency preparedness efforts by coordinating extensive drills to practice our response to potential situations. 6) Participation in the Paola Pathways organization, a group responsible for developing walking and biking trails in and around Paola, and the Mound City trail project.
SCHEDULE H, PART III, SECTION A, LINE 2The costing methodology used to calculate the amounts contained in the table of Part III, Line 2, of Schedule H, is a cost accounting system.
Schedule H, Part III, SECTION A, Line 3The amount included on Line 3 that could be attributed to patients eligible under the organizations financial assistance policy was estimated by analyzing all private pay patient charges at cost less the cost for those patients that were eligible for financial assistance. This amount was then multiplied by the percentage of what has been historically collected from uninsured patients. The majority of the uninsured patients do not have the ability to pay and if the patients completed the financial assistance application they would be eligible for financial assistance.
SCHEDULE H, PART III, SECTION A, LINE 4See page 9 of the attached audited financial statements.
SCHEDULE H, PART III, SECTION B, LINE 8Medicare allowable costs were calculated using a cost accounting system. Shortfalls arise from payments that are less than the costs to provide the services. We accept all Medicare patients knowing that the cost of providing the care may exceed the payment we receive from the Medicare program. The shortfalls should be considered community benefit as Medicare shortfalls must be absorbed by the hospital in order to continue treating the elderly in our community. It is also implied in Internal Revenue Service ruling 69-545 established the community benefit standard for tax-exempt hospitals and indicates that participation in publically-financed programs, such as Medicare, is evidence that a hospital meets the community benefit standard.
SCHEDULE H, PART III, SECTION B, LINE 9BBefore placement with collection agencies, MCMCI Patient Financial Services offers various options for patients with financial difficulties. In addition to offering our own financial assistance to qualifying individuals, MCMCI contracts with an agency that screens individuals for eligibility for programs like Medicaid, Disability and Crime Victims Compensation and then helps them through the application process. The use of propensity to pay software also helps MCMCI identify individuals that may need assistance that have not completed an application. MCMCI has language written in their collection agency contract that states, "If the collection agency learns a patient/debtor has neither sufficient income nor assets to meet his financial obligation, the agency will not pursue collection activities against the patient/debtor, unless or until the agency learns that the patient/debtor has income or assets to meet the obligation." If the collection agency is notified by the patient/debtor that this created an undue financial hardship or burden due to a change in their income or assets, the agency will notify MCMCI and await further direction from the hospital.
Schedule H, Part VI, Line 2Miami County Medical Center, Inc. (MCMCI) is continually assessing the health needs of the communities it serves in a variety of ways. Informally, MCMCI is involved in multiple health fairs and other community outreach activities that involve screenings, questionnaires and general conversation that help us understand the needs of groups and individuals. Also, the MCMCI Community Advisory Council meets on a regular basis with representatives from a diverse group of individuals who provide helpful input from the organizations and communities they represent. In 2015, MCMCI continued its formal process of assessment. In partnership with the Miami County Department of Health, and with the help of VVV Research and Development, MCMCI conducted a community health needs assessment for its primary service area (Miami County). This was done by performing research, collecting health data for the area and actively seeking input from the community through a survey and town hall meetings. The board approved the CHNA in November 2015. The research, survey and town hall meetings helped develop a clearer picture of our service areas and the health priorities of residents. Below are the top three health need priorities in the primary service areas of MCMCI. Health Need Priorities 2014-2016- MCMCI 1. Increase access to care by making available Urgent Care and Walk-In services for residents of Miami and Linn counties. 2. Increase access to affordable pharmaceuticals. 3. Decrease obesity rate among residents and increase access to healthy food. In the fall of 2013, MCMCI developed a Community Health Improvement Plan to address the top health-need priorities that were identified through the assessment described above. It was approved by the MCMCI Board of Trustees in October 2013. This three-year plan began implementation in 2014. It speaks to each health need priority, outlining MCMCIs initiative(s) to address the need and the anticipated community impact. You can read the full Community Health Needs Assessment and Community Health Improvement Plan documents on our website at olathehealth.org/community. In 2014, MCMCI began implementation of its Community Health Improvement Plan. The main priority for the MCMCI community was access to walk-in and/or urgent care. MCMCI added walk-in hours through Associates in Family Care Paola, and also added space within the clinic dedicated to providing this care. The goal for 2015 was to increase the number of patients seen in the walk-in clinic. That goal was not met and the clinic saw a decrease in the number of patients. While this service was opened to address this need, there are still challenges the organization continues to face and improve, such as provider availability and hours of service. In 2016, the walk-in clinic saw a six percent increase in number of patients utilizing this service. Despite not meeting the goal for increasing number of patients utilizing this service by 20 percent, the increase over previous was progress toward the goal. There are still many efforts within the community to educate the public about this service and when it is appropriate to go to walk-in versus emergency care. Access to affordable pharmaceuticals was another area of focus identified by the community with patients. In 2014, the inpatient pharmacy at MCMCI began working with local pharmacies to enhance the quality of communication. MCMCI also continued to support the Health Partnership Clinics 340B drug pricing program, which gives patients access to the medications they need through reduced pricing from the drug manufacturers. In 2015, MCMCI began a prescription medication assistance program for patients to access upon discharge. In 2016, MCMCI continued to work with the Health Partnership Clinic for patient referrals for testing and work with MCMCIs financial policy for assistance. To address obesity in 2015, MCMCI distributed information related to the 1-2-3-4-5 FIT-astic program within the Jayhawk-Linn school district. The Fit-astic Program teaches healthy habits such as limited screen time and appropriate servings of fruits and vegetables. The goal for 2015 was to provide information to 75 percent of the elementary schools in the MCMCI service area with Fitastic program information. While MCMCI did not meet this goal, it continues to partner with school districts in its primary service area to not only distribute materials, but help educators incorporate these principals into their curriculum. MCMCI representatives also talked with the Louisburg and Osawatomie school district representatives to implement the 1-2-3-4-5-Fit-Tastic program within those districts. The programs were not launched in 2016, but there are ongoing discussions about implementing them in the future and incorporating them into the physical education curriculum. MCMCI also utilized its primary care clinics to integrate goals from this program into patient electronic medical records. Due to limitations within the electronic medical records, the goals for this initiative were not trackable in the system. Additional efforts to combat obesity within the MCMCI service area include supporting the walking trail programs in Paola and Mound City, financial support to the Ozone Fitness Center to provide scholarships for people who cannot afford memberships and hosting a bicycle safety rodeo to distribute free bicycle helmets and teach safe riding habits. In 2016, MCMCI updated its goal to increase the number of pediatric patients who receive nutritional counseling to 35 percent. By the end of 2016, 66 percent of pediatric patients received this counseling. In 2017, MCMCI will launch its second Community Health Improvement Plan that will carry on with some of these initiatives. Health Need Priorities for 2017- MCMCI 1. Increase access to behavioral health, substance abuse and crisis intervention resources. Expand areas of prevention and education. 2. Improve and expand access to primary care services. 3. Decrease tobacco use. 4. Provide affordable healthy eating options and nutritional education to decrease obesity rates in Miami and Linn counties.
