FORM 990, PART I, LINE 1Peak Vista served over 90,000 people living in the Pikes Peak and East Central Plains regions of Colorado in 2016. Our niche in the these regions health care marketplace is as a non-profit multi-specialty provider of integrated medical, dental, and behavioral health primary care services, mostly for working families. In this role, Peak Vista has endeavored to serve populations that are under or uninsured as well as those on public programs. Partnerships are a significant component of Peak Vista's ability to successfully meet its mission. We frequently collaborate with organizations of similar mission to better serve our mutual populations.
FORM 990, PART III, LINE 1Peak Vista offers numerous ancillary services and programs to benefit our patients to include: First Visitor which provides information to parents before and after their baby is born with trained volunteers making home visits designed to connect parents with community resources and early childhood materials. Health education supports patients with management of chronic diseases such as diabetes, high blood pressure, obesity, and asthma. A full service on-site laboratory and pharmacy provide easy access to testing and medications. Well Child Waiting Areas welcome little ones into a secure and child-friendly environment so that patients can attend appointments worry free.
FORM 990, PART III, LINE 22016 Peak Vista Community Health Centers Accomplishments include: Patient Growth Peak Vista served the largest number of patients in our history in 2016 serving over 90,000 patients or an average of 1,300 patient visits per day (in 2006 the average was 600). Continuing Access to Care - In late 2016, Dr. Charles Johnson, a well-known and established physician in the Colorado Springs area announced his plans for retirement. Peak Vista was honored to adopt his medical practice. As Peak Vistas 27th health center, the organization assumed care for nearly 10,000 additional patients. Peak Vista set a new goal in 2016: to answer all incoming calls to the customer service center within three minutes. In order to execute this substantial request, senior leadership and front line staff worked tirelessly to investigate and analyze how to best meet this goal. Peak Vista ensured patient issues were resolved by implementing several operational initiatives to include: Nurse Triage, third party call overflow, extended hours to make an appointment, altering the flow of interpretations, expanding a clinical on-call Registered Nurse and enhanced staff education. The total number of work calls increased from 1,261,779 in 2015 to 1,263,638 in 2016. These process improvements increased Peak Vistas call centers grade of service to an astounding 99% answer rate within three minutes. Many Peak Vista outreach staff are located inside Memorial Hospital and Penrose-St. Francis Health Services urgent care centers and Emergency Departments (ED). These staff members gather referrals for follow-up care, assist patients in navigating the health care system and connecting patients with a Primary Care Provider (PCP). In 2016, Peak Vista expanded the outreach program to the Childrens Urgent Care Center. Since doing so, there has been a significant increase in the appreciation of the emergency department physicians, hospital staff and most importantly, patients. Patients are now able to receive follow-up care and connect to their PCP. In 2017, Peak Vista staff will eagerly work with all local hospitals to continue reducing ED visits for primary care. Dental Program Services - Peak Vistas nine Dental Health Centers work daily to improve the oral health of our patients. The dental teams work with patients to create individual treatment plans to meet their needs, often at a reduced fee. Behavioral Health Services In 2016, through funding from a Federal grant for expanded services, Peak Vista added six behavioral health providers to its staff. As a result, Peak Vista was able to increase behavioral health services from 14,703 visits in 2015 to 29,987 visits in 2016. Team-based Care in Health Center at Union - With support from multiple grantors, Peak Vista piloted a fully integrated, team-based care model in the Health Center at Union. By design, the model aims to reduce patient office visits while also elevating the patients overall health. Patient needs are addressed in care teams consisting of the patient, a primary care provider, a behavioral health provider, a clinic-based nurse/care manager, a medical assistant, a resource navigator, a pharmacist, a health educator, a dental hygienist, receptionists and customer service representatives. This pilot affords Peak Vista the ability to test various care team models, processes and space requirements to plan for the future health care needs. Analysis of testing results provides valuable insight into costs, outcomes, patient satisfaction and staff satisfaction. Peak Vistas Education Health Initiative (EHI) encompassing Peak Vistas Advanced Practice Registered Nurse fellowship, the Family Medicine Residency program and medical student coordination to focus on developing and training new health care professionals. EHI is an effort to build a local solution to the national health care workforce shortage, with the ultimate goal of improving access to quality care in our community. Advanced Practice Registered Nurse Fellowship In the fall of 2016 Peak Vista saw the commencement of Peak Vistas second class of Advanced Practice Registered Nurse (APRN) Primary Care fellowship, a program designed to seamlessly transition recent nurse practitioner graduates from the academic sphere to a clinical setting. The one-year fellowship combines didactic and clinical learning in a team-oriented, patient-centered clinical environment. The inaugural class of APRN fellows consisted of five nurse practitioners graduated in 2016. Our second classes of fellows are scheduled to graduate in fall 2017. Family Medicine Residency Program - Peak Vistas Family Medicine Residency (FMR) is the only primary care residency of its kind in Peak Vistas service area. The three-year program welcomed eight osteopathic doctors in July 2016. The curriculum, program offerings and emphasis of partnership with community agencies and leaders evoked an overwhelmingly positive response. It was also met with strong community support from local hospitals, health care providers and specialty physicians. Residents will learn through patient interaction, bedside teaching and structured educational sessions. Peak Vista received over 230 applications and interviewed 54 candidates for the inaugural class.
