Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - Riverside Medical Center. Riverside Medical Center partners with the Kankakee County Health Department, Presence St. Mary's Hospital, The United Way, and representatives from multiple other service lines within the community to provide leadership, staffing, and funding in the Partnership for a Healthy Community. For this most recent Community Health Needs Assessment (CHNA), this group began its work in 2014 and continued through 2015 to develop a refreshed plan for 2016-2018. The Partnership for a Healthy Community contracted with the Illinois Public Health Institute to facilitate the assessment and development of action plans. The Partnership utilized the standardized process termed 'Mobilizing for Action through Planning and Partnerships' (MAPP) to gather data and develop a strategic plan. This planning process utilized four formal assessments from which to identify the top strategic priorities for the CHNA. Those four processes are: Community Health Status Assessment (CHSA): Forces of Change Assessment (FOCA): Local Public Health System Assessment (LPHSA): Community Themes & Strengths Assessment (CTSA): These four assessments were used to develop the Kankakee County Data Sharing/Strategic Priorities: Riverside Leadership and Participation More than ten representatives from Riverside participated in the assessment and planning process including Matt McBurnie who serves on the Steering Committee to guide the partnership. (Dr. John Jurica and Maggie Frogge served initially on the Steering Committee, but as they transitioned away from Riverside in late 2014 and early 2015 respectively, Matt McBurnie took their place through the remainder of the assessment process).

Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - Riverside Medical Center. Six Prioritized Strategic Areas The MAPP process allowed the community partners to identify six areas of issues, listed in alphabetical order. Since the community has identified these as the most pressing to improve its residents' health, the remainder of this document will articulate Riverside's implementation plans to address them. I. Access to Care II. Chronic Disease III. Education and Employment IV. Mental Health and Substance Abuse V. Teen Pregnancy, Infant Mortality, and STDs VI. Violence and Safety I. Access to Care: * Epic system installed: Riverside successfully installed and implemented Epic systems January 1, 2016. One key feature of Epic is MyChart, which allows patients web portal access to their health information. At the conclusion of 2016, Riverside MyChart users exceeded 8,400 and continues to grow steadily. * Riverside Immediate Care Sites: Riverside continues to operate these services in Kankakee (East Court St), Bourbonnais Wal-Mart, Dwight, Monee, Manhattan (new in 2016), Coal City, and Gilman. * Emergency Department Enhancements: Riverside added the CareNet radio system to aid in better communication. * Ambulance Services and Partnerships: Riverside added Cullom, Kempton and Cabery to its ambulance network. * Riverside accreditations: Riverside was successfully re-designated a Magnet Hospital in 2016 and maintained all other listed accreditations. Hospitalists: Riverside added a number of Hospitalist groups to maximize quality and efficiency of care for our patients. Those groups are: * University of Chicago Pediatricians: Beginning in August, Riverside Medical Center began 24/7 coverage with onsite U of C Pediatricians for children in the inpatient care setting, for births that have elevated risk, and for Emergency Department consults as needed. * University of Chicago Heart Surgeons: Beginning December 1, U of C began providing onsite heart surgeons to maintain heart surgery continuity and ever improve quality as the previously employed surgeons concluded their work at Riverside. * AIM Hospitalists: Beginning October 1, Riverside changed its inpatient hospitalist group to AIM * Intensivist program: Riverside now utilizes an inpatient hospitalist program for patients in the ICU which has dramatically reduced the time a patient is in the ICU care setting. * Neuro Hospitalists: Riverside now employs two neurohospitalists to deliver efficient and high quality care in the inpatient setting for patients with neurological conditions. In addition to better care for those in the inpatient setting, this allows for Riverside Neurologists in the outpatient setting to more quickly see patients in their clinic. * Physician Recruitment: In total, Riverside Medical Group recruited and hired 20 new providers in primary and specialty care to increase ease and accessibility for healthcare in our community bringing the total group to 131. * Language Solutions System: In accordance with the Affordable Care Act, in 2016 Riverside adopted the language accessibility statement and deployed the associated resources to enable ease and accessibility to health care by all. * Healthcare Education and Training: At Riverside 4,070 area students received training in the past calendar year at Riverside-a 26% growth over prior year. * YWCA Kankakee Clinic launch: In March 2016, Riverside Healthcare opened a pediatric specialty clinic inside the YWCA Kankakee. Schedule H, Part I, Line 7 Bad Debt ExpenseBad Debt expense was not booked as an expense in Part IX Statement of Functional Expenses so this amount is properly reported at zero. The bad debt was netted with revenues on the Part VIII Statement of Revenue.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountBad debt expense is determined using the cost to charge ratio. Payments on patient accounts is a reduction of bad debt expense.
Schedule H, Part III, Line 3 Bad Debt Expense MethodologyNone of the amount the organization reported as bad debt was attributable to patients eligible under the organization's financial assistance policy.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteThe medical center analyzes its past history and identifies trends for each of its major payor sources of revenue to estimate the appropriate allowance for Uncollectible accounts and provision for bad debts. Management regularly reviews data about these major sources of revenue in evaluating the sufficiency of the allowance for doubtful accounts. For receivables Associated with services provided to patients who have third-party coverage, the medical center analyzes contractually due amounts and provides an Allowance for doubtful accounts and a provision for bad debts, if necessary (for example, for expected uncollectible deductibles and copayments on Accounts for which the third-party payor has not yet paid, or for payors who are known to be having financial difficulties that make the realization of amounts due unlikely). For receivables associated with self-pay patients (which includes both patients without insurance and patients with deductible and copayment balances due for which third-party coverage exists for part of the bill), the medical center records a significant provision for bad debts in the period of service on the basis of its past experience, which indicates that many Patients are unable or unwilling to pay the portion of their bill for which they are financially responsible. The difference between the standard rates (or the discounted rates if negotiated) and the amounts actually collected after all reasonable collection efforts have been exhausted is charged off against the Allowance for doubtful accounts. For uninsured patients that do not qualify for charity care, the medical center recognizes revenue on the basis of its Standard rates for services provided (or on the basis of discounted rates, if negotiated or provided by policy).
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsTHE COSTING METHODOLOGY USED IS COST TO CHARGE RATIO. THE COST TO CHARGE RATIO IS FIGURED BY TAKING THE TOTAL EXPENSES EXCLUSIVE OF BAD DEBT AND DIVIDING THAT BY GROSS PATIENT REVENUE AND OTHER OPERATING REVENUE. ANY SHORTFALLS THAT RESULT FROM MEDICARE ALLOWABLE COSTS EXCEEDING MEDICARE REVENUE CAN BE CONSIDERED A COMMUNITY BENEFIT IF THE ACTIVITY IS SOMETHING THAT the MEDICAL CENTER IS DOING TO CREATE THE AWARENESS AND WELL-BEING OF THE HOSPITAL COMMUNITY.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistanceWhen a patient has been approved under the hospital's Charity Care Policy for a partial discount, the hospital will work with the patient or responsible party to establish a reasonable payment plan. The plan will take into account available income and assets, the amount of the discounted bill(s), and any prior payments. The hospital will also advise the patient of his or her responsibilities under these collection practices guidelines. Riverside may offer prompt payment discounts to charity care patients who are eligible for partial discounts on their hospital bills. The hospital will not pursue legal action for non-payment of bills against charity care patients who have clearly demonstrated that they have neither sufficient income nor assets to meet their financial obligations. Legal action, including the garnishment of wages, may be taken by the hospital to enforce the terms of the payment plan. Such measures would only take place when there is evidence that the charity care patient or responsible party has sufficient income and/or assets to meet his or her obligation. The hospital will not place a lien on a charity care patients primary residence. The hospital will not execute a lien by forcing the sale or foreclosure of a charity care patients primary residence to pay for an outstanding medical bill. The hospital will not use body attachment to require the charity care patient or responsible party to appear in court, however Riverside recognizes that the court system may take this action independently. The hospital will ensure that the guidelines outlined above are followed by any external collection agency engaged to assist in obtaining payment on outstanding bills from charity care patients. Additionally, external collection agencies will identify patients that may qualify for charity care and will assist in communicating Riverside's financial assistance policy and mail applications to patients requesting assistance.

Schedule H, Part VI, Line 2 Needs assessmentRiverside Medical Center conducted an Analysis of Community Health Needs in 2015 and developed a multi-year prospective Community Health Plan. Riverside Medical Center collaborated on this endeavor with the Kankakee County Health Department and Provena St. Mary's Hospital. The research for the study was conducted by Health Systems Research of the University of Illinois College of Medicine at Rockford, which specializes in community needs assessments for health and human services organizations. Key community health concerns were identified through an analysis using formal data, assessments, health surveys, periodic physician surveys and trend and treatment analysis. The assessment led to the identification and prioritization of key community health issues. Several key community leaders and health and human service providers were assembled to develop an actionable plan to address the identified priorities and implement community based initiatives to address the challenges. The key issues identified through the Assessment of Community Health Needs included: (a) access to health care, (b) mental health needs, and (c) reducing risk factors for chronic disease-obesity.
Schedule H, Part VI, Line 3 Patient education of eligibility for assistanceRiverside uses various tools to communicate the availability of charity care to all patients. Examples include, but are not limited to: Signage, Brochures, Placing a note on the hospital bill and statements regarding how to request information about financial assistance. This includes statements from our bad debt agency. Designated individuals who can explain the hospital's charity care policy, including a Spanish-speaking representative. Reference in our patient handbook Notice on in-house television Lobby Kiosks Website Employee Charity Lesson Riverside also screens identified patients for eligibility for various federal, state or local government programs. We use both internal and external staff to assist in the identification of patients and to assist with the education of our patients.

Four key indicators of well-being are assessed in each of Illinois' 102 counties: high school graduation rates, unemployment rates, teen birth rates, and poverty rates. Kankakee's Poverty Warning List placement elevates he county from its previous Watch status to a level requiring corrective action initiatives.
