The Presentation Sisters joined the Catholic Health Corporation of Omaha in 1980 and later became part of Catholic Health Initiatives. In 2014 and 2015, CHI St. Alexius Health Carrington was recognized as a "Health Strong Hospital" by being one of the top 100 Critical Access Hospitals in the United States. Additionally, CHI St. Alexius Health Carrington was named one of the top 20 Critical Access Hospitals in 2014 by the National Rural Health Association. CHI St. Alexius Health officially announced the formation of its regional health care system on April 19, 2016. The system is the largest health care delivery system in central and western North Dakota and is comprised of a tertiary hospital in Bismarck, and critical access hospitals (CAHs) in Carrington, Dickinson, Devils Lake, Garrison, Turtle Lake, Washburn and Williston and numerous clinics and outpatient services. CHI St. Alexius Health manages four CAHs in North Dakota: Ashley, Elgin, Linton, and Wishek, as well as Mobridge Regional Medical Center in Mobridge, S.D. As a tax-exempt 501(c)(3) facility, CHI St. Alexius Health Carrington is committed to serving the needs of community members without regard to one's ability to pay. However, CHI St. Alexius Health Carrington will not deny care if the individual and/or the individual's family becomes unable to pay the facility charges. CHI St. Alexius Health Carrington provides a wide array of health services for our area including a Critical Access Hospital, Emergency Care, Level 5 Trauma Center, ambulance services, inpatient care (acute care, swing bed, respite care), pharmacy, respiratory therapy, surgical services (general, endoscopy - colonoscopies and gastroscopies, ophthalmology, orthopedics, vein ablation), outpatient services including cardiac rehab, pulmonary rehab, stress testing, cardiac support group, diabetic education, laboratory, medical nutrition therapy, weight management support group, IV therapy, physical therapy, occupational therapy, speech therapy, sleep disorder services, telemedicine, back and joint injections, imaging services (CT and DEXA scans, echocardiograms, EKG, fluoroscopy, general x-ray, digital mammography, MRI, cardiac stress testing with nuclear medicine, ultrasound), volunteer auxiliary services, social ministries, and other services available by referral (mental health, hospice, and home health). CHI St. Alexius Health Carrington also provides a Medical Clinic (family practice, elder care, health maintenance exams, DOT exams, nursing home rounds, pediatric and well child exams, phone nurse, prenatal obstetrics, preoperative exams, and women's health) and an outreach clinic in Eddy County. Two physicians, a general surgeon, three physician assistants and many outreach specialists provide needed local services for which our patients would otherwise have the inconvenience of traveling long distances to receive. The programs and services described throughout this report promote a healthy community and each year those services seem to expand in some form. CHI St. Alexius Health Carrington has a governing Regional & Executive Board of Directors as well as a Quality Committee-Patient Family Advisory Council and a Foundation Advisory Council. CHI St. Alexius Health Carrington is also committed to the promotion of healthier communities. CHI St. Alexius Health, Carrington is proud of its Catholic heritage and its ability to provide direct and indirect support to the communities it serves. CHI St. Alexius Health Carrington is included in the Official Catholic Directory as a tax-exempt hospital.
Schedule H, Part VI LINES 2, 4, & 5 AND PART II: NEEDS ASSESSMENT AND COMMUNITY INFORMATIONII. COMMUNITY BENEFIT APPROACH CHI St. Alexius Health Carrington Medical Center is located in a frontier area and is licensed as a critical access hospital with two provider-based clinics. One clinic is attached to the Carrington hospital and the other is located 16 miles to the north in New Rockford. Carrington is located in east central North Dakota, just two hours from four major cities in North Dakota: Fargo, Minot, Grand Forks, and Bismarck. Counties served by CHI St. Alexius Health Carrington include Foster County and Eddy County in their entirety, plus portions of Stutsman and Wells Counties. Other hospitals are located in both Stutsman and Wells counties. This service area is defined based on the location of the medical facilities, the geographic distance to other hospitals, and the history of usage by consumers. Located in the hospital's service area are the communities of Bowdon, Carrington, Cathay, Fessenden, Glenfield, Grace City, McHenry, New Rockford, Pingree, Sykeston, and Woodworth. While the population of North Dakota has grown in recent years, the populations of the four counties studied were stable with modest changes between 2010 and 2014. The data show that the area is rural and that its residents are older than the state as a whole and are less likely to have completed a four-year degree, which can have workforce implications. Eddy and Stutsman counties had higher levels of children in poverty than North Dakota overall, while Foster and Wells counties experienced lower rates of children in poverty. ND Census estimates the population for 2016 in Eddy and Foster Counties are 5,739 collectively and have a greater percentage of individuals over the age of 65 than the ND average. CHI St. Alexius Health, Carrington considers the service area of the facility to be roughly 8,000 people. The four counties also have a higher median age than the state median age. The percentage of residents aged 65 and older in Eddy, Foster, and Wells counties is substantially higher than the state average. This likely signifies an increased need for medical care in most of the service area due to an aging population. All four counties have rates below the state average in terms of individuals with a high school diploma. With respect to those aged 25 or older with a bachelor's degree or higher, Eddy, Foster, and Wells counties' rates are substantially lower than the state average. Eddy and Stutsman counties have rates of persons living below the poverty line that exceed the state average. Foster and Wells County's rate is lower than the state average. Much of CHI St. Alexius Health, Carrington's service area is very rural. Eddy, Foster, and Wells counties had 3.8, 5.3, and 3.3 people per square mile, respectively. Stutsman County is closer to the state average with 9.5. The generally rural area has implications for the delivery of services and residents' access to care. Transportation can be an issue for rural residents and others as can isolation, which can have many effects on health status. Health Professional Shortage Area (HPSA) designations are used to identify areas and population groups within the United States that are experiencing a shortage of health professionals. There are three categories of HPSA designation based on the health discipline that is experiencing a shortage: 1) primary medical; 2) dental; and 3) mental health. For each discipline category, there are three types of HPSA designations based on the area or population group that is experiencing the shortage: 1) geographic area; 2) population group; and 3) facility. A geographic HPSA indicates that the entire area may experience barriers in accessing care, while a population HPSA indicates that a particular subpopulation of an area (e.g., homeless or low-income) may be underserved. Finally, a facility HPSA is granted to a unique facility that primarily cares for an underserved population. The primary factor used to determine a HPSA designation is the number of health professionals relative to the population with consideration of high need. According to HRSA (Health Resources and Service Administration), CHI St. Alexius Health Carrington is in a Primary Care Health Professional Shortage Area (HPSA); Mental Health Professional Shortage Area and in a Medically Underserved Area/Population. Foster County and surrounding service areas are also designated as frontier in the North Dakota Frontier Counties (less than 6 persons per square mile), based on the 2010 Census. Frontier areas face challenges in providing access to health and human services even greater than the challenges faced by less isolated rural communities. Access to an adequate supply of health care providers is a concern in both North Dakota and nationally. Process Prairie Health Partners, a Grand Forks-based consulting firm, working closely with the CRH, provided substantial support to CHI St. Alexius Health, Carrington and Foster County Public Health in conducting this needs assessment. Professionals from Prairie Health Partners have conducted dozens of comprehensive community health needs assessments and community development activities in a wide variety of communities, including many rural communities. The CRH is one of the nation's most experienced organizations committed to providing leadership in rural health. Its mission is to connect resources and knowledge to strengthen the health of people in rural communities. As the federally designated State Office of Rural Health (SORH) for the state and the home to the North Dakota Medicare Rural Hospital Flexibility (Flex) program, the Center connects the School of Medicine and Health Sciences and the university to rural communities and their health institutions to facilitate developing and maintaining rural health delivery systems. In this capacity the Center works both at a national level and at state and community levels. The assessment process was collaborative. Professionals from both CHI St. Alexius Health Carrington and Foster County Public Health were heavily involved in planning and implementing the process. They met regularly by telephone conference and via email with representatives from Prairie Health Partners. The process closely followed a model used during the last community health needs assessment cycle. CHI St. Alexius Health Carrington did not receive any written comments from the public on the previous community health needs assessment or its most recent implementation strategy. In response to the previous assessment findings, CHI St. Alexius Health Carrington implemented a number of programs and initiatives, as detailed in Appendix D. Periodic updates to the implementation strategy included in Appendix D have been highlighted in blue. As part of the assessment's overall collaborative process, Prairie Health Partners spearheaded efforts to collect data for the assessment in a variety of ways: * A survey solicited feedback from area residents; * Community leaders representing the broad interests of the community took part in one-on-one key informant interviews; * The Community Group, comprised of community leaders and area residents, was convened to discuss area health needs and inform the assessment process; and * A wide range of secondary sources of data was examined, providing information on a multitude of measures including demographics; health conditions, indicators, and outcomes; rates of preventive measures; rates of disease; and at-risk behaviors. Summary of Assessment Findings The Community Group met on May 9, 2016. Sixteen community members of the group attended the meeting. A representative from Prairie Health Partners presented the group with a summary of this report's findings, including background and explanation about the secondary data, highlights from the survey results (including perceived community assets and concerns, and barriers to care), and findings from the focus group and key informant interviews. Following the presentation of the assessment findings, and after consideration of and discussion about the findings, all members of the group were asked to identify what they perceived as the top four community health needs. The results were totaled, and the concerns most often cited were: * Obesity/overweight * Adequate childcare services * Youth alcohol use and abuse * Adult cyber bullying * Adult alcohol use and abuse * Lack of mental health providers
Schedule H, Part VI LINES 2, 4, & 5 AND PART II: NEEDS ASSESSMENT AND COMMUNITY INFORMATIONIII. QUALITATIVE DESCRIPTION OF COMMUNITY BENEFIT An important part of CHI St. Alexius Health Carrington's mission is building healthier communities through programs designed to serve the needs of people who are poor and disadvantaged in our community. This type of community benefit has the added, positive side-effect of delivering an improved quality of life for everyone. Community benefit often takes place outside the walls of our facilities and involves partner organizations in the community. CHI St. Alexius Health Carrington's community benefit activities are designed to: * Identify and increase awareness of health needs in Carrington * Provide resources to meet those needs * Create collaboration between community organizations that focus on health and well being * Tackle problems such as poverty, homelessness, or behaviors (such as domestic violence or underage drinking) that can contribute to poor health Community Outreach for the Poor CHI St. Alexius Health Carrington offers financial assistance to patients who cannot afford to pay for health care. A Patient Financial Counselor is available to help with the financial assistance application and to apply for Medicaid, Medicaid Expansion Programs, and other programs. The goal is to award financial assistance rather than turn patients over to collection for healthcare claims. Eligibility is based on household income as well as other assets. In addition, CHI St. Alexius Health Carrington's Patient Financial Counselor conducts insurance authorizations and gives estimated out of pocket costs for procedures. CHI St. Alexius Health Carrington has a Special Needs Fund that provides assistance for health related medical supplies for any community member. Requests for special needs assistance can be requested in person or anonymously in writing. Carrington's Daily Bread, the local food pantry, is supported by CHI St. Alexius Health Carrington through food and supply donations as well as employee volunteerism. Carrington's Daily Bread assists people living in poverty with nutritional needs as well as children in need through a backpack program in partnership with the public school. CHI St. Alexius Health Carrington's Employees donate Christmas gifts for Children ages birth to 18 years of age through the Angel Tree project. The program is facilitated in part by the Mission and Spiritual Care Department at CHI St. Alexius Health Carrington. Foster County Public Health distributes these gifts to children living in poverty or those in need throughout Foster County. In FY14 the CHI Fargo Division Philippines Outreach Program received a three-year grant from the CHI Mission & Ministry Fund to cover the cost of sending shipping containers full of medical equipment and supplies to the Indigenous People's Hospital, CHI St. Alexius Health Carrington's sister hospital, in the Philippines. In addition to this, CHI St. Alexius Health Carrington fundraises $2000 each year to the Mission and Ministry fund to build the fund and benefit this cause. Three to four times per year, CHI St. Alexius Health Carrington donates supplies to the Healthcare Equipment Recycling Organization (HERO) in Fargo, ND. HERO is a non-profit organization that collects and redistributes donated healthcare supplies to help those in need locally as well as globally. Community Outreach for the Broader Community The majority of CHI St. Alexius Health Carrington's community benefit efforts are directed toward the broader community. A more detailed description of these programs will be included in the subsequent sections. Community Health Improvement Services Community Health Education is offered by CHI St. Alexius Health Carrington at no charge to the broader community. Educational opportunities encompass a wide span of patient types including presentations, newsletters, programs, career fairs, and activities provided to groups without providing clinical or diagnostic services. A few examples of what CHI St. Alexius Health Carrington provided were bereavement/grief material, FIA newsletters, community education classes, emergency preparedness coordination planning, advance directives education, violence prevention programming, educational demonstrations at the Foster County Fair, and a weekly Health Talk segment on the local radio station, to benefit the local community. The CHI St. Alexius Health Carrington Healthy Communities Coalition was re-formed in June of 2008. The Healthy Communities Coalition collaborates with several community organizations to advocate and improve community health. Several individual community members sit on the coalition as well as larger community groups such as: Foster County Public Health, Foster County Social Services, Carrington Public Schools, Veterans Services, the Chamber of Commerce, and local churches. The coalition has grown to be a visible influence in the community providing many educational opportunities and services to the community. The Healthy Communities Coalition offers community education classes for adults ranging from genealogy to wood carving to painting. The coalition has become involved with the local school system in several capacities. Programming in the school system includes: Sources of Strength, Red Ribbon Week, the Backpack Program, Safe Dates programming, the Youth Track Meet, Imagination Library, and the Kid Experience. CHI St. Alexius Health Carrington partners with the Center for Rural Health annually to host R-COOL Scrubs Camp for local eight grade students. Students spend the day doing hands on activities and learning about health care careers. Exposing young children to health care professions and encouraging older youth to explore health careers is fundamental to successfully 'growing our own' rural healthcare workforce. The rotations include activities in emergency services, lab, surgical services, imaging services, physical therapy, public health, dentistry, chiropractics, and optometry. Administration modeled an adult camp after this youth camp. The adult scrubs camp is geared towards educating a selection of community members each year. CHI St. Alexius Health Carrington developed and implemented a Wellness and Care Management program in 2011. Impacting chronic disease and lowering the incidence of acute care due to neglected chronic disease is a multi-sector effort. For the community at large, data indicates that for every $1 spent on healthcare prevention and wellness, healthcare costs are decreased by $7. Community Based Clinical Services are provided by the CHI St. Alexius Health Carrington Cardiac Rehab Department. Supervised exercise for patients with special health concerns is offered at an affordable cost in the outpatient setting and is available to all community members through the Cardiac Rehab Phase III Program. This program operates at a loss; however, it provides an affordable and needed service to the community to promote wellness for all types of patients. Health Care Support Services are given to assist community members. This category includes assistance with Medicaid and Social Security Disability applications and an annual blood drive sponsored by CHI St. Alexius Health Carrington and North Dakota United Blood Services. Support Groups are also included in this category and are typically established to address social, psychological, or emotional issues related to specific diagnoses or occurrences. Staff at CHI St. Alexius Health Carrington lead the Cardiac Support Group, the Weight Management Support Group, and the Diabetes Support Group. These support groups are free to the public. Health Professions Education CHI St. Alexius Health Carrington, partners with several educational institutions to provide hands on health professions education given at no charge to various disciplines in the health care field such as medical, laboratory, and nursing students. The CHI St. Alexius Health Carrington Auxiliary fundraises annually for scholarship money to be awarded to four local students entering into health related fields. CHI St. Alexius Health Carrington also aids in the administration and management of the Meadows Memorial Fund. This fund's intent is to provide financial assistance that will directly enhance both the long term health and health care available to the citizens of Carrington and area serviced by CHI St. Alexius Health, Carrington, through educational scholarships, training, equipment, and educational programs.
Schedule H, Part VI LINES 2, 4, & 5 AND PART II: NEEDS ASSESSMENT AND COMMUNITY INFORMATIONSubsidized Health Services Subsidized health services are provided by CHI St. Alexius Health Carrington despite a negative margin because the services are needed in the community and would otherwise not be available to meet community needs. As a critical access hospital, cost based reimbursement plays a key role in the services provided. One service line that unfortunately is not reimbursed based on cost is the Ambulance Service. Because the Ambulance falls under the umbrella of a critical access hospital, in addition to direct expenses, there are indirect costs that must be allocated to that department. This, in effect, is a transfer of administrative and general (overhead) expenses away from Medicare reimbursable departments. The result is a service line that continually operating at a loss subsidized by the hospital. The City of Carrington and Foster County acknowledge the importance of the ambulance service and mill levies help to fund a portion of the ambulance service. Research Programs CHI St. Alexius Health Carrington did not have any research community benefit. Financial and In-Kind Contributions CHI St. Alexius Health Carrington provides financial and in-kind contributions to individuals and the community at large. In-kind services include hours donated by staff to the community while on work time. Supplies and equipment are sent to Indigenous People's Hospital and HERO. Clinic based phone a nurse services are offered free of charge to patients for answering questions, providing medical information, communicating with medical staff, and assistance in prescription refills. The Ambulance service also provides standby support for community events such as football games, Fourth of July fireworks, and rodeos. This is just a portion of the activities in this category. In-kind contributions are given to the Faith in Action Program for overhead, office expenses and program coordination. The CHI St. Alexius Health Carrington Faith in Action Committee was developed after a needs assessment in 2009 and continues to meet bi-monthly. The purpose of the Faith in Action Program is to aide older adults and others in need living in the community to obtain access to health care services and allow community members to maintain independence as long as possible. Foster County is a frontier county thus transportation to medical appointments is an indispensible service. The Faith in Action Committee raises money to pay mileage to volunteers that provide patient transportation to medical appointments locally as well as to the larger tertiary care centers in North Dakota. Community Building Activities Community building activities include in-kind contributions and budgeted expenditures for the development of community health programs and partnerships. CHI St. Alexius Health Carrington staff involvement on various committees, boards, and coalitions such as the Carrington Development Corporation, Job Development Authority, Carrington Convention and Visitors Bureau, Carrington Chamber of Commerce, Kiwanis, Lions, Ministerial Association, among others, builds relationships in the community with the medical center. Environmental improvements are supported by CHI St. Alexius Health Carrington through an onsite recycling program. CHI St. Alexius Health Carrington also hosts a robust Health Occupations Class in collaboration with the public school and other health care providers in the community. The class is geared towards workforce development. High school seniors are able to enroll in the class to obtain real life and some hands on exposure to health related careers with rotations in: acute care, cardiac rehab, central supply, clinic, dental, diabetes education, emergency services, laboratory, imaging services, infection prevention, materials management, nursing home, nutritional services, operating room, pharmacy, physical therapy, public health, risk, respiratory therapy, and wellness. Community Benefit Operations Community benefit operations include costs associated with the dedicated staff, the community health needs assessment, the ongoing implementation strategy and planning to address the current community needs, and other costs associated with community benefit operations. CHI St. Alexius Health Carrington, a nonprofit, tax-exempt facility, continues to take the necessary measures to be compliant in determining what to report, how to report, and also conducting periodic community health needs assessments. IV. QUANTITATIVE DESCRIPTION OF COMMUNITY BENEFIT Human Investment Statistics Number of programs: 84 Staff hours: 6,415 Volunteer hours: 10,224 The Total Quantifiable Community Benefit cost for FY16 is $499,456 serving 24,322 people. The FY16 community benefit is 3.3% of CHC's expenses and 2.8% of CHC's revenues.
Schedule H, Part I, Line 3c Eligibility criteria for free or discounted careWhen Catholic Health Initiatives (the ultimate parent organization to Carrington Health Center established its financial assistance policy it was determined that establishing a household income scale based on the HUD very low income guidelines more accurately reflects the socioeconomic dispersions among urban and rural communities in 18 states served by CHI hospitals and health care facilities. In comparing HUD guidelines to the Federal Poverty Guidelines (FPG), we find that on average HUD guidelines compute to approximately 200% to 250% (and sometimes 300%) of FPG. Carrington Health Center bases its financial assistance eligibility on HUD's 130% of Very Low Income Guidelines based on geography, and affords the uninsured and underinsured the ability to obtain financial assistance write-offs, based on a sliding scale, ranging from 25%-100% of charges. An individual's income under the HUD guidelines is a significant factor in determining eligibility for financial assistance. However, in determining whether to extend discounted or free care to a patient, the patient's assets may also be taken into consideration. For example, a patient suffering a catastrophic illness may have a reasonable level of income, but a low level of liquid assets such that the payment of medical bills would be seriously detrimental to the patient's basic financial (and ultimately physical) well-being and survival. Such a patient may be extended discounted or free care based upon the facts and circumstances.
Schedule H, Part I, Line 7g Subsidized Health ServicesThere are no physician clinics included in subsidized health services.
Schedule H, Part I, Line 7 Bad Debt Expense excluded from financial assistance calculation472270
Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceThe organization's cost accounting system was used to report financial assistance and community benefits in Part I, Line 7. The organization's cost accounting system addresses all of the patients served by Carrington Health Center.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountThe organization has reported bad debt expense at gross charges written off. The organization's bad debt expense represents amounts billed to patients that was deemed uncollectible and does not include any charges that were ultimately reimbursed or discounted. Patient discounts are recorded in contractual allowance or financial assistance, as appropriate, as an offset to gross revenue and are not included in bad debt expense.
Schedule H, Part III, Line 3 Bad Debt Expense MethodologyCarrington Health Center does not believe that any portion of bad debt expense could reasonably be attributed to patients who qualify for financial assistance since amounts due from those individuals' accounts will be reclassified from bad debt expense to charity care within 30 days following the date that the patient is determined to qualify for charity care.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteCarrington Health Center does not issue separate company audited financial statements. However, the organization is included in the consolidated financial statements of Catholic Health Initiatives. The consolidated footnote reads as follows: "The provision for bad debts is based upon management's assessment of historical and expected net collections, taking into consideration historical business and economic conditions, trends in health care coverage, and other collection indicators. Management routinely assesses the adequacy of the allowances for uncollectible accounts based upon historical write-off experience by payor category. The results of these reviews are used to modify, as necessary, the provision for bad debts and to establish appropriate allowances for uncollectible net patient accounts receivable. After satisfaction of amounts due from insurance, CHI follows established guidelines for placing certain patient balances with collection agencies, subject to the terms of certain restrictions on collection efforts as determined by each facility. The provision for bad debts is presented on the consolidated statement of operations as a deduction from patient services revenues (net of contractual allowances and discounts) since CHI accepts and treats all patients without regard to the ability to pay."
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsCarrington Health Center is designated as a Critical Access Hospital ("CAH"). CAHs are rural community hospitals that are certified to receive cost-based reimbursement from Medicare. The reimbursement that CAHs receive is intended to improve their financial performance and thereby reduce hospital closures. CAHs are certified under a different set of Medicare Conditions of Participation (CoP). Shortfalls are created when a facility receives payments that are less than the costs of caring for program beneficiaries. Because shortfalls are based on costs, not charges, Carrington Health Center, due to their designation as a CAH, received cost-based reimbursement for Medicare purposes, Carrington Health Center will not experience Medicare related shortfalls. Although not presented on the Medicare cost report, in order to facilitate a more accurate understanding of the "true" cost of services (for "shortfall" purposes) the CHI Workbook allows a health care facility not to offset costs that Medicare considers to be non-allowable, but for which the facility can legitimately argue are related to the care of the facility's patients. In addition, although not reportable on the Medicare cost report, the CHI workbook includes the cost of services that are paid via a set fee-schedule rather than being reimbursed based on costs (e.g. outpatient clinical laboratory). Finally, the CHI Workbook allows a facility to include other health care services performed by a separate facility (such as a physician practice) that are maintained on separate books and records (as opposed to the main facility's books and records which has its costs of service included within a cost report). True costs of Medicare computed using this methodology: Total Medicare Revenue: $7,776,414 Total Medicare costs: $8,322,110 Surplus or Shortfall $ (545,696) Carrington Health Center believes that excluding Medicare losses from community benefit makes the overall community benefit report more credible for these reasons: Unlike subsidized areas such as burn units or behavioral-health services, Medicare is not a differentiating feature of tax-exempt health care organizations. In fact, for-profit hospitals focus on attracting patients with Medicare coverage, especially in the case of well-paid services that include cardiac and orthopedics. Significant effort and resources are devoted to ensuring that hospitals are reimbursed appropriately by the Medicare program. The Medicare Payment Advisory Commission (MedPAC), an independent Congressional agency, carefully studies Medicare payment and the access to care that Medicare beneficiaries receive. The commission recommends payment adjustments to Congress accordingly. Though Medicare losses are not included by Catholic hospitals as community benefit, the Catholic Health Association guidelines allow hospitals to count as community benefit some programs that specifically serve the Medicare population. For instance, if hospitals operate programs for patients with Medicare benefits that respond to identified community needs, generate losses for the hospital, and meet other criteria, these programs can be included in the CHA framework in Category C as "subsidized health services." Medicare losses are different from Medicaid losses, which are counted in the CHA community benefit framework, because Medicaid reimbursements generally do not receive the level of attention paid to Medicare reimbursement. Medicaid payment is largely driven by what states can afford to pay, and is typically substantially less than what Medicare pays.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistanceCarrington Health Center's debt collection policy provides that Carrington Health Center will perform a reasonable review of each inpatient account prior to turning an account over to a third-party collection agent and prior to instituting any legal action for non-payment, to assure that the patient and patient guarantor are not eligible for any assistance program (e.g. Medicaid) and do not qualify for coverage through Carrington Health Center community assistance policy. After having been turned over to a third-party collection agent, any patient account that is subsequently determined to meet the Carrington Health Center community assistance policy is required to be returned immediately by the third-party collection agent to Carrington Health Center for appropriate follow-up. Carrington Health Center requires its third-party collection agents to include a message on all statements indicating that if a patient or patient guarantor meets certain stipulated income requirements, the patient or patient guarantor may be eligible for financial assistance. All of Catholic Health Initiatives' hospitals' contracts with third party collection agencies include the following standards: * Neither CHI hospitals nor their collection agencies will request bench or arrest warrants as a result of non-payment; * Neither CHI hospitals nor their collection agencies will seek liens that would require the sale or foreclosure of a primary residence; and * No Catholic Health Initiatives' collection agency may seek court action without hospital approval. Finally, collection agencies are trained on the Catholic Health Initiatives Mission, Core Values and Standard of Conduct to make sure all patients are treated with dignity and respect.

Schedule H, Part VI, Line 3 Patient education of eligibility for assistanceCarrington Health Center includes information concerning its financial assistance policy on its website. In addition, Carrington Health Center prominently displays its financial assistance policy in both English and Spanish in obvious locations throughout the hospitals, including the emergency rooms and other patient intake areas, as well as in Carrington Health Center outpatient facilities. In addition, Carrington Health Center registration clerks are trained to provide consultation to those who have no insurance or potentially inadequate insurance concerning their financial options including application for Medicaid and for financial assistance under Carrington Health Center's financial assistance policy. Upon registration (and once all EMTALA requirements are met), patients who are identified as uninsured (and not covered by Medicare or Medicaid) are provided with a packet of information that addresses the financial assistance policy and procedures including an application for assistance. Carrington Health Center registration clerks read the organization's medical assistance policy to those who appear to be incapable of reading, and provide translators for non English-speaking individuals. Carrington Health Center's staff will also assist the patient/guarantor with applying for other available coverage (such as Medicaid), if necessary. Counselors assist Medicare eligible patients in enrollment by providing referrals to the appropriate government agencies.
Schedule H, Part VI, Line 6 Affiliated health care systemCarrington Health Center, along with its affiliated outpatient facilities, are part of Catholic Health Initiatives. Catholic Health Initiatives (CHI) is a national faith-based nonprofit health care organization with headquarters in Englewood, Colorado. CHI's exempt purpose is to serve as an integral part of its national system of hospitals and other charitable entities, which are described as market-based organizations, or MBOs. An MBO is a direct provider of care or services within a defined market area that may be an integrated health system and/or a stand-alone hospital or other facility or service provider. CHI serves as the parent corporation of its MBOs which are comprised of 104 hospitals, including four academic medical centers, and 30 critical access facilities; community health service organizations; accredited nursing colleges; home health agencies; and other facilities that span the inpatient and outpatient continuum of care. Together, these facilities provided $1.1 billion in financial assistance and community benefit in the 2016 fiscal year, including services for the poor, free clinics, education and research. CHI provides strategic planning and management services as well as centralized "shared services" for the MBOs. The provision of centralized management and shared services - including areas such as accounting, human resources, payroll and supply chain -- provides economies of scale and purchasing power to the MBOs. The cost savings achieved through CHI's centralization enable MBOs to dedicate additional resources to high-quality health care and community outreach services to the most vulnerable members of our society. Carrington Health Center operates with its wholly owned affiliates and community partners, along with its fundraising arm, the Carrington Health Center Foundation, to serve the health care needs of the Carrington, North Dakota communities.

Compensation of the senior most executives is reviewed annually. The Korn Ferry Hay Group reviews both cash and total compensation for overall reasonableness, for adherence to CHI's compensation philosophy, and for comparability to the not-for-profit healthcare market. This independent review is delivered by Korn Ferry Hay Group to the HR committee of the CHI Board of Stewardship Trustees annually at their September meeting and minutes are shared with the full board at the December meeting. The last review was September 13, 2016. In addition, Korn Ferry Hay Group completed a comprehensive review of all positions at the level of vice president and above in the fall of 2014 to determine and validate appropriate compensation levels. Any such changes are not re-submitted to the board.
Form 990, Part VI, Line 12c Conflict of interest policyCatholic Health Initiatives ("CHI") has a Conflicts of Interest ("COI") policy in place to maintain the integrity of all of its activities. The policy applies to CHI Board of Stewardship Trustees and members of its committees; all board and board committee members of CHI Entities; all CHI employees; all CHI physicians (both employed and non-employed) and all physician administrators and leaders; advanced practice clinicians (both employed and non-employed); and all CHI research personnel (both employed and non-employed). Disclosure, review and management of perceived, potential or actual conflicts of interest are accomplished through a defined COI disclosure process. Each person has a general ongoing obligation to promptly and fully report to his/her direct manager, supervisor, medical staff office, board or board committee chair any situation or circumstance that may create a conflict of interest. The person must report the actual or potential conflict as soon as she/he becomes aware of it. In any situation where the person may be in doubt, a full disclosure should be made to permit an impartial and objective determination. In addition to the general ongoing obligation, there are initial disclosure obligations. The board, board committee members, and new employees are required to make disclosures at the time of their initial hiring/appointment. All non-employed, credentialed or contracted physicians are required to make disclosures at the time of their credentialing and during any subsequent reappointment or recredentialing. All researchers are required to make disclosures upon consideration of affiliation with a research sponsor. In addition to the general ongoing and initial disclosure obligations, there is an annual disclosure obligation. All corporate officers, board and board committee members, employees at the level of manager and above, researchers, supply chain employees, employed physicians, physician administrators and leaders, and employed advanced practice clinicians must complete a new conflict of interest disclosure annually. Disclosures of perceived, potential or actual conflicts involving financial interests are forwarded to the Conflicts of Interest Review Committee ("C-CIRC") or Legal Services Group for review depending on the position of the person involved. The C-CIRC reviews COI questionnaires containing disclosures of perceived or possible conflicts for employees at a level of manager or above, supply chain employees, researchers and physicians, physician administrators and leaders, and advanced practice clinicians (both employed and non-employed). In the determination of a conflict, a COI management plan will be developed for that person. With respect to those audiences for which the C-CIRC has review responsibility, the C-CIRC will facilitate development of any such conflict of interest management plan in collaboration with local CRP staff. A designated CHI Entity staff will be responsible for monitoring the COI management plan and for documenting monitoring activities. At its sole discretion, a CHI Entity may reject a Person's request to enter into the relationship in question, or require the relationship be sufficiently altered to avoid a potential COI. If the C-CIRC determines that there is a potential or actual conflict of interest that does not currently have appropriate controls to address the conflict of interest, it may recommend that the disclosing person be allowed to participate in the activity or transaction subject to restrictions as outlined in the COI management plan. If a Person does not agree with a determination made by the C-CIRC, its interpretation of the Policy or Addenda, or seeks an exemption or exception, the following steps should be followed. The Employee disputing the review decision, interpretation of the Policy, or seeking exemption or exception must present the matter to the Employee's immediate direct manager or supervisor for review and determination. If the Employee and the manager do not agree with the review decision, interpretation of the Policy, or seek exemption or exception, the manager shall consult with the manager's Vice President (or higher if the manager is a Vice President) to reach a determination. If the matter remains unresolved, it shall be referred to the CHI Vice President of Human Resources and the CHI Corporate Responsibility Officer. If they are unable to reach agreement, the matter shall be referred to the CHI General Counsel, whose decision shall be final. Reviews and determinations involving board and board committee members and corporate officers will be the responsibility of the board, board executive committee, or board chair, with guidance from the Legal Services Group (LSG). Annual COI disclosures of all trustee and corporate officers will be reviewed by the CHI Senior Vice President, Legal Services, and General Counsel or his or her designee who will report potential conflicts to the applicable Board Chair. The Board Chair or designee shall make such further investigation of any conflict of interest disclosures as he or she may deem appropriate. If the conflict involves the Board Chair, the Vice Chair will assume the Chair's role. Based on review and evaluation of the relevant facts and circumstances, the Board Chair will make an initial determination as to whether a conflict of interest exists and whether, pursuant to the COI Policy, review and approval or other action by the Board is required. A written record of the Board Chair's determination, including relevant facts and circumstances, will be made. The Board Chair shall then make an appropriate report to the Executive Committee of the Board concerning such review, evaluation and determination. If a difference of opinion exists between the Board Chair and another Trustee as to whether the facts and circumstances of a given situation constitute a conflict of interest or whether Board review and approval or other action is required within the COI Policy, the matter shall be submitted to the Board's Executive Committee, which shall make a final determination as to the matter presented. Such determination, including relevant facts and circumstances, will be reflected in the Executive Committee minutes and will be reported to the Board. When any conflict of interest is considered by the board, the trustee or corporate officer, as appropriate, must disclose all of the material facts to the Board. The trustee shall not vote and the trustee or corporate officer shall not use his or her personal influence on the matter. The trustee or corporate officer shall be excused from the meeting during discussion and vote on the conflict of interest. In reviewing such transactions between CHI or CHI Entities and vendors or other contractors who are, or are affiliated with, Trustees or Corporate Officers, the Board will act as it would in reviewing transactions with unrelated third parties. The transaction is not to be approved unless the Board determines that the transaction is fair to CHI or the CHI Entity. The Board must approve the transaction by a majority of the Trustees on the Board, without counting the vote of any individual who has an interest in the transaction. All determinations of conflicts of interest are reported as required by law, regulations, and CHI policy.
