DescSchoolcraft Memorial Hospital (SMH), a component unit of Schoolcraft County, is organized under Public Act of 230 and is a county hospital corporation that operates a 12-bed acute care facility. SMH provides comprehensive inpatient, outpatient, emergency, medical clinic, and home health services in total. Healthcare is offered in at least 30 specialties, some of which include cardiology, neurology, general and orthopedic surgery, a 24-hour physician staffed emergency room, a walk-in clinic, physical and occupational therapy, cardiac rehabilitation, and a wide range of diagnostic services including a full-service laboratory, x-ray, mammography, CT scans, MRI's, and nuclear medicine. SMH is an equal opportunity provider and employer. (continued on Schedule O)During 2024, SMH had 1,159 acute and swing bed patient days, 1,382 inpatient and outpatient surgeries, 65,822 outpatient visits, and 344 Home Health Admits. The Hospital provides health care services to patients who meet certain criteria under its charity care policy without charge or at amounts less than established rates. Since the Hospital does not pursue collection of these amounts, they are not reported as patient service revenue. The estimated cost of providing these services was $312,000 in 2024, calculated by multiplying the ratio of cost to gross charges for the Hospital by the gross uncompensated charges associated with providing charity care to its patients.Desc

Schoolcraft Memorial HospitalPart V, Section B, Line 7d: The implementation strategy has been shared on our website and was used to bolster behavioral health access and increased community education opportunities.
Schoolcraft Memorial HospitalPart V, Section B, Line 11: In the most recent 2024 Community Health Needs Assessment, community members identified the following top 10 community health issues as most important:1. Shortage of dentists or lack of affordable dental care2. Lack of reliable childcare3. Health insurance is expensive or has high costs for co-pays and deductibles4. Substance use (drug abuse)5. Lack of health insurance6. Lack of programs and services to help seniors maintain their health and independence7. Lack of affordable healthy foods8. Lack of housing and programs for people with Alzheimer's disease/dementia9. Shortage of mental health programs and services10. Access to pediatric servicesThese priorities were determined through mailed community surveys, focus groups, and key informant interviews, combined with secondary data sources highlighting health disparities and service gaps in Schoolcraft County, and the surrounding service area.During the 2024 CHNA implementation planning process, Schoolcraft Memorial Hospital selected Food Access, Pediatric Services, and Substance Use as the top priorities to address. The following needs were identified in the CHNA but were not selected as primary priorities due to existing community efforts, resource limitations, or because other organizations are better equipped to lead:* Health insurance is expensive or has high co-pays and deductibles Why not selected: The health insurance landscape is complex, and SMH recognizes its limited ability to make a broad impact as an independent critical access hospital. With this understanding, SMH focuses on helping patients with price transparency, access, and available resources.* Lack of reliable childcare Why not selected: SMH recognizes that childcare is a community-wide issue influenced by many factors but recognizes limited capacity to lead these efforts. SMH supports and promotes the efforts of local partners already addressing this need and will continue to connect families to available resources.* Lack of health insurance Why not selected: Access to health insurance is complex and influenced by broader policies and systems beyond the hospital's direct role and capacity. SMH remains committed to helping community members understand and connect with available coverage options and resources.* Shortage of dentists or affordable dental care Why not selected: Affordable dental care is a recognized need but comes with challenges that extend beyond SMH's current capacity. The hospital will continue exploring ways to support and partner with organizations that can help improve access.* Lack of programs/services for seniors to maintain health and independence Why not selected: SMH recognizes that meeting the needs of older adults often requires significant community collaboration and resources. SMH will continue to work alongside partners and share education to support healthy aging where possible.* Housing and programs for Alzheimer's/Dementia care Why not selected: Specialized dementia care and housing require long-term planning and broader community investment, which extends beyond the capacity of an independent critical access hospital. SMH will continue to support partnershipswith organizations already providing these services, but is not positioned to lead this effort.* Shortage of mental health programs and services Why not selected: SMH recognizes that expanding mental health care is an ongoing challenge in their rural communities but has limitations in staffing and funding to lead these efforts. SMH has enhanced behavioral health services in the past and will continue to collaborate with regional providers to maintain access. In 2024, Schoolcraft Memorial Hospital completed a new Community Health Needs Assessment. The implementation plan for the newly identified priorities and strategies will be developed in 2025.In-Person Focus Groups:* Business Owners Focus Group (10/21/24)* Older Adults Focus Group (10/21/24)* All Community Focus Group (10/22/24)* Senior Center (10/22/24)* Families with Children Focus Group (10/23/24) Key Informant Interviews: 10/22/24- 11/15/24 Community Survey Sent Out: October 15th, 2024Community Survey Closed: November 10th, 2024 CHNA Draft: Completed December 9th, 2024(Continued on Part V, Page 8)

Part I, Line 3c:Schoolcraft Memorial Hospital offers a self-pay discount of 41% of gross charges for uninsured patients regardless of income. Medical indigency and underinsurance status are also used to determine eligibility.
Part I, Line 7:Financial assistance was calculated using the cost-to-charge ratio. Unreimbursed Medicaid and subsidized health services were calculated using the cost report and analyzing the Medicare ratio of cost to charge for the respective cost centers.
Part I, Ln 7 Col(f):The amount of bad debt expense backed out of total expenses to determine the percentage was $1,052,327.
Part III, Line 2:The organization determines its bad debt expense based on actual charges. Self-pay discounts are applied to patient accounts before balances are deemed to be bad debt.
Part III, Line 3:The amount listed on line 3 is an estimate of community care applications not returned. Approximately 60% of applications were not returned that may have qualified for community care.
Part III, Line 4:The footnote to the organization's financial statements addressing implicit price concessions may be found on page 18 of the attached audited financial statements.
Part III, Line 8:The shortfall should be considered as community benefit as this amount is not included in bad debt and no effort is made to collect this shortfall. In addition, providing services to the Medicare population is a community benefit as the elderly are a charitable class and providing them with healthcare helps the community.

If the patient does not respond the SMH efforts, the account is referred to an external collection agency.
Part VI, Line 2:The CHNA is the only formal assessment of broad scope that the hospital has utilized in recent years to assess the health care needs of the community. The Upper Peninsula also conducts a Community Health Needs assessment for the entire UP but that data was not available in 2024.
