However, since many of the priorities were the same as those identified in 2013, Grady continued that work into 2016. Below are the three activities related to the priorities identified in Gradys 2016 CHNA: 1. Improve coordination of care for Grady patients with diabetes, hypertension, prostate cancer, HIV/AIDS and behavioral health conditions. Grady was a lead partner in establishing a statewide Community Health Worker Convention with the purpose of creating a common understanding among stakeholders on the definition, roles and value of community health workers (CHW); and fostering alignment of CHW efforts in supporting health improvement and healthcare access in Georgia. Grady continued to lead in community collaboration through the Atlanta Regional Collaborative for Health Improvement (ARCHI). Grady has representation on the Steering Committee and has participated in community-wide care coordination planning and implementation for ARCHIs Tri-Cities Pilot Project. Grady has met with the Tri-Cities community through this project to address barriers to access for care. In 2016, ARCHI launched an effort to address a common CHNA priority, diabetes, among several healthcare providers in ARCHI. Grady joined this effort in 2016 and will continue to partner with Kaiser Permanente, Wellstar and Piedmont Healthcare to collaboratively address diabetes among underserved communities. 2. Increase opportunities for all Georgians, with a focus on persons served by Grady Health System, to access healthcare coverage. Grady has engaged a number of resources to assist patients in gaining access to Medicaid and Marketplace health insurance. In 2016, more than 4,100 individuals were approved for Medicaid, and 234 were enrolled in Marketplace insurance (110 in OEP3 January 2016; 124 in OEP4 November-December 2016). Grady partnered with CSRA and Health Initiative In-Person Assisters and Health Insurance Navigators to assist our patients during Open Enrollment. 3. Increase patient and community engagement in healthy behaviors to prevent diabetes, hypertension, HIV, unintentional injuries, and homicide. In 2016, Grady launched a Fruit and Vegetable Prescription (FVRx) Program in partnership with Wholesome Wave Georgia, Open Hand, and Common Market. FVRx is a multicomponent clinic-based program intended to increase access to healthy foods among low-income patients with diet-related chronic diseases. For six months, patients meet with a clinician for a monthly nutrition class, attend six cooking classes, and receive a prescription for fruits and vegetables, which is filled onsite by Common Market. Sixteen patients completed the program in 2016, and Grady is expanding the program in 2017. In 2016, the Injury Prevention Program facilitated 50 injury prevention presentations and educational activities across the state. Topics included safe driving, fall prevention, violence prevention, child physical abuse prevention, and general trauma awareness. More than 1,500 individuals representing the general public (teens, parents, older adults), nurses, doctors, EMS providers, leadership, and others attended
PART V, SECTION B, LINE 11, 3.(CONTINUED)the events. Partnering agencies included the Centers for Disease Control AND Prevention, Childrens Healthcare of Atlanta, Georgia EMS, Latin American Association, US MedClinic, local senior centers, local schools, and more. Grady continues to partner with the Atlanta Beltline to increase access to safe parks, trails and health education. Grady is currently on the Beltline Health Steering Committee which provides guidance on a range of health programming. In 2016, Grady began the process of applying for a Health Grant to support a wellness program for community members in partnership with the BeltLines Westside Trail opening in 2017. Grady continues to operate the largest HIV program in Georgia, providing treatment to more than 5,800 people, approximately 25% of people living with HIV in the state. Because strict adherence to antiretroviral therapy (ART) keeps people healthy and significantly reduces risk of transmission, Grady is committed to providing excellent care to all HIV positive patientS. THE LOWER PRIORITIES OF GRADY'S 2016 CHNA- LOW BIRTH WEIGHT(LBW), OBESITY, breast cancer, poverty, and high school education non-attainment were not included as areas of focus for several reasons. First, Grady IS already offering a robust program to address LBW as well as continuously working to improve our services for breast cancer. For the other priorities, Grady does not have the resources or expertise to significantly address these community health needs. Finally, other ARCHI partners with the resources and expertise are leading various initiatives to address these needs. PART V, SECTION B, LINE 15, BOX E The Financial Counselors will advise eligible patients verbally as to where and how to apply for Food Stamps and determine if they meet criteria for Medicaid. Social Workers will typically provide information regarding housing, Food Stamps, and other services to patients. A Financial Counselor may determine if a patient meets criteria for Presumptive Medicaid, Women's Health Medicaid, Women Medicaid Wavier, Cancer State Aid, Crime Victims Compensation Program, Right From The Start Medicaid for newborns, Emergency Medical Assistance - Medicaid for undocumented women who deliver their newborns, Presumptive Medicaid for Pregnant women, Low Income Medicaid, etc., and complete the application as appropriate. The Financial Counselor may also refer a patient to apply for Medicaid with the Department of Family and Children Services Grady Outreach Unit. GMHC haS vendor partnerships on campus whereby representatives are completing applications for eligible patients for various Medicaid Programs, e.g., Low Income Medicaid, SSI, SSD, ETC. representatives are completing applications for eligible patients for various Medicaid Programs, e.g., Low Income Medicaid, SSI, SSD, ETC.
PART V, LINE 20E:ADMISSIONS REPRESENTATIVE USUALLY NOTIFIES PATIENTS OF FINANCIAL ASSISTANCE ON ADMISSION. NONE OF THE ACTIONS LISTED IN LINE 19 ARE FOLLOWED. PART V, LINE 22D: AFTER INITIAL MEDICAL EVALUATION, FAMILY COMMUNICATES/PROVIDES FINANCIAL PROOF, RESIDENCY INFORMATION, ETC. SO THAT THE ELIGIBILITY REQUIREMENTS FOR FINANCIAL ASSISTANCE ARE MET. THE SLIDING SCALE IS DISCUSSED IN LINE 13A-E.

PART I, LINE 3C:THE FEDERAL POVERTY GUIDELINES(FPG) ARE USED TO DETERMINE THE ELIGIBILITY FOR FREE OR DISCOUNTED CARE. PART I, LINE 6A: 2015 Community Benefit Report was prepared in 2016. PART I, LINE 7G: GRADY SUBSIDIZES THE COST OF CARING FOR UNINSURED PATIENTS INCLUDING THE COST OF PHYSICIANS, OTHER CARE GIVERS, DRUGS AND SUPPLIES. PART I, LINE 7, COLUMN F: THIS IS THE PERCENTAGE OF NET COMMUNITY BENEFIT OF TOTAL EXPENSE OF GRADY. PART I, LINE 7: CHARITY CARE AND CERTAIN OTHER COMMUNITY BENEFIT COSTS WERE DETERMINED USING DATA FROM THE AUDITED FINANCIAL STATEMENTS AND THE 2016 FILED MEDICARE AND MEDICAID COST REPORTS. PART II, LINE 6: Grady is a member organization of the Atlanta Regional Collaborative for Health Improvement (ARCHI). ARCHI is an interdisciplinary coalition working to improve the regions (DeKalb and Fulton Counties) health through a collaborative approach to CHNAs and subsequent health improvement initiatives. Grady holds a seat on the ARCHI Steering Committee for ongoing leadership and connectivity to health improvement initiatives, and has signed the ARCHI membership agreement in support of sustainability and an organization structure. Grady also provides funding to ARCHI to support the staffing, data analytics, and partnership building activities. Grady continues to work with and through ARCHI to conduct its CHNAs in order to maximize the impact of community investment in health improvement.
PART III, LINE 2:THE PROVISION FOR BAD DEBT EXPENSE IS BASED UPON A COMBINATION OF THE PAYOR SOURCE, THE AGING OF RECEIVABLES AND MANAGEMENT'S ASSESSMENT OF HISTORICAL AND EXPECTED NET COLLECTIONS, TRENDS IN HEALTH INSURANCE COVERAGE, AND OTHER COLLECTION INDICATORS. PART III, LINE 4: GMHC ALSO INCURS SIGNIFICANT COST ASSOCIATED WITH CARE FOR THE UNDER AND UNINSURED THAT DO NOT APPLY AND/OR QUALIFY FOR CHARITY CARE ASSISTANCE. GMHC INCURRED BAD DEBT EXPENSE OF APPROXIMATELY $415.7 MILLION VALUED IN GROSS CHARGES. PART III, LINE 8: LINE 6 EXPENSES ARE REPORTED FROM THE MEDICARE COST REPORT CMS-2552-96 FOR THE YEAR ENDED 12/31/16. PART III, LINE 9B: THE ORGANIZATION HAS UNIQUE ELIGIBILITY CODES TO IDENTIFY EACH PATIENT QUALIFYING FOR CHARITY CARE TO ALLOW IT TO WRITE-OFF THE CHARITY CARE AMOUNTS PRIOR TO THE COLLECTION PROCESS.
PART VI, 2. NEEDS ASSESSMENTIn addition to Gradys CHNA, many of the coalitions that Grady participates in, or organizations that Grady works with, also assess the needs of their communities or target populations, many of which align or overlap with Gradys community. Coalitions or partner organizations with assessments that also inform Gradys work include Arthur Blank Foundations Westside on the Rise Initiative, Atlanta BeltLine Partnership, Georgia State Department of Public Health, Atlanta, Regional Commission, United Way, Mercy Care, and the Atlanta Community Food Bank. PART VI, 3. PATIENT EDUCATION OF ELIGIBILITY FOR ASSISTANCE PER STATE REGULATIONS, GMHC PLACES ANNUAL ICTF NOTICES IN THE LOCAL NEWSPAPER AND SIGNAGE IS POSTED, ADVISING PATIENTS OF ALL CHARITY CARE PROGRAMS WITHIN THE ORGANIZATION. BASED ON THE PATIENT'S FINANCIAL CIRCUMSTANCES, AND MEDICAL CONDITION, A FINANCIAL COUNSELOR WILL CONSULT WITH THE PATIENT TO DETERMINE BEST FIT FOR THE CRITERIA OF THE VARIOUS ASSISTANCE PROGRAMS. THE APPROPRIATE APPLICATION IS COMPLETED AND THE FINANCIAL ASSISTANCE PROGRAM IS EXPLAINED TO THE PATIENT SIMUTANEOUSLY.
PART VI, 4. COMMUNITY INFORMATION:GMHC IS THE SAFETY NET HEALTHCARE PROVIDER FOR THE CORE OF METROPOLITAN ATLANTA. THIS AREA INCLUDES FIVE COUNTIES AND MORE THAN 3 MILLION PEOPLE. THROUGH ANALYZING THE AVAILABLE DATA SOURCES, GMHC'S PRIMARY SERVICE AREA IS FULTON AND DEKALB COUNTIES, WHICH CONTAIN 1.7 MILLION PEOPLE INCLUDING THE ENTIRE CITY OF ATLANTA, GEORGIA, (17 PERCENT OF THE STATE OF GEORGIA'S POPULATION). According to Gradys 2016 CHNA, the African-American population constitutes more than half of the population in DeKalb County and more than 40 percent of the population in Fulton County, while the Hispanic/Latino population constitutes about 10 percent in both counties. The DeKalb and Fulton populations are also relatively young. More than 40 percent of the population in both counties is in the 18-44 age cohort and the median age is 35 years. THE PERCENTAGE OF THE POPULATION LIVING AT OR BELOW THE FEDERAL POVERTY LEVEL (FPL)-$24,300 FOR A FAMILY OF FOUR- HAS INCREASED IN BOTH COUNTIES. NEARLY 40 PERCENT AND 35 percent of DeKalb and Fulton residents, respectively, live at 200 percent of the FPL. PART VI, 5. PROMOTION OF COMMUNITY HEALTH: GMHC HAS NUMEROUS COLLABORATIONS WITH FULTON AND DEKALB COUNTIES TO DELIVER PRIMARY CARE. IN ADDITION, GMHC COLLABORATES WITH THE LOCAL FEDERALLY QUALIFIED HEALTH CENTERS AS WELL AS EMORY AND MOREHOUSE SCHOOLS OF MEDICINE. GMHC HAS A NOT-FOR-PROFIT COMMUNITY BOARD PROVIDING OVERSIGHT TO ONE OF THE LARGEST SAFETY NET HOSPITALS IN THE SOUTHEAST AND THE LARGEST TRAUMA CENTER IN GEORGIA. Grady is committed to providing critical services our community and state count on every day. Grady is the exclusive 911 provider for Atlanta covering 132 square miles and handling more than 120,000 emergency and non-emergency ambulance requests annually. Grady is also the 911 EMS provider for six counties across Georgia. Grady continues to operate the Georgia Poison Center (GPC), which has been serving the state of Georgia since 1970. GPC provides free assistance and expertise to diagnose and manage human and animal poisonings. This service provides a benefit of more than $4 million to the people of Georgia. Grady is also always looking for ways to innovate and provide better care for our patients and community. For example, Gradys Mobile Integrated Health service conducts home visits for patients who have a high risk of readmission or frequently use 911 for non-emergent care. In 2016, this service expanded from one shift per week to three or four shifts per week and added new providers.
PART VI, 5. PROMOTION OF COMMUNITY HEALTH (CONTINUED)Lastly, Grady is also working to ensure our economic impact is also benefiting our community. In less than five years, Grady has grown supplier diversity from 5% to 28% and extended diversity requirements to subcontractors. Because economic security directly impacts health access and outcomes, these investments will promote equity and contribute to improvements in health and well-being among those disproportionately affected by health disparities. Gradys award winning Supplier Diversity program is the only program of its kind among hospitals in the region, and we are committed to being a pioneer in this important field. PART VI, 6. AFFILIATED HEALTH CARE SYSTEM WHILE GRADY IS NOT LEGALLY AFFILIATED WITH THE EMORY HEALTHCARE SYSTEM, WE ARE THE PRIMARY TRAINING SITE FOR MORE THAN 1,000 EMORY AND MOREHOUSE INTERNS AND RESIDENTS EACH YEAR. PART VI, 7. STATE FILING OF COMMUNITY BENEFIT REPORT: GMHC FILES A COMMUNITY BENEFIT REPORT IN GEORGIA.
