Form 990, Part IV, Line 34:	NCIRE is affiliated with the U.S. Department of Veterans Affairs in San Francisco, California. This affiliation does not meet the definition of relationship for purposes of Form 990 since NCIRE cannot legally be owned or controlled by the government entity it is affiliated with. The enabling federal statute (Public Law 111-163, Title 38-Subchapter IV, 7361) which established NCIRE as a nonprofit corporation providing a "flexible funding mechanism for the conduct of research and education at the (San Francisco VA) medical center", states the following: (2) A corporation under this subchapter is not (A) owned or controlled by the United States; or (B) an agency or instrumentality of the United States.

The final draft of the 990 is reviewed by the CFO and the Executive Director. The Board receives the final version of the Form prior to filing.

For directors and officers conflict of interest policy, the Executive Director's office requests completion of an annual disclosure form by January 31 for all directors and officers.
