DescOver the course of 2016, America's Agenda: Health Care for All (AAHCA) program services continued to promote and expand delivery of higher quality patient care, while achieving more affordable overall health costs and a substantially improved patient experience. During 2016, in addition to AAHCAs ongoing program emphasis on advanced primary care design and enhanced coordination of patient care across all care settings, AAHCA added a new dimension to our ongoing work: improved affordability of prescription medications.AAHCAs updated program services, described below n more detail with key accomplishments, are:1. Campaign to transform delivery of health care for New Jersey public employees (expanded from the initial AAHCA focus advocacy for high-value primary care design to include advocacy for modernized, cost-saving acquisition of prescription medicines)2. Campaign to transform delivery of care to employees across the packinghouse industry (expanded to include care delivery transformation in other sectors of the food industry)3. Call to Action to Self-funded Employer Health Plans to Lead in Transformation of America's Health Care (a campaign aimed at accelerating regional and industry-wide high-value care delivery transformation through leadership of self-funded health plans)Detailed Program Activities:Throughout 2016, America's Agenda: Health Care for All ("AAHCA") continued implementation of our initiative to transform healthcare delivery for approximately 800,000 New Jersey public employees, dependents, and retirees. The initiative aims to introduce, on a large scale, innovative new strategies for delivering and coordinating high quality care at lower costs that could save hundreds of millions of dollars in State health care spending per year, relieving pressure to increase taxes on New Jersey residents, while producing improved health outcomes for public employees. The primary goal is to create networks of advanced, high-value, direct primary care health centers in communities where high densities of public employees live and work. It aims to allow private sector employers and employees to access the direct primary care medical homes established initially to serve public employees, thereby dispersing the benefits of higher value care across entire New Jersey communities. In 2015, the AAHCA initiative resulted in a landmark agreement between New Jersey public employee unions and the current State administration to establish a Direct Primary Care Medical Home Pilot Program in which about 400,000 New Jersey State and local government employees and dependents would be eligible to participate. In 2016, the AAHCA effort resulted in a corollary agreement that added an additional 400,000 NJ public school employees and dependents. The AACA campaign has focused throughout 2016 on keeping implementation of the Pilot Program on track with the Program design specifications. Finalist primary care providers were selected to deliver care to enrolled public employees and dependents. The first Direct Primary Care Medical Home opened its doors to care for New Jersey public employee and dependent patients in November of 2016. At least five more are scheduled to open their doors to public employee and dependent patients in 2017. Concurrent with the work to transform health care delivery in the New Jersey delivery, AAHCA launched an initiative to significantly reduce the cost of prescription pharmacy benefits that had become the fastest growing component of public and private health care costs in New Jersey and in fact, throughout America. The pricing of prescription drugs in large, multi-drug formularies administered by Pharmacy Benefit Managers (PBMs) is so complex that neither the State nor public employee unions could calculate their actual costs, let alone make meaningful comparisons between proposals. During 2016, the States request for proposals (RFP) process failed without selection of a new PBM, leaving the incumbent contract to be extended by default. Faced with such an expensive pharmacy benefit quagmire, AAHCA proposed that the State create a competitive marketplace for PBM selection through deployment of big data technology that could make reliable estimates and meaningful comparison of the cost of complex PBM proposals to provide pharmacy benefits to approximately 750,000 New Jersey public employees and dependents. AAHCA hypothesized that creation of a technology-supported, dynamically competitive market for PBM bidders an online PBM auction for the States pharmacy benefits account could save the State millions of dollars in public funds. Ultimately, the New Jersey Governor and legislative leaders agreed to attempt to implement the AAHCA-advocated approach in lieu of more draconian alternatives. By the end of the end of 2016, legislation was enacted that would allow expedited State contracting of technology that met AAHCA-proposed guidelines for creating dynamic PBM competition for the States prescription pharmacy account. (See AAHCA work cited in: http://www.njspotlight.com/stories/16/11/03/sweeney-plan-could-save-200m-in-prescription-costs-for-public-workers/ , http://www.njspotlight.com/stories/16/12/01/state-seeks-high-tech-oversight-of-2b-annual-spending-on-drug-benefits/ , and http://senatorsweeney.com/cost-saving-bill-prescription-drugs-goes-governor/Over the course of 2015, AAHCA carried forward an advocacy effort to persuade major corporate employers and unionized employees in the meatpacking or "packinghouse" industry to find common ground in redesigning advanced direct primary care delivery for workers at meat packing plants located in communities across several Midwestern and Western states, including Nebraska, Iowa, Minnesota, and Colorado. As many of the packing house plants are the major employers in their relatively small, rural communities, our mission is to harness their potential to drive care delivery transformation that will reduce health costs and improve health outcomes for the entire populations of these communities by opening access for all local residents to advanced direct primary care that was developed, in the first stage, for unionized packinghouse workers. In 2016, the initial packinghouse employer to have agreed to adopt the AAHCA-proposed advanced primary care delivery design also the largest operator In the US packinghouse industry reported that the AAHCA advocated program had reduced overall health care spending for employees at the three pilot plants of more than 10%. Overall health costs had risen by only 0.49% over the prior year. The realized health care savings enabled significant improvement in worker wages. The employer is expanding the innovative care delivery design to eight additional plants. Other packinghouse operators have adopted variations of the AAHCA high-value care delivery design. (These results and the overwhelming positive response of packinghouse workers to their new healthcare experience are summarized in the video, Revolutionizing Healthcare Across the Packinghouse Industry, viewable online at: www.youtube.com/watch?v=sdIrZ6RX5D8 )The progress of AAHCAs campaign to implement high-value primary care transformation in the packinghouse industry and the savings that have been achieved, to date, provides encouraging evidence of the potential to transform care delivery in partnership with employers and unions across the entire food production industry. In 2016, AAHCA expanded the reach of our campaign from the packinghouse industry to advocacy to corporate leaders in food processing and retail grocery industries, as well. AAHCA views the sponsors and trustees of self-funded, employment-based health plans that provide health care to 1/3 of all Americans as key strategic partners in our campaign to achieve high-value transformation of American health care. Self-funded health plan sponsors have unparalleled flexibility to adopt care delivery innovations that can reduce healthcare costs, improve care quality, and expand access to high-value health care, not only to their own participants, but across their local communities and regions. Bolstered by 2016 reports of impressive savings results achieved in AAHCAs private sector initiatives (i.e. in the packinghouse and building services industries) and the expansion of the NJ Direct Primary Care Medical Home Pilot Program, the AAHCA Call to Action to Self-funded Employer Health Plans campaign expanded our reach into additional public and private sector industries. In the public sector, for example, AAHCA advocated for innovative care delivery re-design to public school board members and education union representatives from some 12 states. In a private sector example, Americas Agenda initiated similar discussions with union and employer self-funded health plans sponsors in the Aerospace industry, where we have asserted that high-value redesign of healthcare delivery is key to competitiveness of American manufacturers in an increasingly comDesc
